Streszczenie
Następstwem uszkodzenia struktur mózgu często są różnego typu zaburzenia percepcyjno-motoryczne. Jednym z nich jest zespół odpychania (ang. Pusher Syndrome, PS). Jest on definiowany jako zaburzenie postrzegania pozycji ciała w płaszczyźnie czołowej, co w konsekwencji utrudnia utrzymanie prawidłowej pionowej postawy. Występuje najczęściej u osób po udarze prawej półkuli mózgu, w tylno-bocznej okolicy wzgórza, a jego częstotliwość szacuje się na około 9–50%. Jest to jednak wciąż mało poznany syndrom i zbyt rzadko diagnozowany. Prawidłowe, wczesne rozpoznanie zespołu pushera oraz wprowadzenie odpowiedniej fizjoterapii pozwala skrócić czas rehabilitacji pacjenta i przyspieszyć jego powrót do sprawności. Celem pracy jest przedstawienie zasad i możliwości terapii osób z zespołem odpychania.

Abstract
Background. Imbalance during standing, which is usually observed as an asymmetry in the weight shifting toward to the unaffected side of the body, is one of the most common factors affecting the independence and quality of life of post-stroke patients. Clinical assessment of imbalance in post-stroke patients is often conducted by visual observation using standardised tools (balance tests). However, quantitative analysis, using a posturographic platform, is more accurate and provides more information about the patient’s functional status. The TYMO® device, used in this study, is a portable posturography platform that measures tilts of the body’s center of mass and reports the results of the rehabilitation process. It enables an objective, direct and quantitative assessment of the patient’s functional status. Such an assessment can contribute to significant effectiveness of physiotherapy and consequently improve the patient’s quality of life and shorten the period of abstinence from work. Objective. Quantitative assessment of imbalance and postural control using a posturography platform (TYMO®) in patients after ischaemic stroke, in the chronic phase, as an important component of the functional diagnosis process and rehabilitation programme design. To emphasise the role of the posturography platform (TYMO®) as a tool for measuring static balance – symmetry of body weight distribution, and a tool for monitoring and reporting the results of physiotherapy treatment. Methods. In the current study, before and after two weeks of rehabilitation, quantitative measurements of balance on the TYMO® platform were made in a group of subjects (n = 60: adults, after ischemic stroke – first stroke episode, in chronic phase – up to 5 years after the stroke incident occurred) undergoing therapy using neurophysiological methods (PNF – Proprioceptive Neuromuscular Facilitation and NDT-Bobath – Neurodevelopmental Treatment according to the Bobath concept) and the SPIDER system (Strengthening Program for Intensive Developmental Exercises and activities for Reaching health capability). Measurements included: the distance marked by the patient’s center of mass while performing the test, the medial-lateral and anterior-posterior tilts of the subject’s body, the area of movement marked by the body’s center of mass, the average speed at which the patient performed the movement to maintain the required position, and the distribution of the subject’s weight. Based on the posturographic results obtained before therapy, it was possible to design a targeted rehabilitation programme. The magnitude of the difference in measurements before and after rehabilitation made it possible to assess the impact of the therapy on the patient’s balance. In addition, it was a specific indicator of the accuracy of the selection of physiotherapeutic treatment (a large difference in the mean results before and after therapy reflected an improvement in the parameters of postural control, hence the effectiveness of the therapy) and determined the direction of the future rehabilitation programme. Results and conclusions. The parameters measured by the TYMO® platform are crucial in assessing the functional status of post-stroke patients, especially with regard to postural control or balance disorders. The results described confirm the validity of using quantitative assessment, using the posturography platform, as an important component of the functional diagnostic process and designing an rehabilitation programme. The TYMO® platform itself is a useful tool for measuring, monitoring and reporting the effects of physiotherapeutic treatment in post-stroke patients.

Abstract
Background. Breast cancer is the most common neoplastic disease in the group of women. In terms of negative consequences of oncological treatment, disorders of the kinematics of the shoulder complex and coexisting disorders of motor control are also distinguished. Aim of the study. The aim of the study is to systematize information on motor disorders in terms of scapula kinematics in the area of the shoulder complex during upper limb elevation movements in the group of women after mastectomy. Methods. The systematic review is based on the search results of Science Direct, Ebsco, and PubMed. The systematic review was performed on the basis of PRISMA guidelines. Results. The analysis of the works showed the existence of disorders in the kinematics of the scapula in the group of women after mastectomy. In this group of patients, the scope of upwards rotation of the scapula during the elevation of the upper limb, its lowering, and functional movements was limited. Conclusions: The analysed articles indicate the existence of disorders of the scapula kinematics in the group of women after mastectomy. The obtained results indicate the necessity of implementing routine evaluation and motor re-education in the field of scapula kinematics disorders in the process of comprehensive rehabilitation in this group of patients.

