Fizjoterapia Polska 2021; 21(2); 122-137

Abstract
Introduction. The 21st century is characterized by the dynamic development of the automotive industry and other industries. Along with them, the pace of our life increases, resulting in an increased trauma rate due to rush and traffic accidents. When looking for attractions, young people often get into conflicts or risk to impress their peers. As a result, traumatology grows into a vast branch of medicine, and mandibular fractures account for an increasing percentage of injuries. Objective. The objective of the study was to assess the usefulness of physiotherapy in patients after orthopaedic and orthopaedic-surgical treatment of mandibular fractures for faster recovery of the masticatory organ function, and to introduce physiotherapy as a standard in treatment of patients after mandibular fractures. Material and methods. The study involved a group of 20 patients aged 18–60 who suffered a mandibular fracture as a result of an injury. The patients were diagnosed and treated at the Clinical Department of Cranio-Maxillofacial Surgery and Oncology of Norbert Barlicki’s University Clinical Hospital no. 1 in Lodz. Results. The obtained results showed significant differences between patients who underwent physiotherapy after treatment of a mandibular fracture and those who did not want to participate in it. Conclusions. This allowed for the conclusions to be made that the problems of patients after immobilization due to a mandibular fracture are significant and should constitute the starting point for the development of an appropriate physiotherapeutic treatment algorithm for these patients. It would also be advisable to promote physiotherapy as standard treatment in such cases.

Liliia Orel, Valentyn Savchenko, Olga Baitaliuk, Nataliia Oblovatska – Legal Aspects of the Informed Consent of the Patient During Physical Therapy in Ukraine. Fizjoterapia Polska 2021; 21(2); 78-83

Abstract
The purpose of the study is to establish the legal features of the use of informed voluntary consent of the patient in providing him/her with physical therapy services in Ukraine. Materials and methods. The analysis of the normative-legal base of Ukraine with the use of methods of analysis, synthesis, comparative-legal method is carried out. Research findings. Currently Ukraine has developed and approved the form of primary accounting documentation “Informed voluntary consent of the patient for diagnosis, treatment and surgery and analgesia” and instructions for filling out this form. This form is completed by a patient who has applied to a health care facility and agrees to undergo diagnostic and treatment measures, and if necessary the patient agrees to surgery and analgesia. However, this form is not suitable for legal relations between a physical therapist and a patient. The main features of the application of this consent are determined and its model for use in the practice of physical therapists is developed. Conclusions. The principle of patient autonomy during physical therapy requires regulation of the patient’s right to provide him/her with full information about physical therapy, coordination and approval of the written form of voluntary informed consent of the patient to physical therapy.

Thirumalaya Balaraman, Zbigniew Śliwiński – The effectiveness of a case based online video assignment in improving practical performance of physiotherapy students – a quasi experimental study. Fizjoterapia Polska 2021; 21(2); 30-34

Abstract
Aim. In this technologically advancing world, online teaching has become omnipresent in all fields. However, teaching online is still a challenge in allied health professional courses where hands-on skills are the core component of the curriculum. Hence, the aim of this paper was to assess the effectiveness of a case based combined online video and written assignment in improving the practical performance and satisfaction of the cardiorespiratory physiotherapy course students. Material and Methods. At INTI International University (INTIIU), we provided a case scenario to the cardiorespiratory physiotherapy course students as an assignment. First, the students need to submit their written answers in the Blackboard (BB) Learning Management System. They also need to record a video of the patient assessment procedure for the given case scenario and upload it in the BB. The student’s practical performance before the online assignment in the coursework Objective Structured Clinical Examination (OSCE) and after the assignment in the Final OSCE was assessed to find the difference. Results. The paired t-test showed a significant difference between coursework (M = 66.47, SD = 7.04) and final OSCE (M = 77.49, SD=10.11) performance; t (13) = −3.48, p = 0.004, which indicates that the students’ performance in the final OSCE was significantly better than coursework OSCE. The students’ perception about the course and their engagement in the BB was better. Conclusions. The case-based video and written assignment is a successful strategy to improve the practical performance of the students in a physiotherapy course where practical skills are imperative.

