Effectiveness of PNF method in improving symmetry of lower extremity loading in late-stage post-stroke patients

Abstract
Early comprehensive multidisciplinary rehabilitation is crucial for achieving satisfactory results in the conservative management of post-stroke patients. Gait re-education is a major goal in post-stroke rehabilitation regardless of how much time has elapsed since the stroke. The study hypothesis was that therapy based on PNF (Proprioceptive Neuromuscular Facilitation) would significantly improve loading symmetry of the lower limbs, thus enabling faster restoration of the support and locomotor function and improving gait efficiency in post-stroke patients. A total of 64 stroke patients were randomly divided into two groups. A Loading Symmetry Index (LSI) of the lower limbs was calculated basing on data recorded in the ‘two-scales’ test. Patients were followed up for a mean of 21 days (during which they stayed at a Neurological Rehabilitation Ward). Group A received conventional rehabilitation while Group B also received individual kinesiotherapy based on the PNF method. Analysis of variance showed significant differences in LSI values (interaction: group x measurement – p=0.0075). Post hoc Tukey test revealed significant intra-group differences in Group B (p=0.0002). There were no significant intra-group differences in Group A or significant differences between the groups. PNF-based rehabilitation of late-stage post-stroke patients significantly improved loading symmetry of the lower extre – mities.

Abstract
The study presents five functional benchmarks (FBs) corresponding to various levels of knee function and physical fitness (from basic locomotion to sports-level performance) in patients undergoing physiotherapy (rehabilitation) after reconstruction of the anterior cruciate ligament (ACL). The FBs presented in the paper complement clinical evaluation of the outcomes of a comprehensive treatment plan for post-ACL reconstruction patients. We introduce our own classification of functional benchmarks (FBs) and propose examination methods which allow fairly objective assessment of the fitness level of the musculoskeletal system at a particular stage of rehabilitation. The following FBs are described: clinical (CFB), minimal (MFB), standard (SFB), optimal (OFB) and sports (SPFB). Two criteria are assumed separately for the sports benchmark, namely an amateur sports benchmark and a professional sports benchmark. The paper presents the author’s experience as well as a review of the literature basing on the experience of other authors.

Abstract
Background. The present paper analyses and compares the factors that motivate female and male students of physiotherapy to take up these studies and examines students awareness of the employment situation in their future profession.
Material and methods. The diagnostic poll method (anonymous and voluntary question naire). The study sample comprised 1st year Bachelor programme students from eleven university-level schools. The statistical analysis was based on question naires obtained from 954 students. The survey was carried out in 2006, at the beginning of the 1st semester of the Bachelor’s programme. Outcome measures The Mann–Whitney U test was used at a significance level of p<0.05. Results. Personal interest was the only factor that influenced the decision to study physiotherapy and differentiated between the male and female students. For a significantly higher percentage of the female students (p<0.001), studying physiotherapy had been a life-long ambition. The vast majority of the students stated that they were not familiar with all opportunities for finding employment as physiotherapists. The vast majority of the students believed that it is easy to find employment as a physiotherapist in other European Union countries, with female students stating so significantly more frequently (p<0.05) than their male counter parts
Conclusion. Men and women share similar expectations of their future job and motivations

Abstract
A sense of professional identity, the perception of the profession in society are important factors of professional development, provide the motivation to remain in this occupation. In the paper systemisation of the knowledge concerning the development of the profession of physiotherapist in Poland has been attempted. Analysis of the available archival texts and national literature allowed to identify the test group in the context of the formation of its professional identity. The following processes were tracked formation of the first training centres for the motor rehabilitation and creating an teaching standards, program of specialisation for physiotherapists. Introduction of the term „physiotherapy in place of the functioning of the term „motor rehabilitation has initiated changes within the domain of legislative activities. In accordance with the Bologna Declaration new levels of education were created, which resulted in the acquisition of certain skills and professional titles. The changes also affected the area associated with specialization. This level of training is being actively undertaken by physiotherapists. Currently 26 centers have been accredited in which the specialization program is carried out by about 400 physiotherapists.

