Effect of foot orthoses on patellofemoral pain syndrome: A systematic review

Abstract

Background. Although foot orthosis may play a role in treatment of patellofemoral pain syndrome, its significance is still conflicting and previous studies show a low level of evidence. Objective. To find the latest evidence regarding the effectiveness of foot orthosis (FO) on adult patients with patellofemoral pain syndrome (PFPS). Methods. A comprehensive electronic database search of PubMed, Cochrane Library Database, the Physiotherapy Evidence Database (Pedro), ProQuest, EBSCo host, and Ovid databases was performed for randomized controlled trials (RCTs) in English language since January 2000 till February 2019 on adults with patellofemoral pain syndrome treated by foot orthosis and physical therapy, with hand searching to reference lists of previous systematic reviews and included papers. At least one primary outcome measure for pain or function must have been reported. Two authors independently reviewed and assessed each citation for inclusion, The Cochrane risk of bias tool was used to rate methodological quality and risk of bias. Data was met analyzed when this was not possible, qualitative data analysis was performed. Results. A total of 9 RCTs with a total number of 415 patients (6 on non-runners and 3 on runners) were included. Findings were summarized in four main comparisons as follows 1)foot orthosis versus flat insoles these results showed the positive efficacy of FO on pain, function and muscular activity, 2) foot orthoses alone versus physiotherapy have a poor indication of PFO alone as an intervention alternative to physiotherapy for PFPS, 3)foot orthoses versus no treatment may provide greater improvements on pain, function and neuromotor changes than natural history concerning foot posture, and 4)foot orthoses combined physiotherapy is better than physiotherapy alone even though the success may relate to clinical foot predictors for foot orthosis. Conclusion. The clinical efficacy of foot orthoses may correlate with targeting specific individuals based on specific foot measures and the comfort perception of the orthosis. On the kinematics, foot orthosis may behave minor neuromotor activity changes at the lower extremity. The effects of combining foot orthoses with physical therapy may be superior to physical therapy alone and result in better outcomes.

Abstract

Background. Cervical radiculopathy (CR) is known as a severe neuromusculoskeletal condition which causes pain and physical disorders that both impair employment and quality of life. Spinal mobilization with arm movements (SMWAMs) is one of the methods in Mulligan’s techniques while residual discomfort is assumed to originate from the spine. Objective. This study aimed to assess the effect of SMWAMs on cervical proprioception and functional abilities in patient with cervical radiculopathy. Methods. Forty subjects of both sexes with chronic unilateral CR participated in the study, they were randomly assigned into two equal groups, Group A (experimental group): received SMWAMs in addition to a guide line protocol, group B (control group): received the guide line protocol, over four consecutive weeks, all subjects received 12 sessions. Outcome assessments included cervical proprioception using the cervical range of motion (CROM) and functional abilities using the cervical disability index (NDI) were assessed at baseline and 4 weeks’ post-intervention. Results. There was a statistical significant improvement in the experimental and control groups 4 weeks post-treatment for cervical proprioception and functional performance (p < 0.05). Concerning groups there was a significant difference between both groups regarding all measured variables in favor to the experimental group (p < 0.05). Conclusion. SMWAMs provides an additional effect in the management of chronic Cervical radiculopathy patients as it proprioception and functional performance.

Abstract

Purpose. This study was conducted to detect the effect of Cryolipolysis and Quadri-polar Radiofrequency on Post-Natal women with Abdominal Skin Laxity. Materials and methods. Forty post natal women complained of postnatal abdominal skin laxity selected randomly from El Kasre El Ainy University hospital in Cairo. They were divided into two equal groups group (A) (n = 20) and group (B) (n = 20), group A treated by eight sessions of cryolipolysis for 8 weeks (1session per/ week to abdominal area) the duration of each session is 20 minutes, while group (B) treated by non-invasive quadri-polar radiofrequency for 8 weeks (1session per/ week to abdominal area) the duration of each session is 20 minutes. BMI (kg/m2), Waist and hip circumference measurements were used for Whole body evaluation while two-point discrimination and photographic scale were used for Skin laxity evaluation before and after intervention. Results. There was significant reduction (p < 0.05) in BMI, waist hip ratio and two-point discrimination at both groups in the post treatment condition compared with the pretreatment. Also there was significant difference of two-point discrimination between both groups in favor to group B. Also There was a significant improvement in the Photographic scale at post-treatment in compared to pre-treatment in both groups (p < 0.05) and there was a significant improvement of the distribution of Photographic scale in favor of group B in compared to group A at post treatment (p < 0.05). Conclusion. Quadri-polar radiofrequency is yield more improvement than cryolipolysis in treating postnatal women with abdominal skin laxity.

