Abstract

Background. The aim of this paper was to show that procedures used within the framework of therapeutic management at the Regional Specialised Paediatric Hospital in Kielce improve the quality of service, which is the main condition for the hospital to remain on the market of health service providers. The high quality of services has been confirmed by the recent conferring of an accreditation certificate which requires that the hospital operate on the basis of a number of documented procedures, instructions and standards. The therapeutic activities of the hospital are based on a set of instructions providing detailed guidelines concerning the management of particular medical conditions. Material and methods. The study included children who were hospitalized or treated on an outpatient basis. Patient management was based on a set of instructions. An anonymous questionnaire concerning the quality of treatment and the level of satisfaction with the service was distributed among the children’s guardians and parents. Results. In the first and the second half of the year 2005, 95% and 86% of parents respectively assessed service quality as very good. 92% of parents gave the rating of very good in the first half of the year 2006 and 79% did so in the second half of the year. These results confirmed the high quality and efficacy of as well as satisfaction with the treatment received by the children. Conclusions. The observance of specified instructions during the therapeutic process improves the quality of service and minimizes the possibility of medical malpractice.

Autonomia zawodu fizjoterapeuty na przykładzie systemu kształcenia w USA

Streszczenie
Uzyskanie autonomii zawodowej potwierdzającej specjalistyczną wiedzę i umiejętności zawodowe jest podstawą działań podejmowanych przez stowarzyszenia fizjoterapeutów działających w USA. Pomimo faktu, iż większość stanowych aktów prawnych daje pacjentom możliwość bezpośredniego skorzystania z oceny i leczenia przez fizjoterapeutów, nie ma aktu prawnego, który jednoznacznie formułowałby zakres autonomicznej praktyki. Stowarzyszenie Fizjoterapii Amerykańskiej (American Physical Therapy Association – APTA) pełni wiodącą rolę w dbaniu o jakość kształcenia i praktykowania fizjoterapeutów. Zdefiniowana przez Zarząd Dyrektorów APTA autonomiczna praktyka opiera się na profesjonalizmie, odpowiednim poziomie kształcenia i możliwości bezpośredniego dostępu pacjenta do usług fizjoterapeutycznych. W USA istnieje kilka ścieżek edukacyjnych pozwalających uzyskać zawód fizjoterapeuty. Dynamiczna natura praktyki fizjoterapeutycznej wpływa na ewaluację programów nauczania. Większość programów zapewnia kształcenie na poziomie magisterskim lub zawodowym doktoranckim. Stopień doktora fizjoterapii uzyskiwany w toku kształcenia nie jest równoznaczny z akademicką nominacją fizjoterapeutów. Programy studiów doktoranckich stanowią 79,4% spośród wszystkich akredytowanych programów kształcących fizjoterapeutów. Przeznaczone są w szczególności dla doświadczonych pracowników klinik, podnoszą umiejętności kliniczne w konkretnej dziedzinie. Finałem ukończenia akredytowanego programu przygotowującego do zawodu fizjoterapeuty jest przystąpienie do Państwowego Egzaminu Fizjoterapii (National Physical Therapy Examination – NPTE). Ogólnokrajowy wynik zdania egzaminu ustalany jest przez Federację Stanowych Władz Fizjoterapii (Federation of State Boards of Physical Therapy – FSBPT). Podniesieniu kompetencji służy również ukończenie specjalizacji klinicznej. Amerykański Zarząd Specjalizacji Fizjoterapii (American Board of Physical Therapy Specialities – ABPTS) oferuje uzyskanie specjalizacji w kilku obszarach zainteresowań, w najbliższej przyszłości również w nowym zakresie jakim jest zdrowie kobiet. Wyrazem dbałości o jakość usług fizjoterapeutycznych jest konieczność ponownej recertyfikacji specjalistów klinicznych po 10 latach.

