Abstract
Introduction. Thyroid cancer is the most common type of cancer in the endocrine system. Thyroidectomy has potential complications. Shoulder dysfunction is the most reported physical complication following neck dissection surgery post thyroidectomy. Various treatment is utilized to diminish or counteract shoulder distortion. The purpose of this study was conducted to determine the effect of Pilates Exercises on Shoulder Dysfunction after Thyroidectomy. Methods. A prospective, randomized, controlled study with the pretest-posttest design was performed; 50 patients who were 6 months to 1-year post unilateral neck dissection and aged 30-50 years (at the time of surgery). They were randomly assigned to 2 groups: Experimental Group (Group- A): (25 Patients, 14 males and 11 females) were treated with Pilate’s exercises in the clinic under the supervision of a physiotherapist, for 12 weeks, 3 sessions /week. Control Group (Group-B): (25 patients, 12 males and 13 females) were treated with conventional therapy. All treatment interventions were applied at the frequency of 3 sessions/week for 12 weeks. The participants were recruited from the National Cancer Institute in Cairo to be treated in the outpatient clinic, Faculty of Physical Therapy, Cairo University at the period from September 2018 and May 2019. Results. Outcome measures were taken with Shoulder Pain and disability index (SPADI) was developed to evaluate the shoulder pain and disability and Shoulder range of motion: Active ROM of shoulder flexion, abduction, and external and internal rotation; was measured by a digital goniometer, before the treatment, and 12 weeks after the intervention. The comparison among the 2 groups revealed that there was a significant decrease in pain, disability and total percentage of SPADI of group A compared with that of group B (p < 0.05); Also there was a significant increase in flexion, abduction, external and internal shoulder ROM of group A compared with that of group B (p < 0.05). Conclusions. It was concluded that Pilate’s exercises have greater efficacy than conventional therapy in the rehabilitation of patients with shoulder dysfunction post thyroidectomy.

Effect of Exercise on Maternal and Neonatal Outcomes in Obese Elderly Primigravida: A Randomized Clinical Trial

Abstract
Background. In obstetric practice, advanced maternal age such as in case of elderly primigravida is known to be associated with adverse maternal and fetal outcomes. Obesity is a commonly occurring risk factor with advancing maternal age. Exercise in pregnancy could prevent and limit adverse maternal and fetal morbidities. Further research was warranted to study the effect of exercise in this high risk group due to lack of research in this area. Objective. To investigate the effect of a specialized exercise program combined with diet conducted early in pregnancy on maternal and neonatal outcomes in obese elderly primigravida. Methods. Design: A randomized, clinical, controlled trial. Setting: The study was conducted at physical therapy Department of Bab El-Sharia University Hospital, Egypt, between May 2016 and May 2017. Participants: 80 obese elderly primigravida participants enrolled into experimental and control groups. Interventions: The experimental group participants were closely supervised to perform the exercise program starting from 14 weeks’ gestation till 37 weeks’ gestation with a moderate restricted diet and received advice specific to each trimester of pregnancy, while the control group participants were provided with specific instructions to perform the exercise program with the same diet and advice. Outcome measures: Primary outcome measure was the last BMI of each participant measured at 37 weeks of gestation while the secondary outcome measures were mode of delivery, neonatal weight and neonatal APGAR score. Results. Eighty obese elderly primigravida (control group n = 40; experimental group n = 40) were randomized. There were statistical significant differences between groups in last BMI, mode of delivery and neonates APGAR scores at 1st minute of life and 5th minute of life (p < 0.05). While, there was no statistical significant difference between groups in neonatal weight (p > 0.05). Conclusion: A specialized antenatal exercise program is very effective in decreasing maternal and neonatal complications in obese elderly primigravidae and their off springs.

Abstract
Background. Menopause causes reduction in bone strength. Wearing shoes with different heights in old women, affecting their performance while they’re working either in erect position or in 90˚ trunk flexion or complete trunk flexion. Purpose. This study was designed to investigate the effect of different heel height shoes on spinal configuration in osteoporotic post menopausal women, in different trunk positions. Methods. This study was carried out upon 40 post menopausal osteoporotic women who were selected from faculty of physical therapy wearing different shoes: flat, sneakers and 3cm heel height.Their mean age was, 49.05 ± 2.14 (years), mean body mass 79.02 ± 7.7 (kg), mean height 163.06 ± 6.17 (cm), and mean body mass index BMI 29.57 ± 1.9 (kg/m2). The spinal angle deviation was measured between 2 definite vertebrae while wearing different heel height and comparing it with the spine deviation using flat shoes. The changes in the spine position were recorded for (flat shoes, sneakers and 3cm heel height shoes) during erect position, flexion trunk 90 degrees and complete flexion trunk by the spinal mouse device. The spinal angle deviation was measured in 4 levels T1–T2, T7–T8, T12–L1, L5–S1 levels. Result. There was a significant effect of different trunk position on deviation of spinal angle. At the level of T1-T2, there was a significant difference in the mean value of spinal angle between the 3 positions using sneakers and 3 cm heel shoes. At the level of T7–T8,there was a significant difference in the mean values of spinal angle between the 3 positions using flat shoes, sneakers and 3 cm heel shoes. At level of T12–L1, there was a significant difference between the mean values of spinal angle while wearing 3 types of shoes, flat shoes, sneakers, 3 cm heel, from 90˚ trunk flexion and a significant difference in the mean value of spinal angle between the 3 positions using flat shoes, sneakers and 3 cm heel shoes. At the level of L5–S1, there was a significant difference between the mean values of spinal angle of the 3 types of shoe, at erect position and significant difference in the mean value of spinal angle between the 3 positions using flat shoes, sneakers and 3 cm heel shoes. Conclusion. At level of T1– T2 and T7–T8, as the trunk flexion increases the spinal angle deviation decrease. At T12– L1 level, when flexion 90˚ the flat shoes is the best, followed by the 3 cm heel height. At the level of L5–S1 at erect position the flat is the best,followed by the 3 cm heel height.