Risk factors of neurodevolopmental disorders in preterm infant

Abstract

Introduction. Preterm infants are at a greater risk for developmental disorders than their term peers. The most cases, it is not possible to determine the specific cause of the cerebral motor disorders. Review of literature indicate that they have no only one cause. Instead, it usually occurs due to a sequence of events or circumstances, so- called risk factor. The risks factors can happen during pregnancy and delivery, so they are usually are divided into following categories: 1) prenatal, 2) natal and 3) postnatal risk factors.
The aim of this study is to recognize the relationship between prematurity and the presence of other risk factors for the development of CMD, related to the course of pregnancy, the course of delivery and the condition of the child after birth.
Participants & Methods. Thirty premature infants from 3 to 5 months, who were qualified to SYNAGIS program included to the study. The present study collected data of burdens and clinical symptoms associated with preterm birth, i.e. the gestational age, birth weight, Apgar score, maternal age, as well as the presence of prenatal, perinatal and postnatal factors of motor disorders in preterm infant.
Results & conclusion. The obtained results confirmed that preterm babies are at a higher risk of developing neurodevelopmental disorders. The obtained results additionally indicated a large share of postnatal factors related to the immaturity of the respiratory system, which is a directly result from preterm labor.

Effect of Concurrent Training on Hand Grip after Gunshot Wounds in Upper Extremity: A Randomized Controlled Trial

Abstract

Purpose. to evaluate effect of concurrent training on hand grip after gunshot wounds in upper extremity. Design. single blind randomized controlled trial. Methods. Forty patients with Gunshot Wounds in Upper Extremity participated in the study, they were recruited from the police hospital, Egypt; their ages ranged from 20-50 years old. Confidentiality was assured. They were assigned randomly into two groups equally in number; 20 patients each: study Group (A) received concurrent training In addition to their traditional physical therapy program while control group (B) received traditional physical therapy program only. All treatments were administered on the affected upper limb, 3 times a week for 8 weeks. Measurement of disabilities of the arm, shoulder and hand questionnaire (DASH) scale was used to evaluate upper limb disability and Jamar hand dynamometer was used to assess hand grip strength (HGS). Descriptive statistics as mean, standard deviation, minimum and maximum were calculated for each group. Unpaired t test was conducted for comparison of (HGS) and (DASH) between both groups. Paired t test was conducted for comparison of (HGS) and (DASH) between pre and post treatment in each group. Alpha point of 0.05 was used as a level of significance. Results. When compared to the pre-treatment condition, HGS / DASH improved in the post test condition in group A and B. Between the groups, There was statistically significant difference was noted between the mean value of all dependent variables in group A and their corresponding value in group B. Conclusion: Concurrent training was effective and beneficial in improving hand grip strength after gunshot wounds in upper extremity as manifested by the highly increased (HGS) and the significant decrease in (DASH).

Occupational burnout of psychiatric nurses vs. characteristics of the work environment – preliminary report

Abstract

Objective. The objective of the study was to assess the level of occupational burnout and the relationship between occupational burnout and characteristics of the work environment in a group of psychiatric nurses.
Material and methods. The study was conducted in a group of 100 nurses (94 women, 6 men) working in psychiatric wards in the Masovian Voivodeship. Variables were assessed using the proprietary sociodemographic questionnaire and the following questionnaires: Link Burnout Questionnaire (LBQ), Psychosocial Working Conditions, part III – Scale of psychosocial support, as well as the Scale of Psychosocial Risk, part D – Healthcare.
Results. All the results in the individual areas of occupational burnout were on an average level. Both the prevalence of psychosocial risks in the workplace and their stress induction were higher than the average score for indicators according to the standards for the health sector. The relationship between psychosocial factors and the areas of occupational burnout as well as received social support and the level of occupational burnout was revealed.
Conclusions. Occupational burnout in the group of psychiatric nurses relates primarily to the lack of commitment to relationships with patients and psychophysical exhaustion. The level of occupational burnout in psychiatric nurses depends on the characteristics of their work environment. Social support in the work environment reduces the risk of burnout in the group of psychiatric nurses. There is a need to identify the characteristics of the work environment that determine burnout in physiotherapists.