Abstract
Introduction. The 21st century is characterized by the dynamic development of the automotive industry and other industries. Along with them, the pace of our life increases, resulting in an increased trauma rate due to rush and traffic accidents. When looking for attractions, young people often get into conflicts or risk to impress their peers. As a result, traumatology grows into a vast branch of medicine, and mandibular fractures account for an increasing percentage of injuries. Objective. The objective of the study was to assess the usefulness of physiotherapy in patients after orthopaedic and orthopaedic-surgical treatment of mandibular fractures for faster recovery of the masticatory organ function, and to introduce physiotherapy as a standard in treatment of patients after mandibular fractures. Material and methods. The study involved a group of 20 patients aged 18–60 who suffered a mandibular fracture as a result of an injury. The patients were diagnosed and treated at the Clinical Department of Cranio-Maxillofacial Surgery and Oncology of Norbert Barlicki’s University Clinical Hospital no. 1 in Lodz. Results. The obtained results showed significant differences between patients who underwent physiotherapy after treatment of a mandibular fracture and those who did not want to participate in it. Conclusions. This allowed for the conclusions to be made that the problems of patients after immobilization due to a mandibular fracture are significant and should constitute the starting point for the development of an appropriate physiotherapeutic treatment algorithm for these patients. It would also be advisable to promote physiotherapy as standard treatment in such cases.

Abstract
Aim. In this technologically advancing world, online teaching has become omnipresent in all fields. However, teaching online is still a challenge in allied health professional courses where hands-on skills are the core component of the curriculum. Hence, the aim of this paper was to assess the effectiveness of a case based combined online video and written assignment in improving the practical performance and satisfaction of the cardiorespiratory physiotherapy course students. Material and Methods. At INTI International University (INTIIU), we provided a case scenario to the cardiorespiratory physiotherapy course students as an assignment. First, the students need to submit their written answers in the Blackboard (BB) Learning Management System. They also need to record a video of the patient assessment procedure for the given case scenario and upload it in the BB. The student’s practical performance before the online assignment in the coursework Objective Structured Clinical Examination (OSCE) and after the assignment in the Final OSCE was assessed to find the difference. Results. The paired t-test showed a significant difference between coursework (M = 66.47, SD = 7.04) and final OSCE (M = 77.49, SD=10.11) performance; t (13) = −3.48, p = 0.004, which indicates that the students’ performance in the final OSCE was significantly better than coursework OSCE. The students’ perception about the course and their engagement in the BB was better. Conclusions. The case-based video and written assignment is a successful strategy to improve the practical performance of the students in a physiotherapy course where practical skills are imperative.

Physiotherapy in patients with congenital haemorrhagic diathesis in the material of the systemic rehabilitation department

Abstract
Aim. The objective of this study is to present the process and results of physiotherapy in patients suffering from congenital haemorrhagic diathesis, treated at the Inpatient Rehabilitation Department of the Krakow Rehabilitation Centre and Orthopaedics (MSOR). Material. The analysis involved 73 patients treated at the Inpatient Rehabilitation Department between 2014 and 2019. The reason for admission was haemophilic arthropathy in the course of congenital haemorrhagic diathesis. Results. The results prove that early postoperative knee rehabilitation is effective in patients with congenital haemorrhagic diathesis after surgery. Deaver’s functional scale: before rehabilitation, the mean result was 3.65, after rehabilitation it was 2.62. Muscle strength: the assessment of muscle strength according to the Lovett scale showed improvement in 64 patients (87,67%). Range of motion: in the extension test, the condition of 57 patients (78%) improved, the mean result was better by 7.52 degrees. Conclusions. 1. The use of early physiotherapy in patients with haemophilia after knee endoprosthetics results in an improvement in gait function analysed according to Deaver’s scale. 2. Early rehabilitation of patients with haemophilia after endoprosthetics results in an increase in the range of motion of the knee joint and in muscle strength. 3. It was also found that pain in the joint undergoing surgery was reduced thanks to the applied physiotherapy.

