Assessment of mobility of the mandible and cervical spine in the stomatognathic system disorders

Abstract
Introduction. The biomechanical connection of the stomatognathic system (SS) with anatomical structures within the head, neck, shoulder girdle, and even with the whole musculoskeletal system signals the need for an interdisciplinary approach to the treatment of dysfunctions of this system.
Objective. Assessment of mobility of the temporomandibular joints and cervical spine in patients with masseter muscle pain. Determination of the relationship between the mobility of the temporomandibular joints and cervical spine. Assessment of bioelectrical activity (sEMG) of masseter muscles in patients with myogenic disorders of the stomatognathic system.
Material and methods. Fifty women (G I) aged 20 to 37 years with diagnosed myogenic disorders of the stomatognathic system and pain were studied. The following examinations were carried out: interview (questionnaire) and physical examination (examination of the stomatognathic system according to Bouman, sEMG of masseter muscles, evaluation of cervical spine mobility, numerical pain scale). The control group (G II) consisted of 50 healthy women.
Results. The level of pain intensity in the masseter muscles was close to the intensity of pain within the sternocleidomastoid muscles. In patients suffering from pain of the stomatognathic system, there was a limited range of mobility of the cervical spine. The largest difference occurred during the rotation of the head to the right and to the left. Between G I and G II, there were statistically significant differences in the values of the masseter muscle sEMG amplitude.
Conclusions. There is a relationship between the limitation of movements of the mandible (opening, lateral movements) and a decrease in the range of mobility of the cervical spine. Myogenic disorders of the stomatognathic system may lead to disturbances in the mobility of the neck.

Evaluation of the importance of previous physiotherapeutic preparation in aspect of using TENS during childbirth among primiparous

Abstract

Introduction. For the purpose of the study has set the definition of what is the efficacy of transcutaneous electrical stimulation of nerves during labor using a portable apparatus without consultation over the use of this therapy by prior training and participation of a physiotherapist.
Materials and methods. The study included 30 primiparous. In the 18 women who start treatment preceded by a description of how the action electrostimulator by a physiotherapist, and 12 patients who have purchased their own devices for electrostimulation.
Results. In 94.43% of the primiparous of the training covered by a physiotherapist, and 50% of patients without consultation, TENS to a lesser or greater extent eliminates the pain of childbirth. In the group of patients after the training has been only one case of pharmacological analgesic administration. However, in the group without consulting the percentage was 25%.
Conclusions. TENS pain therapy is more effective if it is preceded by appropriate training or physiotherapy consultation regarding the use of the TENS machine. Patient after physiotherapy training showed a greater knowledge of the mode of action and use the apparatus to TENS therapy than patients who missed this stage.

Evaluation of chosen electrotherapy methods efficiency in lower spine diseases in aspect of lowering usage of painkillers

Abstract

Background. While aging, degenerative phase of spine and circular joints deepen, also traumatic diseases in movement system – it is all inseparably combined with pain. Alternatively to painkillers we can use methods such as electrotherapy. The purpose of the researches was to verify the efficiency of chosen methods in electrotherapy in field of fighting with pain in lower part of spine as well as to lower the amount of pharmacological painkillers that are taken.
Material and methods. The group that was researched was 74 patients (46 women and 28 men) suffering from lower part of spine disease. Each of these people had of three procedures of electrotherapy: TENS, Nemec currents, Bernard currents. There was an individual interview (questionnaire) contained basic information about the patient, diagnosis, information about painkillers taken and evaluation of pain ailment that was conducted 4 times. Individually for each patient there were chosen parameters allowing to evaluate the efficiency of the therapy in time period.
Results. The biggest group that stated long-term positive influence in overcoming pain (60.9%) was noticed in TENS group. The biggest group that stated positive temporary and immediate effect in overcoming pain (56%) was noticed in IF group, similarly like the most frequent positive subjective evaluation of the therapy (84%). Lowering the amount of medicines (painkillers) was noticed on similar level (50-60%) for therapy with interference current and TENS.
Conclusions. The most effective long-term method in therapy of treatment pain in the lower part of spine is TENS. The biggest effectiveness in immediate action and short-term in overcoming pain in spine lumber vertebra shows therapy with interference stream – that can be a reason to use it directly before kinesitherapy. Electrotherapy can and should be used as an alternative way for pharmacological painkillers.

