Assessment of the legal situation of physiotherapists regarding the continuation of professional qualifications

Abstract

The profession of physical therapist is subject to specific legal regulation under the Act on the Profession of Physical Therapist of 25 September 2015, the adoption of which was motivated by the need to introduce comprehensive legal solutions taking into account the specific social character of that profession. Until that act took effect, i.e. until 31 May 2016, Poland had had no statutory provisions regulating the principles of pursuing that profession.
Analysis of the adopted legal regulations indicates numerous doubts that make it difficult to issue an unequivocal legal assessment of the factual conditions occurring in practice. The above statement refers, in particular, to the persons meeting the statutory criteria that want to continue the acquired professional qualifications.
The objective of this study is to demonstrate the doubts resulting from the adopted normative provisions, to attempt to explain them, and to formulate the de lege ferenda conclusions that would allow to respect that condition.

Assessment of physical endurance in students of the 1st and 2nd grade of secondary school

Abstract

Introduction. Physical endurance is a very important element of everyone’s health. It is a health aspect comprising the factors which may affect the level of physical endurance. That is why it is important to know what allows to keep it as high as possible.
Objective. The purpose of this study was to demonstrate the differences in the levels of physical endurance of students of 1st and 2nd grade of secondary school.
Materials and methods. The studies were conducted on secondary school students. It covered 70 students, including 37 women and 33 men. The physical endurance level was examined using the Harvard Step-Up Test. The study included analysis of the effects of sex, age, place of residence and BMI on the level of physical endurance.
Results and conclusions. The research results allowed to draw the conclusions that sex, place of residence or BMI are not statistically significant for the level of physical endurance of the persons studied. As for age, the research results may indicate that at 5% significance level, age is statistically relevant for the level of physical endurance – older students demonstrate more good and very good fitness results, while the number of average and poor physical endurance results decreases.

Abstract

Under the Act on the Profession of Physical Therapist, preventive physical therapy is one of the health services provided within physical therapy activities. Its task is to promote pro-health behaviours and to develop and maintain fitness and endurance of people of different age, for the purpose of preventing disability.There are three levels of preventive physical therapy – primary, secondary and tertiary. Primary preventive physical therapy is addressed at the whole population or at susceptible groups. Its purpose is to provide the knowledge of and skills for maintaining health using proper types of physical activity. Secondary preventive physical therapy covers the activities undertaken after the occurrence of the first signs of disease. Its purpose is to prevent the consequences of the diseases or to inhibit the development of the expected but not yet visible, signs. Tertiary preventive physical therapy includes the activities undertaken in the case of a developed disease or dysfunction which are not possible to cure. Its purpose is to prevent the exacerbation of signs and to provide optimum compensation mechanisms.The following persons are statutorily obliged to develop the life and special skills directed towards (propedeutic) competences in the scope of preventive physical therapy: parents, teachers, health promoters, caretakers, nurses, physicians and physical therapists.

Abstract

Patella stiffness is performed in situations when the panel of surgical procedures aimed at restoring the support function of the limb in the extensive deformation of the lower leg with pantalarne chronic pain syndrome has been exhausted. In the years 2010-2017, 31 comprehensive patella stiffness were performed. Treatments were performed in 27 men and 4 women, who underwent pre and post-operative physiotherapy and selected physiotherapy treatments. The work discusses the necessary information about comprehensive patella stiffness and details of the physiotherapeutic treatment of these patients.

Incidence of degenerative and proliferative changes in the spine

Abstract

The purpose of this paper is to conduct a retrospective analysis of the medical documentation concerning the patients treated by the Physiotherapy Unit of the Teodor Dunin Mazovian Hospital in Rudka in 2017, in order to determine the frequency of degenerative and proliferative changes in the spine depending on the subject’s age group, place of residence and sex.
Materials and methodology. The retrospective method was used to determine the incidence of spinal degeneration, which made it possible to examine a large numer of subjects – 1032 women and 553 men, 1589 subjects in total. The data was entared in the Excel database. The parcentage was calculated separately for all women and for all men. The Chi2 test was used for statistical analysis. Women and men were compared in terms of the distribution of results in the scope of particular analysed variables. Statistically significant results at level p < 0,05 are indicated by *.
Results. It has been concuded that degenerative and proliferative changes in the spine are the most common disease. They usually occur in 61-70 age group degenerative and proliferative L-S spinal changes are more often reported among women living in the country side, and more often for men living in urban areas.
Conclusions:
1. Degenerative and proliferative changes in the spine constitute the most common disease.
2. Degenerative and proliferative changes are more common in the L-S spine
3. The men are more likely to have degenerative and proliferative in LS spine than women.
4. The women are more likely to have degenerative and proliferative in Cspine than men.
5. People in 61-70 age group are most often affected by the degenerative disease of the spine.
6. Degenerative and proliferative spinal changes are more common among women living in the country.
7. Degenerative and proliferative spinal changes are more common among men living in urban areas.

