Abstract

Common behavioural and ethical patterns existing in modern medicine are subject to many social debates in which physiotherapist, alongside ethicists, philosophers and doctors, have their say. All of them express their points of view, which more often than not, coincide. The differences in opinions constitute the consequences of applying deontology in certain professional cases. Forming behavioural patterns in the process of academic studies is a hard and long-lasting task. Further development of ethical attitudes of a physiotherapist will be influenced by work. Even though physiotherapists embark upon their professional paths with already formed attitudes, these will be prone to further changes, both negative and positive, depending on the environment.

Abstract

The social role of a physiotherapist must not be reduced to displaying instrumental professional knowledge and propriety; it demands from a physiotherapist to employ humane and ethical streak in his or her practice. A physiotherapist, in collaboration with the whole medical team, creates proper conditions for full recovery. Physiotherapists develop professional identity and awareness by furthering their education, raising their professional competence, belonging to physiotherapist associations and building bonds between other representatives of this profession. What influences the level of professionalism of the group and all the same determine the quality of physiotherapeutic services and a proper approach towards the patient are mutual attempt to standardise education, create role models as well as professional culture and ethical values. The aim of this study is to present physiotherapy as a profession, and the physiotherapist both as a professional who has the highest level of professional command to treat the ill and needed, and as a member of the rehabilitation (therapeutic) team – a humanist who treats a patient as a summum bonum of medicine.

Abstract

Introduction. Dynamic taping is enjoying ever-growing popularity as an effective aid for treating injuries both in professional and non-professional dancers. The aim of the study was to evaluate the effectiveness of the application of dynamic taping in improving the selected motor characteristics in hypermobile dancers. Materials and methods. The study included a total of 46 female concert dancers between 18 and 25 years of age (M = 20.06, SD = 2.67), specialized in jazz dancing, who had been diagnosed with joint hypermobility syndrome using the Sachs scale. Within the examined group, dynamic taping was applied following the screening tests, and the applications were changed three times (every 7 days) during the therapy. The observed changes in motor characteristics were assessed using the muscle flexibility tests, the muscle strength measurements performed with a dynamometer and the hip joint mobility measurements carried out with a digital inclinometer. Results: The application of dynamic taping produced statistically significant results in the restoration of normal muscle flexibility. The analysis results confirmed the effectiveness of applying dynamic taping in increasing muscle strength as well as active mobility range of the hip joint in dancers (p < 0.05). Findings: The multidisciplinary physical therapy of dancers should include a variety of techniques and methods, with particular emphasis on the appropriate selection of dynamic taping applications. The increase in muscle tone with the simultaneous increase in the active mobility range of the hip joint and the reduction of subjective pain sensation confirm the beneficial impact of dynamic taping on the changes of motor characteristics related to the joint hypermobility syndrome.

Anxiety among therapists working with children with autism

Abstract

Introduction. The anxiety among the therapists working with the children diagnosed with autism is associated with the fear and concerns associated with planning and conducting the therapeutic process. The objective of the study is to analyse the situations in which therapists experience discomfort and anxiety during classes and exercises with the persons diagnosed with autism. The studies conducted included observations of the causes of anxiety behaviours, the degree of their intensity and elimination.
Materials and methods. The analysis covered 16 therapists working in the Polish Autism Society. The research was conducted from December 2014 to January 2015. The therapists were aged between 22 and 45. The research made use of a diagnostic survey conducted using questionnaires.
Results. The most frequent symptoms of anxiety among the therapists working with autistic children are nervousness and higher heart rate. The best way of handling anxiety is relaxation. The most frequent cause of anxiety among the therapists working with autistic children is physical and verbal aggression demonstrated by children and absence of effects of work.
Conclusions. An important think to remember when working with autistic children is that work problems should not be brought home. The point is to eliminate occupational burnout. An important element that eliminates anxiety is finding the time for relaxation.

