Multidimensional effects of dual-task-focused training added to structured neurological rehabilitation in individuals with chronic stroke: a randomized controlled trial

Abstract
Purpose. This randomized controlled trial aimed to investigate the multidimensional effects of dual-task-focused exercises added to structured neurological rehabilitation on balance, mobility, anxiety, cognitive function, and quality of life in individuals with chronic stroke.
Materials and Methods. In this evaluator-blinded randomized controlled trial, forty individuals with chronic stroke were assigned to two groups. Both groups received six weeks of structured neurological rehabilitation, while the experimental group additionally completed dual-task training. Balance, walking speed, mobility, anxiety, verbal fluency, cognitive function, and quality of life were assessed before and after treatment.
Results. Baseline demographic and clinical characteristics were similar between groups. Compared with controls, the dual-task group showed significantly greater improvements in mobility, anxiety, and quality of life, with particularly notable gains in anxiety and quality of life. Both groups improved in balance and cognitive function, with some measures favoring the control group. No significant between-group difference was found in walking speed.
Conclusions. Adding dual-task-focused exercises to structured neurological rehabilitation provides significant benefits in mobility, anxiety, and quality of life in individuals with chronic stroke. Variability in balance and cognitive outcomes indicates the need for further research to determine optimal protocols and long-term effects.

Abstract:
Background. Stroke is the most common clinical manifestation of diseases which occur motor, sensory, perceptual, or cognitive deficiency and have serious impact on independence, functioning and quality of life of the stroke survivors.
Objective. To investigate the effect of core stability exercise on symmetrical weight bearing in chronic stroke patients.
Methods. This study involved the recruitment of forty chronic stroke patients, comprising individuals of both genders aged between 50 and 65 years. Data regarding demographics were meticulously collected, and the diagnosis was confirmed by a neurologist. The chosen patients were split into a pair of equal groups at random, the control group (B) and the study group (A). The subjects were assessed by weight scale by standing on one weight scale with both legs to measure the accurate body weight of the patient then put one leg on each weight scale to capture the weight of the affected and non-affected legs. Prior to treatment, the Berg Balance scale was used to evaluate each patient. Further measurements were conducted following the course of treatment. The treatment was done for six weeks, day after day. The study group (A) received core stability exercises and selected program for physical therapy (gentle strengthening exercises for upper limb & lower limb muscles, mobilization for scapula & pelvis, stretching exercises, range of motion exercises and balance exercises). The control group (B) received only the same selected physical therapy program.
Results. The comparison among pre-treatment as well as post-treatment within group A revealed a substantial difference for the affected weight limb and non-affected weight limb, respectively, (P = 0.0001* and 0.0001*), and group B showed a significant difference for the affected weight limb and non-affected weight limb, respectively, (P = 0.0001* and 0.0001*). Results from comparing the two groups post treatment revealed no substantial differences (P = 0.622 and 0.183) for the affected weight limb and non-affected weight limb, respectively.
Conclusion. Core stability exercises significantly affected symmetrical weight bearing in chronic stroke patients. However, there was no substantial difference among the study group (core stability exercises) and the control group (selected physiotherapy exercises) on symmetrical weight bearing among chronic stroke patients.

Abstract:
Background. Stroke is one of the leading causes of disability that can result in spasticity, gait disturbances, and balance disorders. Land-based rehabilitation methods have been explored, and concerns about joint and muscle stress have led researchers to investigate alternative approaches, including water-based exercise.
Aim. To summarize the available data related to the clinical picture of whether there is a difference in the effect of water-based exercises with or without external buoyancy devices in influencing balance improvement in stroke.
Material and methods. The method used in this study is Scoping Review, which aims to find answers to questions from researchers related to the literature on the research topic. The collected articles will be synthesized using the TIDIER checklist, and articles will be searched through various databases.
Results. After conducting the article selection process, 20 suitable articles were found. The details of articles related to population, interventions, and outcome measures are listed.
Conclusion. Water-based exercises are efficient for stroke patients. These exercise methods showed effectiveness in improving balance in stroke patients. Many practical water-based exercise methods must be analyzed, especially for stroke patients, in future research.

