Abstract
Background. The Achilles tendon is the strongest tendon in the human body. Despite the high incidence of injuries and a well-known problem of treating heel tendon injuries, there has been no consensus on the proper treatment, (both surgical and rehabilitation) in professional literature so far. Choosing an operating method that gives “better” treatment results can facilitate the choice of a more effective treatment. It should also be remembered that a properly conducted rehabilitation process is an extremely important element following the surgery.
Aim of the study. The study aimed to evaluate the effectiveness of the surgical treatment of patients with Achilles tendon injuries using the minimally invasive and classic methods.
Material and Methods. The study material included 62 patients treated surgically for Achilles tendon injury in the Department of Orthopedics and Traumatology of CSK MSW in Warsaw in the years 2011-2016. 31 patients were operated using the minimally invasive method (the study group) and 31 patients using the conventional method (the control group). The research group consisted of 27 men and 4 women. The comparative group consisted of 27 males and 4 females. The following research tools were used to evaluate the treatment results: 1. Author’s questionnaire 2. AOFAS score (American Orthopedic Foot and Ankle Society Score) 3. Measuring instrument for maximum force moments in ankle joint JBA “Staniek” 4. HUR stabilographic platform 5. Ultrasonography.
Results. 4 patients, (12.9%) in the control group re-ruptured the tendon after operative treatment. In the study group, 2 patients (6.5%) experienced a complete heel tendon rupture in the postoperative period. The problem of postoperative wound healing disorders was observed among 2 patients (6.5%) from the study group. In the group treated with the classical technique, 5 (16.1%) patients experienced a delay in the healing of the surgical wound due to bacterial infection. Paresthesia, from the sural nerve was observed in only 1 patient in the MIS (Minimal Invasive Surgery) group. 27 patients (87.1%) returned to their sports activity after percutaneous surgery, and 22 patients (71%) after conventional treatment. On average, patients from both groups needed 6 months to return to sports activities.
Conclusions. 1. The number of renewed ruptures after minimally invasive treatment of a damaged Achilles tendon is low and comparable to that of a group of patients treated with the classic technique. 2. Postoperative wound healing disorders are rare in both groups. 3. Calf nerve injury was reported only in one case, while using a minimally invasive procedure. 4. Patients return to work and sports at the same time after surgery in both groups which is influenced by properly implemented physiotherapy 5. Patients presented a similar balance control on the stabilographic platform in both groups. 6. Similar values of maximum force moments in ankle joint were recorded in both groups. 7. In the ultrasound image, there were no morphological differences in Achilles tendon formation in either study group.

Abstract

Diagnosis of human body posture and assessment of functionality, in physiotherapeutic procedures, is mainly carried out by viewing methods and by photogrammetric or videogrammetric methods, which do not provide angular measurements, anthropometric, time-space measurements, etc. Highly specialized imaging tests are medical procedures, which significantly affects their availability. Also, in terms of their economics, application and invasiveness. Thus, their purpose is significantly limited in the initial diagnostics process, and in particular in current physiotherapeutic assessment.
In the world, for over 30 years, gait, balance, selected biomechanics parameters and body functionality have been evaluated by pedobarography. However, other scientific studies of the authors indicate that the method is very little known in Poland by medical staff. Despite the fact that it has been included in the list of guaranteed benefits since 2011, no procedures for its implementation have been specified. An important aspect of pedobarography is the consistency of its directions of use with the standards of training the profession of a physiotherapist. The obvious and logical conclusion is that the lack of knowledge in the area of the method does not allow its development. An important issue is to assess whether education standards are directly consistent with the directions of pedobarography in the rehabilitation process.

