Abstract
Introduction. Hallux rigidus and hallux limitus are characterized by limited range of motion of the first metatarsophalangeal joint in the sagittal plane.
Aim of the study. The purpose of this study was to present the possibilities and evaluate the effectiveness of conservative treatment of hallux rigidus and limitus with consideration of orthopedic supplies based on the available literature.
Material and methods. The available literature was analyzed based on English-language databases PubMed, Scopus, ScienceDirect, Medline and others, using the Google Scholar search engine. Twenty-one articles were analyzed out of 51 retrieved by the above method. Publications no older than 21 years were used as search criteria using the following keywords: hallux rigidus, hallux limitus, orthopedic supplies, physiotherapy, orthopedic insoles, surgical treatment.
Results. After analyzing the papers, it was not possible to find a clear answer to determine which conservative treatment of hallux rigidus is the most effective, while each of the mentioned methods brings benefits to the patient.
Conclusions. Contemporary studies indicate that conservative treatment methods for hallux rigidus and limitus are effective. Both orthopedic insoles, drug treatment and physiotherapy provide relief from pain. When a patient is referred for surgery, the doctor has a choice of different treatment methods depending on the stage of the hallux rigidus.

Abstract
Patellar dislocation is an emergency, requiring immediate medical intervention, followed by the cooperation of the orthopedist and physiotherapist in returning to full health, as well as reducing the risk of relapse. The kneecap is an integral part of the knee joint and is responsible for its proper functioning. Among the methods of treatment used, there are conservative and surgical methods. The article is an attempt to answer the question whether there is an advantage of some methods over others. Materials available on the PubMed and Cochrane Library websites were used to obtain answers to the question asked. Due to different standards of management, the advantage of surgical over conservative has not been established, but there are researchable factors that may help in making therapeutic decisions.

Abstract

The authors have developed a two-stage kinesitherapeutic model for the conservative rehabilitation of compression fractures of the spine in children and youth. The first phase, called the “hospitalization phase”, lasts 4-6 weeks, while the second, the “ambulatory phase”, lasts from the 2nd to the 12th month after injury.The orientation in medical rehabilitation presented by the authors of this article combines elements typical for conservative treatment reinforced by Kempf exercises with elements of periodic application of prostheses, in the form of light orthopedic corsets. The best results are achieved by creating an autogenic muscle girdle, which simultaneously protects the spine against flexion movements by forcing extension of the spine, and by using correctional and hypercorrectional positions in the orthoses used, especially in multiple fractures. The primary purpose of kinesitherapy is to strengthen the muscle girdle and to shape the strength and endurance of the postural muscles, using positions that relieve pressure on the spine. The isometric exercises in the pressure-relieving position lead to a systematic increase in the strength of the postural muscle. Other important elements of kinesitherapy include learning to feel one’s own body, proper protection of the spine, and shaping the correct body posture. Full recovery was attained in 70% of cases after one year of modified therapy. In the control group – traditional therapy – positive results were obtained in 56%.

Abstract
Background. The purpose of our research was to measure the impact of cessation of the recommended rehabilitation program on the clinical status of patients with herniated discs in long-term follow-up. Material and methods. We examined 36 patients diagnosed by computer tomography with herniated disk, treated conservatively at the Physical Medicine Department in 1998. Examinations were performed at one year and two years after the resolution of symptoms. The subjects were divided into 3 groups: Group I consisted of 18 patients (50%) who were still maintaining the recommended rehabilitation program at home; Group II included 9 patients undergoing periodic rehabilitation in an outpatient program; Group III was comprised of the remaining 9 patients, who had entirely abandoned the recommended program. Results. After one year 66,6% of the patients in groups I and II had moderate pain symptoms (up to 5 points on the Laitinen Scale), and 33,4% had pain symptoms periodically preventing work (6-11 points). In group III, 33,3% of the patients had >5 points, 33,3% had 6-11 points, and 33,4%had pain systems permanently preventing a return to work and independent living (>11 points).

