Abstract
Purpose. To determine the effect of Transdermal Delivery of Insulin by Low Intensity Ultrasound in healing of chronic lower limb venous ulcer. Methods. Sixty males and females participants who had chronic lower limb venous ulcer participated in this study. Their ages ranged from 40 to 60 years, distributed randomly into: group (A), 30 participants who received Transdermal Delivery of Insulin by Low Intensity Ultrasound in addition to regular medical care, and group (B), 30 participants who received topical insulin in addition to regular medical care. Both groups were evaluated pre-treatment, after 4 and 8 weeks from the beginning of the treatment by using (image j) software for measuring the ulcer surface area and normal saline for measuring the ulcer volume. Results. By comparison between both groups post treatment there were statistically significant decreases (P < 0.05) in wound surface area and wound volume in favor of group (A). Conclusion. transdermal delivery of insulin is a safe, non-invasive and beneficial modality in the treatment of lower limb venous ulcer and can be added to the treatment protocols for these debilitating conditions.

Factors determining the knowledge of women aged 30-40 from the Rzeszów poviat about urinary incontinence

Abstract
Introduction. Urinary incontinence is any incident of uncontrolled leakage of urine from the urethra, regardless of the cause. The aim of the study was to assess the dependencies of se-lected factors with the level of knowledge about urinary incontinence in women.
Material and methods. The diagnostic survey using the author’s questionnaire covered 207 women aged 30 to 40 from the Rzeszów poviat, The data were analyzed based on Pearson’s Chi-square test (χ2).
Results. A statistically significant dependence was found between the level of education and the level of knowledge about urinary incontinence (p = 0.001).
Conclusions. Women with higher education have more knowledge about urinary incontinence than women with secondary and vocational education. Educational activities should be aimed mainly at women with secondary and vocational education. The elimination of risk factors should be encouraged, as well as the necessity of early diagnosis of urinary incontinence and immediate treatment.

Abstract
Background and objective. Postmenopausal women suffer from clinical symptoms such as vasomotor, psychological, physical, and genitourinary symptoms. 77% of postmenopausal women suffer from muscle and joint problems, loss of muscle strength and flexibility. This study was conducted to see effect of Pilates exercise on postmenopausal symptoms and lumbar flexibility.
Methods. Forty postmenopausal women at least 1 year from their last menstruation aged from 50-59 years there BMI does not exceed 30 kg/m2 were assigned randomly to 2 equal groups: Group A (study group) received calcium 500 mg/day, cholecalciferol (vitamin D3) (0.2mg)/day and Pilates exercises (hundred, saw, side kick spine stretch) for 45 minutes/ session, three times per week for 12 weeks. Group B (control group) only received calcium 500 mg/day and cholecalciferol (0.2 mg)/day for 12 weeks. Their menopausal symptoms were assessed through menopausal rating scale (MRS) and lumbar flexibility assessed by (modified Schober test, sit to reach test and fingertip to floor test) before and after treatment period.
Results. There was a significant decrease in MRS post treatment results in the study and control group compared with that of pretreatment ones (p > 0.01). there was a significant decrease in the post treatment results of MRS of Group A (study group) compared with that of Group B (control group). Regarding flexibility There was a significant increase in sit and reach distance, modified Schober test distance and significant decrease in fingertip-to-floor distance post treatment results in the study and control group compared with that pretreatment results (p < 0.001), there was a significant increase in lumbar flexibility of Group A (study group) compared with that of Group B (control group) post-treatment results.
Conclusion. Pilate’s exercise has a positive effect on postmenopausal symptoms and lumbar flexibility.

Aerobic exercise versus mediterranean diet on insulin resistance in obese prediabetic postmenopausal women: A randomized controlled study

Abstract
Purpose. To find out the effect of Mediterranean diet and exercise on weight reduction and their influences on insulin resistance in obese prediabetic postmenopausal women.
Methods. Randomized controlled trial. This study was carried out on 40 obese postmenopausal women with sedentary life and with insulin resistance. They were selected from Cairo University Medical Hospitals and divided randomly into two equal groups; group A, Diet group (n = 20), enrolled on a Mediterranean diet for 12 weeks and group B, Exercise group (n = 20), enrolled on exercise training for 12 weeks. For each patient the weight, height, body mass index (BMI), waist and hip circumferences and ratio and Hemoglobin A1c were estimated and recorded before and after intervention.
Results. After intervention, the Mediterranean diet and exercise groups showed significant weight reduction, lower BMI, waist and hip circumferences and ratio and HbA1c. However, there was significant differences between both groups. Mediterranean diet showed significant reduction in all weight parameters and HbA1c than exercise group.
Conclusion. The Mediterranean diet resulted in better weight reduction and improvement of insulin resistance than exercise in obese prediabetic postmenopausal women.

