The role of hippotherapy in children rehabilitation

Abstract
Hippotherapy is one of the forms of animal therapy (zootherapy). It is defined as a therapeutic action to improve human health and its functioning, with the help of a horse and riding on its back. The aim of the study was to present current reports on hippotherapy, as well as its popularization as a method that supports traditional rehabilitation. The theoretical part of the thesis discusses the mechanism of the horse’s therapeutic impact on humans, including its impact on the physical, emotional, motivation, cognitive and social spheres. The basic assumptions of hippotherapy are also presented – indications, contraindications, choice of a horse, the profession of a hippotherapist and the principles of conducting therapeutic activities with children. In the research part, in order to present up-to-date reports, a review of scientific articles from Poland and around the world, published in 2015-2019, on the effectiveness of hippotherapy activities and their impact on the psychomotor development of children with CPR, ASD and ADHD. These articles were found in the PubMed medical database. Twenty publications were selected that met all the search criteria. Most of the analyzed studies showed a positive effect of hippotherapy on the functioning of ill children. The most common effects are, the improvement of motor functions (including body posture, gait and balance), improvement of behavior, and better social functioning. It was confirmed that hippotherapy can complement comprehensive rehabilitation among children with CPR, ASD and ADHD, as well as motivate to therapy and exercise. On the basis of the discussed data, conclusions were also drawn that further research is needed, which would take into account larger groups of respondents and would show the long-term effects of therapy with the participation of a horse.

Abstract
Zootherapy has been introduced in recent years as an adjunct approach to traditional physiotherapy in patients with cerebral palsy (CP). Hippotherapy is less popular in Poland compared to other European countries or the USA, with few reports of investigations carried out in accordance with the EBM paradigm. The aim of the present study was to evaluate the usefulness of hippotherapy in comprehensive rehabilitation of children with cerebral palsy.The study involved 40 cerebrally palsied children who were randomly divided into two equal-sized groups (A and B). Both groups received home-based rehabilitation according to the Proprioceptive Neuromuscular Facilitation (PNF) method. Group A additionally received hippotherapy. The children’s motor function was determined using the Gross Motor Function Measure (GMFM).In Group A was 0.02 for ChP=5.226, 1 df. The parameter determined in the study exceeded this critical value, thus allowing the conclusion that the effect of home-based rehabilitation combined with hippotherapy on overall health improvement was statistically significant (p<0.05). In Group B, the right-tailed critical value (p) was 0.429 for Chi2=0.625, 1 df. The parameter determined in the study did not exceed the critical value, thus not allowing the statement that home-based rehabilitation had a significant effect on overall health improvement.1. The study shows significantly better results of rehabilitation in children treated with combination of PNF and hippotherapy. 2. The results indicate the need for randomized prospective studies involving larger test groups.

Abstract
Background. The parents’ satisfaction with life and their acceptance of their child’s illness are important factors that directly and / or indirectly influence the process of rehabilitation of children with cerebral palsy. The aim of this study was to evaluate the effect of hippotherapy on the level of satisfaction with life and acceptance of illness among parents of children with cerebral palsy. Material and methods. The study was conducted in 2008-2010 among 94 parents of cerebrally palsied (CP) children partici­pating in hippotherapy sessions in Warsaw. The study used an original questionnaire with questions dealing with the effect of hippotherapy on selected psychological factors in the parents. The questionnaire was completed by the parents twice: before and after a one-year hippotherapy course. Two psychometric scales: the Satisfaction with Life Scale (SWLS) and the Acceptance of Illness Scale (AIS), were also used, both adapted for Polish by Juczyński. The results were analyzed statistically.Results. Overall satisfaction with life among the parents increased significantly (p<0.05) during the year-long hippotherapy course. There was also a statistically significant increase in the parents’ acceptance of their children’s condition. Conclusion. Hippotherapy in CP children significantly increases their parents’ satisfaction with life and acceptance of their children’s illness.

Wpływ hipoterapii na wybrane czynniki fizyczne i psychiczne u dzieci z autyzmem

Streszczenie
Wstęp. Autyzm jest uznawany za zaburzenie rozwojowe, prawdopodobnie determinowane biologicznie i przejawiające się w różnych sferach funkcjonowania dziecka.
Cel. Celem badań była ocena wpływu rocznej hipoterapii na funkcjonowanie fizyczne i psychiczne u dzieci z autyzmem w wybranych obszarach.
Materiał i metody. Rodzice i dzieci chore na autyzm w wieku 5–7 lat, którzy w ostatnich 2 latach przebyły terapię z wykorzystaniem hipoterapii. W celu dokonania oceny wpływu rocznych oddziaływań wychowawczo-edukacyjnych na poszczególne sfery rozwojowe dzieci z autyzmem skorzystano ze zmodyfikowanej skali PEP-R, uzupełnionej kwestionariuszem własnej konstrukcji na temat cech demograficzno-społecznych oraz w zakresie przebiegu choroby. W analizie statystycznej stosowano test t-Studenta oraz współczynnik korelacji Pearsona. Za statystycznie znamienne przyjmowano te wyniki testów, dla których poziom istotności był mniejszy lub równy 0,05 (p < 0,05).
Wyniki. W toku rocznego procesu terapeutycznego z wykorzystaniem hipoterapii nastąpiła poprawa w rozwoju motoryki i reakcji emocjonalnych u dzieci z autyzmem. Wysoka motoryka dzieci z autyzmem ma pozytywny wpływ na sferą emocjonalną. Niższy stopień autyzmu cechuje wyższy poziom funkcjonowania fizycznego i psychicznego dzieci z autyzmem niż dzieci z umiarkowanym i dużym stopniem autyzmu.
Wnioski. Hipoterapia jest jednym z obszarów terapii osób z autyzmem.
