Lęk wśród terapeutów pracujących z dziećmi z autyzmem

Streszczenie

Wstęp. Lęk wśród terapeutów pracujących z dziećmi ze zdiagnozowanym autyzmem dotyczy strachu oraz obaw związanych z zaplanowaniem i przeprowadzeniem procesu terapeutycznego. Celem pracy była analiza sytuacji, w której terapeuci odczuwają dyskomfort i lęk podczas zajęć oraz ćwiczeń z osobami ze zdiagnozowanym spektrum autyzmu. W przeprowadzonych badaniach dokonano obserwacji przyczyn występowania zachowań lękowych oraz stopnia ich nasilania lub eliminowania.
Materiał i metody. Analizie badawczej poddano 16 terapeutów pracujących w Krajowym Towarzystwie Autyzmu. Badania odbywały się od grudnia 2014 r. do stycznia 2015 r. Terapeuci byli w wieku od 22 do 45 lat. Do badania został użyty sondaż diagnostyczny z wykorzystaniem techniki kwestionariusza ankiety.
Wyniki. Najczęstszymi objawami lęków wśród terapeutów pracujących z dziećmi autyzmem to zdenerwowanie oraz przyspieszenie akcji serca. Najlepszym sposobem radzenia sobie z lękiem jest relaks. Najczęstszy powód lęku wśród terapeutów pracujących z dziećmi z autyzmem jest agresja fizyczna i słowna ze strony dzieci oraz brak efektów pracy.
Wnioski. Ważnym elementem w pracy z dziećmi z autyzmem jest fakt, by nie przenosić problemów z pracy do domu. W ten sposób eliminowany jest stan wypalenia zawodowego. Istotnym czynnikiem eliminującym lęk jest wygospodarowywanie czasu na relaks i odprężenie.

Abstract

Introduction.  Surgical procedure involving total hip alloplasty is a common consequence of the absence of progress of rehabilitation in patients with significant degenerative lesions accompanied by severe pain and significantly limited mobility. An individual program for patient rehabilitation is established during the first 24 hours after the procedure. This is of particular importance as proper planning and implementation of physiotherapy allows to relieve pain and to achieve full functional joint mobility as well as to strengthen the muscles. We make sure that the proper muscle control and stability within the joint will restore patient’s locomotion and self-care both at the initial and later stages of rehabilitation. There are many ways and methods to improve the above mentioned parameters. Muscle energy techniques, which represent a non-invasive therapy in patients with total hip replacement, are one of these methods. These techniques, including poizometric relaxation of contracted muscles as well as active relaxation of hip joint muscles, myofascial techniques, also in combination with elements of PNF techniques, allow for effective treatment and rehabilitation of patients. Aim. The aim of the study was to compare the effects of standard physiotherapy in patients after cemented hip arthroplasty with those of physiotherapy using muscle energy techniques.
Materials and Methods. A total of 66 patients with advanced degenerative arthritis of the hip, who received surgical treatment in the form of endoprosthetics, were qualified for the study between 2013 and 2014. The patients were divided into two groups  (group I and II), 33 subjects each. Pain level, the range of hip motion as well as muscle strength were assessed in each patient. Group I received muscle energy techniques, including poizometric relaxation of muscles, active muscle relaxation and elements of PNF method. Also, myofascial techniques targeting muscles with different strengths and direction of pressure were used. Group II received standard physiotherapy.
Results. Significant improvement of pain, an increase in the range of functional motion as well as a significant improvement of muscle control of the joint were observed in the group receiving muscle energy treatment, which had beneficial effects on the locomotion of patients.
Conclusions. Muscle energy techniques had significant effects on the improvement of muscle function and stabilization in the pelvic girdle of the joint with endoprosthesis. Poizometric relaxation combined with elements of the PNF method as well as myofascial techniques effectively improved balance, coordination and gait pattern. In conclusion, physiotherapy using muscle energy techniques was shown to be a more effective method in the treatment of hip osteoarthritis as compared to the method based on standard rehabilitation program.

The level of anxiety and the attitudes of physiotherapists towards mentally disabled children

Abstract

Aim: The aim of this paper is to analyse the attitudes of physiotherapists towards mentally disabled children and to analyse the influence of their attitudes on their anxiety level.
Method: The study comprised 67 respondents; 37 students of physiotherapy and 30 physiotherapists who work in special care centres. Both research groups were asked to fill out an STAI Questionnaire which determines the level of anxiety as a state and anxiety as a trait, as well as author’s original questionnaire (Scale of Attitudes of Physiotherapists Towards Mentally Disabled Children).
Results: Both research groups present positive attitudes towards mentally disabled children. The research indicates that a more positive attitude towards mentally disabled children is presented by physiotherapists who work in special care centres, as compared to the other research group. A higher level of anxiety as a state is experienced by the students of physiotherapy.
Conclusions: Anxiety may influence the attitudes of physiotherapists toward mentally disabled children. It may be inferred that the higher the score of assessment of positive attitude towards mentally disabled children, the lower the level of anxiety in physiotherapists and students of physiotherapy.