Abstract
Introduction. One of the more common ailments of young women in the reproductive period is dysmenorrhea (MSM). More than 50% of women complain of menstrual pain, and 10% of them are forced to give up their daily activities for the first 1-3 days of the menstrual cycle.
Objective of the work. The aim of the research was to check to what extent the selected techniques on soft tissues in strictly defined places will reduce the intensity of pain sensations occurring in the painful menstruation syndrome.
Material and methods. 23 women aged 19 to 48 participated in the study. Women with cyclical IBM-related pains that had been repeated for at least a year were invited to the study. Pain complaints were examined according to the VAS scale, which allows the assessment of pain on a scale of 1-10. Each visit began with the completion of a questionnaire. Thanks to it, pain sensations and other ailments or observations related to the menstrual cycle for 3 consecutive menstrual cycles were checked.
Results. The most common complaints were pain in the area of the lower abdomen, lumbar spine or head. There was a positive difference in the assessment of pain scale, pain duration and the assessment of improvement in health during menstruation. There was also a significant decrease in pain medication intake from 86 at the first treatment meeting, where only 21 was indicated at the last meeting.
Conclusions. The results of the presented studies have shown that the therapy of soft tissues at specific points has a positive effect on chronic menstrual pain and the reduction of painkillers. The author, through a detailed analysis of the subject of UBM, hopes that the presented content will increase the prevention and awareness of patients in the subject of UBM.

Abstract
Purpose. To investigate the effect of aerobic exercise and ketogenic diet on type 2 diabetic obese pre-menopausal women.
Methods. Thirty premenopausal obese women diagnosed as having type2 diabetic mellitus. They were selected from outpatient clinic of gynecology in t AL Kasr AL Ainy hospital Cairo University.. Their BMI was between 30 and 35 kg/m2 and their ages ranged from 45 to 48. They were divided into two groups equal in number, the study group (A) was treated by aerobic exercise, ketogenic diet and oral hypoglycemic drugs prescribed by physician, while control group (B) was treated by oral hypoglycemic drugs prescribed by physician. Blood glucose levels were evaluated by fasting blood glucose test and body mass index was evaluated by weight and height scale before and after performing study for 4 weeks.
Results. The obtained results showed a statistically significant decrease in weight, BMI and fasting blood glucose level post treatment in the study group compared with pre treatment (p < 0.001) while there was a significant decrease in fasting blood glucose in the control group post treatment (p < 0.001) and no significant changes in weight and BMI of the control group (p > 0.05). There was significant decrease in fasting blood glucose, weight and BMI of the study group compared with that of the control group (p < 0.001).
Conclusion. Ketogenic diet and Aerobic exercise have a significant effect in decreasing Weight, BMI and Fasting blood glucose level in Type-2 Diabetes Mellitus, obese pre-menopausal women.

Abstract
Introduction. Our study aims to present the state of Polish outpatient physiotherapy covered by the National Health Fund (NHF) based on a set of original indices to parametrise the structure of services and the physiotherapy process.
Materials and methods. The study used the NHF reporting data on outpatient physiotherapy collected between January 2017 and October 2019. The analysis used a set of originally developed indices.
Results. Of all outpatient physiotherapy cycles, generally provided on consecutive working days, 70% lasted 10 days and 84% were provided to patients without a formal confirmation of severe disability. From the patient’s perspective, 41% of the time of services is provided in a one-on-one basis (index of the structure of services), while the kinesiotherapy accounted for 30% of total time of services. The dominant group of services, about 70% in quantitative terms, is physical therapy.
Conclusions. To satisfy the health needs of patients by publicly funded outpatient physiotherapy is still a challenge and creates a perspective to develop a new way of organising and financing outpatient physiotherapy. The proposed indices of service structure and process enable an accurate diagnosis of the current situation, development of optimal system solutions and monitoring their implementation.

