Santrauka
Fonas. Daugkartinio kiaušidžių sindromas (PCOS) yra dažniausia endokrininė būklė reprodukcinio amžiaus moterims ir yra laikomas rizikos veiksniu kardiometabolinei ligai. Priklasomai nuo taikomų diagnostinių kriterijų, maždaug 10–15% reprodukcinio amžiaus moterų kenčia nuo PCOS. Tikslas. Ištirti, kaip vidaus organų manipuliavimas veikia hormoninį profilį moterims, sergančioms PCO. Dizainas. Perspektyvus atsitiktinės imties vienakryptis kontroliuojamas tyrimas. Vieta. Sadato bendrosios ligoninės ambulatorijos.
Metodai. Trisdešimt PCO moterų pacienčių buvo pritrauktos ir atsitiktine tvarka suskirstytos į dvi lygias grupes: kontrolinė grupė gavo mitybos gaires terapijos pavidalu, t.y., hipokalorinę Viduržemio jūros dietą 12 savaičių, o tyrimo grupė gavo tas pačias kontrolinės grupės intervencijas papildomai su vidaus organų manipuliavimu (VM). Biocheminė analizė (FSH ir LH) ir KMI buvo pagrindinis rezultatas. Visi kintamieji buvo išmatuoti bazinėje linijoje ir po 12 savaičių intervencijos.
Rezultatai. Statistinė analizė buvo atlikta naudojant porinių skirtumų testą, kuris parodė reikšmingą abiejų grupių pagerėjimą. Todėl, naudojant mišrų dizaino MANOVA ir kelis porinius palyginimo testus (post hoc testus) KMI ir LH/FSH santykio po gydymo atveju, buvo nustatyta, kad LH/FSH santykis žymiai sumažėjo (p=0.007) naudai B grupei, palyginti su A grupe, su nereikšmingais KMI skirtumais tarp abiejų grupių (p=0.443), rodydami, kad VM grupė (B) yra efektyvesnė nei grupė (A) biocheminėje analizėje (p<0.05).
Išvada. Naudojant vidaus organų manipuliavimą su standartine hipokalorinės Viduržemio jūros dietos programa turi daugiau naudingų poveikių hormoniniam profilui moterims, sergančioms PCO.

Streszczenie
Wstęp. Ból miednicy o podłożu miofascjalnym (MFPP) charakteryzuje się obecnością wrażliwych punktów spustowych mięśniowo-powięziowych (MTrPs) w mięśniach i powięzi dna miednicy. Terapia manualna dna miednicy jest często proponowana jako pierwsza linia leczenia MFPP.
Istniejąca literatura potwierdza korzystne wyniki terapii manualnej w leczeniu MFPP i oferuje zwięzłe podsumowanie, jak te techniki są stosowane u pacjentów z MFPP. Literatura zawiera dziesięć technik; (1) uwalnianie punktów spustowych miofascjalnych, (2) masaż Thiele, (3) wewnętrzny samomasaż, (4) masaż krocza, (5) łączone techniki manualne, (6) mobilizacja miofascjalna miednicy, (7) manualna terapia narządów wewnętrznych, (8) manipulacja tkanki łącznej, (9) uwalnianie blizny, oraz (10) wewnętrzna manipulacja kości guzicznej.
Metody. Przeprowadzono przegląd narracyjny w celu podsumowania dostępnych dowodów na temat technik terapii manualnej dna miednicy dla MFPP.
Kluczowe treści i wyniki. Po przeglądzie aktualnych badań, dwadzieścia pięć badań spełniło kryteria włączenia. Dostępne dowody sugerują, że terapia manualna dna miednicy jest skuteczna w leczeniu MFPP.
Wnioski. Chociaż obecnie dostępne badania są ograniczone pod względem liczby i mają pewne ograniczenia w projekcie badania, terapia manualna jest uważana za obiecującą, skuteczną i bezpieczną opcję leczenia MFPP.

Laser Acupuncture: Could it be a New Complementary Intervention in Management of Peripheral Arterial Disease?

Abstract
Laser acupuncture has produced promising results in the management of many medical conditions; it has been used as a non-invasive therapy with no puncture pain which represents a major advantage over traditional acupuncture or electroacupuncture. The research question of this study is: Does the Aluminum Gallium Arsenide (AlGaAs) LASER acupuncture affect the lower limb blood supply and functional walking ability in patients with peripheral arterial disease (PAD)?. Thirty male patients’ diagnosis with PAD were recruited from Kasr El-ainy Hospital, and were randomly divided into two equal groups: study group and control group. Both groups received the routine medical treatment whereas study group received laser acupuncture therapy twice weekly for five weeks. Ankle brachial pressure index (ABPI), six-minute walk test (6MWT), and rating of perceived exertion using modified Borg scale were measured. The 2×2 mixed design MANOVA showed that laser acupuncture has induced highly statistically significant improvements in all the measured outcomes (p < 0.05). However, results revealed no significant difference between the two groups except for modified Borg scale. It was concluded that, AlGaAs Laser acupuncture could be an effective non-pharmacological intervention for improving circulation and functional walking performance in PAD patients and may be recommended as a complementary therapy in the management protocols of PAD.