Abstract
Purpose. Comparison of the effect of either physiotherapy or injection of platelet-rich plasma (PRP) on management of lateral epicondylar tendinopathy (LET) chronicity. Methods. A number of 27 participant complaining of LET at least for 3 months were enrolled into our study and were randomly assigned into two separate groups; group A (PT group) which undergone physiotherapy and group B (PRP group) which undergone injection of PRP. Assessment of all patients was done through a blinded assistant pre and post six weeks of treatment. We used the visual analogue scale (VAS) to help in pain evaluation and also the hand-held dynamometer to help in measurement of the strength of wrist extensor muscles. Results. Significant improvement was found in pain and wrist extensors strength in the two groups when before and after treatment mean values were compared. Comparison of the mean values of post treatment in the two groups revealed that there was a significant pain reduction in as well as significant improvment in wrist extensors strength in favor to the PT group. Conclusion. Although improvement percent was better in patients of the PT group, both physiotherapy and PRP injection were effective in pain reduction and improvement of wrist extensors strength in patients suffering from chronic LET.

Abstract
Purpose. Determination of Electronic Balance Board effect on Balance in Women in their postmenopausal period was the aim of this study. Subjects and Methods. Our study design was a randomized, single blind controlled trial. A total of 40 sedentary, medically stable women in their postmenopausal stage complained of poor life quality. The range of the participated women ranged between 50 to 65 years that were randomly allocated into 2 equal number groups: group (A), that participate in the electronic balance board training program in addition to balance rehabilitation program and aerobic exercise, and group (B), which received balance rehabilitation program and aerobic exercise. Each group received 3 sessions weekly for one month. The stability index, AP index and ML index was recorded before and after the intervention. Results. There were a significant reduction in overall stability index, AP index and ML index in group A in compared to group B at the stage of post treatment. Conclusion. A program of four weeks’ electronic balance board besides balance rehabilitation program and aerobic exercise yield improvement in the balance of the Biodex Balance System than a balance rehabilitation program alone in treating problems of postmenopausal balance.

Abstract
Background. Incontinence in the urine is a common problem. Its prevalence increases as age increasing , especially among older women..Conservative management as its first line treatment is recommended. Physical therapies are the cornerstone of such conservative management, especially pelvic floor muscle exercise.
Objective. Prospective study to evaluate objectively the advantages of pelvic floor exercises for the treatment of urinary stress incontinence associated with breathing exercises.
Materials and Methods. Fifty female elderly patients were selected for this study at age 50-65 diagnosed with urinary stress incontinence (SUI), their body mass index (BMI) was < 30 kg / m2.They were allocated randomly into two groups equally (A and B). they recieved the same treatment which is pelvic floor training but in group (A) the breathing exercise were added to the program For 12 weeks, the treatment program was done twice a week. Evaluation of all subjects for both groups (A and B) was conducted pre and post treatment program during the evaluation of the perineometer change in vaginal squeeze pressure.The revised scale of urinary incontinence (RUIS) was used to assess the alter in symptoms of severe incontinence.
Results. Analysis of the data reported marked improvement in squeezing vaginal pressure and urinary incontinence scale of both groups when comparing their mean values before and after treatment. Significant improvement in the study group, however, was noted when comparing the mean values of the study group after treatment with the control group.
Conclusion. Pelvic floor exercises associated to breathing exercises is an Appropriate and secure way to alleviate postmenopausal stress urinary incontinence.

Abstract
Aim. Aging resulted in decrease in respiratory muscle strength which limits the exercise performance and quality of life. The purpose of this study was to evaluate the clinical and the functional outcomes of inspiratory muscle training in elderly.
Material and methods. 48 nonsmoker’s elderly from both genders aging 60 to 70 years were recruited. They were assigned randomly into two groups of equal number. The study group received training with threshold inspiratory muscle trainer (IMT) at 40% of the PImax measured during the initial evaluation which was readjusted weekly by evaluating the PImax while the control group received training with threshold IMT at the lowest pressure offered by the device, 3 times/week for 10 weeks. Diaphragmatic mobility was measured by ultrasonography, Maximal inspiratory pressure measurement and the functional capacity was measured by 6MWT before and after the end of the training program for both groups.
Results. The diaphragmatic mobility, Maximal inspiratory pressure measurement and the distance walked during 6MWT increased significantly within the study group at the end of the study (p < 0.05) with non-significant change in the control group (p > 0.05). Comparison between both groups at the end of the study revealed significant increase in diaphragmatic mobility and Maximal inspiratory pressure measurement in study group compared with control group (p <0.05) with non-significant difference between both groups regarding the distance walked during 6MWT (p > 0.05).
Conclusion. Inspiratory muscle trainer should be a part in elderly daily routine as it has a positive effect on the clinical outcome measure and there is a tendency to improve the functional outcome.
