Analysis of pain and quality of life in women after radical and sparing mastectomy

Abstract
Objective. The objective of the study was to analyse pain and quality of life in women after radical and sparing mastectomy. The quality of life after radical mastectomy compared to sparing mastectomy was characterized, assessing difficulties in everyday life and work, as well as limitations in undertaking various activities caused by pain after the procedure. The study verified whether the type of mastectomy significantly impacted selected aspects of life.
Material and method. The questionnaire-based study was carried out among 100 patients of Holy Cross Cancer Centre, aged 29 to 88 years, after radical and sparing breast cancer treatment. In order to compare the values of quantitative and qualitative variables between the groups, Student’s t-test, chi-square test, Mann-Whitney test and Shapiro-Wilk test were used. A significance level of p <0.05 was adopted in the course of the analysis. Statistical analysis was performed using Statistica 13.1 by StatSoft; the data was collected in Excel 2016.
Results. The analysis showed no statistically significant differences in the level of pain intensity in each analysed area between the groups of women, however, it confirmed a statistically significant correlation between the type of surgery performed and the onset of pain as well as limitations in everyday functioning. The analysis showed no statistically significant differences in pain intensity depending on the treatment period.
Conclusions. The level of pain after radical and sparing mastectomy was similar. After radical mastectomy, the patients’ quality of life was lower than after sparing mastectomy. Rehabilitation reduced pain intensity and improved the patients’ quality of life.

Abstract
Introduction. Osteoarthritis of the hip joint is one of the most common chronic diseases of the locomotor system. Total hip replacement (THR) is a treatment option of choice alongside conservative treatment. The aim of the study was the long-term assessment of the quality of life of patients after total hip replacement surgery and after conservative treatment.
Material and methods. The study group consisted of 35 patients diagnosed with many years of osteoarthritis of the hip who underwent THR with posterolateral surgical approach. The assessment was carried out at the Department of Orthopedics and Traumatology of the University Teaching Hospital WAM – CSW in Łódź. The control group consisted of 15 patients with long-term osteoarthritis of the hip, treated conservatively. Medical data were obtained from the history of the disease and the author’s questionnaire, while the World Health Organization Quality OF Life (WHOQOL) – BREF questionnaire was used to assess the quality of life.
The obtained results were statistically analyzed with the use of MS Office and the Student’s T-test, considering the results below p <0.05 as statistically significant.
Results. The assessment of domains was similar in both groups, the greatest dissonance was observed in the assessments of the somatic and social domains. Based on WHOQOL-BREF questionnaire, the majority of patients in the study group rated the quality of life from 60% to 70% of the maximum value, the highest psychological sphere, the least somatic. In the control group, the social domain obtained 78% of the maximum value, other domains – 74% of the maximum value.
Conclusions. Patients treated conservatively assessed their quality of life higher in all domains than patients who underwent THR. In both studied groups, the “positive” variant of the answer was the most frequently chosen in the WHOQOL-BREF questionnaire.

Abstract
Achieving a high quality of life should be the basic treatment goal in chronic dialysis. The best means to achieve this aim is a rehabilitation program aimed at restoring the patient’s capacity to return to work, while in the case of older patients the primary goal is to attain functional independence in performing activities of daily living. The achievement of these goals makes renal replacement therapy cost-effective. In the future it will be necessary to constantly update, plan, and execute the rehabilitation program and to monitor the beneficial long-term outcomes of its application.

Abstract
Background. The quality of life of the patients after renal graft transplanted is not only medical, but also a social problem. One of the suggested ways of their treatment is a holistic approach to their rehabilitation, as well as analysis of their evaluation of life conditions. Material and methods. The research involved 20 patients after renal graft transplanted, during the process of physiotherapy. This group has been scrutinized during a questionary examination (incluling SWLS, AIS) and also during a specified standard questionary. The received results have been statistically analysed according to interpretation standard adapted to Polish conditions.Conclusions and Results. The conducted examinations showed, the usefulness of the scrutiny thanks to which high interpreting results have been accomplished. The results show the acceptance of poor health conditions by the patients and low level of life satisfaction caused by the end-stage of their renal failure.