Abstract
The complex pathophysiology of spasticity depends on the interaction between facilitation and inhibition of spinal and supraspinal neural centres and pathways. Contemporary medicine has developed several methods to treat spasticity. However, none of them has offered superior efficacy. This study aimed to investigate differences in the degree of spasticity between patients subjected to traditional post-stroke rehabilitation and rehabilitation based on the PNF method.A total of 64 stroke patients were enrolled. They were randomly divided into Group A and Group B. The modified six-grade Ashworth scale was used to evaluate the level of spasticity. During a 21-day follow-up, all subjects underwent intensive comprehensive rehabilitation. Group A received traditional stroke therapy. Group B received kinesiotherapy based on the PNF method. Both groups demonstrated significant improvements. In Group A, the degree of improvement in the final examination was 1.8% compared to baseline. In Group B, an improvement of 7.8% was registered already after the first session. The degree of improvement at the end of treatment was 16.86%. No significant inter-group differences were noted at baseline. The final evaluation showed significant differences.A regimen based on the PNF method resulted in greater reduction in spasticity than the traditional approach.

Dariusz Białoszewski, Izabela Korabiewska, Monika Lewandowska, Katarzyna Wasiak – The Usefulness of Hippotherapy in the Rehabilitation of Cerebrally Palsied Children. Pilot Study. Fizjoterapia Polska 2011; 11(2); 175-181

Abstract
Zootherapy has been introduced in recent years as an adjunct approach to traditional physiotherapy in patients with cerebral palsy (CP). Hippotherapy is less popular in Poland compared to other European countries or the USA, with few reports of investigations carried out in accordance with the EBM paradigm. The aim of the present study was to evaluate the usefulness of hippotherapy in comprehensive rehabilitation of children with cerebral palsy. The study involved 40 cerebrally palsied children who were randomly divided into two equal-sized groups (A and B). Both groups received home-based rehabilitation according to the Proprioceptive Neuromuscular Facilitation (PNF) method. Group A additionally received hippotherapy. The children’s motor function was determined using the Gross Motor Function Measure (GMFM).In Group A was 0.02 for ChP=5.226, 1 df. The parameter determined in the study exceeded this critical value, thus allowing the conclusion that the effect of home-based rehabilitation combined with hippotherapy on overall health improvement was statistically significant (p<0.05). In Group B, the right-tailed critical value (p) was 0.429 for Chi2=0.625, 1 df. The parameter determined in the study did not exceed the critical value, thus not allowing the statement that home-based rehabilitation had a significant effect on overall health improvement.1. The study shows significantly better results of rehabilitation in children treated with combination of PNF and hippotherapy. 2. The results indicate the need for randomized prospective studies involving larger test groups.

Analysis of career development among Physiotherapy graduates based on a profiled electronic survey. Pilot study

Dariusz Białoszewski, Joanna Gotlib

Dariusz Białoszewski, Joanna Gotlib – Analysis of career development among Physiotherapy graduates based on a profiled electronic survey. Pilot study. Fizjoterapia Polska 2011; 11(3); 213-226

