Abstract

Three groups of children and youth with clinical signs of first-degree scoliosis were studied. The range of active movements in the shoulder and hip joints and in the lumbar and thoracic segments of the evaluated twice – once before rehabilitation was commenced (baseline examination) and again after the predetermined rehabilitation period was completed (outcome examination). During the period that elapsed between the baseline and outcome examinations, all the girls and boys from both groups received a three-week cycle of rehabilitation, three 45-minute sessions per week. The entire procedure was supplemented by an individually realized program of home rehabilitation. In experimental group A, some active mobilization exercises (automobilization) were added to this rehabilitation program. In experimental group B, however, corrective procedures were supplemented by passive mobilization of particular spinal segments from Th1 to L5. The three-week rehabilitation cycle made as essential contribution to significant improvement in the mobility of the majority of the analyzed movements in all the observation groups. Better outcomes were observed in groups where passive or active spinal mobilization was performed.

Abstract
Since 2800 BC, when ancient doctors suggested that extension of the leg should be used to examine sprained vertebrae, many authors have given opinion on how to examine disorders in the hip, pelvic and lumbar region by movements of the leg. This article does not attempt to offer yet another interpretation of the Straight Leg Raising test (SLR), but rather a review of several interpretations and differences in the test. Further, we describe the sequence of “diagnosis by treatment” using SLR as a device for differential diagnosis, and how it can help in the choice of technique for therapy.

Abstract
Background. The use of orthopedic inserts to either correct true or pseudo-anisomelia or to help correct disordered body statics is a method of controlled external compensation often used in the corrective therapy of scoliosis. It has been proven that the use of orthopedic inserts to correct pseudo-anisomelia may lead to serious decompensation of the defect. These observation pertain to advanced structural distortion. The aim of this article, then, is to present a three-surface analysis of the influence of orthopedic inserts, lengthening one of the limbs, on the quality of the posture of children and youth with scoliosis. This analysis involved less advanced irregularities, which means that they had a larger component of functional changes and considerably greater corrective potential. Material and methods. The research was performed on a group of 82 children and young people ranging in age from 6 to 19 years (x = 12.0 +/- 3.1) who had the clinical symptoms of first-or second-grade scoliosis (x Cobb’s deviation = 22.8 +/- 9.5). The analysis pertained only to those children who had been advised to use leveling orthopedic inserts. The evaluation of posture was carried out twice – with and without an insert – using a computer set for testing body posture. Results. As essential connection was found between the height of the insert and the reduction of some of the first-, second-, and third-class characteristics of scoliosis. The improvement of body posture quality affected especially the frontal plane, and was accompanied by essential changes in the sagittal plane. In the transverse plane no univocally positive changes of body posture were recorded.

Abstract
Background. When the knee joint is immobilized, the sliding of joint surfaces is quickly reduced, primarily as a result of the periartricular contraction of the soft tissues (especially the articular capsule). This kind of mechanical imbalance between gliding and rolling is additionally strengthened by the disturbance of muscle equilibrium, both between different muscle groups (the femoral quadriceps and the ischio-crural muscles) and within the thigh muscle itself (between the vastus medialis and the rectus femoris). In the latter case, this status may also lead to the physiological route of knee-cap slide. In view of the complex arthromechanics of the knee joint and the proper distribution of muscle strength for its functioning, the purpose of our research was to study the impact of selected techniques of manual therapy and some of the motion patterns of the PNF method on improving the activity of the knee joint patients with injuries in the vicinity of this joint surgically treated by internal osteosynthesis. Material and methods. The study involved two groups of 17 people who were treated surgically for injuries to the vicinity of the knee joint. After a period of immobility the active mobility of this joint was assessed in all patients, along with the strength of the muscles functionally connected with the knee joint and subjective pain complaints. The studied patients from both groups underwent routine rehabilitation. Selected traction and mobilization techniques for the knee joint were additionally introduced for the patients from the main group, with exercises based on the notion of post-isometric relaxation and some motion patterns taken from PNF models. Results. The statistical analysis of the quantitative data we acquired pointed to a larger range of improvement in the mobility of the knee joint, associated with a somewhat larger simultaneous increase in the strength of the evaluated muscle groups among the patients from the main group.

Abstract
Background. The Hanke Method, also known as „E-Technik” (Entiwicklungskinesjologischer Technik), is among the less wellknown methods used to treat neuromuscular disorders. Since the elements of methods with a strictly neurophysiological mechanism of action are rarely used in corrective procedures, and the possible efficiency of these elements is typically stated in purely descriptive form, the aim of this article is to trace the impact of the Hanke method on the changes in selected parameters of the radiological evaluation of malformation. Material and methods. The research involved a group of 17 children and youth aged 7-15 (x = 10,1 +/- 2,7) with clinical and radiological symptoms of scoliosis. With only one exception these were first-degree malformations. Before and after a 10-month period of rehabilitation enhanced by certain elements of the Hanke method, X-ray photographs were subjected to evaluation. The Cobb angles, the costovertebral angles, and the Harrington indexes were calculated. All the data collected in this way were statistically analyzed. Results. The reduction of the angular value of primary curvature, associated with the simultaneous leveling out of differences in the costovertebral angles on the concave and convex sides, was obtained in 11 cases (64,7% of the subjects). Only in one case was distinct progress of malformation noted (5o). Conclusions. The results obtained point to the considerable therapeutic value of the Hanke method. The possibility of direct control over reflex reactions, leading in effect to normalization (symmetrization) of muscle tension constitutes the hallmark effects obtained by the application of methods for stimulating by reflex the neurophysiological mechanisms for controlling body posture require further thorough research.

