Effect of Upper Limb Exercises on Blood Flow in Hemodialysis Patients: A Randomized Controlled Trial

Abstract
Background. Kidney failure patients exhibit a_considerable decline in blood flow (in diameters, end-diastolic velocity & peak systolic velocity of ulnar, radial, brachial arteries & cephalic vein) & also functional capacity (in data of handgrip strength, heart rate, oxygen saturation) that had profound effects on patients’ basic needed activities during the day. Physical exercises might be effective in slowing the rate or even reversing that decline. Aim. To investigate the exercises effect including the upper limb (Wrist flexion-extension, Elbow flexion-extension, & Hand open-close) on blood flow in those patients receiving hemodialysis treatment. Materials and Methods. Sixty men patients who performing hemodialysis were participated in the existing study, collected from Al-Badrashin central hospital in Giza, the age of the patients ranged from 40 to 50 years, CKD. They were randomly allocated into two groups of equal number. The study & control groups, study group who received medications, hemodialysis & breathing& upper limb exercises three times each week for a_duration of six weeks and the other control group who received also medications, hemodialysis but, without training. Assessment of the Patients were done with the aid of by ultrasound machine sonoscape, pulse oximeter & hydraulic hand dynamometer prior & after the study. Results. The results revealed that all evaluated parameters except heart rate were have a_statistical significant that observed as an improvement in the study group if compared to the other control group in terms of oxygen saturation, diameters, end-diastolic velocity & peak systolic velocity of brachial, ulnar, radial arteries & cephalic vein & strength of handgrip (5.87, 11.17, 30.39, 11.11, 19.44, 12.89, 10.14, 16.11, 6.55, 22.32, 10.82, 17.11, 0.75 & 36.32 respectively). There were no observed changes of significant value after treatment between both groups concerning heart rate, the end-diastolic velocity of ulnar& radial arteries. Conclusion. It can be concluded that exercises of upper limb enhance the flow of blood in patients receiving hemodialysis.
