Abstract

Background. The aim of our research was to evaluate and compare the analgesic effectiveness of thermal therapy in conjunction with magnetotherapy and active exercises without weight load to improve the functional efficiency of the knee joint with degenerative changes. Material and methods. The level of pain, range of movement, and strength of muscle groups in the knee joint were tested before and after rehabilitation in 48 patients, divided randomly into two groups of 24 people. Results. In both groups there was a reduction in the average level of reported pain, and in the flexion deficit in the knee joint. No changes were noted in the increase of average muscle strength in the flexors and extensors of the knee. Conclusions. In respect to analgesic action and improvement of function in a knee with degnerative changes, a rehabilitation program that includes local cryotherapy proved to be more effective that a program using Sollux lamp irradiation.

Beata Olesiak, Agnieszka Przedborska – Analysis of selected factors affecting rest pain in knee joint osteoarthritis. Fizjoterapia Polska 2021; 21(2); 206-214

Abstract
Introduction. Knee pain is a clinical sign of osteoarthritis and an ongoing inflammatory process in the joint. Aim of the thesis. The purpose of this study was to evaluate the effect of BMI and degradative changes in patellofemoral joint cartilage on the level of knee pain at rest. Material and methods. The study group consisted of 150 patients with patellofemoral joint cartilage lesions. The classification of cartilage degradation according to Outerdridge was done by ultrasound. The level of pain at rest was assessed according to the VAS scale. The data was collected with usage of a self-made study protocol. Results. There was no statistically significant correlation between BMI and VAS pain complaints (rho = 0.099, p = 0.228). There was also no statistically significant correlation between degradative changes in the cartilage of the patellofemoral joint and the level of pain on the VAS scale (rho = 0.076, p = 0.353). Conclusions. BMI does not affect the severity of knee joint pain at rest. The extent of cartilage wear in the patellofemoral joint does not correlate with knee pain during physical inactivity.

Joanna Fidut-Wrońska, Robert Latosiewicz, Karolina Janikowska, Krzysztof Sokołowski – Assessment of the effectiveness of local cryotherapy in treatment of gonarthrosis. Fizjoterapia Polska 2012; 12(3); 241-250

Abstract
The purpose of the study was to compare the effect of cryotherapy combined with other physical therapy methods. The study group consisted of 50 patients with gonarthrosis, treated by means of local cryotherapy and other physical therapy methods. The control group included 38 matched patients with gonarthrosis, treated with the same physical therapy methods, but without cryotherapy. Evaluation methods were based on the authors` tailored questionnaire, VAS scale and WOMAC questionnaire. The physical examination consisted of the measurements of body height and weight, the range of motion (ROM) in the knee joints and knee circumference and a series of functional tests. The results indicate that ROM of the knee joints increased by 11.3° in the study group and by 3.1° in the control group. The circumference was reduced by 1.1 cm in study group and by 0.4 cm in control group. The value of WOMAC index was diminished by 17.3% in the study group and by 11.9% in control group. After the treatment, the study group covered 8.7 stairs more during ascending and 9.2 more during descending whereas the control group covered 3.9 and 4.3 stairs respectively. Similarly, after the treatment patients from the study group did 5.5 squats more compared to 1.7 squats performed by the control group subjects. Cryotherapy combined with other physical therapy methods in treatment of gonarthrosis has better effects than application of physical therapy methods only, without cryotherapy.

P. Kozieł, M. Kiljański – Assessment of the effectiveness of local laser therapy in treatment of gonarthrosis. Work on habits. Fizjoterapia Polska 2018; 18(4); 26-33

Abstract
Introduction. Gonarthrosis is one of the most frequent diseases of the locomotor system. It is characterized not only by destruction of joint cartilage, but also by lesions in the subchondral bone layer, in the synovial membrane, joint capsule and periarticular structures. It leads to damage to the anatomical structure of the joint and to debilitation of its function which is so important in everyday functioning. Local laser therapy is one of the treatments applied in fighting with gonarthrosis. That procedure has anti-inflammatory, analgesic and edema-reducing effects. The objective of this study is to assess the results of local laser therapy, its impact on the scope of mobility and painfulness in patients suffering from gonarthrosis.
Materials and methods. The study covered 50 persons, suffering from gonarthrosis, divided into two groups, control group and study group, each with 25 members. The control group, with 19 women and 6 men, was subject to the following physical therapy procedures: cryotherapy, magnetotherapy and TENS currents. The study group, with 19 women and 6 men, was additionally subject to local laser therapy. All the persons were subject to a series of 10 procedures, from Monday to Friday, for two weeks. Before the series of procedures and right after completion thereof, the patients filled out anonymous surveys which contained the VAS pain scale, there were conducted mobility measurements of the knee joint flexure in the sagittal plane and linear measurements of the knee joint circumference.
Results. The results from both groups were subject to statistical analysis. After treatment, in both groups an improvement was noted in knee joint mobility, a decrease in knee joint circumferences and a decrease in the level of pain measured with the VAS scale in comparison with the condition from before treatment. The group with added local laser therapy demonstrated better mobility ranges and smaller knee joint circumferences.
Conclusions. Local laser therapy is an effective procedure applied in treatment of gonarthrosis. The level of pain decreased both in the control group and in the study group. The decrease was greater in the control group. The difference was statistically relevant In both groups, an improvement in knee joint mobility was observed. More improvement was noted in the study group. The difference was statistically relevant The knee joint circumference decreased both in the control group and in the study group. The decrease was greater in the study group. The difference was statistically relevant.
