Thermovisual evaluation of temperatures of the upper limbs following cryostimulation in post-stroke patients

Abstract
Background. Cerebral stroke is a very serious life-threatening condition regardiess of the clinical picture at the onset. In most cases cerebral stroke is a consequence of systemic vascular diseases such as atherosclerosis or hypertension. Material and methods. The study group comprised 103 patients, including 52 post-stroke patients with a paretic upper limb and 51 healthy participants. The group of stroke patients included 18 women and 34 men, while the healthy group consisted of 41 women and 10 men. The mean age of the male patients was 55.6 and of the female patients 66.1 years. The mean age of the control group was 37.2 years. Local cryostimulation was applied to the hand and forearm of the paretic limb in the stroke patients and to the hand the forearm of the right upper limb in the healthy group. The limb was exposed to cold for 6 minutes. The temperatures were measured using an Agema 570 thermovision camera.Results. Subjecting only one forearm to cryotherapy procedures results in a decrease in temperature. In the post-stroke patients the return to the basseline value after the procedure was more rapid. This is possibly connected with more efficient mechanisms protecting the body against loss of warmth, since in this group temperature values were higher. In the control group the temperature in the non-cooled limb decreases by an average of 1.6 st.C, compared to only 1.3 st.C in the stroke patients. Conclusions. The temperatures of an area in the forearm exposed to 6 minutes of cryotherapy were different in the post-stroke patients and in the healthy controls. This is probably attributable to stroke-related brain damage. Contralateralization, i. e. the decline of temperature in the non-cooled limb, occurred both in the post-stroke patients and in the healthy controls. Temperature decreases produced by the exposure of limbs to cold were higher in the stroke patients than in the healthy participants. The temperature returned to baseline more rapidly in the post-stroke patients. The temperature in the cooled limb dod not return to, or exceed, baseline value fifteen minutes after the procedure.

Abstract
Background. High-tone power therapy is a new method of nerve and muscle stimulation with electric current of a frequency corresponding to the acoustic band of electric oscillation between 4 400 Hz to 12 300 Hz. The usa of two pairs of electrodes serves to generate a dynamic electric field deep inside the tissues. The aim of the study was to assess upper limb function in hemiparetic stroke patients who undrwent high-tone power therapy in the course of rehabilitation. Materials and methods. The study group consisted of 16 men aged 46-72 years (mean 54) and 4 women aged 67-74 years (mean 71) who had suffered strokes of various aetiologies. Arm function was determined on the basis of grip strength testing, ranges of motion (ISOM) and spasticity level (Ashworth scale). Results. Grip strength increased with simultaneous improvement in angular ranges of motion in the radiocarpal joint of the paretic limb. Spastic tone in the limb decreased by 0.35 on the Ashworth scale. Analysis of linear measurements revealed statistically significant changes of circumferences only at the upper arm level. Conclusions. Comprehensive physiotherapy including horizontal therapy performed in stroke patients produced significant functional improvements in the paretic upper limb. High-tone power therapy is well tolerated by hemiparetic patients with despite the long duration of the procedure.

Abstract
Background. The results of a thermovision study of the body surface of patients subjected to various physical therapy procedures are presented. Pain intensity is also assessed using a Yisual Analogue Scale (VAS). The aim ofthe paperis to compare the efficacy of these procedures. Material and methods. The study involved 38 patients (15 males and 23 females) aged 19-78 years (mean age 58.1 years) with diagnosed degenerative changes and pain syndromes ofjoints of the lower extremities (13 patients), upper extremities (7 patients) and the vertebral column (18 patients). Ali patients were divided into 4 groups and subjected to NSAID iontophoresis (Group 1), anodę galvanisation (Group 2), galvanisa-tion followed immediately by an NSAID rub-in (Group 3), and NSAID rub-in alone (Group 4). Pain intensity was assessed using a subjective scalę (VAS) on the first, fifth and tenth procedurę day The distribution of temperature in the treated area was also recorded with an Agema 570 thermo-vision camera on the day of the first and last procedure. Results. A subjective reduction in pain intensity was recorded in all groups. Patients from the first group reported 50.2% improvement, patients from Group 2 reported 44.9% improvement, Group 3 reported 62.2% improvement, and Group 4 reported 38.9% improvement over baseline (first day of treatment). Changes in skin surface temperature were also observed but were not significant for Group 4. Conclusions. In the subjective opinion of the patients, the most efficacious procedure was anode galvanization immediately followed by an NSAID rub-in. The superior efficacy of this procedure appears to be related to the higher dose of medication being introduced into the treated area and greater skin hyperaemia compared to iontophoresis alone. Medication rub-in following galvanisation is effective and more cost-effective as the-re are no drug losses due to the drug remaining in the vehicle, as is observed during an iontophoresis procedure.

Abstract
Swallowing is a complicated, multi-stage process, which consists of forms of volitional and automated activities of our body. Irregularities in its scope may have a neurological basis, not related to the morphology and function of the nervous system, or may be associated with the physiological aging process.
Dysphagia, presbyphagia and aphagia may have complications in the form of aspiration pneumonia, malnutrition, dehydration, and deterioration of the quality of life. Multidisciplinary treatment, therapeutic and rehabilitation treatment, as well as involvement of a patient’s family and caregivers in this process is extremely important for patients with dysphagia.