Abstract

Background. Urinary incontinence and disturbances of perineal muscle and nerve activity in women after gynecological surgery have a major influence on the patients’ quality of life. Using global EMG data, it is possible to customize parameters for spinal FES and perineal EMS treatments and to objectively assess treatment outcomes. Material and methods. 15 women after gynecological surgery with disturbances of micturition underwent gynecological and urodynamic assessments, sonography, global EMG with a vaginal probe and magnetic field-induced motor evoked potentials (MEP) tests. The parameters for EMS and FES were customised on the basis of those diagnostic tests and the treatments were self-administered by patients for 2 months. Quality of life changes were assessed with Prof. Kowalik’s quality of life assessment scale at baseline and at the end of the treatment. Results. In 12 patients, EMG and MEP recordings revealed that the disturbances were caused by axonal degeneration of motor fibres rather than by impaired impulsation at the spinal centre level. Mean values of perineal muscle resting potentials were 2.52 pV before and 1.87 pV after treatment. Contraction strength rose from 14.7 pV to 16.9 pV The self-reported 20-domain quality of life scale showed increased current and future-oriented satisfaction with life. Conclusions. A two-month cycle of FES and EMS improved motor unit function at maximum contraction. MEP studies revealed improved conduction from spinal motor centres to muscles, especially if EMS of perineal muscles had been supplemented with lumbo-sacral FES.

Abstract
Background. Kidney transplantation is an acknownledged method of treatment of end-stage failure. Rehabilitation of kidney transplant aims to restore physical and mental fitness. This study set out to assess the relationship between kidney transplantation and quality of life and the relationship between participation in a rehabilitation programme and quality of life after kidney transplantation. Material and methods. The study involved 67 patients aged 17-73 years (x = 47.5 years) who were divided into a rehabilitated and control group. The quality of life was evaluated with the SF-36 questionnaire, which the patients completed twice to reflect their situation prior to transplantation (period of dialysis therapy) and one year after transplantation. Results. 1. Quality of life after transplantation was regarded as higher compared to the evaluation prior to the transplantation (p&lt;0.05 for all scores) by the entrie patient population, by the rehabilitated group and the control group taken separately. 2. More positive changes were noted in the rehabilitated group. 3. A comparison of the results obtained in individual patients before and after the transplantation showed that: 25% of the patients reported no improvement in Global Quality of Life, including 15% reporting a worsening of Global QoL- almost 38% of the patients did not experience improvement in the general physical health domain- 27.6% did not experience positive changes in the general mental health domainConclusions. Kidney transplantation and participation in the rehabilitation programme positively influenced the patients’ subjective evaluation of the quality of life. About 1/3 of the kidney transplant recipients did not note any improvement in functioning.

Abstract
Background. Post-mastectomy women are pessimistic about their life owing to a number of adverse changes in the upper limb and shoulder girdle on the operated side and the possibility of mental disturbances caused by a fear of disability, cancer relapse or death. Following a radical mastectomy, women indeed lead a stressful life. The stress may by caused by uncertainty about the efficacy of the treatment, cancer relapse anxiety, physical disability (limitation of motor function, lymphedema, decrease in musclepower), breast loss (half-woman complex), personal, family =, professional, and postoperative sequelae-related problems. All the above-mentioned situations have a deleterious effect on the women’s psyche, which is dominated by fear of disability and death, anxiety, about their future life in society and family, meticulous analysis of their own medical case, distrust of medical staff, exaggerated sensitivity and reactions to the behaviour of those around them and everyday problems. The aim of this study was to assess the quality of life of post-mastectomy women.Material and method. The study group consisted of thirty women after mastectomy from the ‘Amazons’ Post-Mastectomy Women’s Club in Wroclaw. The patients filled in a sixty-question inquiry form. Conclusions. According to subjective evaluation, breast amputation was followed by different degrees of deterioration of the quality of life for the majority of the patients (82% of the study group). Mastectomy forced the women to give up active ways of spending their free time, which were replaced by passive pursuits, and to completely abandon some of their housework.