Abstract
Frailty syndrome, otherwise known as weakness or decline in physiologic reserve syndrome, is an important health problem for an aging population. According to the definition, it is a syndrome characterized by a decrease in the body’s physiologic reserves as a result of the accumulation of reduced efficiency of many organs and systems. The prevalence of frailty increases with age, it is intensified by the burden of chronic diseases and limitation of physical fitness. Symptoms of frailty may appear before the age of 65, although the incidence worsens in people who are 70 and older. The consequence of frailty is limited resistance to stressors and the associated susceptibility to disproportionate deterioration of the patient’s health. It has been shown that the frailty syndrome is a dynamic and partially reversible condition, therefore, it is important to introduce preventive measures which will reduce the development of frailty and contribute to improvement of health. One of the most important interventions in the prevention and treatment of frailty syndrome is regular and properly planned physical activity.
The aim of the publication is to summarize the current knowledge and guidelines on planning an optimal physical activity program for this group of patients.

Abstract
The aim of the study was to analyse factors which motivate students to commence university studies as well as to analyse the level of satisfaction with studying among a group of Master’s degree students in their final semester. 1942 second year Master’s degree students. Voluntary and anonymous questionnaire studies. The approval of the Ethical Review Board of WUM was not necessary. The questionnaire was developed by the authors and comprised 74 questions. STATISTICA 10.0 (license WUM), tests: Chi-square, Kruskall-Wallis, and Mann-Whitney U (p<0.05). The students declared the level of satisfaction with studying at PM: 3.44, PWF: 3.54 and IP: 3.52 (H=17.167; p<.000). A half of the study group would choose a Physiotherapy course again, 30% would choose the same university. 36% got acquainted with the curriculum of a Master’s degree course (p=NS). 56% reported that the curriculum content concerning orthopaedics was repeated most often, manual therapy and massage was repeated least often (34%). The largest proportion of the students declared that a Master’s degree course improved their skills. 1. The modification of curricula seems to be necessary. It should consists in prolongation of the duration of a Bachelor’s degree course and in putting a greater emphasis on a strictly occupational training and more efficient use of hours devoted to the work with patients. 2. The modification may be performed with the use of the curricula which are consistent with the guidelines of European and National Qualification Frameworks. The emphasis there is put mainly on outcomes of education and e-learning. 3. The assessment of the level of satisfaction with learning at a Master’s degree Physiotherapy course among students who had changed the educational profile of a university, as well as a detailed analysis of the curricula of a Master’s degree course seem to be important in order to extend the study.

Abstract
The professionalization of the physiotherapist profession is a dynamic process in Poland, which can be seen in the number of academic organizational units, formation of PTF, and development of academic research. The study aimed to analyse the process of professionalization among a group of final semester Master’s degree students of Polish university-level schools of different educational profiles.1942 second year Master’s degree students took part in the study. 1600 students were qua­lified to the statistical analysis: 570 PM students, 464 PWF students, and 566 IP students. Voluntary and anonymous que­stionnaire studies. The questionnaire was developed by the authors and comprised 74 questions. STATISTICA 10.0 program (Warsaw Medical University license) and the following non-parametric statistical tests: Chi-square, Kruskall-Wallis, and Mann-Whitney U (p<0.05).Only 6% of students engaged themselves in the activity of a Student Research Society (SRS) (p<.000). Only 28% of students actively took part course in a scientific conference (p<0.834). As little as 18% of students actively parti­cipated in a research study/research project (p<.000). The vast majority of students declared that they did not publish any article. 40% of the students declared participation in additional occupational courses. The vast majority of students had found out about the courses from the Internet (H=7.896, p<0.019) and declared that they would like to get objective infor­mation on the courses during their university classes (H=0.247, p<0.883). 1. University-level should develop programmes to pick up outstandingly talented individuals in order to provide them with an individual path of scientific development and build closer tights between these graduates and the school in the future. 2. Physiotherapy students seem to have a practical attitude towards the post-graduation training, which is more oriented to the content and usefulness and less oriented to temporary trends in physiotherapy. 3. There is an urgent need that university-level schools educating prospective physiotherapists develop educational programmes which would deal to a greater extend than it is at present with modern methods used in physiotherapy and which would constitute specific guidebooks in which students would be able to find information on the best parallel and post-graduate directions of further studies.

Usability of sEMG analysis for the evaluation of the muscles stimulation sequence in the lumbosciatic area in patients suffering from pain in lumbar spine

