Abstract

Background: Cardiorespiratory physiotherapy specialty is a developing practice in Nigeria and other West African countries and its specific patterns for its practice are yet to be established.This study therefore sets to explore and document the practice pattern of physiotherapy in acute cardiorespiratory care in Nigeria.
Material and Methods: A self designed well-structured and validated questionnaire was administered to 66 physiotherapists pooled from the six geo-political zones of Nigeria.
Results:The results revealed that only 3 (5.3%) of the respondents specialized in cardiopulmonary physiotherapy, 32 (56.1%) were involved in intensive care unit (ICU) practice/coverage, 22 (38.6%) and 24 (42.1%) were involved in pre-operative and post-operative cardiac surgery rehabilitation management respectively. 32 (56.1%) of the respondents reported being on-call duty in the ICU with only 11 (19.3%) of them formed part of patients’ holistic care and discharge decision making process of acute or ICU patients in their practice settings.
Conclusions: The findings from this study suggest that there is still a low level of utilization of physiotherapy in acute cardiorespiratory care practice in Nigeria, possibly resulting from availability of only few specialists. It is therefore concluded thatphysiotherapists need to develop specialist skills and expertise with regard to cardiorespiratory assessment and management of critically ill patients in this area.

Abstract

Introduction. Sedentary lifestyle connected with the civilizational development and the work in sitting position cause glenohumeral joints dysfunction, most often in the form of overload-pain ailments.
Purpose of this study. The purpose of this study was to compare the physiotherapeutic effects with the application of mobilization with motion according to Brian Mulligan and the pectoral girdle muscle tapping, and the used circumarticular injections.
Material and Methods. The research included 27 patients. The first group (16 persons) was treated with the circumarticular injections of medicines of the corticosteroids group. In the second group (11 persons) the manual therapy according to Brian Mulligan’s concept and the muscle tapping were used. The examinations were performed before the therapy and after three weeks. The used measurements included the shoulder pain and disability index SPADI, provocative tests HIN, HIB, POP, Job, “painful arc” (70°-120°), “belly–press”, goniometric measurement and the subjective pain feeling at the night time. In order to check if the used therapy type had any effect on the upper limb function improvement, the statistical analysis was made in the variance with a_mixed scheme, where the inter-group factor was the type of therapy, and the intra-group factor was the measurement moment: before and after completion of the therapy. Data were processed using Microsoft Excel and Statistica software.
Results. The applied therapy methods influenced improvement of the glenohumeral joint movement range in both groups. The group 2 had better results in reduction of periodic pain ailments and sleep disturbances.
Conclusions. Both used therapeutic programs improves functioning and reduce pain ailments in the investigated group, however the use of 2 therapeutic techniques in the group 2, according to Mulligan’s concept and the muscle tapping, brought significantly greater improvement in the clinical condition evaluation and in the patients’ subjective assessment.

Abstract

Aim. The aim of this study was to present the general principles of massage therapy during pregnancy, as well as indications and contraindications for performing the treatment.
Basic assumptions. Pregnancy, as a_period of the great change in woman’s body, results in a_number of different ailments and because of that, can be determined as a_therapeutic problem. The most common symptoms are pain and tension from musculo-skeletal system. The relaxing massage treatment, performed regularly during pregnancy is able to prevent or alleviate already present symptoms. Unfortunately, it is performed very rarely, due to many apprehensions about the safety of that treatment and because of the fact that not every massage parlour or beauty salon is ready to provide help for pregnant women. Prenatal massage, in order to be safe for pregnant woman, must be always performed in a_special way, so that women could feel at ease, relaxed and won’t have to worry about their health and of their unborn child. It is extremely important, for the massage specialist, to rule out any contraindications before the treatment, to respect the rules of performing it and care about the well-being of the pregnant.