Assessment of changes in the electrical activity of muscles in classic squat with a barbell in powerlifting athletes and people training Crossfit

Abstract
Aim. Assessment of the degree of muscle involvement during a squat with a barbell in powerlifters and Crossfit trainers using surface electromyography (sEMG).
Material and methods. The study included two groups of 30 people exercising Crossfit and powerlifting, with an average age of 24 ± 2.8 years. Inclusion criteria: male gender, age range-22 to 28 years and a minimum of two years of training experience. Muscle tension measurements were made using the EMG FREEEMG300 electromyograph, in accordance with the SENIAM guidelines. The study consisted of a total of twelve repetitions of squats with a barbell, in two series of frontsquat and backsquat techniques.
Results. The mean and mean maximum muscle tone, each time statistically significant differences were noted – in the Crossfit group it was higher than in powerlifting.
Conclusions. The involvement of muscles during the squat with the bar and with the load equal to the body weight, performed using the “back squad” and “front squat” techniques is significantly higher in competitors training Crossfit. The analysis of the subjective assessment of difficulties in performing the exercises showed that the level of difficulty depends on the technique of performing the exercises. Reported difficulties of exercises – 50% of crossfiters using the “back squat” technique and 100% of powerlifters using the “front squat” technique.

Abstract
Purpose. This study aimed to investigate the effect of intermittent stretching of inferior capsule of frozen shoulder patient on shoulder range of motion (ROM) (shoulder flexion, abduction, and external rotation), pain intensity level, shoulder function, and handgrip strength. Materials and Methods. Randomized controlled trial. Overall, 30 patients of both genders with ages ranged from 40-55 years with a frozen shoulder in the second stage were recruited randomly and divided into two equal groups; Group (A) (control group), received conventional treatment of frozen shoulder for 4 weeks three times per week, day after day and Group (B) (experimental group), received the same conventional treatment plus intermittent stretching with mobilization for 4 weeks three times per week, day after day. Pre- and post-treatment assessment of the pain intensity level was done by numerical pain rating scale (NPRS), patients’ ROM was assessed by Goniometer-Pro App, handgrip strength was assessed by Jamar hand dynamometer and shoulder function by The American Shoulder and Elbow Surgeons evaluation form. Results. There were statistically significant differences between pre and post-treatment of each group, and significant differences post-treatment between the two groups, where statistical analysis showed greater improvement in pain intensity level, shoulder function, ROM, and handgrip strength (p < 0.05) in favor of the experimental group (B). Conclusion. Intermittent stretching of the inferior capsule is more effective when used with conventional physical therapy than conventional physical therapy alone in adhesive capsulitis patients.

Immediate effects of kinesiotaping of the neck on lung function and respiratory muscle strength in stable COPD patients

Abstract
Background. Chronic obstructive pulmonary disease (COPD) is complicated by the presence of frequent and recurrent acute exacerbations. The patients are asymptomatic and stable in the early stages. They develop weakness of primary respiratory muscles and use accessory muscles which lead to postural changes. The forward head posture in the neck is very common in COPD patients due to increased usage of accessory muscles and aging changes. The objective of this study is to determine the immediate effect of Kinesio-taping on lung function and respiratory muscle strength in stable COPD patients and age matched healthy individuals.
Methods. 20 stable COPD patients and 20 age matched asymptomatic participants were assessed for the pulmonary function test by Spiro lab machine and respiratory muscle strength using Micro RPM machine before applying the KT. The outcomes were repeated 30 minutes after applying the KT for correcting the forward head posture. The with-in group and between group comparison of the variables before and after the application of KT was done using student t-test.
Results. A significant correlation was found in the baseline parameters of age (p = 0.011); PEF (p = 0.004) and physical activity levels
(p = 0.039). The application of Kinesio tape proved to be beneficial by improving the lung and respiratory parameters of FVC (p = 0.002) and MEP (p = 0.22) among the COPD group.
Conclusions. Our study concluded that, application of kinesio-tape can be an adjunct to conventional breathing exercises for COPD patients. There is improved lung function and reduced rate of perceived exertion after application of kinesio-tape.