Abstract
The complex pathophysiology of spasticity depends on the interaction between facilitation and inhibition of spinal and supraspinal neural centres and pathways. Contemporary medicine has developed several methods to treat spasticity. However, none of them has offered superior efficacy. This study aimed to investigate differences in the degree of spasticity between patients subjected to traditional post-stroke rehabilitation and rehabilitation based on the PNF method.A total of 64 stroke patients were enrolled. They were randomly divided into Group A and Group B. The modified six-grade Ashworth scale was used to evaluate the level of spasticity. During a 21-day follow-up, all subjects underwent intensive comprehensive rehabilitation. Group A received traditional stroke therapy. Group B received kinesiotherapy based on the PNF method.Both groups demonstrated significant improvements. In Group A, the degree of improvement in the final examination was 1.8% compared to baseline. In Group B, an improvement of 7.8% was registered already after the first session. The degree of improvement at the end of treatment was 16.86%. No significant inter-group differences were noted at baseline. The final evaluation showed significant differences.A regimen based on the PNF method resulted in greater reduction in spasticity than the traditional approach.

The Usefulness of Hippotherapy in the Rehabilitation of Cerebrally Palsied Children. Pilot Study

Abstract
Zootherapy has been introduced in recent years as an adjunct approach to traditional physiotherapy in patients with cerebral palsy (CP). Hippotherapy is less popular in Poland compared to other European countries or the USA, with few reports of investigations carried out in accordance with the EBM paradigm. The aim of the present study was to evaluate the usefulness of hippotherapy in comprehensive rehabilitation of children with cerebral palsy.The study involved 40 cerebrally palsied children who were randomly divided into two equal-sized groups (A and B). Both groups received home-based rehabilitation according to the Proprioceptive Neuromuscular Facilitation (PNF) method. Group A additionally received hippotherapy. The children’s motor function was determined using the Gross Motor Function Measure (GMFM).In Group A was 0.02 for ChP=5.226, 1 df. The parameter determined in the study exceeded this critical value, thus allowing the conclusion that the effect of home-based rehabilitation combined with hippotherapy on overall health improvement was statistically significant (p&lt;0.05). In Group B, the right-tailed critical value (p) was 0.429 for Chi2=0.625, 1 df. The parameter determined in the study did not exceed the critical value, thus not allowing the statement that home-based rehabilitation had a significant effect on overall health improvement.1. The study shows significantly better results of rehabilitation in children treated with combination of PNF and hippotherapy. 2. The results indicate the need for randomized prospective studies involving larger test groups.

Abstract
Physiotherapist carries out the medical profession. Physiotherapy is a long-term action and requires special preparation, fulfilling high professional and ethical requirements necessary in health care. Relevance and usefulness of knowledge is a requisite for professionalism. Every profession strives to achieve autonomy, reflected in the internal control exercised by professional organizations. The aim of the undertaken research is to know physical therapists’ subjective opinion on their job in the context of its professionalisation.The results of research in the group of 338 employed physiotherapists and 125 students of physiotherapy. The study was multicenter, included 11 institutions (universities and rehabilitation centers).The aim of the study was carried out based on a realized survey. The questionnaire of the survey consisted of questions arranged by topic. Respondents presented opinions on the quality of education, level of satisfaction with career choice, the status of the profession, the place it occupies in the medical team and the development of academic and professional physiotherapists.The development of scientific and professional skills indicates the progress made by Polish physiotherapy.Physical therapy is perceived as a responsible, dynamically developing and satisfying job. The work on the act on the profession of physical therapist must be completed. The quality of academic education must be enhanced as regards practical preparation for the job.