Evaluation of manual dexterity after treatment with botulinum type A(B), selected kinesitherapeutic methods: a case study

Abstract
Introduction. Botulinum treatment involves the reduction of spasticity in patients qualified for this type of treatment. The main aim of this treatment is to increase the quality of life by improving functionality. In some situations, however, objective tests indicate a deterioration of movement parameters, and at the same time the patient reports improved performance of basic self-care activities. In this case, the decision to continue the therapy depends on the patient’s subjective feelings.
Material and methods. The study concerns a 20-year-old patient with right-sided spastic paresis resulting from the surgical treatment of left thalamic haemangioma in 2008. The patient was qualified for botulinum toxin treatment under the NFZ programme in 2019. The above-mentioned therapy was applied in combination with selected methods of neurophysiological rehabilitation. The aim was to maximize the positive effects of the therapy: to improve the range of mobility and reduce associated reactions, with minimal loss of manual activities of the right upper limb. Tests were performed to assess muscle tone according to the Ashworth scale, and precision grip according to the “Guidelines of the National Council of Physiotherapists for Health Services”. Additionally, the range of motion in the hand joints was measured using the vision system. A subjective 10-degree scale was proposed to assess the incidence of associated reactions during everyday activities.
Results. The results obtained indicated discrepancies between the objective results of standard tests and the patient’s subjective evaluation. According to the patient, despite the loss of some of her hard-earned mobility patterns, her quality of life improved.
Conclusion. Botulinum injection combined with physiotherapy slightly increased the range of motion in the hand joints, decreased the associated reactions, and slightly improved manual dexterity.

Abstract
Place of physical therapy with particular emphasis on exercises and massage in the treatment of patients with Ankylosing SpondylitisThe process includes the sacroiliac joints, spinal joints and ligaments, fibrous rings and peripheral joints. From 0.1% to 0.5% of the population in Central Europe suffer from AS. Treatment of patients with AS requires, first of all, combining pharmacotherapy with non-pharmacological treatment including mainly: physical exercise, massage, participation in self-help groups, education, manual therapy, hydrotherapy, electrotherapy, acupuncture, balneotherapy, spa therapy, systemic cryotherapy, lifestyle modification. So far, no single, proper algorithm for non-pharmacological treatment of patients with AS has been developed.

Abstract
Diseases of the temporomandibular joints (TMJ) constitute an interdisciplinary problem with many symptoms related to the masticatory muscles and the structures that make up the joint.
Pain is the main reason why patients seek help from specialists. Due to the specificity of the area and the complicated structure, the temporomandibular joint is exposed to various types of dysfunctions. Most diseases or dysfunctions of the temporomandibular joint (85–90%) are treated conservatively. Physical therapy plays an important role in the process of restoring fitness and improving the quality of life. It is an increasingly appreciated method of treating disorders within the masticatory system, also due to the problematic area for radical (surgical) procedures.

Abstract
Elderly people constitute the largest group of beneficiaries of physical therapy. It results from the higher incidence of chronic diseases in this age group, as well as the decline in functional performance, and thus increasing dependence on other people. Therefore, geriatric rehabilitation to a large extent bears the hallmarks of disability prevention. The problem of an elderly patient is usually very complex. This results from multiple morbidity, lower rehabilitation potential, lack of motivation, social and financial problems, etc. As a result, in the case of an elderly person, recovery of fitness similar to levels observed before illness is often not possible. Great emphasis in the management of an elderly patient should be placed on the prevention of hypokinesia and its effects. Due to the consequences of immobilization, patients should avoid staying in bed, and if this is not possible, try to reduce this time, quickly mobilize and verticalize themselves. The effects of immobilization may in many cases be more serious than the underlying disease. The purpose of this article is therefore to show the point of view of physical therapy in geriatrics.

Abstract

Achilles tendon is the largest and strongest tendon in the human body. Due to its key function in motor skills and the currently observed increase in popularity of various sports disciplines, it is subject to more frequent injuries. This requires the introduction of appropriate therapeutic measures. The main goal of the whole treatment process is to ensure the full recovery of tendon function. In cases of a full tendon ruptures, the treatment of choice is surgery unless it is contraindicated for general medical reasons. In both cases, proper physiotherapy is needed. In the article, the authors present an algorithm for physiotherapy after traumatic injury to the Achilles tendon.