Cryotherapy as Part of the Post-Stress Disorder Rehabilitation Complex

Abstract

Aim. The aim of our investigation is to estimate cryotherapy and complex rehabilitating procedures effectiveness for the patients who suffer of some post stress disorders. The aim of psychotherapeutic treatment for PSD suffering patients is to relieve such patients from some traumatic event intrusive memories and following emotional pains. Every patient should have got possibility to be involved in present day social and economic activities.
Material and methods.
Research approaches and materials: the clinical and experimental psychological investigations have been conducted with 114 patients, who suffered of various post stress disorders on the basis of psychoneurological department of the State Institution “Scientific Practical Medical Rehabilitating Diagnostic Centre of the Ukrainian Health Ministry” Konstantinivka city, Donetsk region, Ukraine. The main (experimental) group consisted of post stress disorders suffering patients. This group consisted of 74 patients, who, along with corresponding medical treatment according to the clinical medical treatment protocol, attended course of general extreme air cryotherapy (GEACT). The control group consisted 70 patients.
The equipment to create extreme low temperature (beneath -100°С), was cryochamber “Cryo Therapy Chamber” Zimmer Midizin Systeme (Germany). It was possible to treat the patients both individually and in groups (3-4 persons). All the procedures were conducted according to O.A. Panchenko’s methodology. The patients had been selected according to inclusion-exclusion criteria. All the patients were volunteers, they were properly informed about extremely low temperature influences and about such medical investigation conditions.
Conclusions.
Today, cryotherapy is widely spread in various fields of medical science. This methodology constant technical improvement and any side effects absence promote this methodology to be popular around the Globe. General extreme air cryotherapy is an ideal model of psychophysiological stress. As a result, patients’ psychophysiological and psychological conditions improvements have been observed.

Analysis of selected body posture parameters in female volleyball players

Abstract

Introduction. Habitual posture of professional athletes is closely connected with the demands of sport training, and its characteristics depend on the type of sport involved. The purpose of this study was to assess the posture and foot loads of female volleyball players from the KSZO Ostrowiec team playing in the Orlen Liga (the Polish Women’s Volleyball League) in comparison to women in a similar age group who do not play sports professionally.
Materials and methods. The study was conducted between 2014 and 2015, at the start of the volleyball season, and used the DIERS Formetric 4D analysis system. The results of the female volleyball players were compared with healthy women in the same age group who do not play sports professionally.
Results. The results obtained in this study indicate that female volleyball players have lower values of the lumbar lordosis angle in comparison to the control group. The values of the trunk inclination angle (relative to the vertical plane) were also significantly different between the two groups, with the analyzed group having higher values. With regard to the other analyzed variables i.e. kyphosis and foot loads, the study found no statistically significant differences between the groups.
Conclusion. The assessment of the habitual posture of female volleyball players conducted in the present study suggests that the demands of sport training can have an influence on how the body posture of professional athletes develops.

The use of local cryotherapy and PNF method in the procedure that improves the functioning of patients with subacromial impingement syndrome

Abstract

Considering the serious nature of complications pertaining to the trauma of the glenohumeral joint, the priority is to address the underlying cause as well as symptoms. This research was conducted in order to evaluate the effectiveness of localized cryotherapy and the PNF method on the mobility of the glenohumeral joint, in patients presenting with subacromial impingement syndrome. Each group (test and control) consisted of 18 patients. Patients in the test group were subjected to treatment of local cryotherapy and PNF therapy in the treatment of the shoulder girdle for fifteen days whilst the control group were treated with local cryotherapy procedures for the duration. There is little doubt that local cryotherapy is and effective treatment for various kinds of dysfunction however when combined with other treatment methods (such as PNF) a better outcome is achieved.

Abstract

Introduction. The back pain is frequently caused by spine ligament weakness, changes associated with intervertebral joints, overload, degenerative disc disease and degeneration in the elderly. One of the most crucial results of the local cryotherapy on the surgical area is an analgesic effect. Kinesiology Taping method is based on the alignment of muscle tone on the application area. This technique contributes to increase range of motion and muscle strength.
Material and methods. The study group included 90 patients with degenerative disease of the lumbar spine. They were from Rehabilitation Clinic in Szczecin. All subjects underwent the examination of mobility of the spine and were asked to rate their pain on the Visual Analogue Scale (VAS) on the first and fifteenth day of treatment. The I group was subjected to Kinesiology Taping method and local cryotherapy 15 times. We used fascia technique in accordance with the methodology. The patients from the II groups were also subjected to local cryotherapy. The III group consisted of the patients who were waiting for the treatment. They underwent the examination of mobility of the spine at the same time as the other groups.
Results. We observed significant pain reduction and significant improvement in motion of lumbar spine in the I group, which was subjected to Kinesiology Taping method in conjunction with local cryotherapy. In the group which was subjected to local cryotherapy, a slight improvement in motion of lumbar spine and in reduction in pain could be observed.
In the III group, which consisted of the patients awaiting the treatment, we observed no significant improvement in flexion or extension of lumbar spine.
Conclusions. Kinesiology Taping method in conjunction with local cryotherapy results in the improvement of lumbar spine mobility. The use of the Kinesiology Taping method together with local cryotherapy has a significant influence on decreasing the intensity of pain.  The application of local cryotherapy alone has significantly lower therapeutic effects than its application together with Kinesiology Taping method.