Magnetic field stimulation and the quality of life in MS patients

Abstract
Background. The aim of our study was to evaluate the effectiveness of magnetic field stimulation and exercise on the quality of life in multiple sclerosis patients undergoing home-based rehabilitation. Material and methods. The study involved a group of 87 patients with advanced multiple sclerosis (minimum grade 6 in the Expanded Disability Status Scale). Patients were divided into two groups. In Group I magnetic field was applied from a Viofor JPS apparatus once daily for 20 days. Group II (controls) did not receive magnetic field therapy. Patients from both groups carried out exercises at home. The clinical status was evaluated using Cendrowski’s numerical scale and the Expanded Disability Status Scale. Quality of life was examined by using an extended Testa and Simonson questionnaire. Patients were evaluated twice: before and after 20 days of rehabilitation at home. Results. An improvement was obtained in Group I, where magnetic field stimulation was applied, with regard to everyday functioning and such activities as personal hygiene, getting dressed, eating meals, or locomotion. There was a statistically significant improvement in muscle strength, sphincter control, a decrease in muscle tone, fatiguability, sensory disturbances and balance dis&shy;tur&shy;bances. There was also a significant improvement in the patients’ mental state and emotional function, less depression and fear and increased satisfaction with life.Conclusions. Magnetic field stimulation had a significant impact on the quality of life in multiple sclerosis patients. Exercise and magnetic field therapy in patients with advanced disability improved not only their motor ability but also mental state. Magnetic field stimulation supported and facilitated rehabilitation exercises performed at home by the MS patients.

Abstract
The purpose of the above study was to evaluate the quality of life and the type of late postoperative complications in women after breast cancer surgery. The research group consisted of 30 women who underwent treatment for breast cancer. The women were members of the “Amazons” Post-Mastectomy Women’s Club in Tarnów (50%) and in Kraków (50%). The questionnaire dealt with the quality of life and health and was based on the EORTC QLQ-30 questionnaire. The examination chart determined the range of active movement in the joints of the shoulder girdle, limb circumference, potential contracture of the pectoral muscles, measurement of muscle strength and hand strength. The quality of life in the examined women who underwent mastectomy did not change dramatically. In the research group, the range of movement in the shoulder joint on the side of the operated breast became limited in all directions and similarly, the strength of the analyzed muscles decreased. The upper limb lymphoedema was registered in half of the examined women. Pain and sleep disorders were the two postoperative complications that most strongly affected the quality of life in post-mastectomy women.

The Efficiency of Physiotherapy for Hip Osteoarthritis in the Light of Self-assessed Health-related Quality of Life

Abstract
The aim of this study was to assess the effectiveness of a 4-week osteoarthritis rehabilitation program in terms of the amount of change in pain severity, range of motion in the hip affected as well as changes in function subscales as determined by SF-36 Health Survey, version 2 (SF-36v2) both in the total study population and in relation to gender. Correlations between the changes and range of motion and pain indices were also analyzed. The study group comprised 30 patients with hip osteoarthritis participating in a 4-week reha­bilitation program. Both active and passive. range of motion (ROM) in the hip joint was assessed Pain severity was measur­ed using the Numerical Rating Scale (NRS). Health-related quality of life was determined by means of SF-36 Health Survey, version 2 (SF-36v2) before and after the rehabilitation program. The analysis of mean scores in SF-36v2 function subscales obtained before and after therapy demonstrated major improvements in health-related quality of life subscales except general physical function. Pain severity decreased as suggested by the bodily pain subscale of the SF-36v2 and numerical rating pain scale. Statistically significant changes in active range of motion after therapy compared to the baseline values observed in the total study population as well as in female and male subgroups seem to confirm the effectiveness of physiotherapy for hip osteoarthritis.