Standards of competence in the physiotherapist profession

Abstract
Background. Work on standards of professional aualifications was started in Poland in the 1990’s. The standards contain a general description of professional tasks in the context of the particular profession together with an outline of the associated skills and qualifications. A total of 253 qualifica-tions standards had been developed by the end of 2007, including standards for physiotherapists, physiotherapy assistants and massage specialists. Material and methods. Five levels of qualifications have been defined in the National Professional Oualification Standard in accordance with EC provisions, taking under consideration the complexity of work, the difficulty of the skills, the degree of responsibility and the level of professional hierarchy which students are prepared to. The sets of skills, knowledge and psychophysical characteristics were assigned to four groups of qualifications: extra-professional, general-professional, basie professional and specialised professional skills, with indMdual groups differing in terms of scope and degree of generalny. On the basis of the above criteria, a questionnaire was designed and a survey was carried out. The questionnaire, which had over 30 pages, was first tested in a pilot study involving 8 persons and then used in a target study of 40 respondents. Both studies involved active physiotherapists as well as the management of the facility where they were employed. The results of the suney served to develop a preliminary version of the standard, which was later assessed by three evaluators during a session with the authors. Following incorporation of justified comments and emenda-tions, the standard was sent to reviewers, whose remarks were incorporated before the standard was submitted to a relevant committee for approval. Results. The standard of the physiotherapist profession was elaborated, including the legal basis of the profession, a synoptic description of the profession, work place, professional tasks, and a specification of general-professional, basie and specialized qualifications for the profession. Conclusions. 1. The standard is a single document specifying the range of knowledge and skills, professional tasks and the scope of professional responsibility. 2. The standard of the physiotherapist profession can be used by interested persons and institutions for a variety of purposes, e.g. vocational counselling, employment policies, personnel policy of companies and institutions as well as for social insurance related purpos-es to estimate occupational risk and determine insurance premiums accordingly and in jurisprudence and to determine social insurance.

Abstract
A sense of professional identity, the perception of the profession in society are important factors of professional development, provide the motivation to remain in this occupation. In the paper systemisation of the knowledge concerning the development of the profession of physiotherapist in Poland has been attempted. Analysis of the available archival texts and national literature allowed to identify the test group in the context of the formation of its professional identity. The following processes were tracked formation of the first training centres for the motor rehabilitation and creating an teaching standards, program of specialisation for physiotherapists. Introduction of the term „physiotherapy in place of the functioning of the term „motor rehabilitation has initiated changes within the domain of legislative activities. In accordance with the Bologna Declaration new levels of education were created, which resulted in the acquisition of certain skills and professional titles. The changes also affected the area associated with specialization. This level of training is being actively undertaken by physiotherapists. Currently 26 centers have been accredited in which the specialization program is carried out by about 400 physiotherapists.

Abstract
Physiotherapist carries out the medical profession. Physiotherapy is a long-term action and requires special preparation, fulfilling high professional and ethical requirements necessary in health care. Relevance and usefulness of knowledge is a requisite for professionalism. Every profession strives to achieve autonomy, reflected in the internal control exercised by professional organizations. The aim of the undertaken research is to know physical therapists’ subjective opinion on their job in the context of its professionalisation.The results of research in the group of 338 employed physiotherapists and 125 students of physiotherapy. The study was multicenter, included 11 institutions (universities and rehabilitation centers).The aim of the study was carried out based on a realized survey. The questionnaire of the survey consisted of questions arranged by topic. Respondents presented opinions on the quality of education, level of satisfaction with career choice, the status of the profession, the place it occupies in the medical team and the development of academic and professional physiotherapists.The development of scientific and professional skills indicates the progress made by Polish physiotherapy.Physical therapy is perceived as a responsible, dynamically developing and satisfying job. The work on the act on the profession of physical therapist must be completed. The quality of academic education must be enhanced as regards practical preparation for the job.

