Shoulder joint function evaluation after reverse total shoulder arthroplasty – preliminary reports

Abstract
Introduction. Physiotherapy is an important part of the comprehensive treatment of patients after reverse total shoulder arthroplasty (RTSA). Patients qualified for RTSA struggle with shoulder joint dysfunctions of various aetiologies – most often with irreparable damage to the rotator cuff tendons. The aim of physiotherapy after RTSA is to restore the lost functions of the shoulder joint based on a targeted rehabilitation protocol taking into account the changed biomechanical conditions of the shoulder.
Materials and methods. The study material consisted of 6 patients after RTSA. Patients were examined before the procedure and after rehabilitation with the use of the Constant-Murley Score. The rehabilitation model was implemented on the day before surgery and continued for 12 weeks.
Results. Each patient experienced an improvement in shoulder mobility and a reduction in pain intensity.
Conclusions. Rehabilitation after RTSA improves the functions of the shoulder joint expressed by an increased range of joint mobility, elimination of pain and improved quality of life.

Abstract

Background. Functional disorder of the shoulder joint on the paretic side is among the more common post-stroke complications. Rehabilitation can be hindered by the associated pain, limited joint mobility, and muscle distension during the period of flaccid paresis (or plegia). The purpose of our study was to evaluate certain shoulder joint structures in post-stroke patients in inpatient rehabilitation, to compare the structural image of the shoulder joint in the affected and healthy limbs, and to identify the impact of rehabilitation on the tested joint structures. Material and methods. In a group of 40 post-stroke patients we performed ultrasonographic examination of the shoulder joints. The study group included patients who were hospitalized from December 2000 to March 2001 in the Department of Rehabilitation at Voivodeship Hospital no. 2 in Rzeszów, Poland. Examinations were performed on the first and last day of hospitalization. Results. Pathological changes were found in the shoulder joints. Over a 3-4-week rehabilitation period some improvement was noted. Conclusions. Changes in shoulder joint structure in post-stroke patients with hemiparesis occur more often on the paretic side (esp. thinning of the rotator cone, the deltoid muscle, and the tendon of the long head of the shoulder biceps, calcification, subluxation in the shoulder joint, and fluid in the bursae). Comprehensive hospital rehabilitation has a positive impact on the tested shoulder joint structures. Ultrasonography is a good method for evaluation of the anatomical structures of the shoulder joint in patients with hemiparesis.

Abstract
The aim of this paper was to evaluate the effect of physiotherapy on upper limb function in post-mastectomy women. Thirty-three women attending the Amazons Post-Mastectomy Women’s Club at the Holy Cross Cancer Centre in Kielce were examined on two occasions at three months’ interval. The participants were divided into Group I of 15 patients regularly attending rehabilitation and Group II of 18 patients practicing rehabilitation occasionally. Bilateral measurement of the angular range of motion in the shoulder was the basic method of assessment. A shoulder range of motion of the upper limb on the operated side smaller by more than 10° in relation to the opposite limb was regarded as ROM limitation. Upper limb circumferences were also measured at 10 cm above and below the lateral humeral epicondyle and in the central metacarpus excluding the thumb to an accuracy of 1 mm. Circumferences of the upper limb on the operated side greater by more than 2 cm compared to the opposite limb were regarded as lymphoedema. Our evaluation of statistical significance of correlations between qualitative parameters (influence of rehabilitation on limitation of the shoulder range of motion and incidence of lymphoedema) relied on the non-parametric Chi-square test (χ²).There was a statistically significant correlation between regular rehabilitation practice and the shoulder range of motion and persistence of upper limb lymphoedema on the mastectomy side.Physiotherapy improves ROM and effectively prevents deterioration of upper limb lymphoedema in post-mastectomy women.

Analysis of the impact of deep tissue massage on the functional state of the shoulder joint after arthroscopy

Abstract
Introduction. Shoulder joint physical therapy after arthroscopy is a complex process, and one of the possible actions used by therapists is deep tissue massage.
Purpose. The purpose of the study was to analyze the impact of deep tissue massage on the functional state of the shoulder after arthroscopy.
Materials and methods. Patient examinations were carried out from July 1, 2014 to February 28, 2015 among patients of the Artros physical therapy clinic in Bialystok. We used a questionnaire containing 10 open questions. The study group included 16 patients who underwent shoulder arthroscopy – 9 men and 7 women.
Results. The largest increase in muscle strength measured by the Lovett scale was recorded during abduction of the upper limb (by 2.5 ± 1.15); in terms of mobility the best results were obtained in abduction (by 79.5° ± 49.40°) and flexion of the limb (by 76° ± 48.33°).
Conclusions. Deep tissue massage, combined with kinesitherapy and physical therapy, increases muscle strength and range of motion in the shoulder joint and reduces pain.

Functional assessment of the shoulder joint in volleyball players from different age groups

Abstract

Objective. The aim of the paper was to perform functional analysis and assessment of the range of movement of the joints of the pectoral girdle in the dominant and non-dominant limb in volleyball players. Materials and methods. The study involved 40 competitive volleyball players divided into 2 groups. The shoulder joint movement range assessment involved active performance of specific movements by the participants while lying down. The tool used to take the measurements was a standard goniometer, while the movements were performed in different planes in a specified and systematic way in accordance with the SFTR method. The second stage involved the performance of 3 proposed functional tests to assess the complex range of movements of the shoulder girdle. The tools used to assess the tests performed were a standard goniometer and a flexible tape measure. Results. The results of the tests were subjected to statistical analysis with the use of the Statistica 10 software. Statistical significance was set at p < 0.05. In addition, the average motion ranges and functional test results were compared. Conclusions 1. Both the Youth group and the Senior group showed a significant decrease in the range of internal rotation of the dominant limb as compared to the non-dominant limb. In addition, a deficit was found in the Youth group in horizontal flexion of the dominant limb. 2. The results of the functional tests show worse results of the “safety pin” test for the dominant limb in both groups. In the Youth group, the difference amounted to 7.4 cm and in the Senior group – 10.1 cm as compared to the non-dominant limb. 3. The Seniors achieved higher ranges of flexion in the sagittal plane and the Youth in extension and horizontal extension. 4. Athletes with a higher internal rotation range of the shoulder joint of the dominant limb achieved higher results in the “safety pin” test.

Therapeutic value of local cryotherapy in symptomatic treatment of patients with shoulder joint pain syndrome

Abstract

Introduction. The aim of this study was to determine the effects of the cryotherapy treatment on the mobility of the shoulder joint of the patients with osteoarthritis. We took into consideration the motion of flexion and extension. Additionally, the potential changes in the pain complaints were analyzed.
Material and methods. We examined 54 patients in the study, including 31 women and 23 men. They were put through the local cryotherapy on the shoulder joint 15 times daily. Before participating, all patients gave their informed consent to take part in the study and they were asked to rate their pain in the Visual Analogue Scale (VAS). The range of motion in the shoulder joint of every patient was measured  by goniometer before and after cryotherapy.
Results. All patients who underwent the series of cryotherapy had significant improvement of flexion and extension in the shoulder joint. Taking into consideration the general analysis of pain rated in the VAS score by the participants, we observed a significant reduction in the values of VAS indicated by the patients after the series of treatments.
Conclusions. Local cryotherapy is a reliable method that reduce pain and improve range of motion in the patients with pain syndrome of the shoulder joint.