Abstract
Background. The amended Higher Education Act, mandatory for all Polish university-level schools since 1st October 2011, introduces an
obligation to analyse the career development of graduates as part of procedures aiming to assess the quality of education (Art. 13a). The objective
of this study was to analyse the career development of those graduating from Bachelor-level and Master-level programmes in Physiotherapy using
a profiled electronic survey.
Material and methods. A total of 132 persons (students of 26 university-level schools) responded to an on-line survey profiled especially for
the specific degree programme. Women accounted for 74% of the study group and men represented 21% [A 5% TO OBOJNAKI? ] . The average
age of respondents was 25 years (range: 21-39; SD=3.9). Forty-six per cent of participants graduated from Bachelor-level programmes (AI) and
54% completed Master-level studies (AII). Statistical analysis was carried out with STATISTICA 9.0 (licensed to WMU) and based on the Mann-
Whitney U test (p<0.05).
Results. Thirty-five per cent of graduates started to search for employment immediately after obtaining the diploma (p=NS). Twenty per cent
of the respondents looked for a job for less than one month; most used the Internet for that purpose (p=NS). Most participants worked in
kinesiotherapy (p=NS). The largest proportion of AI and AII found employment thanks to the help of family members. Over 40% of the respondents
would like study towards obtaining a specialisation in physiotherapy, while more than 25% were not aware of such a possibility. Twenty per cent of
AI and thirty-three per cent of AII used the services of a Career Office. A majority considered working abroad but only a small group was familiar
with the conditions of undertaking employment in other countries.
Conclusions. 1. Research on career development among graduates of Physiotherapy degree programmes based on profiled, targeted on-line
questionnaires sent directly to graduates seems to represent a good method of studying their career paths. 2. It is necessary to supplement the
research on career development among graduates of Bachelor-level and Master-level programmes with a survey among their potential employers.
3. It seems necessary to make available wider information about undertaking employment abroad by physiotherapists as well as to intensify and
promote the activity of university-based Career Offices. 4. As informal relations play an important role in the process of finding jobs among
physiotherapists, it is advisable to encourage employers to use job competitions more widely in order to eliminate such practices. 5. The study
results represent a starting point for wider, extended and prospective multi-centre research.

Historical outline of neuromobilisation in Poland and worldwide

Przemysław Zdunek

Przemysław Zdunek – Historical outline of neuromobilisation in Poland and worldwide. Fizjoterapia Polska 2012; 12(1); 77-81

Abstract
The objective of the study is to present a historical outline of neuromobilisation and clinical neurodynamics -in Poland and worldwide. Following the development of non-invasive methods of treatment, the knowledge of nervous system mechanics has become more and more extensive. The first record of performing a neurodynamic test goes back to 2800 BC. Many years later, in the first and second half of 20th century, Bragard, Von Lanz and Wachsmuth described three main neurodynamic tests for an upper limb. The biggest worldwide input in neuromobilisation problems development was made by the following scientists: Geoffrey Maitland, Robert Elvey, David Butler, Michael Shacklock and Alf Breig. David Butler and Michael Shacklock definitely represent the key researchers since it is them who, during their cooperation in the period of 1990-1997, made a great input in the origins of neuromobilisation treatment methods. The concept of nervous system mobilisation in Poland was initiated in April 1998 in Zgorzelec, where the first course run by David Butler himself, was held. In the same year Zbigniew Śliwiński published his first two studies in the journal entitled “Manual Medicine”.At present trainings, courses and numerous research works, focused on nervous system mechanics, are conducted in Poland. The most recent and worth noting publications are these referring to neuromobilisation effectiveness and performed by the team of Dwornik, Białoszewski and colleagues.

Krystyna Boroń-Krupińska, Marek Woźniewski – The impact of relaxation on chronic pain intensity and level of depression. Fizjoterapia Polska 2012; 12(1); 49-57

Abstract
The objective of this study was to demonstrate the relationships between relaxation, pain intensity and level of depression. The study was conducted among forty five (N = 45) patients with pain, including 23 females and 22 males aged 60–80 years. The studied participants were randomized in three groups; each group consisted of fifteen subjects with equal number of males and females. The first group participated in relaxation training and standard physiotherapy, the second group participated only in standard physiotherapy and the control group was a „placebo” group who had conversations with relaxation trainer, without training and standard physiotherapy. Pain intensity was measured twice every day using VAS scale, before and after the activities. The level of depression was verified at the baseline and after two weeks using Geriatric Depression Scale. The analysis of the differences between average values for dependent samples was performed using Wilcoxon’s signed-rank test while the analysis for independent samples was carried out using Kruskal Wallis test. The correlations between the studied parameters were calculated using Spearman’s correlation ratio. The study showed pain intensity decrease in all studied groups, to the large extent in the relaxation and physiotherapy group (49.4% and 44.2% respectively). The level of depression significantly decreased in the relaxation group (7.9%). Depression level is a significant factor related to pain intensity. Relaxation resulted in the decrease in pain intensity and depression level.