Compensation Mechanism in Treatment of Idiopathic Scoliosis with the FED Method – Preliminary Results

Abstract

Introduction. The idiopathic background of scoliosis makes it difficult to treat the disorder in a cause and effect way. This gives a starting point for creation of many physiotherapy treatment methods, among them the FED method. It is a new, but a very promising way of the conservative treatment for scoliosis.
Research Goal. The assessment of impact of the FED treatment for reducing the angle of the primary curve (KTR P) and the secondary curve (KTR W) rotation, and the overall rotation of the spine (SDR).
Materials and Methods. In the research participated girls 11 to 15 years old (M = 13.5 ±1.4), with the diagnosed double curve scoliosis of the second degree. In all the patients, there were carried out measurements of the angle of trunk rotation in the primary curve, secondary curve, cervical segment and the scoliosis coefficient was measured with a Zebris apparatus. There was also analyzed the sum of two rotations (SDR) parameter. The above examinations were performed before and after the three week treatment with the FED method.
Results. After the application of the FED treatment method, we have observed statistically significant decrease in the average value of the trunk rotation angle at the levels of primary curve, secondary curve and cervical segment. Similar observations were made in the case of the scoliosis coefficient in Zebris examination. All the examined parameters have shown a significant decrease in the average output values (p < 0.001).
Conclusions. Treatment of double curve scoliosis with the application of the FED method, for the period of 3 weeks, leads to reduction of the rotation angle of the trunk. This reduction applies to both, the primary and secondary curve, the overall rotation angle and the cervical segment. The FED therapy improves the total rotation of the whole spine.In the course of treatment, there occurs decrease in the angle of vertebrae rotation in the cervicothoracic segment.

Muscle energy techniques after total hip replacement

Abstract

Introduction.  Surgical procedure involving total hip alloplasty is a common consequence of the absence of progress of rehabilitation in patients with significant degenerative lesions accompanied by severe pain and significantly limited mobility. An individual program for patient rehabilitation is established during the first 24 hours after the procedure. This is of particular importance as proper planning and implementation of physiotherapy allows to relieve pain and to achieve full functional joint mobility as well as to strengthen the muscles. We make sure that the proper muscle control and stability within the joint will restore patient’s locomotion and self-care both at the initial and later stages of rehabilitation. There are many ways and methods to improve the above mentioned parameters. Muscle energy techniques, which represent a non-invasive therapy in patients with total hip replacement, are one of these methods. These techniques, including poizometric relaxation of contracted muscles as well as active relaxation of hip joint muscles, myofascial techniques, also in combination with elements of PNF techniques, allow for effective treatment and rehabilitation of patients. Aim. The aim of the study was to compare the effects of standard physiotherapy in patients after cemented hip arthroplasty with those of physiotherapy using muscle energy techniques.
Materials and Methods. A total of 66 patients with advanced degenerative arthritis of the hip, who received surgical treatment in the form of endoprosthetics, were qualified for the study between 2013 and 2014. The patients were divided into two groups  (group I and II), 33 subjects each. Pain level, the range of hip motion as well as muscle strength were assessed in each patient. Group I received muscle energy techniques, including poizometric relaxation of muscles, active muscle relaxation and elements of PNF method. Also, myofascial techniques targeting muscles with different strengths and direction of pressure were used. Group II received standard physiotherapy.
Results. Significant improvement of pain, an increase in the range of functional motion as well as a significant improvement of muscle control of the joint were observed in the group receiving muscle energy treatment, which had beneficial effects on the locomotion of patients.
Conclusions. Muscle energy techniques had significant effects on the improvement of muscle function and stabilization in the pelvic girdle of the joint with endoprosthesis. Poizometric relaxation combined with elements of the PNF method as well as myofascial techniques effectively improved balance, coordination and gait pattern. In conclusion, physiotherapy using muscle energy techniques was shown to be a more effective method in the treatment of hip osteoarthritis as compared to the method based on standard rehabilitation program.