Physiotherapy After the Supraspinatus Muscle Tendon Reconstruction – Case Study

Abstract

Introduction. Trauma or degenerative disorders associated with age are the most common cause of loss of functions within the structures of the rotator cuff. The arthroscopic treatment of the rotator cuff is a therapy applied more and more often, due to the potential benefits of very small incisions.
Research Goal. The aim of the study is to present the effects of physiotherapy after the arthroscopic treatment of damage to the supraspinatus tendon, within the period from 5th to 16th week after the surgery.
Materials and Methods. The patient (47 years old) diagnosed with the supraspinatus tendon rupture of 7.5mm, had undergone the arthroscopic suturing of the supraspinatus muscle, with the use of the right shoulder joint anchor. Within the study, there have been performed: pain assessment using the Laitinen Scale, VAS, evaluation of the joint mobility, Hawkins Test, Neer Test, Jobe Test, Drop Arm Test and the Constant Shoulder Score. The patient has been subjected to a rehabilitation treatment within the period from 6th to 16th week after the surgery.
Results. After 16 weeks of the rehabilitation treatment, there has been achieved the range improvement of adduction, abduction and rotation, and the decrease of pain sensation. Hawkins Test – negative; Neer Test – negative; Drop Arm Test – negative; in the Jobe Test, the patient was not able to abduct the arm against resistance to the degree of 90°, there was no pain sensation. In the assessment of the Constant Shoulder Score, the patient scored 94 out of 100 points available.
Conclusions. The physiotherapy, applied within the period from 6th to 16th week after the arthroscopic treatment of the rotator cuff damage, has caused the reduction of pain sensation, improved mobility range, muscle strength and better functioning of the treated shoulder joint.

The rehabilitation of patients after the cerebral stroke including socio-demographic factors

Abstract

Introduction. According to the World Health Organization, cerebral stroke constitutes the third largest cause of death just after heart disease and cancer. It is estimated that every year 4.6 million people die due to stroke, including 3.2 in developing countries and 1.2 in developed ones. It has been observed that the effectiveness of physiotherapy in patients after a stroke depends on biological factors but also on their socio-economic condition.
The dissertation goal. The goal of this dissertation is to analyze the role of socio-demographic factors in the process of physiotherapy for patients after a cerebral stroke.
Materials and methods. The research has been carried out at the neurological rehabilitation ward of the Wrocław Center for Rehabilitation and Sports Medicine. The research group consisted of 120 patients after cerebral stroke. The examination was conducted at two time intervals – on the first day of the patient’s treatment at the neurological rehabilitation ward, and after three weeks of rehabilitation. All the patients participated in the systematic physiotherapy, which is a standard procedure at the neurological rehabilitation ward, where they had a defined schedule of frequency and time of rehab. A full research cycle lasted for three weeks for every patient. The following methods were used in the research: Barthel Scale (Barthel Index, BI), Lawton Scale (Instrumental Activity of Daily Living (IADL), Rivermead Mobility Index (RMI), Mini-Mental State Examination (MMSE), Geriatric Depression Scale (GDS), Generalized Self – Efficacy Scale (GSES), Acceptance of Illness Scale (AIS), and Visual Analogue Scale (VAS).
Results. Factors regarding the biological nature of the patient’s condition such as age, time since the stroke, type of stroke, and number of coexisting diseases have a definite impact on the low effectiveness of the physiotherapy conducted at the neurological rehabilitation ward. Socio-demographic factors, marital status, professional activity, and kind of job, also have an impact on the low effectiveness of the conducted physiotherapy.
Conclusions. Comprehensive rehabilitation of neurological patients after the cerebral stroke, taking into account socio-demographic factors is essential to achieve high effectiveness of physiotherapy.

Comparative analysis physiotherapy in children with atypical autism

Abstract

Introduction. The autism is an all-embracing development disorder that affects all areas of the child functioning. It causes disturbances in the interpersonal communication. It leads to difficulties in submission to social rules due to the lack of their understanding.
Material and Methods. In this research the case of two children has been described, Zazanna and Aleksander, who took part in the physiotherapeutic sessions with the physiotherapist at the National Autistic Society in Kielce, once a week for 1 hour.  The research was held in the period from November 2015 to March 2016. The children were 4 years old. The PEP model with a scale 0-5 was used to evaluate the children.
Results. The children with atypical autism have disturbances in perception, communication and speech.  The least difficult are for them tasks related with gross motor and visual-motor coordination. The examined boy was more promising comparing to the girl.
Conclusions. The children with diagnosed atypical autism differ in terms of psychophysical development depending on their gender. According to the psychoeducational profile PEP, the physiotherapy reduces disorders resulting from the diagnosed autism spectrum. The gender has significant influence on the fine and gross motor development in children with ASD.

