Abstract
Introduction. A common cause of heel pain is the presence of a calcaneal spur. This condition may disrupt the posterior superficial fascia chain and can also lead to problems in distant areas of the body. Among the physical methods used, shockwave therapy is often applied. The aim of the study was to assess the effectiveness of shockwave therapy and its effect on the tension of the gastrocnemius muscle, which is part of the posterior fascia chain.
Materials and Methods. A 52-year-old female patient diagnosed with a left lower calcaneal spur underwent a series of six radial shockwave therapy sessions, performed twice a week. The method for assessing the therapy’s effectiveness involved measuring pain on the Visual Analogue Scale (VAS) and changes in tissue temperature before, immediately after, and five minutes post-therapy. Electromyographic (EMG) examination was used to assess the tension of the gastrocnemius muscle.
Results. An average decrease in temperature was observed immediately after the treatment (from 25.3°C to 24.45°C). After five minutes, the temperature increased to 27.05°C compared to the initial measurement. The resting tension of the gastrocnemius muscle, as measured by EMG, decreased as follows: channel 1: from 0.6 µV to 0.4 µV, channel 2: from 4.3 µV to 3.2 µV. Pain decreased from a level of 7 to 2 on the VAS scale.
Conclusions. Shockwave therapy is an effective solution for alleviating pain in the case of a calcaneal spur. The reduction of resting tension in the gastrocnemius muscle leads to a decrease in elevated tension within the posterior myofascial chain, which may help reduce the risk of secondary pathologies in distant areas along the chain.

DOI: https://doi.org/10.56984/8ZG5608E5F

Abstract
Background. Tibial eminence fracture is an injury referred to the interarticular portion of the neighbouring plateau of the tibia. Non-operative treatment is recommended for type I and II. The study aimed to investigate the effect of virtual reality (VR) training combined with a physical therapy program (PTP) in the management of children after tibial eminence fractures. Methods. Forty eight children suffering from tibial eminence fracture (type I and type II). Their age ranged from 7 to 14 years old. The control group (G1) received only a physiotherapy program (PTP) and the experimental group (G2) received the same PTP plus virtual reality (VR) in form of Wii Sport Training. Both groups received the treatment protocol for six successive months (3 sessions per week). Participants were assessed before and after the treatment for six successive months. The outcomes were range of motion (ROM) of knee joint using goniometry, knee pain using the visual analogue scale (VAS) and balance by Pediatric Balance Scale (PBS). Results. Post-treatment, there was a a statistically substantial difference in pain, PBS, knee flexion and extension ROM in both groups (P-value < 0.05). Also, there was a statistically substantial difference in PBS and knee flexion ROM between the two groups (P-value < 0.05), favouring the VR group. Conclusions. These findings from this study reveal the value of physical therapy combined with VR in the rehabilitation of children with tibial eminence fractures. Virtual reality used in conjunction with PTP enhances those children’s knee range of motion and body stability.

Sara Atef Ali, Cecilia Ferreira Aquino, Achraf Ammar, Ashokan Arumugam – Post-operative rehabilitation and return to sport criteria following anterior cruciate ligament reconstruction – a survey of physiotherapists in the United Arab Emirates – Fizjoterapia Polska 2024; 24(2); 344-355

DOI: https://doi.org/10.56984/8ZG5608IV0

Abstract
Background. The rehabilitation and return to sport (RTS) criteria used after anterior cruciate ligament (ACL) reconstruction seem to vary between physiotherapists in different countries. However, no study has been conducted to investigate the post-operative rehabilitation and RTS criteria following ACL reconstruction used by physiotherapists in the United Arab Emirates (UAE).
Aim. The purpose of this study is to summarize the clinical practice and RTS criteria used by physiotherapists treating patients following ACL reconstruction in the UAE, and to investigate the factors associated with the commonly used RTS criteria.
Material and methods. An electronic survey questionnaire was distributed to UAE physiotherapists. The questionnaire included questions related to demographic information, clinical practice, and RTS criteria employed by UAE physiotherapists.
Results. A total of 108 physiotherapists voluntarily participated in the study. Approximately only 25% adhered to the established RTS criteria before RTS. About 88% of physiotherapists base their clearance decisions on how well they do in functional assessments like balance checks, hop tests, and running and sprinting biomechanics. A multidisciplinary team decided on the RTS clearance, according to 47% of physiotherapists. The most often utilized RTS criteria did not significantly correlate with parameters including academic standing, years of experience, nationality, specialty certification, or the type of license held by UAE physiotherapists, according to the study.
Conclusion. Most physiotherapists in the UAE were not adhering to the most recommended RTS criteria post-ACL reconstruction in their clinical practice, and none of the evaluated physiotherapist-related factors showed a significant association with RTS criteria following ACL reconstruction.

