The influence of external loading on weight compensatory changes and pelvic behaviour during walking in scoliotic children

Abstract
Background. Compensatory changes in the spatial arrangement of body segments are a characteristic sign of the development of scoliosis. Additional external loads may lead to intensification of existing signs in both static and dynamic conditions. The manner of carrying heavy objects on a daily basis is a key factor here. The aim of this paper was to assess some changes in static and dynamic parameters associated with carrying the school pack in children with scoliosis.Material and methods. Thirty-six children aged 8-15 years with low-grade scoliosis were examined. Foot pressure in static conditions was recorded using a force platform without additional loading, and with a 4 kg external load carried in a symmetric or asymmetric manner. Three-dimensional gait analysis was also performed using a Zebris® system. Parameters were registered during walking without and with the external load.Results. Signs of asymmetric foot pressures were observed that intensified during external loading of spine (both symmetric and asymmetric). These changes were not clearly related to body posture parameters. There were also signs of gait asymmetry that intensified during walking with asymmetrical external loading. The most marked changes were observed with respect to pelvis-related gait determinants. The magnitude of changes was related to the manner of carrying the school pack rather than to body posture parameters. Conclusions. External loads intensify the asymmetry of foot pressure distribution as a result of spontaneous and unpredictable compensatory dislocations. The manner of carrying the school pack is not irrelevant in children with scoliosis.

Infant crawling and motor coordination level of children aged 4-7 years

Abstract
Introduction. One of the stages of a child’s motor development in the first year of life is crawling, whose positive influence on psychomotor development has been supported by research. However, some children skip this stage of locomotion and start to move immediately in a standing position. The aim of the study was to assess the motor and visual-motor coordination of children who did not crawl in infancy and to compare the results with a group of children who did not omit this stage of motor development.
Material and methods. The study involved 53 healthy children aged 4-7 years attending preschool facilities in Upper Silesia. The control group consisted of 20 children, who did not crawl in infancy. The control group consisted of 33 children who were crawling in infancy. The research consist: author’s questionnaire and Ozierecki’s Test of motor and visual-motor coordination. Questionnaire concern five parts of information about health, development, and current motor skills level of children. The second stage of the study – Ozierecki’s Test (interpreted by A. Barański) was used to assess the motor and visual-motor coordination level of 4-16 years old children.
Results. The group of crawling children had a higher level of visual-motor coordination, but only in one of the examined skills (swinging). The presence of crawling does not significantly affect the motor abilities of an older child. In the study group, it is noted that the higher the value of Apgar points obtained at birth, the higher the motor skills and motor coordination.

5-7岁儿童的偏侧化特性

摘要
偏侧化也被称为“功能不对称”,是每个人心理物理正确发展中必要的一部分,取决于大脑半球的专业化。偏侧化的特性在于身体一侧的效能高过于另一侧及大脑对感觉刺激记录的差异。偏侧化为一重要因素,随我们身体在空间中定向的方式发展,执行动作任务并协助形成躯体感觉及身体的最终形态。
该研究的目的在确定并观察学龄前及学龄前期儿童偏侧化形成的过程。
材料及方法。试验组由55名女孩及55名男孩在内共110名年龄在5-7岁间的儿童组成,他们来自西里西亚省的小学及幼儿园。试验期间进行诊断测试,即随意选择观察眼、耳、上肢和下肢等。
结果。儿童的侧面支配优势确定随年龄而增加,35%的儿童有交叉性偏侧化的失调现象,这种模式最常出现在5岁年龄组的儿童中。
结论。根据所获得的数据显示,5-7岁的儿童偏侧化过程未完成。

5-7岁间患自闭症谱系障碍儿童的平衡评估

摘要
自闭症谱系障碍为一系列广泛症状,呈现出在沟通、社会发展和陈规定型行为模式等领域上的缺乏。自闭症的症状也出现在社交互动、情绪障碍、体感障碍及注意力问题等问题上,患自闭症的儿童经常被诊断出有感觉处理障碍。儿童发展中非常重要的能力是保持平衡,使其能在支撑平面上控制重心,适当的平衡为顺利完成多项日常活动所不可或缺的因素,平衡发展的延迟将阻碍日后的运动发展,并影响孩童与同龄人交流的机会。
研究目的在客观评估患自闭症谱系障碍儿童的同等技能。
材料及方法:共30名5-7岁患自闭症谱系障碍的儿童参与试验,其中包括22名男孩及8名女孩,采用富勒頓高級平衡計分錶規模 (FAB) 测试作为平衡评估。
结果:各年龄组间取得的测试结果不同,但这些差异并不显著,而在各年龄组的串联步态测试中则出现显著差异。
结果:患自闭症谱系障碍的儿童中出现同等技能失调。

