Abstract
This report aims to alert clinicians to the importance of comprehensive evaluation starting with collecting the details of patient’s history. This case also highlights the need to prioritize interventions starting with conservative treatments based on severity and urgency. We report a case of cervicalgia and bilateral cervical radiculopathy of 62 years old female who did not respond to conservative physical therapy treatment. The specialist used total assessment- reassessment and evaluation using biokinesiology (TAREK) approach for clinical reasoning and judgment to come up with tentative physical therapy diagnoses. The manifestations were confirmed by magnetic resonance imaging report. The clinical manifestations were in harmony with the radiological findings. The patient was poorly responding to conservative treatment with many weeks of no response. Surgery was offered to ease up patient’s symptoms. Finally, clinicians should be familiar with the both the primary and the secondary pathological changes of cervical radiculopathy and put in consideration the complexity of clinical manifestations and poor prognosis.

Abstract

Background. Cervical radiculopathy (CR) is known as a severe neuromusculoskeletal condition which causes pain and physical disorders that both impair employment and quality of life. Spinal mobilization with arm movements (SMWAMs) is one of the methods in Mulligan’s techniques while residual discomfort is assumed to originate from the spine. Objective. This study aimed to assess the effect of SMWAMs on cervical proprioception and functional abilities in patient with cervical radiculopathy. Methods. Forty subjects of both sexes with chronic unilateral CR participated in the study, they were randomly assigned into two equal groups, Group A (experimental group): received SMWAMs in addition to a guide line protocol, group B (control group): received the guide line protocol, over four consecutive weeks, all subjects received 12 sessions. Outcome assessments included cervical proprioception using the cervical range of motion (CROM) and functional abilities using the cervical disability index (NDI) were assessed at baseline and 4 weeks’ post-intervention. Results. There was a statistical significant improvement in the experimental and control groups 4 weeks post-treatment for cervical proprioception and functional performance (p < 0.05). Concerning groups there was a significant difference between both groups regarding all measured variables in favor to the experimental group (p < 0.05). Conclusion. SMWAMs provides an additional effect in the management of chronic Cervical radiculopathy patients as it proprioception and functional performance.

Abstract

Purpose. To evaluate the effect of neural mobilization with or without mechanical intermittent traction on neck and arm pain, as well as functional disability in patients with unilateral cervical radiculopathy. Materials and methods. Thirty patients suffering from unilateral cervical radiculopathy were randomly assigned into two equal groups. Group (A) received only neural mobilization for 4 weeks (n = 15), while group (B) received neural mobilization plus mechanical intermittent traction for 4 weeks (n = 15). All patients in both groups were evaluated pre- and post-treatment with visual analogue scale (VAS) to assess neck and arm pain intensities, and neck disability index to assess functional disability. Results. Comparing both groups post-treatment showed that there was a_statistically significant reduction in neck and arm pain intensities, as well as functional disability (p < 0.05) in favor of group (B). Conclusions. Neural mobilization plus mechanical intermittent traction yield greater reduction in neck and arm pain intensities, as well as functional disability than neural mobilization alone in cervical radiculopathy patients.