Moza Obaid Saeed Alyahyaee, Jinwoong Cheong, Meeyoung Kim – The feasibility and effect of robot-assisted gait training frequency on gait functions in children with cerebral palsy – A single blinded, randomized pilot study –  Fizjoterapia Polska 2024; 24(1); 297-303

DOI: https://doi.org/10.56984/8ZG2EF8iRS

Abstract
Introduction. The aim of this study is to investigate the feasibility and the effect of Robot-assisted gait training (RAGT) frequency on gait functions in children with diplegic cerebral palsy (CP). We hypothesized that RAGT with the increased frequency (4 times a week) will result in greater improvements than RAGT with the common frequency (2 times a week).
Material and methods. Fourteen participants with diplegic CP were assigned to two groups that received only RAGT at different frequencies. The treatment group (TG) received 24 sessions while the control group (CG) received only 12 sessions over 6 weeks. Gross motor function measure (GMFM) D, GMFM E, walking distance (6 minute-walk test), speed (10 meter-walk test), balance (Pediatric balance scale), and the quality of gait (Edinburgh visual gait score) were assessed. Data was collected twice, before and after RAGT intervention period. Also, the feasibility was assessed by the safety and the rates of recruitment, compliance, and adherence.
Result. There was significant improvement in GMFM D, GMFM E, walking distance, balance, and the quality of gait in both groups (p < 0.05). Walking speed has been significantly improved only in TG. The difference in the quality of gait is greater in TG (p < 0.01) compared to CG (p < 0.05). The recruitment rate was 70%. 100% of recruited participants complied with and adhered to the intervention. No adverse events were reported.
Conclusion. RAGT with the increased frequency could induce greater improvement in gait functions than RAGT with the common frequency in children with diplegic CP.

Maria Dąbrowska, Maciej Biernacki – Sensory integration of the vestibular system as a component of complex rehabilitation of a child with hearing loss. Fizjoterapia Polska 2023; 23(4); 140-143

DOI: https://doi.org/10.56984/8ZG20AA4A

Abstract
Introduction. The vestibular system is one of the most important human sensory systems, morphologically connected with the auditory system. The fetus starts responding to sound at around 16 weeks of development and its inner ear begins developing at 4 weeks. Proper receiving and processing of sound dictates development and comprehension of speech. The organ of hearing can be damaged during fetal development, birth or in the postnatal period. The damage to the organ of hearing affects the sensory systems’ integration. Training of sensory integration can be successfully introduced as a part of integrated rehabilitation of a child with hearing loss.
Purpose. The work describes Sensory Integration used in speech and hearing therapy. Examples of hearing exercises have been included. The therapy was conducted in years 2019-2021 in Polish Association of the Deaf Toruń and Bydgoszcz facilities.
Rehabilitation of the organ of hearing. The organ of hearing can be damaged during fetal development, birth or in the postnatal period. The rehabilitation of the organ of hearing begins as soon as the damage is recognized. The child, after being supplied with hearing aids or an cochlear implant, sometimes before that, is referred for speech and hearing therapy. In the process of widely understood rehabilitation, if complex action is required, included are physiotherapy and Sensory Integration- which was done at Polish Association of the Deaf Toruń and Bydgoszcz facilities.
Sensory Integration in children with hearing loss. In children with hearing loss parallel to speech, pedagogical and psychological therapies conducted are other such as Sensory Integration, physical therapy or other forms of rehabilitation individually chosen depending on the needs of the child.
Conclusion. Hearing exercises in accordance with the concept of Sensory Integration applied parallel to other forms of therapy can be used as a part of complex rehabilitation of a child with hearing loss.
