摘要
背景与目的。前庭康复是一种基于运动的方法,旨在最大化中枢神经系统(CNS)在前庭核及其他CNS水平对前庭病理的补偿。仅有少数研究记录了前庭康复对双侧前庭功能减退(BVH)患者恢复速度的影响。本研究的目的是调查结构化前庭康复(VR)计划对慢性BVH患者的眩晕严重程度、运动恐惧症、平衡能力、睡眠质量、日常生活活动(ADL)和生活质量(QoL)的有效性。
病例描述。本研究纳入了三名被诊断为BVH的参与者,他们患有眩晕严重程度、运动恐惧症、平衡能力、睡眠质量、ADL和QoL问题。
干预。一个结构化的VR程序每周一次,每次50分钟,作为家庭练习程序每周3天,持续8周。
结果。参与者使用视觉模拟量表(VAS)评估眩晕严重程度,使用坦帕运动恐惧症量表(TSK)评估运动恐惧症,使用半跟踪、跟踪和站立测试评估平衡能力,使用匹兹堡睡眠质量指数(PSQI)评估睡眠质量,使用前庭障碍日常生活活动(VADL)评估ADL,使用眩晕障碍库存(DHI)评估QoL,在研究开始时(T0)、第4周(T1)、第8周(T2)和第20周(T3)进行评估。
结论。总之,一个为期十二周的结构化VR程序可能会增强慢性BVH参与者的眩晕严重程度、运动恐惧症、平衡能力、睡眠质量、ADL和QoL。

Abstract
Background. Imbalance during standing, which is usually observed as an asymmetry in the weight shifting toward to the unaffected side of the body, is one of the most common factors affecting the independence and quality of life of post-stroke patients. Clinical assessment of imbalance in post-stroke patients is often conducted by visual observation using standardised tools (balance tests). However, quantitative analysis, using a posturographic platform, is more accurate and provides more information about the patient’s functional status. The TYMO® device, used in this study, is a portable posturography platform that measures tilts of the body’s center of mass and reports the results of the rehabilitation process. It enables an objective, direct and quantitative assessment of the patient’s functional status. Such an assessment can contribute to significant effectiveness of physiotherapy and consequently improve the patient’s quality of life and shorten the period of abstinence from work. Objective. Quantitative assessment of imbalance and postural control using a posturography platform (TYMO®) in patients after ischaemic stroke, in the chronic phase, as an important component of the functional diagnosis process and rehabilitation programme design. To emphasise the role of the posturography platform (TYMO®) as a tool for measuring static balance – symmetry of body weight distribution, and a tool for monitoring and reporting the results of physiotherapy treatment. Methods. In the current study, before and after two weeks of rehabilitation, quantitative measurements of balance on the TYMO® platform were made in a group of subjects (n = 60: adults, after ischemic stroke – first stroke episode, in chronic phase – up to 5 years after the stroke incident occurred) undergoing therapy using neurophysiological methods (PNF – Proprioceptive Neuromuscular Facilitation and NDT-Bobath – Neurodevelopmental Treatment according to the Bobath concept) and the SPIDER system (Strengthening Program for Intensive Developmental Exercises and activities for Reaching health capability). Measurements included: the distance marked by the patient’s center of mass while performing the test, the medial-lateral and anterior-posterior tilts of the subject’s body, the area of movement marked by the body’s center of mass, the average speed at which the patient performed the movement to maintain the required position, and the distribution of the subject’s weight. Based on the posturographic results obtained before therapy, it was possible to design a targeted rehabilitation programme. The magnitude of the difference in measurements before and after rehabilitation made it possible to assess the impact of the therapy on the patient’s balance. In addition, it was a specific indicator of the accuracy of the selection of physiotherapeutic treatment (a large difference in the mean results before and after therapy reflected an improvement in the parameters of postural control, hence the effectiveness of the therapy) and determined the direction of the future rehabilitation programme. Results and conclusions. The parameters measured by the TYMO® platform are crucial in assessing the functional status of post-stroke patients, especially with regard to postural control or balance disorders. The results described confirm the validity of using quantitative assessment, using the posturography platform, as an important component of the functional diagnostic process and designing an rehabilitation programme. The TYMO® platform itself is a useful tool for measuring, monitoring and reporting the effects of physiotherapeutic treatment in post-stroke patients.