Analysis of statics of the pelvis in radicular and pseudoradicular syndrome of the lower part of the spine

Abstract

Introduction. Radiculopathies and pseudoradiculopathies are the most common pain syndromes of the lumbar spine. This issue became a civilization disease and affects people of various ages. Goal. The goal of this work was to compare analgetic effectiveness of physiotherapy in radiculopathies and pseudoradiculopathies and to identify an association between syndromes of vertebral origin disorders of muscular elasticity as well as to assess the effects of therapy on pain-induced discomfort.
Material and methods. Study included 40 patients divided into two groups. Group A consisted of patients with radiculopathy, while group B comprised pseudoradiculopathy subjects. Muscle energizing techniques (MET), Hold Relax method and classical massage of musculofascial structures were applied in therapy. It also involved physiotherapeutic procedures (Interference currents, Tens, Iontophoresis, UD, Laser, Sollux). Results. Based on the Oswestry questionnaire it was determined that among group A patients level of disability was reduced by 38%, while in group B it decreased by 31%. In both study groups therapy contributed to reduction of pain measured in VAS scale by 58%. Lumbar spine mobility measured with Schober test increased by 11% in group A and by 5% in group B. Administered physiotherapy contributed to normalization of length and tension of musculoligamentous structures. Conclusions. Study results show that radiculopathies and pseudoradiculopathies affect the disorders of pelvic statics presenting with shortening of muscle fibers and increased activity of connective tissue structures in this region. Physiotherapeutic methods applied in treatment of radiculopathies and pseudoradiculopathies significantly reduce pain. Some techniques (MET) restore muscle elasticity, contributing to lessening of discomfort.

Abstract

Cryotherapy is one of the modern therapeutic methods in physiotherapy, in which the impact of cryogenic temperatures (-110ºC to -160ºC) on the human body is used.
Aim of the study.
The aim of this study is to evaluate the effect of cryogenic temperatures on the functional status of patients with lumbar spine pain syndrome.
Material and methods.
The study involved 40 patients with lumbar spine pain syndrome: 24 women and 16 men, from 35 to 55 years old. The patients were divided into two groups. There were patients who underwent systemic cryotherapy and 30-minute analytical kinesitherapy (study group) and patients who underwent only kinesitherapy (control group). The therapy lasted for 10 days. In the study, subjective methods (assessment of intensity of the experienced pain according to VAS scale, assessment of muscle strength according to Lovett scale) and objective methods (measuring the range of lumbar spine flexion – Schober’s test, measuring lumbar spine rotation) were used.

Assessment of the Therapy Effectiveness in Patients with the Lumbar Degenerative Disc Disease Treated with the McKenzie Method

Abstract

Introduction. The lower back pain syndromes can be defined as a series of ailments associated mainly with the lumbosacral section of a spine. The intervertebral discs disorders and degenerative processes are being  considered the major cause of these syndromes. A lower back pain may also be caused by the anatomical structures of a spine. The treatment of a lumbosacral pain is always a complex procedure. It includes kinezytherapy, physiotherapy and aims to reduce the pain, strengthen the muscular corset, restore the range of movement, muscle strength, and to re-educate patients in the areas of the correct posture patterns and the physical activities. Materials and Methods. In the research participated 38 patients with the diagnosed lumbar degenerative disc disease. The patients had been divided into 2 groups – control group and study group. All the participants, before they took part in the research, had been subjected to various forms of treatment and had been taking pain and anti-inflammatory medications. The study group had undergone treatment according to the McKenzie method, while the control group had  applied the Maigne syndrome exercises. Both groups were treated with the physiotherapeutic procedures. After completing the exercises, each group filled out a questionnaire regarding the treatment. Objective: To determine the impact of treatment according to the McKenzie method on changes in the functional status of patients with the lumbar degenerative disc disease. Conclusions. The research has shown, that the application of the McKenzie method yielded positive results in terms of the increased range of movement within the lumbosacral spine section, in all patients. McKenzie method exercises do have an impact on the peripheral joints of the spine. The physiotherapy treatment according to the McKenzie method, applied in the study group, resulted in a reduction of pain in the l-s spine section, in 100% of patients. The Maigne method application in patients with the diagnosed lumbar degenerative disc disease, only in a mild way alters the patients’ sensation of the lower back pain.
