Abstract
Background. Damage to the ligamental and capsular apparatus of the knee, especially the anterior cruciate ligament (ACL), leads to instability of the knee, manifesting functionally as unsatisfactory joint coordination and slippage of the plateau of the tibia. The choice of treatment should be dictated by the extent of damage incurred by the knee and the patient’s age and degree of physical activity. Surgical reconstruction of the ACL restores joint coordination, but the final outcome is largely determined by post-surgical kinesitherapy. In planning the rehabilitation program after surgical reconstruction of the ACL, it is necessary to bear in mind the impact exercises will have on knee stability, as well as on the work and load of the passive and dynamic stabilizers of the knee.Material and methods. In this article the effects of exercises in systems based on open and closed kinematic chains are subjected to statistical analysis, as well as the impact of such exercises on the relative movements of the individual structural elements of the knee.Conclusions. The results of the present study showed that kinesitherapy in open and closed kinematic chains has a positive impact on rehabilitation outcome after ACL reconstruction, while the decisive factor is the skillful combination of exercises from both types of kinematic chain.

Abstract
Background. Hip joint alloplasty is a very complicated therapeutic problem. The cooperation of the orthopedic surgeon with they physiotherapist is very important in successful treatment. The aim of current treatment strategies is to restore the function of the hip joint. This article describes the model of rehabilitation treatment we apply in our clinical practice.Material and methods. We examined 18 patients after hip alloplasty using a cementless endoprosthesis, operated in the Orthopedic and Traumatology Clinic of the Medical University of Bydgoszcz, and rehabilitated in the Rehabilitation Clinic of the Medical University of Bydgoszcz. On the basis of differences in limb length, the patients were divided into three subgroups: A) no difference in limb length; B) operated limb shorter; C) operated limb longer.Results. Significant differences among the subgroups were found in terms of the percentage of weight burden on the operated limb.Conclusions. Rehabilitation after hip alloplasty must consider each patient individually. Outcome depends not only on good surgical technique, but also the nature and quality of the rehabilitation program after surgery.

Abstract

Background. The treatment of post-traumatic contracture is beset with many difficulties, caused in large part by the anatomical complexity of this joint. Case histories. This article presents three patients treated for persistent post-traumatic contracture of the elbow. The rehabilitation problem consisted in the fact that when extenion was increased, flexion was decreased, and vice versa. In order to overcome this mechanism, which maintained an unchanged range of movement by alternately shifting the extreme positions of flexion or extension, we attempted to increase the total range of movement by using plaster extensors to fix the progress of maximum movement. Plaster casts holding the joint in the maximum, extreme position were changed every two hours. Therapy with physical agents and mild exercises was simultaneously administered. New pairs of double casts were made twice, each time achieving further improvement. Conclusions. In selected cases of contractures resistant to standard rehabilitation procedures, when the use of a redressing rail is impossible, the method of serial casts in alternating extreme redressment as here described should be considered.

Abstract

Background. Injury to the ligaments and capsule of the knee joint, especially the anterior crucial ligament, leads to joint instability, manifested functionally by unsatisfactory joint mobility and a tendency for the tibial plateau to give way under load. Surgical treatment generally restores stability but the outcome is determined by rehabilitation, focused on recovery of function. The aim of our research was to evaluate outcome one year after surgical reconstruction of the anterior crucial ligament. Material and methods. We tested 20 randomly selected patients under treatment in the Department of Orthopedics and Traumatology of the Motor Apparatus in 2001 after post-traumatic surgical reconstruction of the anterior crucial ligament. The research involved a patient interview that included the pathomechanism of the injury, and a subjective evaluation of the rehabilitation outcome. Results. The most common cause of this injury proved to be sport. Two-thirds of the subjects evaluated the outcome of rehabilitation after surgical reconstruction as good or excellent. Conclusions. The program described here gives good effects in the rehabilitation of patients recovering from surgical reconstruction of the anterior crucial ligament.

Abstract

The diversity of available strategies enables the choice of the optimal method leading to effective and safe clinical work. One such strategy is the PNF concept, which is characterized by a holistic approach to the patient and his problems. This makes it possible to treat the patient in his/her own environment, using familiar behaviors and situations. This article describes examples of the use of the MTD Control Balance Platform as an auxiliary device in an intensive program of functional exercises based on the PNF concept. Seven patients are described, all treated at the Rehabilitation Clinic of the Rydygier Academy of Medicine in Bydgoszcz. The examples include neurological and orthopedic disorders, as well as sports injuries. Only the patient’s adaptation to independent living is proof of the effectiveness of rehabilitation. This effect can be achieved by conducting therapy in the patient’s own environment, using familiar behaviors and situations. The examples of PNF therapy selected for presentation here, supplemented with the patient’s independent exercising on the MTD Control Platform, satisfy these conditions and can be recommended.