Effect of Minds in Motion program on vestibular function and balance in children with hemiplegia: A randomized controlled study

Abstract
Purpose. To investigate the effect of Minds in Motion (MiM) program on vestibular function and dynamic balance in children with hemiplegia.
Methods. Thirty children with hemiplegic cerebral palsy (CP) of both genders with age ranged from 6-12 years were randomly assigned into two groups of equal number; Group (A) (control group), received specially designed physical therapy program for 3 months, Group (B) (study group), received MiM vestibular exercise program in addition to the specially designed physical therapy program given to the control group for 3 months. Vestibular functions and stability indices were assessed for all children before and after treatment using cervical vestibular evoked myogenic potentials (cVEMPs) device and biodex balance system (BBS) respectively.
Results. The results of the present study showed statistically significant improvement within both groups when comparing their pre and post-treatment mean values of stability indices and VEMP amplitude. Post-treatment significant improvement was also observed in the amplitude asymmetry ratio (AAR) in the study group compared with the pre-treatment mean values while no significant change was detected in the control group. Also, no significant changes were detected in P1, and N1 latencies in both groups after treatment. Statistically significant differences were observed in all measured variables between both groups in favor of the study group while there was no significant difference between both groups when comparing the post-treatment mean values of P1 and N1 latencies.
Conclusion. Minds in motion program is an effective modality that can be used for improving the vestibular functions and balance capabilities in children with hemiplegic CP.

Prevalence of dynamic instability among football players in the UAE: A cross-sectional study

Abstract
Although dynamic balance is one of the most important components of stability, it is not considered properly during athletic evaluations.
Aim. To investigate the prevalence of dynamic instability among a group of football players in the UAE. In addition, to investigate the possible association between dynamic instability and player position, weight, and body mass index (BMI). Materials and Methods. 142 Emirati football players from different emirates, between the ages of 16-24 years free from any upper or lower limb injuries at least 3 months prior to the study. The participants were assessed for dynamic balance using the overall stability index during single leg stance measured by the Biodex balance system Results. revealed a high prevalence of dynamic instability among football players in the UAE as 73.2% of the participants showed to be unstable. In addition, there was a strong negative correlation observed between field position and single leg stability index (P = 0.002), however, there was no correlation found between dynamic instability and weight (P = 0.106) or BMI (P = 0.212). Conclusions. Study results suggest the prevalence of dynamic instability among UAE football players with a strong negative correlation between dynamic instability and player position with no significant correlation with weight or BMI.

Abstract

Background. The maintenance of stability and the performance of any motor task is possible thanks to the control function of the nervous system, which collaborates closely with both active and passive components of the locomotor apparatus. The aim of this study was to find out whether stabilizing exercises executed in closed kinetic chains (influencing active elements) and rotational mobilizations (engaging passive structures) could improve body balance. Material and methods. Twenty young, healthy, physically active women were randomly divided into two equal groups. The first group (I) performed stabilizing exercises and the second (II) was subjected to rotational mobilizations. Every woman was examined twice (before and after the exercise procedure). Stability in double-feet quiet stance with and without visual control was examined using a dynamometric platform. The resulting data were analysed statistically. Results. More evident changes could be seen in the frontal plane. These were more noticeable in the active stabilization exercise group (especially with reference to stiffness, where the differences were statistically significant). Conclusions. Some changes of the viscoelastic system were seen in response to both passive and active exercise procedures. More distinct changes were seen following active exercises in a closed kinetic chain (Group I), suggesting that this kind of exercise has a greater effect on postural reactions.

Abstract
Background. The aim of the study was to analyse the relationship between lateral spinal curvatures and MLPX and MLPY among children aged 12 to 15 years. Material and methods. The sample consisted of 503 children chosen from among attending Promary School No. 13 and Junior Secondary School No. 4 in Starachowice. The spatial photogrammetric method, based on Moire’s topography, was used. Balance was examined on a stabilographic platform. The MLPX and the MLPY were used to describe balance findings. Results. Analysis of variance showed a significant effect of the testing options. In the test with eyest closed, MLPX increased in all three defect groups in the frontal plane. Closing eyes increases MLPX, which reflects an increased pressure of the left leg. Increased instability in this plane is usually effectively compensated for by the hip joint strategy. Conclusions. Although the analysis of variance did not show any essential effects, the value of MLPY with eyes closed was hightest in curvatures >10o. This group also showed the biggest difference in Romberg’s test.