Abstract

Introduction. In last years there has been considerable development of physiotherapy methods used in dentistry. Among them is the manual therapy as a_form of treatment of functional disorders of the stomatognathic system muscular origin. This method is still very popular among dentists. This is because a_small number of literature, and a_small amount of physiotherapists involved in manual therapy in order to eliminate the symptom of pain.
Aim of the study. determined the effect of relaxing and analgesic masseter muscles under the influence of the use of manual therapy.
Material and methods. study was performed in 20 females, aged from 40 to 45  pain diagnosed as a_disorder of the stomatognathic system. In patients conducted physical examination and clinical examination, electromyographic study of masseter muscles and pain assessment using the VAS scale. Then, the test group received manual therapy treatments.
Results. After completion of therapy and analysis of the results showed a_decrease in the electrical potentials studied muscle and decrease of the VAS scale

Application of the physiotherapy Kinesiology Taping method in treatment of pain of the cervical spine in young dentists

Abstract

Introduction. Dentists, due to the conditions of their work, are exposed to many loads both physical and psychological. Considering the ergonomics of work, health status of this professional group should be considered primarily in terms of diseases of the spine, especially the cervical. This applies to all items in the work, both independently, as well as in team techniques, where apart from monotypy and monotony, there is always forced posture, and large static load, that contributing to a lot of degenerative arthritis changes of the cervical spine disorders, which are the result of the above-mentioned degenerations. Material and method. The study were conducted in the Department of Propedeutics and Dental Physicodiagnostics of the Pomeranian Medical University. The study group consisted of 36 fifth-year students of dentistry at the Faculty of Medical-Dentistry, Pomeranian Medical University in Szczecin with chronic pain of the cervical spine. Group of examined persons was subjected to the treatment with Kinesiology Taping method,  in which were used muscular and ligaments technique , accordance with the methodology. In the study has been used diagnostic survey method – a questionnaire. To assess the degree of feeling pain by the subjects were used a visual analogue scale VAS in the horizontal format in the range of 0-10.
Results. Considering the range of motion of the cervical spine flexion, the Kinesiology Taping method has improved in this movement, and extension range in the cervical spine improved more than the range of flexion. Taking the scope of the movement of the bend into the side, as well as the turn of the cervical spine in the test group were significantly improved, both in motion toward the right as and left. Examined in terms of assessing the level of the declared values of pain on VAS observed a significant reduction of this disease. Conclusion. Kinesiology Taping application also favorably influenced the range of motion of the cervical spine, while improving comfort work. Kinesiology Taping method applied among young dentists had a positive effect on the declared value of pain on the VAS, which transfers into a reduction. Considering the sex of patients, in the group of women has significantly improved the range of extension, while in men the range of flexion.

Impact of the deep electromagnetic stimulation on the level of the pain sensation in patients with the rheumatoid arthritis

Abstract
Introduction. Treatment of the rheumatoid arthritis includes the simultaneous actions against the pain, the inflammation and to regulate the muscle tension. The search for new and effective methods of treatment is continuously under way. There appeared this new physical method of deep electromagnetic stimulation. The objective of this study is to evaluate the effectiveness of the treatment of pain with the deep electromagnetic stimulation method, in the rheumatoid arthritis disorder cases.
Materials and Methods. The research has been conducted on a group of 50 patients with the rheumatoid arthritis, who have undergone a series of 10 deep electromagnetic stimulation treatments, applied to the most painful joints. Evaluation of the effectiveness and efficiency of the therapy has been done on the basis of the VAS scale, the Laitinen scale, the assessment of duration of the morning stiffness, and the subjective assessment of the result of the treatment made by the patients.
Results. After the series of treatments, the statistically significant (p <0.0001) reduction of the pain intensity has been found, measured with the VAS and the Laitinen scale  (the respective median (IQR): 8 (7-8) before the treatment vs 4 (3-5) after the treatment in the VAS scale, and 10 (8-12) before the treatment vs 6 (4-8) after the treatment in the Laitinen scale. Also observed, has been the statistically significant (p <0.0001) reduction of the duration of the morning stiffness, respectively 108±27 minutes before the treatment vs 67±34 minutes after the treatment.
The factors significantly affecting the result of the treatment were: age, BMI and the severity of the disorder.
Conclusions. Deep electromagnetic stimulation is an effective therapy method in patients with the rheumatoid arthritis, it significantly reduces the intensity of pain, the amount of painkiller medications being taken, and the duration of the morning stiffness.