Abstract
The aim of the study was to analyse factors which motivate students to commence university studies as well as to analyse the level of satisfaction with studying among a group of Master’s degree students in their final semester. 1942 second year Master’s degree students. Voluntary and anonymous questionnaire studies. The approval of the Ethical Review Board of WUM was not necessary. The questionnaire was developed by the authors and comprised 74 questions. STATISTICA 10.0 (license WUM), tests: Chi-square, Kruskall-Wallis, and Mann-Whitney U (p<0.05). The students declared the level of satisfaction with studying at PM: 3.44, PWF: 3.54 and IP: 3.52 (H=17.167; p<.000). A half of the study group would choose a Physiotherapy course again, 30% would choose the same university. 36% got acquainted with the curriculum of a Master’s degree course (p=NS). 56% reported that the curriculum content concerning orthopaedics was repeated most often, manual therapy and massage was repeated least often (34%). The largest proportion of the students declared that a Master’s degree course improved their skills. 1. The modification of curricula seems to be necessary. It should consists in prolongation of the duration of a Bachelor’s degree course and in putting a greater emphasis on a strictly occupational training and more efficient use of hours devoted to the work with patients. 2. The modification may be performed with the use of the curricula which are consistent with the guidelines of European and National Qualification Frameworks. The emphasis there is put mainly on outcomes of education and e-learning. 3. The assessment of the level of satisfaction with learning at a Master’s degree Physiotherapy course among students who had changed the educational profile of a university, as well as a detailed analysis of the curricula of a Master’s degree course seem to be important in order to extend the study.

Abstract
The term „occupational burnout” is used to define mental an emotional exhaustion, especially due to a long term exposure to stress. Studies show that occupational burnout is typically found in such professions as: physicians, physiotherapists, nurses, psychologists and social workers. In women, occupational burnout is manifested by emotional exhaustion. The objective of the study was to assess the level of life and job satisfaction among female physiotherapists, with particular reference to occupational burnout phenomenon. The sample included 30 female physiotherapists and the control group was composed of 30 female pharmacists. The research tools included Life Satisfaction Questionnaire (LSQ) and Burnout Scale Inventory (BSI). Statistical analysis was based on arithmetic means, standard deviations (SD) and the significance of differences between the mean values obtained from each occupational group. Significance level was established using Student’s-t test. The results indicate differences in the global life satisfaction score between the study and control groups. The studied female physiotherapists were diagnosed with chronic fatigue, requiring prompt preventive measures.

Abstract
The professionalization of the physiotherapist profession is a dynamic process in Poland, which can be seen in the number of academic organizational units, formation of PTF, and development of academic research. The study aimed to analyse the process of professionalization among a group of final semester Master’s degree students of Polish university-level schools of different educational profiles.1942 second year Master’s degree students took part in the study. 1600 students were qua­lified to the statistical analysis: 570 PM students, 464 PWF students, and 566 IP students. Voluntary and anonymous que­stionnaire studies. The questionnaire was developed by the authors and comprised 74 questions. STATISTICA 10.0 program (Warsaw Medical University license) and the following non-parametric statistical tests: Chi-square, Kruskall-Wallis, and Mann-Whitney U (p<0.05).Only 6% of students engaged themselves in the activity of a Student Research Society (SRS) (p<.000). Only 28% of students actively took part course in a scientific conference (p<0.834). As little as 18% of students actively parti­cipated in a research study/research project (p<.000). The vast majority of students declared that they did not publish any article. 40% of the students declared participation in additional occupational courses. The vast majority of students had found out about the courses from the Internet (H=7.896, p<0.019) and declared that they would like to get objective infor­mation on the courses during their university classes (H=0.247, p<0.883). 1. University-level should develop programmes to pick up outstandingly talented individuals in order to provide them with an individual path of scientific development and build closer tights between these graduates and the school in the future. 2. Physiotherapy students seem to have a practical attitude towards the post-graduation training, which is more oriented to the content and usefulness and less oriented to temporary trends in physiotherapy. 3. There is an urgent need that university-level schools educating prospective physiotherapists develop educational programmes which would deal to a greater extend than it is at present with modern methods used in physiotherapy and which would constitute specific guidebooks in which students would be able to find information on the best parallel and post-graduate directions of further studies.