Abstract
Cerebrovascular stroke has become the third most common cause of death in many developing countries and one of main causes of long term disability. The aim of the study was to evaluate an impact of the degree of paresis in the upper extremity and the side of paresis on balance and the lower extremity weight bearing in patients with ischemic stroke. The study group included 51 stroke patients, admitted for treatment to the Provincial Hospital No.2 in Rzeszow. The control group included 51 subjects in the same age group with no stroke history. We assessed functional efficiency of the hand according to the Brunnström scale, balance and symmetry of the lower extremity loading using stabilometric Zebris platform. Statistical analysis did not show any significant differences between the mean values obtained for the examined balance parameters in regard to the side of paresis. However, a statistically significant difference was found between the mean values obtained for the examined parameters of lower extremity weight bearing symmetry and the side of paresis. The degree of the upper extremity paresis correlates with balance parameters and lower extremity weight bearing symmetry in stroke patients with left-sided hemiparesis. The side of paresis affects the symmetry of lower extremity weight bearing.

Abstract
Background. A fault posture of head can disturb the body balance. Forward head posture (FHP) is one of common faulty postures seen among university students. Whole Body Vibration (WBV) is a quick method of neuromuscular training used to increase muscle strength, improve dynamic balance control and eventually correct posture.
Purpose. A randomized controlled trial was designed to investigate the effect of WBV training on craniovertebral angle and dynamic balance control in subjects with forward head posture.
Methods. Forty-five participants (11 males and 34 females, 18-23 years old) were randomly allocated into 3 equal groups: group (A) received traditional treatment (stretching and strengthening exercises) + postural advices, group (B) received whole body vibration training + postural advices, group (C) received traditional treatment + whole body vibration training + postural advices, 3 sessions /week for 4 weeks. Outcome measures included craniovertebral angle (CVA), overall stability index (OSI), anteroposterior stability index (APSI) and mediolateral stability index (MLSI) that were assessed at baseline and 4 weeks post-intervention.
Results.  Comparing all groups post training revealed that there were statistically significant increases (p < 0.05) in all measured variables (CVA, OSI, APSI and MLSI) in favour of group (C), while there were statistically non-significant differences between group A & B (p > 0.05).
Conclusion. The conjugation of WBV training with traditional treatment of FHP improved craniovertebral angle and dynamic balance control in subjects with forward head posture.

摘要
自闭症谱系障碍为一系列广泛症状,呈现出在沟通、社会发展和陈规定型行为模式等领域上的缺乏。自闭症的症状也出现在社交互动、情绪障碍、体感障碍及注意力问题等问题上,患自闭症的儿童经常被诊断出有感觉处理障碍。儿童发展中非常重要的能力是保持平衡,使其能在支撑平面上控制重心,适当的平衡为顺利完成多项日常活动所不可或缺的因素,平衡发展的延迟将阻碍日后的运动发展,并影响孩童与同龄人交流的机会。
研究目的在客观评估患自闭症谱系障碍儿童的同等技能。
材料及方法:共30名5-7岁患自闭症谱系障碍的儿童参与试验,其中包括22名男孩及8名女孩,采用富勒頓高級平衡計分錶規模 (FAB) 测试作为平衡评估。
结果:各年龄组间取得的测试结果不同,但这些差异并不显著,而在各年龄组的串联步态测试中则出现显著差异。
结果:患自闭症谱系障碍的儿童中出现同等技能失调。

Abstract

The treatment and selected diagnostic tests performed inpatient cardiac patients often require immobilization. The procedures used in the treatment of these patients may have an impact on the control of postural and balance after immobilization and involve disturbances at the level of activities of daily living, preventing or hindering the maintenance of upright posture.
The aim of the study was to evaluate body balance control in patients with acute coronary syndromes and after coronary angiography
Examined 70 patients, including 35 after acute coronary syndrome treated with interventional cardiology ward of the Regional Medical Center. The study was conducted using a modified scale Berg showing deterioration in the balance in the group of patients who underwent angiography.
