Fizjoterapia Polska 2021; 21(2); 122-137

Abstract
Introduction. The 21st century is characterized by the dynamic development of the automotive industry and other industries. Along with them, the pace of our life increases, resulting in an increased trauma rate due to rush and traffic accidents. When looking for attractions, young people often get into conflicts or risk to impress their peers. As a result, traumatology grows into a vast branch of medicine, and mandibular fractures account for an increasing percentage of injuries. Objective. The objective of the study was to assess the usefulness of physiotherapy in patients after orthopaedic and orthopaedic-surgical treatment of mandibular fractures for faster recovery of the masticatory organ function, and to introduce physiotherapy as a standard in treatment of patients after mandibular fractures. Material and methods. The study involved a group of 20 patients aged 18–60 who suffered a mandibular fracture as a result of an injury. The patients were diagnosed and treated at the Clinical Department of Cranio-Maxillofacial Surgery and Oncology of Norbert Barlicki’s University Clinical Hospital no. 1 in Lodz. Results. The obtained results showed significant differences between patients who underwent physiotherapy after treatment of a mandibular fracture and those who did not want to participate in it. Conclusions. This allowed for the conclusions to be made that the problems of patients after immobilization due to a mandibular fracture are significant and should constitute the starting point for the development of an appropriate physiotherapeutic treatment algorithm for these patients. It would also be advisable to promote physiotherapy as standard treatment in such cases.

Dariusz Białoszewski, Izabela Korabiewska, Monika Lewandowska, Katarzyna Wasiak – The Usefulness of Hippotherapy in the Rehabilitation of Cerebrally Palsied Children. Pilot Study. Fizjoterapia Polska 2011; 11(2); 175-181

Abstract
Zootherapy has been introduced in recent years as an adjunct approach to traditional physiotherapy in patients with cerebral palsy (CP). Hippotherapy is less popular in Poland compared to other European countries or the USA, with few reports of investigations carried out in accordance with the EBM paradigm. The aim of the present study was to evaluate the usefulness of hippotherapy in comprehensive rehabilitation of children with cerebral palsy. The study involved 40 cerebrally palsied children who were randomly divided into two equal-sized groups (A and B). Both groups received home-based rehabilitation according to the Proprioceptive Neuromuscular Facilitation (PNF) method. Group A additionally received hippotherapy. The children’s motor function was determined using the Gross Motor Function Measure (GMFM).In Group A was 0.02 for ChP=5.226, 1 df. The parameter determined in the study exceeded this critical value, thus allowing the conclusion that the effect of home-based rehabilitation combined with hippotherapy on overall health improvement was statistically significant (p<0.05). In Group B, the right-tailed critical value (p) was 0.429 for Chi2=0.625, 1 df. The parameter determined in the study did not exceed the critical value, thus not allowing the statement that home-based rehabilitation had a significant effect on overall health improvement.1. The study shows significantly better results of rehabilitation in children treated with combination of PNF and hippotherapy. 2. The results indicate the need for randomized prospective studies involving larger test groups.

Ewa Godula, Dariusz Szurlej, Bartosz Szurlej – Extracorporeal Membrane Oxygenation (ECMO) treatment and physiotherapy of the patient with Acute Distress Syndrome (ARDS). Fizjoterapia Polska 2012; 12(4); 397-403

Abstract
Acute Respiratory Distress Syndrome (ARDS) is a life-threatening condition, requiring urgent medical intervention. Pneumonia, shock, sepsis, multiorgan trauma or other morbidities may cause an alveolar-capillary barrier failure, which leads to ARDS. Sometimes the etiology of ARDS is not fully known, what makes the treatment difficult. In some cases, the usage of conventional mechanical ventilation techniques may be insufficient. The patients may have a chance of survival through using Extracorporeal Membrane Oxygenation (ECMO). Although this procedure does not treat the pulmonary disease, it gives more times for the lungs to regenerate and the approach enables essential treatment and gives the physician an ability to find the factor causing acute respiratory failure. Nevertheless, the use of ECMO is not completely harmless to human body and entails the risk of numerous complications of which the most dangerous are bleeding into the body and bleeding from the cannulation areas. The team dealing with ECMO consists of a cardiac surgeon, an anesthesiologist and a perfusionist. Experience of the team and proper cooperation enables the treatment to be as effective as possible. Well selected and conducted rehabilitation is the element of the treatment, which reduces effects of a long-term hypokinesia, has a positive effect on a psychological area, and fast motor and respiratory mobilization can shorten the time of the treatment and reduce the costs of expensive therapy. The paper presents the case of a 19-year old woman with acute respiratory failure, who was qualified for veno-venous ECMO therapy and the authors’ own experiences in physiotherapy during the treatment.