Vacuum Assisted Closure Therapy Versus Polarized Light in The Treatment of Pressure Ulcer: A randomized controlled trial

Abstract

Purpose. This study was conducted to determine the effect of vacuum assisted closure therapy (VAC) versus polarized light or bioptron light therapy (BLT) in the treatment of pressure ulcers (PUs). Materials and Methods. Sixty patients of both sexes suffering from pressure ulcers classified between grade II and III according to the European Pressure Ulcer Advisory Panel (NPUAP), undergoing traditional medical treatment, participated in this study and were randomly assigned into two groups; group A, received polarized light 3 sessions per week for 8 weeks, and group B, received vacuum assisted closure therapy daily for 8 weeks. In addition to wound culture recording, wound surface area (WSA) and wound depth assessment were conducted before starting the treatment and at the end of treatment after 8 weeks of intervention. Results. Statistical analysis showed reduction in ulcer surface area and depth after treatment for both groups with high significant improvement in ulcer surface area and depth using polarized light. Conclusion. It can be concluded that both polarized light and vacuum assisted closure therapy had significant effect on pressure ulcers healing with high significant improvement in ulcer surface area and depth using polarized light compared with vacuum assisted closure therapy.

Abstract
Background. Interstitial cystitis is a range of urological manifestations that are characterized by bladder, pelvic and urethral pain, just as irritative voiding side effects. It is characterized by the International Continence Society as the protest of suprapubic pain, identified with bladder filling combined by different manifestations, for example, expanded day time and evening time recurrence, without demonstrated urinary contamination or different evident pathology of the lower urinary tract. Objective. To investigate the effect of interferential electrical stimulation on pain perception and disability level on interstitial cystitis. Participants and methods. A total of 40 volunteering women diagnosed with interstitial cystitis. Their ages were ranged between 25 to 40 years old and their body mass index was > 30 kg/m2. Participants were randomly assigned to two equally numbered groups; group (A) receiving interferential current at the lower abdomen, in addition to a routine medical intervention, or group (B) receiving solely routine medical intervention for 8 successive weeks. Participants were assessed for pain using visual analog scale (VAS), related disability index using levels using O’Leary–Sant Symptom Index or Interstitial Cystitis Index (ICSI), and blood cortisol concentration. Measurements were taken before and after eight weeks of intervention. Results. The analysis of the results revealed a significant reduction in VAS score (P < 0.0001), ICSI scores (P < 0.0001), and the plasma cortisol concentration (P < 0.0001) in the participants of group (A) at the end of the treatment, compared to group (B). Conclusion. These results concluded that adding IC therapy to routine medical intervention had an excellent effect on the management of interstitial cystitis associated signs and symptoms particularly pain, plasma cortisol levels, related disabilities.

Mechanical Response of Hip and Knee Muscles Following Randomized Crossover Trials in Patellofemoral Pain Syndrome

Abstract
Background. Faulty hip kinematics during weight bearing activities is proposed to contribute to patellofemoral pain syndrome (PFPS). However, limited information exists to determine the effectiveness of exercises programs that not only act on the knee joint, but also on the hip joint in patients with PFPS. Purpose. The purpose of this study was to compare between the effect of the starting with hip strengthening exercises before knee exercises program and the starting with knee exercises program before hip strengthening exercises on pain intensity, Kujala questionnaire scale, quadriceps angle (Q-angle), anteversion angle, and eccentric peak torques (PT) for hip abductors, external rotators, and knee extensors in patients with PFPS. Materials and Methods. Twenty-four patients suffering from PFPS were randomly assigned into two equal groups of twelve. Group (A): their mean age, weight, height and BMI values were 23.33 ± 5.39 years, 71.16 ± 13.05 kg, 164.75 ± 4.5 cm, and 26.21 ± 4.71 kg/m² respectively. They received hip abductors and external rotators strengthening exercises for three weeks followed by knee extensors strengthening exercises and stretching exercises for hamstring quadriceps, iliotibial band and gastrocnemus for another three weeks. Group (B): their mean age, weight, height and BMI values were 23.16 ± 6.33 years, 69.41 ± 18.14 kg, 164.66 ± 7.27 cm, and 25.2 ± 6.2 kg/m² respectively. They received knee extensors strengthening exercises and stretching exercises for hamstring, quadriceps, iliotibial band and gastrocnemus for three weeks followed by hip abductors and external rotators strengthening exercises for another three weeks. Both groups received three to four sessions per week for six weeks. Pain level, Kujala scale, Q-angle, anteversion angle, and isokinetic eccentric PT for hip abductors, external rotators, and knee extensors were recorded before, after three weeks, and after six weeks of exercises. Results. 2×3 Mixed Design MANOVA revealed that there was a significant reduction in level of perceived pain and improvement of Kujala scale in group (A) compared with group (B) after six weeks of exercise (p < 0.05). However, there was no significant difference in the Q-angle, anteversion angle, and isokinetic eccentric PT for hip abductors, external rotators, and knee extensors between the tested groups after six weeks of exercise (p > 0.05). Conclusion. Starting rehabilitation program with hip strengthening exercises before knee exercises program is more effective than starting with knee exercises program before hip strengthening exercises in reducing pain and improving knee function in patients with PFPS. Consequently, this may help physiotherapists in designing the most effective and efficient prevention and rehabilitation programs for patients suffering from PFPS.

Abstract
Aim: to explore the effect of forward head posture (FHP) on temporomandibular joint (TMJ) proprioception in post-pubertal females.
Material and Method. Forty post-pubertal females participated in this study. They were classified in accordance with the craniovertebral angle (CVA) into two groups. The Forward head posture (FHP) group had CVA of less than 48° (n = 21), while the control group had CVA of more than 48° (n = 19). TMJ repositioning accuracy had been measured by vernier caliper for females in both groups.
Results. Statistical analysis revealed that there were no significant (p > 0.05) differences between both groups regarding TMJ repositioning errors in all directions (mouth opening, protrusion and lateral movements) with eyes open or closed.
Conclusion and Discussion. In post-pubertal females, FHP has no negative effect on TMJ proprioception in all directions whether the eyes are open or closed.