Abstract

In children with Cerebral palsy (CP), according to the definition, motor disorders occur.
The primary aim of the rehabilitation in this group of children is to support the development of the best quality of motor and posture patterns. Presented study describes the effects of non-invasive physical therapy, based on Neurodevelopmental Treatment (NDT) connected with serial inhibiting casts application, on postural control and gait pattern improvement in 5 years old child with spastic hemiplegia.
Functional assessment, before and after treatment, included: clinical measures of spasticity, weight – bearing distribution between body sides, posturography testing and gait analysis. Applied therapy influenced on: reduction of spasticity in the affected limb and improvement in postural control and gait parameters. Both range of movement (ROM) deficit and spasticity angle (AOS) in knee and ankle decreased and effected on postural control and gait improvement.
Obtained results demonstrate that non-invasive procedures of physical intervention, connected with serial inhibiting casts application, can be successful method of spasticity reduction, postural control and gait improvement in young children with spastic unilateral CP. Early detection of spasticity of triceps contracture and proper physical intervention may eliminate the need for invasive intervention and its associated risks in this population.

Musculoskeletal disturbances in women with Turner’s syndrome – case report

Abstract

Turner’s syndrome term defines the most commonly encountered genetic aberration, connect-ed with genital chromosomes. In girls and women with Turner’s syndrome, characteristic ex-ternal appearance features (called Turner’s phenotype) occur. Moreover, patients also exhibit disturbances directly related to musculosceletal system. Presented case study is based on the results of the functional assessment of 18 years old – randomly selected – patient from Paediatric, Endocrinology and Children’s Diabetology, where on the basis of cytogenetic examination high concentration of gonadotropins typical for Turner’s syndrome was found. As part of the research, following exams have been made: ob-jective photogrammetric assessment of body posture, spirometry measurement, computerized evaluation of feet arches, assessment of body weight distribution and three dimensional gait analysis. The results of this study revealed various functional disturbances in: postural control, pelvic space orientation, weight bearing distribution and in gait pattern. However, there were no spirometry and basic stabilography measures disturbances. Received results may suggest the presence of other disturbances in Turner’s syndrome than those described in literature. Confirmation of musculosceletal functional disorders would indicate the need for early functional diagnostic procedures and early physiotherapy intervention in patients with Turner’s syndrome starting in infancy.

Physiotherapist – occupational predispositions. Level of flexibility

Abstract

Background. Possibly high level of functional efficiency is not only the foundation of human welfare, but it also plays an important role in fulfilling social functions, including professional functions. Among professions in which physical fitness is an important element of the quality of work stands out the profession of physiotherapist. Flexibility is one of the components of physical fitness. In those professions, which are characterized by volatility of positions and different effort load, the appropriate level of flexibility seems to be of particular importance.
Aim of the study. The aim of the study was to determine the level of mobility (flexibility) of the trunk among physiotherapy students. It was also decided to investigate how such variables as gender, age, morphological parameters, past and current physical activity are related to the present level of flexibility.
Material and methods. 341 people have been examined: 289 women (84.25% of the total) and 52 men (15.25%). The average age of the study group was: 20.26 years (SD = 1.69) for women; 19.85 years (SD = 1.73) for men. All of these persons were physiotherapy students.
Following data have been collected: age, past sports and physical activity – according to the Baecke questionnaire. Anthropometric measurements and three – plane measurement of the flexibility of the spine have been also made.
Results. There have been weak relationships between morphological parameters and flexibility – only in women. Physical activity strongly differentiated the level of flexibility among women in a sagittal plane (p <.05) and a transverse plane (p <.001; p <.002), a similar situation is with past sports: a sagittal plane: (p <.001) and a lateral plane on the left side (p <.05). In men, only past sports differentiated flexibility in a transverse plane on the right side (p <.05).
Conclusions. The level of flexibility varies individually, especially in a sagittal plane. Gender, age and morphological parameters in young adults are poorly associated with the flexibility. Past and current physical activity have a positive influence on the trunk functional efficiency.

The level of burnout among representatives of medical professions

Abstract

Aim of the study. Burnout is a phenomenon which affects many professions, which are based not only on disturbed man-labor relations, close interpersonal relationships, but first of all are based on specific approach to another human being, requiring patience, empathy and commitment. Despite many scientific studies there is no clear solution to the problem of burnout in medical professions, which is a problem underrated and extremely important, especially from the patient’s point of view.
Material and methods. Presented study was designed to determine the prevalence and the level of burnout among medical professionals: doctors, physiotherapists, nurses and midwives. Among 209 medical professionals, evaluation of the level of depersonalization, emotional exhaustion and a sense of their own achievements, using the original questionnaire: Burnout Questionnaire MBI (Maslach Burnout Inventory) by Ch. Maslach has been carried out.
Results. The results showed that the highest percentage of burn out in employees was noted among doctors and nurses. In addition, it was found that the severity of burnout cases among medical staff depends on the specific clinical department, age of the respondents and years worked in the profession.
Conclusions. Results of this study do not allow to definite unequivocal conclusions, however, these results are encouraging enough to continue studies in this field of study in a much wider population of representatives of various professional groups.