Abstract

Background. The transcutaneous electrical nerve stimulation was to evaluate of work usefulness of and examining of applied method of treatment onto level of fear at ill with chronic pain in ground of spine. Material and methods. Investigation degeneration were moved in group of 59 patients with symptoms of chronic pain in after-effect of changes-disfiguring section of loin spine. Level of fear in studied group ISCL were estimated behind help of questionnaire. Heal conduct hugged endeavors with use of impulses TENS as well as heal practices. Results. The statistical tests dependence was showed between level of fear in studied group and results of treatment. Sex was not essential meaning in formation both guilds as state of fear in studied group. Conclusions. Applied method of treatment contributed to lower of level of fear in studied group and results confirmed usefulness in treatment ill with symptoms of chronic pain.

Abstract

Background. The purpose of our study was to analyze the occurrence, strength, and dynamics of depressive symptoms in patients with chronic pain caused by degenerative and deformative changes in the lumbar spine, and to examine the impact of the physiotherapy methods used on the measured parameters. Material and methods. The research material consisted of patients at the Resurs Specialized Rehabilitation Clinic in Wrocław. We examined 106 patients complaining of chronic pain caused by degenerative and deformative changes in the lower spine. The level of depressive symptoms was assessed in all patients with the Beck Depression Inventory. Results. Statistical analysis showed a dependency between the occurrence of depressive symptoms and gender. The impact of various methods of treatment in the group with depressive symptoms was also assessed. Conclusions. Depression occurs in some patients with chronic lower back pain. The level of depression in both examinations was higher among the women. After treatment there were no significant changes in the level of depression in the study group.

Abstract

Background. Contemporary rehabilitation is developing in many different directions. In view of the scope of research and practice, it is a discipline that joins many sciences sharing a common goal: to help the patient. This task is performed by an interdisciplinary rehabilitation team composed of the attending physician and the physiotherapist, along with other specialists. The constantly growing number of patients and increasing financial constraints are making it essential to seek new therapeutic solutions. One of many possible strategies is the PNF concept. This method is characterized by a holistic approach to the patient, based on behaviors and daily life situations with which the patient is familiar, and using available rehabilitation equipment. Case description. This article presents a case study of a female patient admitted for rehabilitation after a spinal cord injury (SCI) to the Department of Rehabilitation at the Bydgoszcz Medical University. Examples are provided of functional exercises used in the intensive rehabilitation program developed for this patient. The ASIA scale was used to assess outcome, along with functional tests for patients with paraplegia, a modified version of the Barthel test. Conclusions. The successful outcome in this case resulted from the cooperation of the entire therapeutic team, and from the exchange of information regarding the patient’s clinical status and progress in treatment. Close cooperation makes it possible to chose the optimal method, ensuring effective and safe work with the patient.

Abstract

Background. The aim of our research was to evaluate the effectivness of two methods of treatment of the lymphoedema after mastectomy operation. Method number 1 – classic method in treatment of lymphoedema after mastectomy operation. Treatment by using: lymphatic drainage, pneumatic massage, exercises in series 10/ 3 times a week. Method number 2 – complex methods that helps in restoration of patency: lymphatic drainage, pneumatic massage, exercises, bandaimy the upper limb, where lymphoedema appeared in series 10, 5 times a week. The examination aim was to evaluate subside of pain druing process of decreasing of lymphoedema. Material and methods. In our research we examinated patients with lymphoedema of upper limb after mastectomy operation, who were treated in Wojewódzkie Centrum Onkologii in Gdansk. Patients were devided into two groups of 15 patients each. In first group patients were treated in serie of 10, 3 times a week by classic method used: lymphatic drainage, pneumatic massage, exercises. In second group patients were treated by complex method of treatment, in series of 10, 5 times a week, by using classic treatment and adding the bandainy of upper limb, were lymphoedema appeared after operation. Results. Treatment of lymphoedema by using two methods had influence on our final results. Lymphoedema in group number 1, who were treated by classic method was located on level of forearm and elbow point. Lymphoedema in group number 2, who were treated by complex method was located over decvanon of vinov bone. Conclusions. Our research shows that conservative methods of rehabilitation had influence in process of decreasing lymphoedema, also helped to relieve pain in upper limb. The complex method is very effective treatment in lymphotic restor patency process.

Improvement of spinal joint mobility and relief of low back pain by soft tissue massage of the spine

Abstract

Background. The aim of this paper is an evaluation of the effect of remedial massage on the improvement of mobility and reduction of pain in the lower back. Material and method. The study enrolled 25 outpatients treated at the Therapeutic Mobilization Laboratory, Ministry of Internal Affairs and Administration Medical Centre in Olsztyn. Of the 25 patients, 13 were diagnosed with cervical discopathy, including 4 with brachial neuralgia (with pain radiating to the arm in 2 patients and to the forearm in the other two), and 12 with lumbar discopathy, including 8 with pain radiating to the foot. Sources of data comprised linear measurements of spine mobility and a modified Melzack Pain Questionnaire. The data were obtained on the first medical day and at the end of a course of 10 procedures. Dry massage was applied to the entire spinal column with heavier pressure on the area where the pain originated. Results. The results indicate a statistically significant increase in mean spinal range of movement after the course of massage procedures. After the massage treatment, a definite change occurred in the location of pain in the patients with left-sided and right-sided sciatic neuralgia. There was also a definite improvement in the patients with brachial neuralgia. Conclusions. The massage treatment brought about a reduction in the pain and altered its quality.