Abstract
Background and Purpose. The vestibular rehabilitation is an exercise-based method, aiming to maximize central nervous system(CNS) compensation at vestibular nuclear and other CNS levels for vestibular pathology. A minimal number of studies have documented the impact of Vestibular rehabilitation on the recovery rate of patients with Bilateral Vestibular Hypofunction(BVH). The purpose of this study was to investigate the effectiveness of structured vestibular rehabilitation (VR) programs on severity of dizziness, kinesiophobia, balance, quality of sleep, activities of daily living(ADL) and quality of life(QoL) in subjects with chronic BVH.
Case Description. Three participants diagnosed with BVH who suffered from severity of dizziness, kinesiophobia, balance, quality of sleep, ADL and QoL were included in the study.
Intervention. A structured VR program was applied in 50-minute sessions once a week and as a home exercise program 3 days a week over 8 weeks.
Outcomes. Participants were evaluated for severity of dizziness with the Visual Analog Scale (VAS), for kinesiophobia with the Tampa Scale of Kinesiophobia (TSK), for balance with the Semitandem, tandem, and standing tests, for quality of sleep with the Pittsburgh Sleep Quality Index (PSQI), for ADL with the Vestibular Disorders Activities of Daily Life (VADL) and for QoL with Dizziness Handicap Inventory (DHI) at the baseline (T0), at 4th week (T1), 8th week (T2), and 20th week (T3) after study started.
Conclusion. In conclusion, a twelve-week structured VR program may enhance severity of dizziness, kinesiophobia, balance, quality of sleep, ADL and QoL in participants with chronic BVH.

Abstract
Purpose. Determination of Electronic Balance Board effect on Balance in Women in their postmenopausal period was the aim of this study. Subjects and Methods. Our study design was a randomized, single blind controlled trial. A total of 40 sedentary, medically stable women in their postmenopausal stage complained of poor life quality. The range of the participated women ranged between 50 to 65 years that were randomly allocated into 2 equal number groups: group (A), that participate in the electronic balance board training program in addition to balance rehabilitation program and aerobic exercise, and group (B), which received balance rehabilitation program and aerobic exercise. Each group received 3 sessions weekly for one month. The stability index, AP index and ML index was recorded before and after the intervention. Results. There were a significant reduction in overall stability index, AP index and ML index in group A in compared to group B at the stage of post treatment. Conclusion. A program of four weeks’ electronic balance board besides balance rehabilitation program and aerobic exercise yield improvement in the balance of the Biodex Balance System than a balance rehabilitation program alone in treating problems of postmenopausal balance.

Evaluation of balance in 5-7-year-old children with autism spectrum disorder

Abstract
Autism spectrum disorder (ASD) represents an extensive class of symptoms manifested as deficiencies in such fields as communication, social development and stereotypical behavioural patterns. Autism symptoms appear in social interactions issues, mood disorders, somatosensory disorders, concentration problems. The conversion disorders are frequently diagnosed in autistic children. Maintaining the standing balance is an immensely important skill in the child’s development, which consists in controlling the centre of gravity over the support plane. Appropriate balance is necessary for successful performance of numerous daily actions. Retarded development of balance impedes the future motor development and affects the opportunity of the children’s involvement in interactions with peers.
The purpose of this paper was an impartial evaluation of balance skills in children with autism spectrum disorder.
Material and methods. 30 children 5-7 year-old with ASD, including 22 boys and 8 girls. Methods: The Scoring Form for Fullerton Advanced Balance (FAB) Scale test was used for evaluation of the balance.
Results. The lowest results were obtained in the tandem walk and stand on one leg test. The test results varied in specific age groups, however, the differences were not significant. Substantial differences were displayed in tandem gait test for specific age groups.
Conclusions. In children with ASD disorders in balance skills occur.
