Sažetak
Cilj. Ova studija je imala za cilj istražiti učinak intervencije dojenja pomoću aplikacija za pametne telefone na ekskluzivno samopouzdanje dojenja kod žena nakon poroda s multiplom sklerozom. Materijali i metode: Istraživači su koristili kvazi-eksperimentalni istraživački dizajn (kontrolna skupina slučajeva). Studija je provedena u centrima za zdravlje majke i djeteta (MCH) u Shebin El-Komu, Menoufiji, Egipat. Ciljna populacija ove studije bila je 50 žena s multiplom sklerozom nakon poroda. Za prikupljanje podataka korištena su tri instrumenta: strukturirani intervjui, skala samopouzdanja dojenja i upitnik o odnosu između dojenja i relapsa multiple skleroze nakon poroda. Rezultati i rasprava: Postojala je statistički značajna razlika između obje grupe u odnosu na samoupravljanje tijekom poslijeporođajnog razdoblja. Poboljšanja su bila povezana s odnosima majki s njihovim pružateljima zdravstvene skrbi i znanjem i informacijama o multipli sklerozi tijekom ove prijelazne faze, što ukazuje na to da je intervencija sestrinstva imala pozitivan učinak na žene nakon poroda s multiplom sklerozom. Zaključak: Žene nakon poroda s multiplom sklerozom imaju zadovoljavajuću razinu samopouzdanja dojenja nakon intervencije sestrinstva pomoću aplikacija za pametne telefone. Stoga, provođenje multidisciplinarnog programa za praćenje i podršku majkama s MS-om poboljšava samoupravljanje tijekom poslijeporođajnog razdoblja.

Abstract
Purpose. To compare between the effect of torso based balance weighting and tailored physical therapy program on balance and postural control in ataxic MS patients. Methods. Ninety patients diagnosed as ataxic multiple sclerosis with impaired balance and postural control were selected from outpatient clinic of two hospitals. The patients randomly assigned into two groups, group (A: n = 30) received torso-based balance weighting and selected physical therapy program and group (B; n = 60) received tailored physiotherapy program and the same selected physical therapy program. Treatment lasted eight weeks, three times per week. Pre- and post-treatment assessment using berg balance scale and overall stability index were done for all patients. Results. Post-treatment comparisons between the two groups showed a statistically significant improvement in Berg balance (p < 0.05) in favor of group (B) and both groups showed clinical improvement. Conclusion. The tailored physical therapy program was more statistically effective in improving balance disorders than torso based balance weighting in ataxic MS patients, and there was clinical effect on improving postural control of both groups.

Magnetic field stimulation and the quality of life in MS patients

Abstract
Background. The aim of our study was to evaluate the effectiveness of magnetic field stimulation and exercise on the quality of life in multiple sclerosis patients undergoing home-based rehabilitation. Material and methods. The study involved a group of 87 patients with advanced multiple sclerosis (minimum grade 6 in the Expanded Disability Status Scale). Patients were divided into two groups. In Group I magnetic field was applied from a Viofor JPS apparatus once daily for 20 days. Group II (controls) did not receive magnetic field therapy. Patients from both groups carried out exercises at home. The clinical status was evaluated using Cendrowski’s numerical scale and the Expanded Disability Status Scale. Quality of life was examined by using an extended Testa and Simonson questionnaire. Patients were evaluated twice: before and after 20 days of rehabilitation at home. Results. An improvement was obtained in Group I, where magnetic field stimulation was applied, with regard to everyday functioning and such activities as personal hygiene, getting dressed, eating meals, or locomotion. There was a statistically significant improvement in muscle strength, sphincter control, a decrease in muscle tone, fatiguability, sensory disturbances and balance dis&shy;tur&shy;bances. There was also a significant improvement in the patients’ mental state and emotional function, less depression and fear and increased satisfaction with life.Conclusions. Magnetic field stimulation had a significant impact on the quality of life in multiple sclerosis patients. Exercise and magnetic field therapy in patients with advanced disability improved not only their motor ability but also mental state. Magnetic field stimulation supported and facilitated rehabilitation exercises performed at home by the MS patients.

Objective quality of life and level of life satisfaction in people suffering from multiple sclerosis

Abstract
The chronic nature of the disease, accompanying numerous ailments and unpredictable pace of dete­rioration in the patient’s condition adversely affect the quality of life in patients with multiple sclerosis (MS). To make a full and reliable assessment of the quality of life of patients with MS, both the objective and subjective measurements should be taken into account.The study included 30 people with MS and 60 healthy individuals (OZ). Objective quality of life was analyzed on the basis of four indicators: education, employment, income, leisure activities. To assess the subjective quality of life the Life Satisfaction Questionnaire was used. In case of objective determinants of quality of life significant differences between groups were reported in terms of employment, income and certain leisure activities (reading books, travelling). People with MS assessed their overall life situation similarly to healthy controls (HC), i.e. as rather satisfactory. For people with MS the most satisfying were relationships with their partners and family life, and for HCs – self care ability. Both MS patients and HCs were the least satisfied with financial and occupational status. MS patients are rather satisfied with life as a whole, probably due to the satisfaction with leisure activities. In addition, they declare significantly lower employment rate and income as compared to HCs, which confirms a very difficult financial situation of MS patients in Poland.

