Assessment of the legal situation of physiotherapists regarding the continuation of professional qualifications

Abstract

The profession of physical therapist is subject to specific legal regulation under the Act on the Profession of Physical Therapist of 25 September 2015, the adoption of which was motivated by the need to introduce comprehensive legal solutions taking into account the specific social character of that profession. Until that act took effect, i.e. until 31 May 2016, Poland had had no statutory provisions regulating the principles of pursuing that profession.
Analysis of the adopted legal regulations indicates numerous doubts that make it difficult to issue an unequivocal legal assessment of the factual conditions occurring in practice. The above statement refers, in particular, to the persons meeting the statutory criteria that want to continue the acquired professional qualifications.
The objective of this study is to demonstrate the doubts resulting from the adopted normative provisions, to attempt to explain them, and to formulate the de lege ferenda conclusions that would allow to respect that condition.

Abstract

Under the Act on the Profession of Physical Therapist, preventive physical therapy is one of the health services provided within physical therapy activities. Its task is to promote pro-health behaviours and to develop and maintain fitness and endurance of people of different age, for the purpose of preventing disability.There are three levels of preventive physical therapy – primary, secondary and tertiary. Primary preventive physical therapy is addressed at the whole population or at susceptible groups. Its purpose is to provide the knowledge of and skills for maintaining health using proper types of physical activity. Secondary preventive physical therapy covers the activities undertaken after the occurrence of the first signs of disease. Its purpose is to prevent the consequences of the diseases or to inhibit the development of the expected but not yet visible, signs. Tertiary preventive physical therapy includes the activities undertaken in the case of a developed disease or dysfunction which are not possible to cure. Its purpose is to prevent the exacerbation of signs and to provide optimum compensation mechanisms.The following persons are statutorily obliged to develop the life and special skills directed towards (propedeutic) competences in the scope of preventive physical therapy: parents, teachers, health promoters, caretakers, nurses, physicians and physical therapists.

Abstract

Patella stiffness is performed in situations when the panel of surgical procedures aimed at restoring the support function of the limb in the extensive deformation of the lower leg with pantalarne chronic pain syndrome has been exhausted. In the years 2010-2017, 31 comprehensive patella stiffness were performed. Treatments were performed in 27 men and 4 women, who underwent pre and post-operative physiotherapy and selected physiotherapy treatments. The work discusses the necessary information about comprehensive patella stiffness and details of the physiotherapeutic treatment of these patients.

Analysis of injuries of musculo-ligamentous-skeletal system occurring among handball players and physiotherapy treatment

Abstract

Introduction. Injuries in team games are an inseparable part of today’s sport. For every athlete it is connected with a decrease in exercise capacity, need for medical help and economic consequences. Professional rehabilitation of athletes, after a traumatic event, requires work with a physiotherapist with high skills and extensive experience and the use of modern physical therapy equipment. The lack of one of those factors can lead to further loss of health by the player, prolonged rehabilitation and thus delayed return to the game
Aim. The aim of the study was to assess the type and frequency of injuries occurring among professional female handball players and the type and methods of treatment used after the injury.
Material and methods. The studied group consisted of 19 women aged 18-31 years, playing handball for at least 5 years. All of the subjects played in the first team of Korona Handball Club in Kielce. In the study a diagnostic survey method was used.
Results. The study showed that 100% of players suffered from at least one injury connected to their professional sports activity. Most often the injury occurred while landing – 33.9%, fouls – 21.4% and throws – 19.6%. 12.5% (7) of the occurred traumas required surgery and the remaining 87.5% ended in conservative treatment. In spite of the fully recovery from the injury, a large proportion of the players – 68.4% were feeling discomfort after the resumption of training and during the matches.
Conclusions. The most common injuries of professional handball female players are sprains and dislocations of the lower limbs. Most athletes undergone conservative physiotherapy treatment including kinesiotherapy and wide range of physical therapy modalities.

The use of deep water running in physiotherapy – possibilities and restrictions

Abstract

Deep Water Running, as a common way to counteract the decreasing cardio-pulmonary efficiency of body, has been known in many West European and North American countries since 1980s. This type of training was and still is willingly used by sportsman who due to injuries were temporarily excluded from normal trainings with a load of bone-joint apparatus.
Long-standing experiences of doctors and physiotherapists show that the skillfully applied Deep Water Running’s training has a lot of advantages, which can be also used by people suffering from such health problems as joints’ degeneration, obesity or physical coordination disorder. Only in a few health and rehabilitation centers in Poland the Deep Water Running is implemented. Due to health benefits of this type of training, it deserves an outline of its advantages and disadvantages as well as a brief description of the main methodological assumptions.

Shaping ethical attitude of physiotherapist

Abstract

Common behavioural and ethical patterns existing in modern medicine are subject to many social debates in which physiotherapist, alongside ethicists, philosophers and doctors, have their say. All of them express their points of view, which more often than not, coincide. The differences in opinions constitute the consequences of applying deontology in certain professional cases. Forming behavioural patterns in the process of academic studies is a hard and long-lasting task. Further development of ethical attitudes of a physiotherapist will be influenced by work. Even though physiotherapists embark upon their professional paths with already formed attitudes, these will be prone to further changes, both negative and positive, depending on the environment.