Fizjoterapia Polska 2021; 21(1); 178-186

DOI: https://doi.org/10.56984/8ZG079497

Abstract
Introduction. When performed according to the most recent standards, rehabilitation and nebulization enable the prevention of progression of cystic fibrosis.
Aim. The aim of the study was to assess the parents’ knowledge concerning the disease, education of pro-health attitudes, taking care of pulmonary rehabilitation equipment, and following the recommendations on how to perform inhalations correctly.
Materials and methods. Seventy-four parents with at least one child affected by cystic fibrosis participated in a survey, where we applied our own questionnaire.
Results. A majority of the parents claim to possess knowledge about the disease and to be familiar with the specialist terminology; however, a control question does not confirm that. Only 50% of the respondents take advantage of a positive influence of physical effort on the respiratory function, whereas 50% of the children assume incorrect positions during nebulization, and merely half of the parents maintain nebulization equipment in the right way.
Conclusions. Parents still need further education concerning their children’s condition, the principles of their rehabilitation, hygiene, the ways of properly maintaining the equipment, as well as dosing the suitable amount of physical activity on a daily basis.

Abstract

Diagnosis of human body posture and assessment of functionality, in physiotherapeutic procedures, is mainly carried out by viewing methods and by photogrammetric or videogrammetric methods, which do not provide angular measurements, anthropometric, time-space measurements, etc. Highly specialized imaging tests are medical procedures, which significantly affects their availability. Also, in terms of their economics, application and invasiveness. Thus, their purpose is significantly limited in the initial diagnostics process, and in particular in current physiotherapeutic assessment.
In the world, for over 30 years, gait, balance, selected biomechanics parameters and body functionality have been evaluated by pedobarography. However, other scientific studies of the authors indicate that the method is very little known in Poland by medical staff. Despite the fact that it has been included in the list of guaranteed benefits since 2011, no procedures for its implementation have been specified. An important aspect of pedobarography is the consistency of its directions of use with the standards of training the profession of a physiotherapist. The obvious and logical conclusion is that the lack of knowledge in the area of the method does not allow its development. An important issue is to assess whether education standards are directly consistent with the directions of pedobarography in the rehabilitation process.

S. Głowiński, K. Łosiński, A. Grochulska, G. Płotka, A. Hałaburda – Evaluation of manual dexterity after treatment with botulinum type A(B), selected kinesitherapeutic methods: a case study. Fizjoterapia Polska 2020; 20(4); 40-48

Abstract
Introduction. Botulinum treatment involves the reduction of spasticity in patients qualified for this type of treatment. The main aim of this treatment is to increase the quality of life by improving functionality. In some situations, however, objective tests indicate a deterioration of movement parameters, and at the same time the patient reports improved performance of basic self-care activities. In this case, the decision to continue the therapy depends on the patient’s subjective feelings.
Material and methods. The study concerns a 20-year-old patient with right-sided spastic paresis resulting from the surgical treatment of left thalamic haemangioma in 2008. The patient was qualified for botulinum toxin treatment under the NFZ programme in 2019. The above-mentioned therapy was applied in combination with selected methods of neurophysiological rehabilitation. The aim was to maximize the positive effects of the therapy: to improve the range of mobility and reduce associated reactions, with minimal loss of manual activities of the right upper limb. Tests were performed to assess muscle tone according to the Ashworth scale, and precision grip according to the “Guidelines of the National Council of Physiotherapists for Health Services”. Additionally, the range of motion in the hand joints was measured using the vision system. A subjective 10-degree scale was proposed to assess the incidence of associated reactions during everyday activities.
Results. The results obtained indicated discrepancies between the objective results of standard tests and the patient’s subjective evaluation. According to the patient, despite the loss of some of her hard-earned mobility patterns, her quality of life improved.
Conclusion. Botulinum injection combined with physiotherapy slightly increased the range of motion in the hand joints, decreased the associated reactions, and slightly improved manual dexterity.
