Efficacy of two different physiotherapeutic procedures in comprehensive therapy of plantar calcaneal spur

Abstract

Background. A plantar calcaneal spur causes pain in the area of the medial process of the calcaneal tuber. The pain is caused by localised pressure in the region of the plantar aponeurosis attachment to the calcaneal bone. Plantar calcaneal spurs are principally treated by physiotherapy techniques, with ultrasound as the most popular procedure. At present, phonophoresis is more and more frequently used. Phonophoresis is an ultrasound treatment in which the substance between the transducer and the skin is replaced with a medicine in gel form. The aim of this paper is to assess the efficacy of and compare ultrasound- and phonophoresis-based therapeutic procedures for calcaneal spur. Materials and methods. 40 patients treated for plantar calcaneal spur participated in the study. The patients were randomised into two groups: Group A was treated by phonophoresis and group B received an ultrasound treatment. The patients were between 35 and 80 years of age (mean age 57.3 years). The enrolment criteria for the study were: absence of concomitant systemic disease or generalised inflammatory conditions and no history of recent injuries. The following criteria of physiotherapeutic treatment efficacy were adopted for the study: subjective pain intensity using the VAS scale, the range of plantar flexion and supination as well as the strength of muscle groups of the longitudinal arch of the foot. In order to provide better insight, taking into account the risk factors for the calcaneal spur, the BMl as well as Bogdanov’s ankle bone angle, which determines the length of the longitudinal arch of the foot, were determined in both groups. Results. Plantar calcaneal spur patients are usually overweight and suffer from slight longitudinal flatfootedness. A significant reduction of pain intensity was observed in group A (at p<0.001), and in group B (p<0.01). There was also an improvement in both foot flexion and supinetion at p<0.001. The strength of foot flexors and supinators increased significantly in both groups, at p<0.001 in group A and p<0.05 in group B. Conclusions. The clinical profile of calcaneal spur patients mostly includes overweight and slight longitudinal flatfootedness. Phonophoresis is a significantly more effective physiotherapeutic procedure than ultrasound in the physiotherapeutic management of plantar calcaneal spurs.

Abstract
The characteristic progress in science and technology of the 21st century is being accompanied by significant research developments in the field physical medicine. Before our very eyes physics is entering more and more courageously into treatment, broadening the therapeutic armamentarium and also reducing the side effects of pharmaceutical agents in some cases. Particularly in recent years one can observe increasing interest and research concerning therapeutic application of new physical methods, including variable fields, which until now have been underused. One such method is the combined application of lasers and magnetic fields, known as magnetolaserotherapy. Magnetolaserotherapy, the combined action of laser radiation and a variable magnetic field, has led to increasingly wider use of this treatment modality in various fields of clinical medicine and rehabilitation. Document positive effects of the use of the biostimulating laser and variable magnetic field in the control of pain, acceleration of wound healing and strengthening of the immune system have already been positively confirmed in a large number of clinical studies, which directly translates into the duration, quality and patient comfort duringtreatment. The present paper is based on information contained in available publications concerning magnetolaserotherapy and the authors’ experience connected with this method. This article is a clear comprehensive review intended for specialists in broadly defined physical medicine.

The PNF method in the treatment of intervertebral disc disease

Abstract
Background. Low back pain syndromes are a social and medical problem affecting 40-50% of the population. The most common cause of low back pain syndromes is changes within the intervertebral disc. The abnormalities may affect the stability of the motion segment. The aim of this work is an analysis of the application of selected procedures of the PNF method and McKenzie method in the treatment of 1st-, 2nd- and 3rd- degree L4/L5 or L5/S1 intervertebral disc injury. Material and Method. Patients were divided into two homogeneous groups. The experimental group (30 people) was subjected to exercises according to the PNF and McKenzie methods and the control group (30 people) utilised only McKenzie exercises. Treatment efficacy was estimated during and on completion of the treatment, and 6 months after completion on the basis of the pattern of pain, range of motion in the sagittal plane, evaluation of the strength of trunk flexors and extensors and the rate of relapse. The results were subjected to a statistical analysis. Results. There was more rapid pain reduction, improvement of range of motion, strength and fewer relapses in the experimental group. The positive effects of the treatment were sustained over the six-month follow-up. Conclusion. The combined application of the two methods in the experimental group was associated with superior efficacy compared to the control group.