Influence of Vestibular Rehabilitation on Balance and Dual-Task Cost of Walking in Patients with Multiple Sclerosis: A Randomized Controlled Trial

Abstract
Background. Balance dysfunction and high dual-task costs are eminent features in multiple sclerosis (MS). Vestibular rehabilitation therapy (VRT) proved to promote functional outcomes; yet, its influence on dynamic balance and dual-task cost of walking (DTCW) in MS needs further research.
Objective. To investigate the effect of adding VRT to aerobic training on dynamic balance and DTCW in people with MS.
Design. Single blinded, parallel randomized controlled trial.
Settings. Out-Patient Clinic at Faculty of Physical Therapy and Kasr Al-Ainy Multiple Sclerosis Unit, Cairo University, Egypt.
Participants. 40 patients with remitting-relapsing multiple sclerosis.
Intervention. Patients were randomized to a control (n = 20) and intervention (n = 20) groups. Over four successive weeks, all patients received stationary bicycle aerobic training. Intervention group received an additional VRT program.
Outcomes measures. Berg Balance Scale (BBS), walking speed tested by the 10 meters timed walk test (10 m-TWT), (DTCW), and overall stability index (SI).
Results. Intervention group showed a remarkable improvement in BBS (p = 0.02), SI at levels four (p = 0.002) and seven (p = 0.03), and DTCW (p = 0.03) compared with control group. Walking speed did not show significant changes post-treatment within or between groups comparison (P > 0.05).
Conclusion. Adding VRT to aerobic training has a positive effect on dynamic balance and dual-task cost of walking in MS.

Džudo kao alternativna metoda rehabilitacije kod multiple skleroze – pilot studija

Abstract
Objective. To evaluate the efectiveness of training judo as a part of a physiotheraphy programme in MS patients
Materials and methodology. 4 female RR-MS patients, aged between 32 and 49, have taken part in a 8-week programme. Their condition was evaluated twice – before and after the therapy. Two surveys have been carried out: one prepared by the authors of the study and Multiple Sclerosis Impact Scale (MSIS-29). In order to evaluate the functional state the following methods have been used: Functional Reach Test, 10 m TW (Time Walking) test and the Lovett test (for the following muscles: rectus femoris, biceps femoris, rectus abdominis, deltoideus).
Results. After the therapy the most persisting ailments decresed considerably. The functional tests showed better results and the patients had a better phycological disposition.
Conclusions. Judo training improved both functional and psychogical condition of the patients in the rehabilitation group. The programme proved to be an alternative form of physiotherapy for MS patients.

Analiza wpływu programu kompleksowej rehabilitacji na równowagę ciała u pacjentów ze stwardnieniem rozsianym i po udarze mózgu na platformie stabilometrycznej

Streszczenie

Cel pracy. Celem pracy jest ocena wpływu programu kompleksowej rehabilitacji na równowagę ciała u pacjentów ze stwardnieniem rozsianym (SR) oraz po udarze mózgu.
Materiał i Metody. Badania zostały przeprowadzone w Oddziale Rehabilitacji Neurologicznej w III Szpitalu Miejskim w Łodzi wśród losowo wybranych osób z rozpoznaniem stwardnienia rozsianego (SR) i po udarze mózgu. W badaniu wzięło udział 50 osób, w tym 30 mężczyzn i 20 kobiet w wieku od 32 do 76 lat. U wszystkich pacjentów przeprowadzono dwukrotne badanie równowagi ciała przy użyciu komputerowej platformy stabilometrycznej. Analizie poddano następujące parametry badania: średnią liczbę wszystkich wychyleń, średnią liczbę wychyleń w przód i w tył oraz w prawo i lewo. Wszystkie parametry wychyleń badano zarówno z zamkniętymi oczami jak i z otwartymi oczami. Wyniki badania uzyskane po rehabilitacji porównano z wynikami badania przed jej rozpoczęciem.
Wyniki. Po przeprowadzonym cyklu programu kompleksowej rehabilitacji wśród pacjentów z SR średnia liczba wychyleń z zamkniętymi oczami uległa istotnemu zmniejszeniu (p < 0,001), z otwartymi oczami zaobserwowano poprawę jednak nie tak znaczną (p < 0,2). Podobnie istotną poprawę zaobserwowano wśród pacjentów po udarze mózgu w średniej liczbie wychyleń z zamkniętymi oczami (p < 0,001) natomiast z otwartymi oczami (p < 0,02) wyniki nie były znamienne statystycznie.
Wnioski. Kompleksowa rehabilitacja ma znaczący wpływ na równowagę chorych na stwardnienie rozsiane (SR) i po udarze mózgu. W badaniu z wykorzystaniem platformy stabilometrycznej lepsze wyniki po cyklu rehabilitacji uzyskali chorzy po udarze mózgu.