Abstract

Background. The treatment of post-traumatic contracture is beset with many difficulties, caused in large part by the anatomical complexity of this joint. Case histories. This article presents three patients treated for persistent post-traumatic contracture of the elbow. The rehabilitation problem consisted in the fact that when extenion was increased, flexion was decreased, and vice versa. In order to overcome this mechanism, which maintained an unchanged range of movement by alternately shifting the extreme positions of flexion or extension, we attempted to increase the total range of movement by using plaster extensors to fix the progress of maximum movement. Plaster casts holding the joint in the maximum, extreme position were changed every two hours. Therapy with physical agents and mild exercises was simultaneously administered. New pairs of double casts were made twice, each time achieving further improvement. Conclusions. In selected cases of contractures resistant to standard rehabilitation procedures, when the use of a redressing rail is impossible, the method of serial casts in alternating extreme redressment as here described should be considered.

Medical massage in the treatment of non-specific low back pain

Abstract

Background. Non-specific low back pain is a serious social problem. Most patients are unable to specify the particular event that caused the pain to appear. The typical treatment methods, i.e. pharmacotherapy and surgery, seldom lead to the desired results. One effective method of treating non-specific low back pain is physiotherapy, in which medical massage plays an important role. This is a therapeutic procedure based on theoretical and clinical research in medicine and related scientific fields. Material and methods. We studied 40 ambulatory patients, 31 men and 9 men, average age 47. The patients were evaluated using a 7-point scale of pain intensity and a patient evaluation chart, containing a comprehensive assessment of the musculo-skeletal system, including the functional status of other systems directly involved in motor function. Results. Before treatment, the patients’ subjective reports of pain intensity averaged 3.57 ± 1.1 points (moderately intense pain). After massage treatment, 77% of the patients (31 persons) reported no pain at all (0 points), and 23% (9 persons) reported minimal pain (1 point). Conclusions. The application of medical massage in this pathology significantly reduced the reported intensity of pain in all the patients we studied. Medical massage can be applied as an effective therapeutic procedure in managing pain for persons with non-specific low back pain.

Early results of conservative treatment of children with idiopathic scoliosis using the SpineCor System

Abstract
Background. The SpineCor method, the effectiveness of which is evaluated in the present paper, was introduced to Poland two years ago and has been used in few centres. The present paper evaluates the effects of dynamic bracing over a period of two years. Material and methods. The subjects were 121 children undergoing treatment at the Paediatric Rehabilitation Hospital in Radziszów. Parameters analysed included Cobb’s angles (radiographic), the Risser test and a clinical evaluation of spinal rotation before and immediately after the placement of the brace, and at one year after placement. Results. Clinical evaluations and a classification based on the SpineCor Assistant System (SAS) revealed a predominance of right-sided thoracic (RT) scoliosis (47.3%), particularly type III (RT3) (24.7%). Significant differences between means after the application of the SpineCor brace were shown in Cobb’s angles in the primary and secondary arches and in clinical rotation. There was a high correlation between Cobb’s angle progression in the primary arch and the Risser test (r=0.46), time from brace placement to first menstruation (PM), r=0.33, and regularity of follow-up (r=0.43). Conclusions. The results indicate a significant change in the clinical parameters of sciolosis following the application of a SpineCor brace as regards halting the progression of the scoliosis angle and reduction in clinical rotation of vertebrae. A reduction in Cobb’s angle within a year of placement of a SpineCor brace was recorded in 7-10-year-old children. Progression or no change in Cobb’s angle was seen in those aged 11-12 years and those older than 17 years of age.

Abstract
Introduction.
Urinary incontinence is a problem recognized by World Health Organization as a social disease. The standards of the International Continence Society (ICS), gynecological and urological societies I and II degree UI begins with conservative treatment conducted by a physiotherapist.
Aim.
The aim of this work is to emphasize the role and possibility of additional diagnosis carried out by physiotherapist.
Conclusions.
Conservative treatment with available technologies, devices, questionnaires allows wide possibilities of insightful approach to the topic which is woman urinary incontinence.