Review of literature on the application of Kinesio Taping (KT) in internal diseases: in surgery, cardiology and pulmonology

Abstract

Introduction. So far, there have been few materials on Kinesio Taping (KT), in particular those meeting the criteria of scientific fact-based materials. The available literature includes articles on the possibility of using KT, but the results presented are not unequivocal. Some hypotheses and theories are contradictory, and many of them are not supported with any study results, and the conclusions presented seem questionable.
Objective. The objective of this study is to present the current state of knowledge of the possibility to apply the KT method in internal diseases: in cardiology and pulmonology.
Material and methods. The first stage was conducted from October to November 2017. It included preliminary selection aimed at identifying the works on the possibility of applying dynamic taping. The second stage of the research was conducted from December 2017 to January 2018, and consisted in a detailed analysis of the selected works on application and assessment of effects of dynamic taping in surgery, pulmonology and cardiology.
Results. When assessing the effects of KT in internal diseases in cardiology and pulmonology, the effects were classified as poor effects (PE), effects (E) and anti-effects (AE), when the results of the trial do not confirm the therapeutic activities. 2 E were discovered in assessing the effects of KT in surgery (2 articles). 3 E were discovered in the study of application in pulmonology (3rd article). No articles of the A reference level, concerning application of KT in cardiology.
Conclusions. Three conclusions were formulated on the basis of analysis of the source materials:
1. There has been a recent progress in the research into the possibility of applying dynamic taping in physical therapy, including in internal diseases: pulmonology and cardiology.
2. There are few prospective, randomized trials associated with application of dynamic taping, conducted on a large enough group of patients.
3. Few articles in the literature that explain the principles of DT, meet the criteria of research based on scientific facts.

Analysis of injuries of musculo-ligamentous-skeletal system occurring among handball players and physiotherapy treatment

Abstract

Introduction. Injuries in team games are an inseparable part of today’s sport. For every athlete it is connected with a decrease in exercise capacity, need for medical help and economic consequences. Professional rehabilitation of athletes, after a traumatic event, requires work with a physiotherapist with high skills and extensive experience and the use of modern physical therapy equipment. The lack of one of those factors can lead to further loss of health by the player, prolonged rehabilitation and thus delayed return to the game
Aim. The aim of the study was to assess the type and frequency of injuries occurring among professional female handball players and the type and methods of treatment used after the injury.
Material and methods. The studied group consisted of 19 women aged 18-31 years, playing handball for at least 5 years. All of the subjects played in the first team of Korona Handball Club in Kielce. In the study a diagnostic survey method was used.
Results. The study showed that 100% of players suffered from at least one injury connected to their professional sports activity. Most often the injury occurred while landing – 33.9%, fouls – 21.4% and throws – 19.6%. 12.5% (7) of the occurred traumas required surgery and the remaining 87.5% ended in conservative treatment. In spite of the fully recovery from the injury, a large proportion of the players – 68.4% were feeling discomfort after the resumption of training and during the matches.
Conclusions. The most common injuries of professional handball female players are sprains and dislocations of the lower limbs. Most athletes undergone conservative physiotherapy treatment including kinesiotherapy and wide range of physical therapy modalities.

Physical activity in hemodialyzed patients of the hospital in Värnamo

Abstract

Introduction. Patients with kidney failure engage in less physical activity. This leads to reduced muscle strength, exercise tolerance, and hemodynamic activity of the heart and the cardiovascular system. Such patients also experience a decreased sense of well-being. Patients who are qualified for hemodialysis (HD) undergo treatment 3-5 times a week. Due to the lengthy nature of the procedure (each session takes about 3-5 hours), it can be a heavy burden for the patients.
Purpose. The purpose of the study was to demonstrate the positive effects of regular physical activity in hemodialyzed patients.
Materials and methods. 23 adult patients (over 18 years old) were included in the study. Two patients did not give their consent to participate, and 8 were excluded due to comorbid conditions (infections, arrhythmia, fistula complications, lower extremity amputation), as they were unable to undergo all of the tests. The remaining 13 patients (57%) were tested for muscle strength, physical fitness, and balance. The tests were conducted before the start of a regular, individual physical training regimen and were administered before / during hemodialysis.
Results. In patients with chronic kidney failure who were undergoing regular hemodialysis at the time of the study, significant improvement in physical fitness (6 patients, 46%), lower extremity muscle strength (8 patients, 61%), upper extremity muscle strength (6 patients, 46%), and balance (4 patients, 31%) was observed after a six-month long physical training regimen, with training sessions of individually adjusted length and intensity conducted 3 times a week.
Conclusion. An improvement in physical fitness, lower and upper extremity muscle strength, and balance was observed in patients with kidney failure, who were treated regularly at the dialysis ward of the hospital in Värnamo (dialyzed at least 3 times a week), and who participated in a regular physical activity training before/during hemodialysis.

Physical therapy in severe cases of Guillain-Barré syndrome – case presentation, management

Abstract

Background. Guillain-Barre syndrome (GBS) is an acute neuropathy with an incidence ranging from 0.6 to 2.2 cases per 100,000. The aetiology of GBS has not yet been explained. 20% of GBS patients do not fully recover.
Material. The paper presents the clinical picture observed in two men with particularly severe GBS and the physical therapy and rehabilitation conducted in these patients.
Results. The patients were discharged from the Department of Rehabilitation after 6 and 18 months of treatment, respectively, and returned to work.
Conclusions. 1. Even though the course of GBS can sometimes be very severe, full recovery is possible. 2. Intensive physical therapy and rehabilitation should be conducted from the onset of the disorder, even if no improvement occurs over several months.

The use of deep water running in physiotherapy – possibilities and restrictions

Abstract

Deep Water Running, as a common way to counteract the decreasing cardio-pulmonary efficiency of body, has been known in many West European and North American countries since 1980s. This type of training was and still is willingly used by sportsman who due to injuries were temporarily excluded from normal trainings with a load of bone-joint apparatus.
Long-standing experiences of doctors and physiotherapists show that the skillfully applied Deep Water Running’s training has a lot of advantages, which can be also used by people suffering from such health problems as joints’ degeneration, obesity or physical coordination disorder. Only in a few health and rehabilitation centers in Poland the Deep Water Running is implemented. Due to health benefits of this type of training, it deserves an outline of its advantages and disadvantages as well as a brief description of the main methodological assumptions.