Predictors for choosing the profession of physiotherapist. A longitudinal study 2011-2016

Streszczenie

Introduction. The aim of this work was to examine the predisposition of the physiotherapist profession selection among the group of students in the College of Rehabilitation. Three variables were considered: personality, mental needs and recognized values.
Methods. The examined group included students of physiotherapy from Warsaw. 427 students have been tested. The research was carried out on groups during the Years 2011–2016 (to check the reproducibility of behaviour and repeatability of predictors).
Methods used to conduct the research:
1. Questionnaire of personality (R. Rohr)
2. Self-portrait Stein (by Choynowski) examining the psychological needs
3. Questionnaire values (M. Rokeach)
Results. Physiotherapy students exhibit a specific personality type (Guardian of Value), for whom the willingness to work and function in a group is extremely important. Group values ​​and group loyalty are also important. Physiotherapy students are also characterized by (Mediator) mediation skills, making contacts – which is very important in working with patients. Important needs governing their life choices are: the need to help, serve others and compensation, which favors ambitions, perseverance in performing tasks and their consistent implementation. For the surveyed students, individual goals are the most important. The values ​​through which they want to implement them are moral.
Conclusions. The research conducted among physiotherapy students in the years 2011–2016 on the factors contributing to professional career choices demonstrated a regular occurrence of Guardian of Value and Mediator personality type. Psychological needs such as the need to assist, to serve others and to compensate also played a major role. Types and hierarchies of values were repeated across the study groups.

Abstract
The correct assessment of functioning and determination of the health needs of the senior using standardized scales and questionnaires is necessary to determine his ability to live independently and to determine the extent of therapeutic intervention and necessary care.
Objective
Assessment of the relationship between physical, emotional and intellectual fitness, and sociodemographic features of residents of the Social Welfare Home (SWH).
Material and methods. 80 people aged 70 to 90 years were examined. The study consisted of three parts: proprietary survey, anthropometric parameters assessment, assessment of functional, intellectual and emotional state of SWH residents.
Results. The obtained variables showed the relationships, i.e. education and physical fitness; gender and physical fitness; gender and tendency to depression; physical activity and functional fitness; physical activity and depression; social and motor activity and depression. In the second group, statistical analysis showed no relationship between age and depression, and education and depression.
Conclusions
1. An increased body mass index has a negative impact on the functional state of the SWH residents.
2. Regular physical activity, positively affects the functional, emotional and intellectual state of SWH residents.
3. Gender and level of education has a significant impact on the functional, emotional and intellectual performance of SWH charges.
4. Physical and social activity of SWH residents has an impact on the reduction of depression symptoms.

Physiotherapy in patients after myocardial infarction

Abstract
Background. Myocardial infarction is a serious social and health problem of modern society. Cardiac rehabilitation is a very important part of management in this group of patients.
Aim. To assess the efficacy of rehabilitation and its effects on cardiovascular and respiratory performance in patients after myocardial infarction.
Materials and methods. The study assessed a group of 30 patients who participated in 3-week day-care cardiac rehabilitation at the District Health Care Institution in Starachowice.
The effectiveness of rehabilitation was analysed using an original study card, divided into two parts (history taking and physical examination). The first part included personal and medical history and a scale for subjective assessment of exertion during daily activities, similar to the Borg scale. The other part included daily blood pressure and pulse measurements taken before and 5 minutes after cycle ergometer training. Dependent variables were verified before rehabilitation and after 15 days of rehabilitation.
Results. After rehabilitation, patients showed permanent cardiovascular adaptation to more intense exercise, reflected by reduced systolic and diastolic blood pressure values and a lower heart rate.
Conclusions. 1. Cardiac rehabilitation helped improve physical performance in patients after myocardial infarction. 2. Systolic pressure was reduced as early as after the first training and permanent cardiovascular adaptation to more intense exercise was seen in study patients, which was reflected by normal blood pressure and pulse values. 3. Rehabilitation had a positive influence on patient functioning in everyday life.