Influence of whole-body cryotherapy on hemodynamic parameters in patients with motor organs disease

Abstract
Background. Whole-body cryotherapy is a physical method that is increasingly being employed in rehabilitation. The procedure consists in stimulating the body with temperatures below -100ºC. The most common applications of cryotherapy is in the treatment of diseases or injuries of the locomotor system. Material and methods. The study comprised 222 patients: 114 females (51.3%) and 108 males (48.6%) selected to the study group, diagnosed with discopathy (107 persons; 48.1%), lumbalgia at L5-S1 (57 persons; 25.6%), and spinal column degeneration (58 persons; 26.1%). The patients underwent series of 10 whole-body cryotherapy procedures, the sessions, each lasting 1-3 minutes, were conducted every day for 10 days. Changes in the body surface temperature (before therapy and after its completion) were recorded and analyzed, in addition pulse, and arterial blood pressure after 1, 5 and 10 procedures, with respect to diagnosed disease and the patient’s gender were also analyzed. Results. Whole-body cryotherapy procedures caused significant reduction in body temperature (p&lt;0.01). Evaluation of pulse and arterial blood pressure parameters showed that the group of patients with discopathy as well as spinal pain syndrome registered the highest changes in pulse rate value (p&lt;0.01). Higher pulse rate fluctuations were observed in women. Results on arterial blood pressure values revealed a statistically significant increase in systolic blood pressure parameters only (p&lt;0.01).Conclusion. Whole-body cryotherapy lead to significant drop in body surface temperature and increased pulse rate and systolic blood pressure.

Wpływ hipoterapii na wybrane czynniki fizyczne i psychiczne u dzieci z autyzmem

Streszczenie
Wstęp. Autyzm jest uznawany za zaburzenie rozwojowe, prawdopodobnie determinowane biologicznie i przejawiające się w różnych sferach funkcjonowania dziecka.
Cel. Celem badań była ocena wpływu rocznej hipoterapii na funkcjonowanie fizyczne i psychiczne u dzieci z autyzmem w wybranych obszarach.
Materiał i metody. Rodzice i dzieci chore na autyzm w wieku 5–7 lat, którzy w ostatnich 2 latach przebyły terapię z wykorzystaniem hipoterapii. W celu dokonania oceny wpływu rocznych oddziaływań wychowawczo-edukacyjnych na poszczególne sfery rozwojowe dzieci z autyzmem skorzystano ze zmodyfikowanej skali PEP-R, uzupełnionej kwestionariuszem własnej konstrukcji na temat cech demograficzno-społecznych oraz w zakresie przebiegu choroby. W analizie statystycznej stosowano test t-Studenta oraz współczynnik korelacji Pearsona. Za statystycznie znamienne przyjmowano te wyniki testów, dla których poziom istotności był mniejszy lub równy 0,05 (p < 0,05).
Wyniki. W toku rocznego procesu terapeutycznego z wykorzystaniem hipoterapii nastąpiła poprawa w rozwoju motoryki i reakcji emocjonalnych u dzieci z autyzmem. Wysoka motoryka dzieci z autyzmem ma pozytywny wpływ na sferą emocjonalną. Niższy stopień autyzmu cechuje wyższy poziom funkcjonowania fizycznego i psychicznego dzieci z autyzmem niż dzieci z umiarkowanym i dużym stopniem autyzmu.
Wnioski. Hipoterapia jest jednym z obszarów terapii osób z autyzmem.

Abstract

Achilles tendon is the largest and strongest tendon in the human body. Due to its key function in motor skills and the currently observed increase in popularity of various sports disciplines, it is subject to more frequent injuries. This requires the introduction of appropriate therapeutic measures. The main goal of the whole treatment process is to ensure the full recovery of tendon function. In cases of a full tendon ruptures, the treatment of choice is surgery unless it is contraindicated for general medical reasons. In both cases, proper physiotherapy is needed. In the article, the authors present an algorithm for physiotherapy after traumatic injury to the Achilles tendon.