Abstract
Background. The bioelectrical activity of the spinal muscles was evaluated, as expressed by the frequency of the biopotentials, their average amplitude, and the number of returns on either side of the primary scoliosis. Material and methods. Our research involved 110 boys and girls aged 6 to 16, with a clinical diagnosis of scoliosis. The measurements were made in a relaxed position during an attempt at active correction and in the course of performing certain corrective exercises. Both symmetric and asymmetric exercises were chose for this analysis. Some exercises were conducted with additional information provided about the degree of fault correction through monitoring, light signals, and sound signals. A computer EMG set was used, composed of a 4-channel „Medicor MG-440” electromyograph equipped with a digital analogue, cooperating with a computer appropriately programmed for the automatic analysis of integrated electromyograms. Results. The results we obtained indicate certain corrective potential, expressed by the symmetrization of bioelectric muscle activity in the course of an attempt at active correction of scoliosis. The suitability of the majority of the analyzed corrective exercises was also indicated in the normalization of muscle tension on both sides of the curvature, and especially those which additionally use the mechanism of substitute feedback.

Abstract
Both the values of the spirometric parameters in people with low-degree scoliosis and their changes in course of the rehabilitation treatment aiming to correct the body posture are subjects rarely mentioned in literature. Just sparse articles give evidence for the presence of respiratory disturbances even in early stages of the development of scoliosis. The hypothesis claiming that normalisation of the values of selected spirometric parameters, decreased earlier, during attempts of correction of the body posture could be obtained was brought forward for needs of the presented report. 60 children with scolioses of Ist and IInd degree and without any coexisting health problems participated. The evaluation of the values of selected spirometric parameters was performed twice: at the beginning of the treatment (initial test) and after its termination (final test). The time of therapeutic influence averaged 28,77 days (SD = 9,47) and daily load with corrective exercises about 3 hours with supplementary procedures. During the period of time between initial and final tests most of the assessed spirometric parameters showed significant improvement increasing their values and bringing them closer to standards (FVC EX, FVC IN, FEV1, PEF, PIF, VC, IC). The ERV parameter didn’t show any important alterations. The volumes TV and MV as well as Tiffeneau coefficient maintained their previous rise tendencies.According to the stated hypothesis the corrective rehabilitation treatment improved values of selected spirometric parameters, which were decreased in course of low-degree scoliosis. Basing on the results some practical clues concerning corrective treatment of this postural fault may also be formulated.

Abstract
Background. The objective of the present study was to evaluate the therapeutic effectiveness of a new type of vibration traction (Glisson type) as compared to traditional cervical traction (Glisson type) in patients diagnosed with degenerative disc disease or spondylarthrosis of the cervical spine. Material and methods. Two groups of ambulatory patients treated for spondylosis of the cervical spine were included in this study. The study group (n=35) and the control group (n=33) were subjected to the standard comprehensive treatment (massage therapy, short wave diathermy, infrared therapy, electrotherapy using Bernard’s current, ultrasound therapy). The cervical traction set used in the study group was additionally equipped with a device generating vibration (frequency 100 Hz, amplitude 0,2 mm), installed just above the cervical loop. Results. The statistical analysis (Chi2 = 9,672, for a = 0,046) revealed considerable correlation between the type of cervical traction used and the results of the treatment, indicating higher effectiveness of vibration traction. Statistical analysis also proved increased range of motion in the cervical spine in the all planes, as well as improved left and right hand grip function. Conclusions. The study proved that the treatment of cervical traction (Glisson type) equipped with additional vibration device was well tolerated by patients. The use of this type of traction resulted in sooner subsidence of existing symptoms such as pain, increased muscle tone and hypersensitivity to touch of cervical spine musculature.

Abstract

Kinesitherapeutic methods are in more and more frequent use by physiotherapists. The significance of treatment methods based on movement is indicated by, among other things, the popularity of various kinds of short courses and post-graduate training programs in this field. In the literature one can find many different treatment programs called „kinesitherapeutic”. However, given the fact that these methods have been popular for only a few decades, there has been to date no comprehensive systematization of this component of kinesitherapy. The present study describes the authors’ definition of treatment by movement and a classification of 47 kinesitherapeutic methods, using their theoretical foundations as the criterion for classification.

Abstract

Background. Movement based-therapy is becoming increasing popular. In fact, it can be said to be dominant modality in contemporary kinesitherapy. One of the most important criteria justifying the use of a particular therapeutic method is its clinical suitability. In this article, we attempted to establish the effectiveness of mechanical methods on the basis of a literature search. For this purpose we used our own system of evaluating the quality of published reports. Material and methods. We analyzed 20 articles from the literature devoted to the problem of evaluated the effectiveness of mechanical methods. The research reports were then ranked on the basis of the authors’ own method of evaluating their quality. Results. The results of our meta-analysis suggest that kinesitherapeutic methods are more effective than individual treatment methods used in isolation. Conclusions. A literature search in the available data bases produces numerous works discussing therapeutic techniques or diagnostic methods. There are relatively few articles, however, that evaluate the effectiveness of movement-based therapy. There are no unified criteria for the evaluation of effectivenessm which means that in the analysis of outcomes achieved from using one and the same kinesitherapeutic method there are major discrepancies in the results obtained by different authors.