Eligibility of COPD patients for physiotherapy based on a six minute walk test and treadmill test

Abstract

Objective. The aim of the paper was to assess the results of physiotherapy of COPD patients whose elgibility for the relevant rehabilitation model was assessed with the help of the 6-MWT test and the treadmill test according to the modified Bruce protocol.
Materials and methods. 80 individuals suffering from COPD treated as in-patients at the MSWiA Specialist Hospital in Głuchołazy between May 2017 and September 2017 were examined. After a 3-week comprehensive physiotherapy, the exercise stress test, spirometry test and dyspnoea assessment on the modified 10-point Borg scale were repeated in both groups of COPD patients.
Results. Calculations were performed using the IBM SPSS software. In order to assess consistency between the distribution of the results with the normal distribution, the Kolmogorov-Smirnov test was used. As the distribution differed from the normal distribution, nonparametric methods were used for further analysis. In order to assess the significance of differences between the two groups in terms of quantitative variables, Mann-Whitney tests were used, while Wilcoxon tests were used for dependent samples to assess the significance of changes over time in the same patients (bound pairs). The statistical significance threshold was set at p < 0.001.
Conclusions
1. A significant improvement was noticed in the level of exercise stress tolerance after physiotherapy both among patients assessed according to the modified Bruce test and patients assessed with the 6-minute walk test.
2. A significant improvement was shown in the values of spirometric indicators after physiotherapy both among patients assessed according to the modified Bruce test and patients assessed with the 6-minute walk test.
3. A significant decrease was shown in the level of dyspnoea after physiotherapy both among patients assessed according to the modified Bruce test and patients assessed with the 6-minute walk test.

Abstract

Purpose. The purpose of the study was to evaluate thickness and symmetry of Transversus Abdominis muscle (TrA) during rest and activation with the use of Rehabilitative Ultrasound Imaging (RUSI) and the assessment of sonofeedback effectivness in learning and control of TrA activity.
Material and methods. The study was conducted on 28 healthy subjects (17 women and i 11 men) aged 19-22 years (mean 21±1,08 years). The subjects undergone RUSI assessment considering thickness of TrA in rest and during contraction. Basing on results the Activation Ration and Percent of Muscle Change were calculated.
Results. Mean thickness of TrA in rest was 3,19 mm among women and 4,37 mm among men. During controlled activity thickness of TrA increased by 69% on right side and by 64% on left side and respectively by 70% and 64% among men. The results showed a significant difference in TrA thickness between right and left side at rest and during contraction among men. During sonofeedback recorded thickness of TrA was significantly higher comparing to results without control of sonofeedback. TrA activity was lower in the group with low back pain and it was 64%, while in pain-free group the activity was 72%.
Conclusions.
1. RUSI is an effective method to evaluate the morphology and activity of the Transversus Abdominis muscle at rest and during controlled activation;
2. The decrease in thickness and activity impairment of TrA is related to the occurrence of low back pain;
3. The use of sonofeedback enables significant improvement in activation and equalization of TrA asymmetry.

Assessment of physical activity effectiveness in pain alleviating of the lumbar spine pain during pregnancy

Abstract

Spine pain is common affliction during pregnancy. It is caused by body mass increase, which creates additional burden for structures of lumbar-sacral spine section and pelvis. Centre of gravity and body position are changed, which causes balance disorder of motor system. Asymmetry in mobility of sacroiliac joints appears, as well as irritation of sacroiliac ligaments.
The treatment of this problem includes pharmacological methods, physiotherapy and most important, variety of physical exercises.
The main goal of this study was assessment of different therapies effectiveness on lumbar spine pain during pregnancy.
Material and methods. The study group consisted of 41 women, who gave birth at least once. To gather information a survey method was used.
Results. Physical activity was pointed as one of the most efficient method of preventing spinal pain – 47,4. Hydrotherapy was chosen by 21% respondents, while massage and kinesiotaping only 15,8% both. According to 15,2% of questioned pain was gone as a result of systematical exercises.
Conclusions. Systematical physical activity before pregnancy was declared by the vast majority of respondents, while during pregnancy by half of questioned.
According to respondents, physical activity before and during pregnancy does not affect lumbar spine pain.

Abstract

Introduction. Kinesiology Taping is a popular method used not only by physiotherapists and physicians, but also by trainers, due to the lack of patient involvement and the economic factor. Numerous scientific publications on kinesiotaping are based on a randomized, usually double-blind tests. Unfortunately, in most scientific articles there is no information on the tape used in the tests, which makes it impossible to repeat the test and obtain the clinical response proven by the authors.
Aim of the tests. The implication for the tests was the comparison of several types of tapes used in dynamic taping in terms of selected mechanical properties.
Material and methods. Seven types of tapes were used for the tests: Nitto Denko, Acu Top, Cure Tape, Nastara, Kinesio Tex Gold, Novama, 3NS Tex. Instron E 3000 endurance machine was used for testing of mechanical properties. Two types of tests were carried out in the tape testing: monotonic stretching test and cyclic stretching test. The obtained results were analyzed using Office Excel 2010 and Statistica 12.
Results. Nitto Denko (154.90 N) and Novama (118.41 N) tapes had the highest static strength, while Acu Top (25.19 N) and 3NS Tex (18.54 N) tapes had the lowest static strength. The results from the cyclic stretching test showed higher values of force necessary for the Nitto Denko and Novam tapes (6.78 and 19.97 N, respectively). The lowest force needed for tape stretching was shown by 3NS Tex and Nastara tapes (3.95 and 4.68 N, respectively). The level of difference between the tested tapes was statistically significant (p < 0.001).
Conclusions. The tapes available on the Polish market differ in terms of mechanical properties.
