Abstract

In the opinion of many authors, surgical reconstruction of the shoulder enables optimal recovery of functionality. The problem of post-surgical rehabilitation remains a point of however, due to differences of opinion regarding the optimal time to commerce rehabilitation, the order which exercises are introduced, and the need to modify physiotherapeutic procedures. Long-term cooperation within the treatment team and good cooperation with the patient can guarantee a positive rehabilitation outcome. The rehabilitation program should be individually tailored and modified at each stage of treatment. On the basis of a review of the literature and their own experience, the authors have developed a basic post-surgical rehabilitation program, which enables the recovery of optimal joint function. The article is illustrated by photographs showing the proposed exercises.

The mobility of the lumbar spine in persons with low back pain

Abstract

Background. The purpose of our study was to examine changes in the mobility of the lumbar spine in persons with L5-S1 diskopathy and the changes in mobility associated with different modalities of treatment. Material and methods. We studied 150 persons 40-50 years of age, 64 men and 86 women. Due to variations in management the material was divided into two groups: Group I, treated only pharmacologically, and Group II, treated with both pharmacological methods and physiotherapy. The patients were examined twice, once before treatment and again after 6 weeks of treatment. The research methods included clinical interviews and measurement of thoraco-lumbar and lumbar mobility on all planes. Non-steroid anti-inflammatory drugs were used in Group I. In Group II, comprehensive physiotherapy was also applied, including physicotherapy, kinesitherapy, and massage. Results. Our analysis of the material showed that the limitations in spinal mobility were similar in both groups at baseline. After a 6-week course of treatment, significant improvement in the mobility of the thoraco-lumbar and lumbar spine was noted only in Group II (pharmacotherapy and physiotherapy), involving all the tested ranges of movement. Conclusions. An evaluation of treatment outcome in persons with L5-S1 diskopathy indicates that persons treated both with pharmacotherapy and physiotherapy show greater improvement. The fact that significant improvement was seen only in this group may result from the fact that the drugs used prepared the way for better results in subsequent physiotherapy. These results indicate that pharmacological treatment in these patients should be supplemented with physiotherapy.

Abstract

Background. Due to the complex etiopathogenesis and the diversity of treatment approaches, spinal pain syndromes constitute a serious medical problem. The chronic pain and functional disturbances accompanying discopathy have motivated a search for more effective physiotherapeutic methods, such as McKenzie’s method of biomechanical diagnosis and treatment and laser therapy. The goal of the present study was to evaluate the analgesic effectiveness of continuous and fractionated doses of laser radiation in chronic lumbo-sacral pain syndrome. Material and methods. Our research involved 88 patients with chronic spinal pain syndrome qualified for McKenzie kinesitherapy. All subjects received kinesitherapy according to the authors’ method, selected according to the biomechanical diagnosis, along with 10 laser therapy procedures. Group A (43 patients) received a continuous dose of 9 J per point, while Group B (45 patients) received a fractionated dose of 3J + 6J per point. 37 points in the lumbo-sacral region were irradiated. Results. Group B showed a significantly greater decrease in pain intensity on the VAS scale (0-10 points), and in pain frequency. Both groups were found to have a reduced deficit in range of movement according to the McKenzie Institute lumbar checklist. Conclusions. Laser therapy with fractionated doses proved to be more effective in pain relief than continuous doses. The method of dosing laser radiation had no effect on improved range of movement in the lumbar spine. Biomechanical treatment using the McKenzie method combined with laser therapy significantly reduces pain and improves the range of movement in patients with chronic lumbar pain syndrome.

Abstract

Background. This article presents outcome in the treatment of osteoporosis using two different therapeutic models. Specifically, traditional pharmacotherapy (bisphosphonians, calcitonine, hormone replacement therapy) is compared with physicotherapy, including kinesitherapy in combination with magnetotherapy and laser biostimulation.Material and methods. We examined a group of 120 women ranging from 47 to 73 years of age, all of whom were at least 2 years after natural menopause. In each case, bone mineral density (BMD) was more than 2.5 standard deviations below the average for young women. The subjects were divided into groups on the basis of the treatment modality: bisphosphonian, intranasal calcitonine, HRT, and physicotherapy. The comparison group consisted of patients who received only calcium and vitamin D3. The experiment lasted for two years.Results. Increased BMD was observed in all groups except the comparison group, though statistically significant improvement occurred only in the bisphosphonian and intranasal calcitonine groups. There was a significant reduction in the tested markers for bone turnover, osteopoiesis, and resorption in all groups. In the physicotherapy group the reduction in bone turnover was more linear, though the largest changes were still seen after the first 6 months. Side effects occurred primarily in the bisphosphonian group, and mainly involved digestive symptoms.Conclusions. Systematic physical exercises, combined with properly chosen physicotherapy, can stimulate processes leading to inhibition of resorption and increased bone mass. Physical agents can thus be useful not only in prevention, but also in the treatment of osteoporosis.