Abstract

Objective. The objective of the study was to assess the effectiveness of the proposed treatment programs for patients with 1st and 2nd degree spondylolisthesis.
Material and method. The study involved 40 patients aged 50-80 with radiologically diagnosed 1st and 2nd degree spondylolisthesis. The subjects were randomly divided into two groups: a group treated according to the standard program used in NFZ (National Health Fund) institutions and a group treated according to the proprietary rehabilitation program. In each of the patients, the intensity of pain was measured, and stabilographic measurements, torque measurements during flexion and extension of the torso under static conditions were carried out.
Results. Mann-Whitney U analysis showed significant differences for most variables measured between treatment groups (p < 0.05). According to the results of descriptive statistics, patients treated using the method of a routine rehabilitation program applied at an NFZ institution achieved significantly higher results in the measurement of the intensity of pain PO and during stabilographic tests, standing on both legs with both eyes closed PO [mm] than patients treated using myofascial trigger point therapy (p < 0.05). On the other hand, significantly higher results in stabilographic measurements with open eyes PO [mm] and when straightening the torso under static conditions (Nm) were achieved by a group of patients treated using myofascial trigger point therapy (p < 0.05).
Conclusions. The results of the study allowed for the evaluation of the effectiveness of the proposed treatment programs for patients with 1st and 2nd degree spondylolisthesis, and then to decide on the modification of routinely used treatment programs for this disease in NFZ institutions.

Abstract

Undoubtedly, the ever frequent use of physical medicine methods in many fields of medicine is a fact. A wide spectrum of procedures enables instituting suitable treatment, reducing treatment time, as well as social and economic costs of treatment. The above applies to pathological states and injuries of the locomotor system, soft tissues, as well as chronic wounds. The pharmacological treatment applied proves to be insufficient, costly, and lengthy in many cases. The paper reports positive result of treatment of a 17-year old patient with persistent chronic wound after removal of long fibular muscle (due to its necrosis) in the right leg, as consequence of a traffic accident. In the treatment, magnetostimulation (variable magnetic field) procedures were applied for 12 weeks, leading to complete healing of the surgical wound. The wound healing allowed the patient to move about unaided (without elbow crutches, feeling no pain), while the positive outcome of treatment enhanced the patient’s quality of life. Physiotherapeutic methods commenced on admission to the complex treatment in patients on dermatology ward conduct to total healing wound.

Physiotherapy in the treatment of lymphoedema

Abstract

Lymphoedema is a chronic disease caused by the malfunction of the lymphatic system, leading to the accumulation of lymph in the interstitial space. The consequence of a primary or secondary lymphoedema is an unesthetical appearance of the limb, limited activity, and sometimes a significant distortion with a deformation of osteoarticular structures and muscle atrophy. Emotional problems often appear, affecting interpersonal relationships and the quality of life of the patients. A complete decongestive physiotherapy is still considered to be the most effective method of conservative treatment of lymphoedema, where a corresponding plan and implementation of the treatment throughout different degrees of severity of the disease brings good therapeutic results.

Application of the Kaltenborn-Evjenth Orthopaedic Manual Therapy Concept in the Treatment of Shoulder Impingement Syndrome

Abstract
Introduction. The aim of the study has been to evaluate the impact of the manual therapy according to the Kaltenborn-Evjenth Concept on the range of motion, joint play and the muscle strength in the shoulder joint, in patients with the shoulder impingement syndrome. In addition, the changes of the intensity of pain have been analysed.
Materials and Method. The total of 53 persons have been examined. The test group consisted of 42 patients with the shoulder impingement syndrome caused by the shoulder damage, with the diagnosis code of ICD 10: M75, while the control group encompassed 11 patients with the shoulder impingement syndrome caused by the cervical radiculopathy, diagnosis code of ICD10: M50. The patients had undergone the therapy every other day, for the period of 4 weeks. Measurements of the range of motion had been taken, the joint play and muscle strength in the shoulder joint had been evaluated. The pain experienced had been assessed according to the VAS scale. Before launching the therapy, and after it was completed, the patients filled out a questionnaire regarding the treatment. The results obtained constituted the indication for the direction of the orthopaedic manual treatment techniques, according to the Kaltenborn-Evjenth Concept.
Resulsts. The results of the research have shown, that the application of the Kaltenborn-Evjenth manual therapy has led to the increased motor activities and has helped to decrease the intensity of the shoulder pain in 100% of the examined patients.
Conclusions. The therapy according to the Katlenbor-Evjenth OMT Concept, applied in the cases of the shoulder impingement syndrome, has had a significant impact on the functional improvement of the shoulder joint and has contributed to the lowering of the intensity of pain.