Effect of acupressure on prostaglandin F2α in primary dysmenorrhea: A randomized controlled trial

Abstract

Objectives. It is well documented that acupressure has a beneficial effect on reducing pain severity of primary dysmenorrhea; however, none of the previous studies had investigated the effect of acupressure on prostaglandin levels in females with primary dysmenorrhea. This study aimed to investigate the effect of acupressure on prostaglandin F2α (PGF2α) in primary dysmenorrhea.
Methods. A total of 50 adult females with primary dysmenorrhea participated in this study. Their ages ranged from 19 to 27 years and their body mass index (BMI) ranged from 20 to 25 kg/m2. They were randomized into 2 equal groups. Group (A) received nutritional modification for 3 consecutive menstrual cycles, while group (B) received the same nutritional modification in addition to acupressure at the liver point (LIV3) for 3 consecutive menstrual cycles. The primary outcome was plasma levels of PGF2α while the secondary outcome was short form of McGill pain questionnaire. The outcome measures were evaluated pre- and post- treatment.
Results. Comparing both groups post-treatment revealed that there were significant reductions in PGF2α levels and short form of McGill pain questionnaire scores (p < 0.0001) in favour of group (B).
Conclusions. Acupressure is effective in treating females with primary dysmenorrhea through reducing levels of PGF2α and scores of short form of McGill pain questionnaire.

It is difficult to imagine the modern world without information technology, and people without computers, laptops or smartphones. The threats posed by modern technologies are largely related to posture disorders. In the article, on the basis of the collected English-language literature, I consider the most typical of these threats, which is “text neck”, a disease little-known in Poland – text neck syndrome. This article tries to answer the question what “text neck” is.

Vacuum Assisted Closure Therapy Versus Polarized Light in The Treatment of Pressure Ulcer: A randomized controlled trial

Abstract

Purpose. This study was conducted to determine the effect of vacuum assisted closure therapy (VAC) versus polarized light or bioptron light therapy (BLT) in the treatment of pressure ulcers (PUs). Materials and Methods. Sixty patients of both sexes suffering from pressure ulcers classified between grade II and III according to the European Pressure Ulcer Advisory Panel (NPUAP), undergoing traditional medical treatment, participated in this study and were randomly assigned into two groups; group A, received polarized light 3 sessions per week for 8 weeks, and group B, received vacuum assisted closure therapy daily for 8 weeks. In addition to wound culture recording, wound surface area (WSA) and wound depth assessment were conducted before starting the treatment and at the end of treatment after 8 weeks of intervention. Results. Statistical analysis showed reduction in ulcer surface area and depth after treatment for both groups with high significant improvement in ulcer surface area and depth using polarized light. Conclusion. It can be concluded that both polarized light and vacuum assisted closure therapy had significant effect on pressure ulcers healing with high significant improvement in ulcer surface area and depth using polarized light compared with vacuum assisted closure therapy.

Attitudes of high school students towards people with disabilities

Abstract

Introduction. Social attitude is a relatively permanent system of beliefs, emotions, feelings and behaviors in relation to a given object/person, which is the result of interactions that an individual is subjected to throughout their life. The aim of the study was to assess attitudes of upper secondary school students towards people with disabilities.
Materiał i metody. 287 students (122 women and 165 men) were included in the diagnostic survey using the author’s survey and the Scale of Attitudes Towards Disabled People in upper secondary schools. The data were analyzed based on Chi-square test, Mann Whitney U test, ANOVA Kruskal-Wallis test, post hoc test for multiple comparisons.
Results. Statistically significant relationships were found between gender and irony and mockery behaviors for people with disabilities (p < 0.001) and participation in actions for the disabled (p < 0.001). Gender differentiated the attitude towards charity actions for the benefit of people with disabilities (p < 0.001), and the type of school diversified attitudes of high school youth towards the disabled (p = 0.003).
Wnioski. Men more often directed irony and mockery at people with disabilities. Women had a better attitude to charity campaigns for the disabled and participate in them more often. High school students had definitely more positive attitudes than representatives of vocational schools and technical secondary schools.