Abstract

Aim of the study: 1.  Evaluation of the relationship between the change in the stimulation sequence of muscles in the lumbosciatic area and the occurrence of functional pain in the L section of the vertebral column, as compared to the Janda’s model.
2. Analysis of the differences in the superficial electromyography (sEMG) record of a correct and disturbed pattern of muscle recruitment during the prone hip extension test in two sub-groups of patients: suffering from pain (sub-group A) and without pain (sub-group B).
3. Evaluation of the suitability of the sEMG measurement for making diagnoses in patients with pains in the lumbar section of the vertebral column.
The material and method: For the research, 61 persons were qualified, aged 20-35 years, with the average age 25 years (+/- 6). The tested group was divided into two sub-groups. 33 persons were qualified to sub-group A, and to sub-group B (reference group) 28 persons were qualified. Among the participants, a questionnaire was spread, prepared for the needs of the survey, to be filled in. For testing the sequence of stimulations, the measurements were used which had been obtained from the sEMG record during the Prone Hip Extension (PHE )Test.
Results: The results obtained during the examination of the muscles recruitment pattern in PHE test showed a changed sequence of muscles activation in the Patients from sub-group A vs. these from sub-group B. The results of chi-square test (χ²) confirmed the statistically significant dependence: the persons with pain symptoms in the lumbar spine statistically less frequently showed the correct sequence of muscles engagement:  χ²=5.05,  p=0.041, R= – 0.59 (strong negative dependence).
Conclusions: Changes in the pattern of muscles recruitment during the hip joint extension lead to overloads in the lumbosciatic area, consequently causing pain in the lumbar section. Superficial electromyography, because of the possibility of obtaining the quantitative outcome, is a useful method of evaluating muscles performance.

The physiotherapeutic estimation of the act of temporal-mandibular joints among students of Medical University of Gdańsk

Abstract

Aim of the study. 1. Evaluating the activity of temporomandibular joints manifesting features of dysfunction in students of the Medical University Gdańsk, 2. Quantitative analysis of the disorders occurring in temporomandibular joints. 3. Proving the correlation between the investigated parameters and the analysed dysfunctions in temporomandibular joints
The materials and research method. For the examination, 40 students in the age of 19 to 27 years (21 women and 19 men) were classified, representing the average age of 23 years.  For the needs of this study, a card of patient examination had been constructed. The evaluation covered, among others, such parameters as: the mobility range of the jaw, the Rozencwaig’s dyskinesia index, the movement of the joint head during the jaw abduction, acoustic symptom, pain index, the presence of trigger points, the cervical spine mobility range.  The obtained results were subjected to statistical analysis with the application of Statistica 10 software .
Results. Analysis of the investigated parameters revealed that dysfunctions of mobility range, at least in one direction, occur in 87,5% (n=35) of the examined persons. The Rozencwaig’s dyskinesia index was proven in 37,5% (n=15). Asymmetric movement of the joints heads occurred in 67,5% (n=27) of the examined persons; and the disorders of the jaw guiding track and the presence of the acoustic symptom were revealed in 80% (n=32) out of the total 40 of the examined persons. Among 57,5% (n=23) of the examined, the presence of trigger points was revealed within the masseter and the temporalis muscles, or in one of them. Dysfunction of the temporomandibular joints correlates, among others, with a reduction of the mobility range in the cervical spine, with acoustic symptoms and presence of parafunctions.
Conclusions. A significant percentage of the temporomandibular joints dysfunctions was observed among students. Attention was attracted by the high percentage of people performing parafunctions. Correlations statistically significant for the temporomandibular joints were proven.

Zastosowanie terapii wymuszeniem ruchu u pacjentów po udarze mózgu

Streszczenie
Co roku, około 10,5 mln osób na świecie doświadcza pierwszego udaru mózgu, czego częstym skutkiem jest niepełnosprawność fizyczna. Około 70-80% doznaje, w różnym stopniu, zaburzenia funkcji kończyny górnej, co znacznie pogarsza codzienne funkcjonowanie. W ostatnich latach rozwijano wiele różnych sposobów usprawniania po udarze, zwłaszcza tych, które wpływają na plastyczność mózgu. Należy do nich m.in. terapia wymuszeniem ruchu (CIMT, Constraint-Induced Movement Therapy), która bazuje na aktywnym udziale pacjenta. Podczas intensywnych treningów niedowładnej kończyny, druga, mniej zajęta kończyna, jest unieruchamiana. Wszystkie komponenty CIMT wspomagają zjawisko plastyczności mózgu. Przeprowadzono wiele badań, w których udowodniono, że terapia CIMT (oraz jej modyfikacja mCIMT) uważana jest za jedną z najbardziej efektywnych form kinezyterapii poprawiającą funkcję niedowładnej kończyny górnej po udarze. Terapia CIMT jest przeznaczona głównie dla pacjentów z łagodnym i umiarkowanym niedowładem kończyny górnej, stosowana jest zwykle w okresie przewlekłym po udarze mózgu.

Streszczenie
Cukrzyca jest narastającym problemem XXI wieku. Przewlekła hiperglikemia jest przyczyną wielu zmian patologicznych w postaci mikro i makroangiopatii, neuropatii oraz zespołu stopy cukrzycowej. Program leczenia powinien uwzględniać: dążenie do wyrównania glikemii, naukę samokontroli, walkę z otyłością a także postępowanie fizjoterapeutyczne. Artykuł ma na celu podkreślenie faktu że fizjoterapia powinna być integralną częścią programu terapeutycznego cukrzycy o czym się często zapomina. Omówiono w nim ogólne współczesne zasady postępowania fizjoterapeutycznego u chorych z cukrzycą zgodnie z wytycznymi z 2014 r. ich korzyści i niebezpieczeństwa a także szerzej poruszono zagadnienie kinezyterapii w tej grupie pacjentów.
