Abstract
This report aims to alert clinicians to the importance of comprehensive evaluation starting with collecting the details of patient’s history. This case also highlights the need to prioritize interventions starting with conservative treatments based on severity and urgency. We report a case of cervicalgia and bilateral cervical radiculopathy of 62 years old female who did not respond to conservative physical therapy treatment. The specialist used total assessment- reassessment and evaluation using biokinesiology (TAREK) approach for clinical reasoning and judgment to come up with tentative physical therapy diagnoses. The manifestations were confirmed by magnetic resonance imaging report. The clinical manifestations were in harmony with the radiological findings. The patient was poorly responding to conservative treatment with many weeks of no response. Surgery was offered to ease up patient’s symptoms. Finally, clinicians should be familiar with the both the primary and the secondary pathological changes of cervical radiculopathy and put in consideration the complexity of clinical manifestations and poor prognosis.

Physiotherapy in the preoperative treatment of lumbar discopathy

Abstract
Background. This article analyzes the therapeutic methods used in the conservative treatment of lumbar discopathy among patients treated surgically for this disorder at the Neurosurgical Clinic of the Lublin Medical University from 1998 to 2000. Material and methods. Our research involved 349 randomly chosen patients referred for surgical treatment of lumbar discopathy. The patients were interviewed in detail and their medical records analyzed on the basis of an authorial questionnaire. The results were subjected to statistical analysis using the contingency analysis tables is the STATISTICA software program (StatSoft, USA). Results. Our research indicated that long-term conservative treatment was limited primarily to pharmacotherapy. Methods of physical treatment, i.e. kinesitherapy and physicotherapy, as well as stationary rehabilitation in health resorts were not put to adequate use by the majority of patients.

Abstract
Based on the experience of the authors’ rehabilitation center is Lublin, the authors present the model of medical rehabilitation they have developed for patients with spinal injuries not involving damage to the central nervous system. The model is dependent on the type of treatment used for spinal injury (surgical or non-surgical). In the case of patients requiring surgery using internal fixation, medical rehabilitation includes preparation of the patient for surgery (over and above nursing activities) by means of respiration exercises, followed by kinesitherapy and physicotherapy after surgery in order to restore physical fitness as quickly as possible. In non-surgical procedures, medical rehabilitation includes physiotherapy, broadly conceived, involving one of three schemes: functional treatment without internal fixation, functional treatment after the application of internal fixation (young persons), and functional treatment for persons with cervical spinal injuries, using direct cranial traction. The authors point out the necessity for cooperation in the entire treatment team in proper rehabilitation procedure.

Abstract
This article presents the newest systematics of physiotherapy that has been compiled and developed by the author. The previous systematics and divisions, including charts and schemes from the available scientific literature, have also been described and characterized, with the special approach to historical aspects of the so-called physical medicine, later termed physiotherapy. The innovative changes in the systematics as these in shape of new methods and techniques that have been incorporated seem indispensable and have been thoroughly explained by the author.

Abstract
The aim of the study is to consider the available evidence related to clinical effectiveness of several types of physical modalities used in the management of painful shoulder. A systematic critical review from basic science studies evaluating therapeutic effects includes the following treatments: superficial heat, cryotherapy, laser therapy, impulsivemagnetic field therapy, short wave diathermy, transcutaneous electric nerve stimulation (TENS), interferential current therapy, ultrasound therapy, iontopphoresis and phonophoresis. There seems to be evidence from basic science research to suggest that many of the therapies could have potentially therapeutic effects. It is important to note that these conclusions are limited by the methodological considerations of the included studies. However, in the managemet of the painful shoulder treating pain is often the basis of the treatment, it should be implemented with a broad range of physiotherapeutic techniques and manual therapy as well when appropriate.

The mobility of the lumbar spine in persons with low back pain

Abstract

Background. The purpose of our study was to examine changes in the mobility of the lumbar spine in persons with L5-S1 diskopathy and the changes in mobility associated with different modalities of treatment. Material and methods. We studied 150 persons 40-50 years of age, 64 men and 86 women. Due to variations in management the material was divided into two groups: Group I, treated only pharmacologically, and Group II, treated with both pharmacological methods and physiotherapy. The patients were examined twice, once before treatment and again after 6 weeks of treatment. The research methods included clinical interviews and measurement of thoraco-lumbar and lumbar mobility on all planes. Non-steroid anti-inflammatory drugs were used in Group I. In Group II, comprehensive physiotherapy was also applied, including physicotherapy, kinesitherapy, and massage. Results. Our analysis of the material showed that the limitations in spinal mobility were similar in both groups at baseline. After a 6-week course of treatment, significant improvement in the mobility of the thoraco-lumbar and lumbar spine was noted only in Group II (pharmacotherapy and physiotherapy), involving all the tested ranges of movement. Conclusions. An evaluation of treatment outcome in persons with L5-S1 diskopathy indicates that persons treated both with pharmacotherapy and physiotherapy show greater improvement. The fact that significant improvement was seen only in this group may result from the fact that the drugs used prepared the way for better results in subsequent physiotherapy. These results indicate that pharmacological treatment in these patients should be supplemented with physiotherapy.