The impact of health resort treatment on improvement of upper limb function in patients with rheumatoid arthritis

Abstract

Background. The frequency of upper limb dysfunction in rheumatoid arthritis and the disability associated with progressive deformation make it necessary to search for treatment solutions. Treatment in a health resort makes comprehensive physiotherapy available to patients, including kinesitherapy, physiotherapy, massages, diet, and climatic treatment. The aim of our study was to estimate the effectiveness of health resort treatment in producing functional improvement of the upper limbs in patients with rheumatoid arthritis. Material and methods. 23 women and 7 men, age 25-65, took part in a comprehensive physiotherapy program in Świeradów-Zdrój, a Polish health resort. Range of movement in the joints was evaluated by goniometry; Lovett’s test was used to measure muscle strength; joint and muscular circumferences were measured; manual efficiency was tested; and the subjects completed a pain questionnaire. All measurements were taken before and after treatment. Results. Improved movement in all upper limb joints was confirmed, especially in flexion and abduction in the shoulder joint (p < 0.001). Moreover, decreased swelling was observed in the elbow and carpal joints (p < 0,001), as well as greater manual dexterity in terms of precision, speed and strength of grip. Insignificant improvement was noted in muscle strength, especially in the extensor muscles of wrist and elbow. No increase in muscle mass was observed. Conclusions. Pain intensity was lower after treatment. Three weeks treatment in a health resort with comprehensive physiotherapy seems to be long enough for significant improvement of upper limb function in patients suffering from rheumatoid arthritis.

Abstract

The purpose of this article is to describe the mechanisms that ensure the stability of the hip joint. In the first part, the passive and active control subsystems are explained, as well as the controlling function of the nervous system. This feed-forward mechanism protects the musculo-skeletal system from dangerous overuse in both static and dynamic conditions. The second part of the article gives the main rules for therapeutic intervention and some examples of stability exercises that can be used in cases of coxartrosis deformans.

Abstract

Background. The article describes diagnostic tests which examine lumbar spine mobility in persons treated for discopathy. The aim of the paper was to monitor lumbar spine mobility in persons treated for lumbar discopathy to study the usefulness of combined physiotherapy sessions, isometric exercises and postisometric muscle relaxation. Material and methods. The study participants were 60 men aged 40-45 years who suffered from chronic spinal ailments. In all patients the main reason for the complaints was lumbar intervertebral disc disorder. Elongation angle measurments were performed with use of „Posturometer S” – computerised set for tridimensional evaluation of examined antropometric points. Results and Conclusions. The analysis of subjective pain evaluation revealed that the effects were similar in both examined groups. Analysis of results indicates significant and positive influence of the implemented kinesitherapy on increase of mobility in examined spinal segment.

Abstract

Aging is an inevitable and dynamic process that leads to a number of adverse changes in both the locomotor system and internal organs as well as the weakening of the body’s regenerative and compensatory abilities. The full recovery of mobility following an illness or dysfunction is difficult and sometimes impossible, affecting the quality of life of the elderly patient. Concomitant psychological problems related to the elderly patient’s attitude to their illness and their expectations about those around them require a modified approach to many aspects of rehabilitation, including physiotherapy, of the elderly. This article describes the tasks that should be addressed in preventative physiotherapy and treatment modalities used in socalled main therapy and secondary prevention of geriatric patients. Emphasis is placed on the principles of planning customised active exercise sessions with particular regard to functional status assessment in individual patients. Besides examples of forms of physical activity and exercise devices for geriatric patients, the article provides comments and descriptions of the most common problems related to selected physiotherapy procedures for the elderly.

Abstract
Background. Cerebral palsy (CP) is a motor and postural disorder resulting from permanent brain damage that occurred at an early stage of development. It is the most common cause of disability in children, who grow up to become adults, but their functional performance decreases over time.
Aim. To present the importance of kinesiotherapy in CP patients.
Material and methods. The study included 20 patients (12 women and 8 men aged 20 to 60 years) with CP who participated in motor rehabilitation sessions conducted at the Occupational Therapy Workshop in Sędek and the Comprehensive Rehabilitation and Health Promotion Centre in Raków. Their clinical presentation was dominated by manifestations of motor and postural abnormalities caused by upper motor neuron and corticospinal tract damage. A diagnostic survey was used as the research method. The study used a survey questionnaire that collected information about problems associated with everyday life and functioning of the study patients and assessed their rehabilitation programme.
Results. The results emphasise the role of kinesiotherapy in the rehabilitation of patients with CP. Systematic motor rehabilitation contributed to an improvement in their physical fitness and everyday functioning.
Conclusions. Physiotherapy allows patients to achieve their maximum functional level and prevents the development of complications associated with CP.

Streszczenie

Celem pracy było przedstawienie stanowiska na temat zastosowania pomiarów, testów i ćwiczeń wykonywanych w warunkach izokinetycznych w fizjoterapii w oparciu o przegląd literatury i własne doświadczenia. Omówiono podstawowe zagadnienia fizjologii mięśni, rodzajów skurczów i pracy mięśniowej oraz zmian napięcia mięśni. Przedstawiono zasady przeprowadzania pomiarów wartości momentów sił mięśni i oceny innych parametrów biomechanicznych wykonywanych w warunkach maksymalnego napięcia izometrycznego i w warunkach dynamicznych – izokinetycznych. Dokonano charakterystyki ćwiczeń przeprowadzanych w warunkach izokinetycznych oraz przeprowadzanych w warunkach izometrii (statycznych) w różnych obszarach fizjoterapii klinicznej. Zwrócono także uwagę na wskazania i przeciwwskazania w zakresie stosowanie tego rodzaju testów i ćwiczeń w praktyce fizjoterapeutycznej w powiązaniu z innymi dziedzinami medycznymi.