Effectiveness of manual therapy in pain syndromes originating from the cervical spine

Abstract
Introduction. Cervical pain is a common problem worldwide and is increasingly prevalent. The structure of the cervical segment, as well as its biomechanics, contributes to this. The highest number of cases has been observed among professionally active people, however, the most vulnerable are people practicing a profession involving long-term sitting in one position for several hours. The spine is often overloaded due to long periods of remaining in unnatural and uncomfortable static positions. Pain is most often the result of changes in the paravertebral tissues occurring on a structural background. This problem affects 60–90% of the population and occurs in people aged 25–65. The main methods of treating pain syndromes include: kinesitherapy, i.e. movement therapy, physical procedures and various special methods, including the McKenzie method, PNF or manual therapy.
Objective The objective of the study was to assess the effectiveness of treating people with pain syndromes originating from the cervical spine with various physiotherapy methods.
Material and methods. The PubMed database was searched electronically using keywords such as ‘manual therapy’, ‘PNF’, ‘McKenzie method’, ‘pain syndromes’, ‘cervical spine pain syndrome’, ‘cervical spine’, ‘neck pain’, ‘neck pain therapy’ and the relevant studies were examined. Twenty-two publications were found, containing treatment of cervical spine pain syndromes using various methods, which were further analyzed. These articles were published between 2006 and 2019. Based on the analysis of titles and abstracts, five studies were excluded. Then another three studies were excluded based on the analysis of full texts. In total, 8 studies were excluded. Finally, the analysis of data contained in 14 studies was performed.
Results. It has been observed that pain is one of the most common symptoms of cervical spine disorders. Most often it is assessed using the VAS visual-analog scale. Based on the analysis of 14 studies, it is found that after the application of special physiotherapy techniques, pain is reduced.
Conclusions. Based on the analysis performed, it is concluded that the most commonly used method of measuring pain in patients before and after therapy is the VAS visual-analogue scale. After analyzing the results of selected studies, it can be clearly stated that special methods have a beneficial effect on reducing pain in patients with cervical pain syndrome, based on the average values before and after therapy.

Analysis of selected body posture parameters in female volleyball players

Abstract

Introduction. Habitual posture of professional athletes is closely connected with the demands of sport training, and its characteristics depend on the type of sport involved. The purpose of this study was to assess the posture and foot loads of female volleyball players from the KSZO Ostrowiec team playing in the Orlen Liga (the Polish Women’s Volleyball League) in comparison to women in a similar age group who do not play sports professionally.
Materials and methods. The study was conducted between 2014 and 2015, at the start of the volleyball season, and used the DIERS Formetric 4D analysis system. The results of the female volleyball players were compared with healthy women in the same age group who do not play sports professionally.
Results. The results obtained in this study indicate that female volleyball players have lower values of the lumbar lordosis angle in comparison to the control group. The values of the trunk inclination angle (relative to the vertical plane) were also significantly different between the two groups, with the analyzed group having higher values. With regard to the other analyzed variables i.e. kyphosis and foot loads, the study found no statistically significant differences between the groups.
Conclusion. The assessment of the habitual posture of female volleyball players conducted in the present study suggests that the demands of sport training can have an influence on how the body posture of professional athletes develops.

Assessment of the Effectiveness of the Physiotherapy in Patients with the Coxarthrosis, Treated in Spa Facilities

Abstract
Introduction. The objective of this study has been to assess the effectiveness of the physiotherapy in patients with the coxarthrosis, treated in Spa facilities. Materials and Methods. The study involved 49 patients (29 women and 20 men), who have been treated in the “Słowacki” Spa in Busko-Zdrój, during their 21 days rehabilitation stay. The patients have had degenerative changes in the hip joint, which have not been surgically treated. The patients have been divided into two groups. To the study group (24 persons) the physiotherapy treatments have been applied. In the control group (25 persons), in addition there has been the kinezytherapy applied. Each of the patients has gone through: questionnaire survey, measurement of the range motion in the hip joints, muscle strength evaluation with the Lovett scale, assessment of the pain severity in the VAS scale, functional tests (Linder 2, abdominal compression test, Patrick Faber test, SLR, Tinetti test,”Up & Go” test, gait analysis according to Perry). Results. After the 21-day rehabilitation treatment, the majority of patients have shown a significant decrease in the subjective pain sensation, a significant improvement of the hip motion functions and a reduced risk of the patient falling down. The carried out research has indicated, that better results have been achieved in the control group, where the kinezytherapy treatment has been applied in addition. Conclusions. 1. Physiotherapy has an impact on increasing the hip joint range of motion, in all directions, particularly in the flexion an abduction movements. 2. The rehabilitation procedure improve the gait stereotype in patients with the osteoarthritis of the hip joint. 3. The Spa rehabilitation therapy reduces the risk of falling down in patients with degenerative changes in the hip joints.

Monitoring the results of scoliosis improvement using the DIERS system

Abstract

The aim of the study was to present a modern method of imaging the posture of the human body, namely, the DIERS Formetric 4D system. This method is used, among others, for non-invasive evaluation of the effects of treatment of patients diagnosed with scoliosis. Using the DIERS Formetric 4D device, the test of body posture can be done almost any time without the use of a temporary regime between studies. This allows assessment of both the effects of the treatment carried out in the short term, as well as the assumptions of the plan for further treatment.
According to the authors, the use of the DIERS Formetric 4D method to control the effects of scoliosis treatment is extremely important because of the possibility of frequent performance this test, its short duration and repeatability of the achieved results.  They are available very quickly and there is a possibility to compare them with the results of previous tests, thereby greatly facilitating the work of physiotherapists.