Evaluation of the effectiveness of Tolpa Physioactive Gel in patients with osteoarthritis

Abstract

Background. The goal of our research was to evaluate the application of Tolpa Physioactive Gel in the treatment of osteoarthritis in peripheral joints. Material and methods. Two cycles of physicotherapy were performed on patients with osteoarthritis of the knee or hip joints. The first cycle was performed with an active substance, Tolpa Physioactive Gel, while the second was performed with a placebo. We assessed the following parameters: range of movement in the affected joint, the WOMAC Scale, the Visual Assessment Scale (VAS) for pain, the Likert scale, and an attempt to walk on a flat surface. Results. Subjectively there was greater improvement after application of Tolpa Physioactive Gel, as assessed on the WOMAC, VAS, and Likert scales. The examiners agreed that improvement was achieved in all patients in respect to range of movement in the affected joints and muscle strength after rehabilitation procedures, both with the physioactive gel and the placebo. Conclusions. Tolpa physioactive gel can be used effectively during iontophoresis in the treatment of patients with osteoarthritis.

Interpretation of the results of physiotherapy for low back pain based on the Ryodoraku electrodermal measurements

Abstract
Background. The aim of this study was to analyse skin conductance of a direct current (galvanic skin response, GSR) in dermatomes corresponding to the level of discopathy and to provide an indirect objective interpretation of several commonly used physiotherapeutic methods: low frequency diadynamic currents, medium frequency interferential currents, low frequency pulsed magnetic therapy and classic acupuncture. Material and methods. Tests of GSR, an indirect measure of sympathetic activity, were carried out in addition to clinical assessment and radiological studies in a group of 164 patients with diagnosed low back pain [65 persons with a pseudoradicular pain syndrome and 99 patients with evident sciatic neuralgia], using the diagnostic component of Yoshio Nakatani’s Ryodoraku method. The essence of the method is the measurement of skin conductance of a 200 uA direct current at 24 points of the body regarded as representative of systemic sympathetic activity. Results. There was no correlation between the clinical characteristics of a pain syndrome and GSR changes. Skin conductance values significantly more often fell within the normal range following exposure to a low frequency magnetic field compared of the other physical therapy methods investigated in the study. Conclusions. The following conclusions can be made on the basis of the observed outcomes: 1. The Ryodoraku method is non-specific and inappropriate as an additional tool for the diagnosis of sciatic neuralgia; 2. Some correlations can be established between conductance measurements and the physical treatment method used.

Trunk asymmetry in people with L5-S1 lumbar discopathy

Abstract
Background. The aim of this study was to investigate and describe trunk symmetry patterns via an analysis of changes in the position of selected anthropometric points and tracing these changes in time relative to type of treatment. Material and methods. The study enrolled 150 people aged 40-50 diagnosed with a chronic spinal pain syndrome inthe course of L5-S1 discopathy. The patients were divided into two groups corresponding to different treatments used. Patients in each group were further divided with respect to the direction of the hemia of the nucleus pulpous. Examinations were carried out twice: before starting treatment and after 6 weeks. Results. The degree of asymmetry in the frontal plane was similar between the study groups. Asymmetry occurred in a considerable percentage of the patients in both groups. Conclusions. Greater improvement of the asymmetry was noted in the patients receiving combination treatment, which may indicate a need for combining physiotherapy with pharmacological treatment in the treatment of discopathy. A complete cure is difficult to obtain in lumbar discopathy patients following a 6-week treatment. Long-term thearpy is needed as well as broadly understood prophylaxis to prevent recurrence of symptoms.

Abstract
Background. The aim of the study was to assess the impact of physiotherapy on selected functional parameters and the process of maintaining body balance in patients after hip joint replacement surgery. Material and methods. The study involved 30 patients, aged 60-75 years, following cementless hip replacement surgery. All subjects underwent a goniometric examination of hip joint range of motion and were assessed with regard to the process of maintaining body balance in a standing position on a stabilographic platform. The treatment regimen comprised kinesiotherapy and physical therapy procedures. Results. The comprehensive programme of physiotherapy improved the range of motion of the hip joints and the process of maintaining body balance in a standing position in the study group. Conclusions. By improving the functional status of joints and the process of maintaining balance, the treatment method used in this study enabled the patients after hip replacement surgery to improve their quality of life.
