Assessment of the effectiveness of local cryotherapy in treatment of gonarthrosis

Abstract
The purpose of the study was to compare the effect of cryotherapy combined with other physical therapy methods. The study group consisted of 50 patients with gonarthrosis, treated by means of local cryotherapy and other physical therapy methods. The control group included 38 matched patients with gonarthrosis, treated with the same physical therapy methods, but without cryotherapy. Evaluation methods were based on the authors` tailored questionnaire, VAS scale and WOMAC questionnaire. The physical examination consisted of the measurements of body height and weight, the range of motion (ROM) in the knee joints and knee circumference and a series of functional tests. The results indicate that ROM of the knee joints increased by 11.3° in the study group and by 3.1° in the control group. The circumference was reduced by 1.1 cm in study group and by 0.4 cm in control group. The value of WOMAC index was diminished by 17.3% in the study group and by 11.9% in control group. After the treatment, the study group covered 8.7 stairs more during ascending and 9.2 more during descending whereas the control group covered 3.9 and 4.3 stairs respectively. Similarly, after the treatment patients from the study group did 5.5 squats more compared to 1.7 squats performed by the control group subjects. Cryotherapy combined with other physical therapy methods in treatment of gonarthrosis has better effects than application of physical therapy methods only, without cryotherapy.

Evaluation of the Efficacy of Local Cryotherapy in Patients with the Degenerative Knee Joint Disease

Abstract

Introduction. Pain, swelling and deformities of joints are the typical symptoms of a degenerative disease. The disease affects more than 60% of persons over 60 years of age. Due to this condition, every thirteenth person takes painkillers on a daily basis. Etiology of the degenerative knee joint disease comprises most often of micro traumas, injuries, overstrain, previous inflammatory conditions, improper weight distribution and congenital anomalies in the joint structure.
Research Goal. The aim of our study has been to evaluate the efficacy of local cryotherapy in patients with the degenerative knee joint disease.
Materials and Methods. The study has been carried out on a group of 86 persons, 20 to 45 years old, treated in the Rehabilitation Unit of the University Clinical Hospital No. 1 of the Medical University in Łódź. The patients have been randomly assigned to group A (40 persons) and B (46 persons.) In all the patients, there have been diagnosed degenerative changes in knee joints. In the study participated persons professionally active, able to walk.
Prior to application of treatment, all the patients were interviewed, had undergone physical and clinical examinations and were asked to fill out the WOMAC (Western Ontario and McMaster) questionnaire. An important purpose of the interview was to gather information about the patients’ lifestyle, their diet, addictions, physical activity and the potential epidemiological threats arising from the type of work they do.
Before and after completing the cycle of 10 treatment sessions, the patients were also examined physically by visual inspection, palpation, percussion, auscultation, measurements of lengths and perimeters, testing of muscle strength and functional tests. The patients were asked to anonymously fill out a questionnaire.
For all the patients were applied same treatments: ultrasounds, magnetic field, self-assisted exercises, knee joint load relief exercises. The differentiating factor in the study were the knee joint local cryotherapy treatments applied additionally for patients in group B. The evaluation was made on the basis of examinations before and after the physiotherapy treatments.
Results. Based on the examinations it can be concluded, that there has been achieved statistically significant reduction in pain according to VAS in both, group A and group B. There has also been a reduction in exudate and temperature of the joint.. There has also been registered an increase in the mobility ranges. Based on results of the test of Western Ontario and McMaster Universities Osteoarthritis Index, we have also observed an improvement in functional capacities in executing daily activities, i.e. for example climbing/descending stairs or putting on/taking off socks.
Conclusions. While interpreting the results, it has been observed that the degree of improvement in the above listed factors in group B, in which the additional local cryotherapy was applied, was statistically significantly higher than in group A, in which the treatment was not used.

Abstract

Introduction. The aim of the study was to evaluate the effect of local cryotherapy treatment for range of motion in the knee joint in patients with osteoarthritis. In addition, analyzed potential changes in symptoms of pain patients.
Material and method. The study group consisted of 50 of the patients. The study involved 39 women and 11 men. Patients were subjected to 10 daily treatments local cryotherapy on the area of the knee. Before treatment was collected interview regarding comorbidities and examined pain according to VAS. In all patients performed to measure of range of motion in the knee joint using a goniometer. It was made before and after treatment.
Results. Considering the range of flexion in the knee joint in both groups, both diagnosed with osteoarthritis of the knee and polyarticular osteoarthritis, after a series of local cryotherapy treatments became significant improvement, while the range of motion in the knee extension improved more in the group diagnosed with polyarticular osteoarthritis.
In assessing the two groups studied, both in the group with a diagnosis of osteoarthritis of the knee, as well as polyarticular osteoarthritis, and made a series of treatments local cryotherapy declared value level of pain on the VAS decreased. Considering the improvement by gender, it can be concluded that in women with a diagnosis of osteoarthritis of the knee improvement was much greater.
Conclusion. Local cryotherapy is a beneficial effect on the range of motion in the knee joint and reduction of pain in patients with gonarthrosis.

Therapeutic value of local cryotherapy in symptomatic treatment of patients with shoulder joint pain syndrome

Abstract

Introduction. The aim of this study was to determine the effects of the cryotherapy treatment on the mobility of the shoulder joint of the patients with osteoarthritis. We took into consideration the motion of flexion and extension. Additionally, the potential changes in the pain complaints were analyzed.
Material and methods. We examined 54 patients in the study, including 31 women and 23 men. They were put through the local cryotherapy on the shoulder joint 15 times daily. Before participating, all patients gave their informed consent to take part in the study and they were asked to rate their pain in the Visual Analogue Scale (VAS). The range of motion in the shoulder joint of every patient was measured  by goniometer before and after cryotherapy.
Results. All patients who underwent the series of cryotherapy had significant improvement of flexion and extension in the shoulder joint. Taking into consideration the general analysis of pain rated in the VAS score by the participants, we observed a significant reduction in the values of VAS indicated by the patients after the series of treatments.
Conclusions. Local cryotherapy is a reliable method that reduce pain and improve range of motion in the patients with pain syndrome of the shoulder joint.