Abstract

Introduction. In physiotherapy, just like in the other medical professions, in addition to acquiring the actual knowledge and gaining the professional skills, the professional ethics is of major importance.
Aim of the Study. The aim of the study was to verify statements made by the physiotherapy students, regarding the humanities (academic disciplines), which should have an impact on formation of their ethical attitudes.
Materials and Methods. In the study participated a group of 141 persons who, at the time, were physiotherapy students. The study was carried out at the turn of 2015 and 2016. The research tool applied was a questionnaire. To interpret the results of the study, the chi-square test (χ2) was used.
Results. The results of the survey show, that there is an average dependency between the humanistic disciplines and their impact on the attitude. Psychology, as well as interpersonal communication, have gained a high level of recognition among the physiotherapy students. Hence, the cause which testifies to the lack of impact of the humanistic disciplines on the formation of attitudes of the physiotherapy students toward their patients, is the lack of interpersonal communication, interchangeable, and therefore no emphasis on the knowledge of psychology during the process of communication.

Cultural Good as the Basis of Culture and Ethics

Abstract

Introduction. The ethical culture of physiotherapists encompass their attitudes and ethical beliefs, at every stage of their education and the actual job performance. The professional culture may be created by introducing the adequate ethics program. Both culture and ethics complement each other, and they reinforce the new principles and attitudes of people. The culture plays an important role in the daily life of every professional group, providing a common language and a shared way of behavior.
Aim of the Study. The aim of the study is to find a method allowing to formulate the definition of cultural good based on the results of used questions.
Materials and Methods. In the study participated a group of 141 persons who, at the time, were physiotherapy students. The study was carried out at the turn of 2015 and 2016. The research tool applied is a questionnaire. The method used to evaluate the results is the chi-squared test (χ2).
Results. The results have shown, that the physiotherapy students considered the following as cultural goods: esteem for health, propriety, tact, kindness, and responsibility, however these values are poorly related to the satisfaction derived from the fact of studying the physiotherapy.
Conclusion. The cultural good serves people, in a positive sense, through existence of the ethical triad, which should lead to the effective, positive man-to-man interaction.

Evaluation of the manual therapy effect on hand functional efficiency in the patients with systemic sclerosis – case description

Abstract

The systemic sclerosis is chronic systemic disease of connective tissue that is characterized by damage of blood vessels, presence of autoantibodies and progressing skin and internal organs fibrination, leading to their failure. The loss of mobility in hand and wrist joints, which often affects patients with the systemic sclerosis, significantly worsens their life quality.
Non-pharmacological treatment of the systemic sclerosis includes: physiotherapy, education, new therapeutic methods. Individually selected therapy counteracts and diminishes dysfunctions in the joints area, soft tissues and skin, and may improve efficiency of internal organs.
Manual therapy perfectly complements comprehensive in this group of patients, by influencing the following receptors: of Golgie, Ruffini, Pacini and interstitial. The manual therapy techniques directed to the skin, fascia, muscles, joints, improve hand efficiency and decrease pain in patients with systemic sclerosis.
Application of manual techniques (skin rolling, deep massage of superficial fascia and muscles responsible for straightening the wrist joint, postisometric muscle relaxation, mobilization in ridge direction in rediocarpal joint, side-to-side mobilization in finger joints) performed in the study improved hand functional efficiency in all tested parameters (Cochin Hand Scale, Hand Mobility in Scleroderma, finger-to-palm-distance).

How do physiotherapists define health? – results of a questionnaire study

Abstract

Introduction. A physiotherapist should know methods used in health assessment, basic definitions and psychosocial mechanism driving health and its prevention. The aim of this study was to assess opinions of physiotherapist about health with aligned basic determinants.
Methods. This cross-sectional study involved 117 subjects, 91 (77.7%) females and 28 (22.3%) males, aged 38 ± 10 years. Polish version of a standardized Ewles and Simnett questionnaire was applied. The poll consisted of 23 questions about 4 components of health: physical (7 questions), mental (6 questions), social (5 questions) and health as a lack of disease (5 questions). Subject chose all of possible 23 answers their agreed with. Percentage of given answers was calculated. On this basis, a dominant domain was established.
Results. Subjects agreed with 64.5% of all opinions of a questionnaire. Most frequently they chose those related to social (80%), then physical (71.4%) and finally to mental (66.7%) components of health. Health as a lack of disease was declared less frequently (60%). After categorization of individual answers, we found that physiotherapists defined health as the bio-psycho-social condition (38.9% of persons). Age, education level, marital status, place of work and number of years in profession were not associated with given opinions.
Conclusions. More than half of investigated physiotherapists define health inappropriately. Gender influences subjects’ opinions about health. Questionnaire formula of the study is its limitation, so the results need to be verified in further researches.
