Abstract
Introduction. Pain in the lumbar spine is a medical and social problem in highly developed countries. In Europe, about 25-43% of people suffer from them. Approximately 25-60% of patients suffer from chronic, progressive ailments, which lead to both incapacity for work and limitation of activities of daily living. The aim of the study was to assess the impact of classic massage on pain and functional state of people with lumbar discopathy.
Material and methods. The study included 61 people aged 45–50, attending therapy at the Independent Public Health Care Facility in Leżajsk due to pain in the lumbar spine. Patients were classified into 2 groups, depending on the applied rehabilitation program. The research tool was the NRS scale and questionnaires: ODI i RMDQ. The Chi-square test, Mann-Whitney U test, Wilcoxon test, Pearson linear correlation and Spearman rank correlation were used for analyses.
Results. After treatment statistically significant reduction of pain and improvement of functional efficiency were noted in both groups. A statistically significant positive relationship was found between the amount of improvement in functional capacity assessed by the ODI questionnaire and the age of patients (R = 0.43; p = 0.026).
Conclusions. Classical massage has an effect on relieving pain and improving functional performance in people with lumbar discopathy. There are relationships between functional performance and age in people with lumbar discopathy. The improvement in functional performance is smaller with age. Body build is not a factor affecting the severity of pain and functional performance of people with lumbar discopathy.

Przemysław Lisiński, Agnieszka Jachowska, Włodzimierz Samborski – Methods of physiotherapy in treatment of intervertebral disc’s pathology in lumbar part of spine. Fizjoterapia Polska 2006; 6(3); 222-227

Abstract
Background. Physiotherapy is very often the first step in a treatment of back pain. The aim of kinesis therapy is a probe of a damaged disc reposition. Beside of this kinesis therapy improve an elasticity of ligaments and makes stronger a paravertebral muscles. Physical therapy procedures plays significant role in a pain treatment in subjective categories of course. Material and methods. All participants of that study were selected by clinical examination and analyses of accessory investigations. Clinical examination was repeated after twenty days of treatment by different physiotherapeutic procedures. So forty patients were included in a treatment. Status of health was calculated by typical orthopedic procedure and total pain pattern questionnaire. The aim of study was to state if physiotherapy is useful method of pain treatment and to compare effects different groups of procedures. Results. All participants of that study state positive effects of a treatment which was used during twenty days. A majority of them described pain after a treatment as a small discomfort or did not feel pain at all. Beside of these findings they concluded that a frequency of pain appearance depressed in near all analyzed cases. Conclusion. Back pain caused by disc derangement positively responded on physiotherapeutic procedures. A selection of particular methods is not so important in effectiveness categories. The treatment is the most successful when kinesis therapy and physical therapy are joined.

Centralisation and peripheralisation of symptoms according to McKenzie Method among patients with chronic low back pain

Aleksandra Truszczyńska

Aleksandra Truszczyńska – Centralisation and peripheralisation of symptoms according to McKenzie Method among patients with chronic low back pain. Fizjoterapia Polska 2011; 11(4); 351-356

Abstract
Background. Diagnosis of lumbar sciatica is based on classical neurological examination and radiographic imaging. In literature, McKenzie described detailed functional tests concerning pain response during mechanical procedures. Centralisation of pain confirmed during examination informs the examiner that conservative treatment is indicated. Contrary to that peripheralisation of symptoms, it confirms that derangement is irreducible. The aim of the study was to show that the method of functional examination enables diagnosis of centralisation or peripheralisation of pain among chronic patients with sciatica. The additional aim was to determine the factors identified from objective evaluation which determine further treatment of these patients. Material and methods. 200 patients with low back pain and sciatica were precisely analysed. There were 84 men (42%) and 116 women (58%), age between 13-79 years (43.0±13,65). Peripheralisation of pain was found in 65 patients (group A), and centralisation – in 135 persons (67.5%). Statistical analysis showed that significant differences between groups included: lateral shift, SLR test, sensory and motor deficits.Conclusions. 1. Precise analysis showed that among chronic patients with disc disease clinical tests lead to centralisation or peripheralisation of pain. 2. Peripheralisation was statistically more often accompanied with lateral shift, neurological deficits and low results of straight leg raise (SLR) test.