Streszczenie
Cele. Zbadanie wpływu terapii wodnej na funkcje oddechowe i jej wpływ na zdolności funkcjonalne chłopców z dystrofią mięśniową Duchenne’a (DMD). Materiał i metody. Do badania włączono dwudziestu czterech chłopców z DMD; ich wiek wahał się od siedmiu do jedenastu lat. Zostali losowo przydzieleni do dwóch równych grup; grupa poddawana terapii na ziemi otrzymała opracowany program fizjoterapii na ziemi, podczas gdy grupa poddawana terapii wodnej otrzymała ten sam program fizjoterapii do wykonania w wodzie na basenie, gdzie prowadzono zajęcia hydroterapii. Wszystkich chłopców poddawano fizjoterapii dwa razy w tygodniu przez trzy kolejne miesiące. Do pomiaru czynności oddechowych, w tym natężonej pojemności życiowej (FVC), natężonej objętości wydechowej pierwszosekundowej (FEV1) i wskaźnika FEV1/FVC% zastosowano spirometr. Testy funkcjonalne w czasie (TFT), w tym test chodu/biegu na 10 m, test wspinaczki lub zejścia po 4 standardowych schodach oraz test wstania z pozycji leżącej, po którym nastąpił test 6-minutowego chodu (6MWT) zastosowano do oceny wydolności funkcjonalnej obu grup, przed i po leczeniu. Wyniki. Wyniki uzyskane po leczeniu nie wykazały statystycznie istotnych różnic dla wszystkich zmiennych (P> 0,05) z wyjątkiem wskaźnika FEV1/FVC%, który wykazał statystycznie istotną różnicę (P = 0,0001) w obu grupach. Wniosek. Terapia wodna nie ma statystycznie istotnego wpływu na funkcje oddechowe i możliwości funkcjonalne chłopców z dystrofią mięśniową Duchenne’a.

Ocena skuteczności usprawniania pacjentów z dyslordozą szyjną z wykorzystaniem testów neurodynamicznych i zabiegów fizykalnych

Streszczenie
Wstęp. Celem pracy była ocena skuteczności kompleksowego postępowania fizjoterapeutycznego usprawnianie pacjentów dyslordoza szyjną materiał i metody badania objęto grupę 40 osób, u których stwierdzono zmiany zwyrodnieniowe w obrębie szyjnego odcinka kręgosłupa. Grupa badana stanowiąca 20 osób została poddana serii zabiegów fizykalnych (biostymulacja laserowa i galwanizacja) oraz serii zabiegów mobilizacji i poizometrycznej relaksacji mięśni z ćwiczeniami izometrycznym. W grupie kontrolnej pacjenci korzystali tylko zabiegów fizykalnych (laser i galwanizacja). Oceny rezultatów zastosowanych terapii dokonano wykorzystując testy neurodynamiczne. Wyniki. Z przeprowadzonych badań wynika że terapia zabiegami fizykalnymi połączonymi z neuromobilizacją i poizometryczna relaksacją mięśni z ćwiczeniami izometrycznym stanowi ważny element w terapii leczenia dyslordozy szyjnej. Przemawia za tym fakt zmniejszenia się średniego bólu odczuwanego przez pacjentów oraz zwiększenie ruchomości kręgosłupa szyjnego. Wnioski. Terapia dyslordozy szyjnej z wykorzystaniem zabiegów fizykalnych (biostymulacja laserowa i galwanizacja) połączona z neuromobilizacją i poizometryczna relaksacją mięśni daje lepszy efekt terapeutyczny niż usprawnianie tylko z wykorzystaniem metod fizykalnych. Kompleksowe postępowanie fizjoterapeutyczne przyczyniło się do zmniejszenia natężenia bólu oraz do poprawy ruchomości kręgosłupa w odcinku szyjnym.
