Abstract

This article begins with a description of the pathophysiology and symptomatology of spasticity. The pathophysiology of spasticity is complex, depending on interactions between opening and closing, stimulating and inhibiting pathways and centers located both in the spine and above the spine. Spasticity results from dysfunction of the upper motor neuron, i.e. structures lying in the vicinity of the cerebral cortex, the subcortical regions, the brainstem, and the spinal cord, and the numerous connections between them. In the clinical picture of spasticity we can distinguish disturbances in the regulation of muscle tension and strength, difficulties in maintaining body posture, and the occurrence of spastic cramps and clonus. As a consequence of these symptoms the patient may experience pain and limb deformity. The article describes several different methods of evaluating spasticity (including a modified version of the 6-point Ashworth Scale). Based on an extensive review of the literature, the authors discuss various methods of managing spasticity, with particular attention to physicotherapy. Some of these techniques have a mechanism of action that is already well understood and constitute a valuable means to supplement the treatment of spasticity; others are only just being introduced to therapy or are still in the research phase. Among the techniques used in the treatment of spasticity the authors discuss kinesitherapy, thermotherapy, electrotherapy, and magnetotherapy.

The application of Zeel T as adjuvant therapy in the kinesitherapy of female patients with arthrosis

Abstract

Background. The preparation known as „Zeel T” shows chondroprotective action and inhibits the degenerative process in joints as effectively as hyaluronic acid. Due to its anti-inflammatory and analgesic action, dosages of non-steroid anti-inflammatory drugs (NSAID) can be reduced. Material and methods. Our research involved 65 women, between 25 and 55 years of age. All these women took part for one month in regular kinesitherapy sessions. This group received oral Zeel T in droplet form. In the comparative group of 60 persons, only NSAIDs were used in addition to kinesitherapy exercises. Other 60 women in comparative group were subjected to exercises together with nonsteroid medicines. Results. In both the experimental group and the control group there was a significant reduction in the Ritche index and an improvement in clinical status. An improvement in strength indices and increased range of movement were observed in the experimental group. Conclusions. Homeopathic preparations show good effects as adjuvant therapy with kinesitherapy in arthrosis.

The application of high-tone therapy in spinal pain syndromes

Abstract

Background. The tasks of rehabilitation for the elderly include improving the efficiency of the cardiovascular and respiratory systems, the musculo-skeletal system (increasing the range of joint movement and muscle strength), and the nervous system and kinesthetic analyzer (using exercises to improve equilibrium and coordination). In order to realize these assumptions, combined methods of medical rehabilitation should be used, including kinesitherapy and high-tone therapy, using alternating electromagnetic fields (medium-frequency current in the high-tone range, ca. 4 – 12 kHz. The purpose of the present study was to evaluate the effectiveness of kinesitherapy combined with high-tone therapy in patients with spinal pain syndrome. Material and methods. The experimental group consisted of 31 patients (group A) 50-85 years of age. The control group (B) included 31 patients in the same age range. Kinesitherapy was applied in Group A for 16 weeks, twice a week, along with high-tone therapy. Group B received only a program of kinesitherapy, identical to Group A. Results. After treatment was completed, we found increased range of movement in the lumbar and thoracic spine in Group A, measured by plurimeter. The comfort index and dynamic equilibrium were significantly higher. The control group achieved lower values. Conclusions. The favorable effects of high-tone therapy as a supplement to kinesitherapy, associated with stimulation of tissue regeneration processes and improved cell metabolism. The results we obtained justify further research on the effectiveness of a rehabilitation program based on combined procedures in persons with spinal pain syndrome, in order to improve their quality of life.

Evaluation of chosen electrotherapy methods efficiency in lower spine diseases in aspect of lowering usage of painkillers

Abstract

Background. While aging, degenerative phase of spine and circular joints deepen, also traumatic diseases in movement system – it is all inseparably combined with pain. Alternatively to painkillers we can use methods such as electrotherapy. The purpose of the researches was to verify the efficiency of chosen methods in electrotherapy in field of fighting with pain in lower part of spine as well as to lower the amount of pharmacological painkillers that are taken.
Material and methods. The group that was researched was 74 patients (46 women and 28 men) suffering from lower part of spine disease. Each of these people had of three procedures of electrotherapy: TENS, Nemec currents, Bernard currents. There was an individual interview (questionnaire) contained basic information about the patient, diagnosis, information about painkillers taken and evaluation of pain ailment that was conducted 4 times. Individually for each patient there were chosen parameters allowing to evaluate the efficiency of the therapy in time period.
Results. The biggest group that stated long-term positive influence in overcoming pain (60.9%) was noticed in TENS group. The biggest group that stated positive temporary and immediate effect in overcoming pain (56%) was noticed in IF group, similarly like the most frequent positive subjective evaluation of the therapy (84%). Lowering the amount of medicines (painkillers) was noticed on similar level (50-60%) for therapy with interference current and TENS.
Conclusions. The most effective long-term method in therapy of treatment pain in the lower part of spine is TENS. The biggest effectiveness in immediate action and short-term in overcoming pain in spine lumber vertebra shows therapy with interference stream – that can be a reason to use it directly before kinesitherapy. Electrotherapy can and should be used as an alternative way for pharmacological painkillers.
