Sacro-lumbar orthosis in lower back pain syndromes – literature review

Abstract
Purpose of study. The purpose of this study is to analyze the effect of sacral-lumbar orthoses on the occurrence of pain in patients with low back pain (LBP) and the effects of these orthoses on the muscles that stabilize the trunk based on the available literature.
Methods. For the purpose of this study, the following databases were searched: Pubmed, Scopus, ScienceDirect, Medline, and others using the Google Scholar search engine using the following keywords: low back pain, lbp exercise, low back pain epidemiology, low back pain orthosis, low back pain therapy, LBP treatment, lumbar sacral orthosis.
Results. 26 of the 92 articles searched by the above method were analyzed in relation to the subject of the use of sacral-lumbar orthoses in patients with spinal pain syndrome. The analysis did not show that wearing a corset negatively affects muscle strength. Most current studies recommend using a corset in combination with exercise and/or pharmacotherapy for LBP.
Conclusion. There are not enough studies to conclusively determine the effect of lumbar sacral orthoses on the sensation of pain in the lumbar spine. Among the main limitations of the analyzed studies are difficulty in measuring pain, as pain is a subjective sensation, and the significant number of possible causes of LBP, which are sometimes difficult to specify.

Abstract
Background. In results from data in literature and our observations that 2% of patients with the low back pain requires operational treatment. The work presents algorithm of rehabilitation of patients after the operation of removing the nucleus pulposus in the lumbosacral section. Material and methods. Twenty patients were examined, who in a period of November 2000 till March 2001 were operated at the Neurosergical Department and rehabilitated in the Physiotherapy Laboratory of the Rehabilitation Department of the District Hospital No. 2 in Rzeszów. In order to evaluate pain a visual analogue scale VAS and five-points Likert’s scale were used. The examination was performed three times: before and after the surgery and after a three weeks’ period of rehabilitation. Results and conclusions. The algorithm of procedure in the post-surgery period was proposed, in which complex rehabilitation was taken into consideration, among others physiotherapy, psychotherapy, pharmacotheraphy, professional advising, teaching ergonomy of labour and rest.

Influence of auto-therapy on efficiency of treatment of group pain backbone

Abstract

Background. The authors worked out the therapeutic programme for the backbone pain syndromes of the lumbar spine, broadened by the programme of auto-therapy- that is the set of exercises designed for everyday, independent practice at the home. Authors took into consideration the fact that a certain group of muscles has the tendency of staying in the contracture which causes the disorder of muscular balance to happen. As a result it gives a feeling of discomfort and if such state tends to last longer can be cause pain of the backbone syndromes. The programme created for patients, was a result of the conducted research concerning norms of the elasticity of muscles, and its task was to restore the disturbed muscular balance. Making patients aware of their responsibility and taking part in that therapeutic success through the introduction of auto-therapy allows to accelerate the process of recovering. The main aim of the authors of this therapeutic programme was to stimulate the patients to the independent, systematic work to improve oneself everyday. The systematic care of the muscular balance guarantees full recovery of the backbone pain syndrome. Material and methods. In the programme participated 40 patients at the age from 28 to 56 years. All the patients were characterized by similar ailments, among which predominated the constant pain or appearing in certain situations. Patients were divided into two research groups. The first group was improved according to the basic programme under the therapist direction, meanwhile second group except for the basic programme mounted the programme extended by independent practices made at the home, that is so called auto-therapy. Results and Conclusions. The extension of the basic programme by auto-therapy, that is the independent self-work in home conditions allows the acceleration of the treatment of the backbone pain syndrome. Moreover maintaining patient’s muscular balance reduces the risk of quick recurrences of pain ailments. (He is liberated active part and for the efficiency of the therapy patient responsibility the additional energy which strengthens the patient both from the physical side, how and psychical side). What strengthens the patient is the energy triggered by an active participation and the patient’s responsibility for the effectiveness of the therapy. This energy ameliorates both patient’s physical and psychical side.

Evaluation of efficacy of interferential current and ultrasound therapy in patients with spinal pain

Abstract
Background. Comparative evaluation of analgesic efficacy of interferential current and ultrasound for the relief of pain radiation to the lower limbs in patients with a chronic spinal pain syndrome in the course of osteoarthritis. Material and methods. The study involved 100 individuals divided into two equal groups. Group A (n=50) patients received 10 ultrasound therapy sessions (0.8 W/cm2, 10 minutes). Group B (n=50) patients received 10 interferential current (IC) treatment sessions (10 minutes, 90-100 Hz). All volunteers completed questionnaires containing 42 questions. A subjective pain evaluation was performed using Visual-Analogue Scale of pain (VAS). The Oswestry Disability Scale was used to evaluate lumbosacral spine dysfunction.Results. The incidence of pain radiating to the buttocks decreased in both groups. In the ultrasound group, there was an insignificant reduction of pain radiation to the posterior part of the thighs, while after interferential current therapy pain radiation decreased by 50%. Pain radiation below the knees was reduced significantly in the group treated with interferential current. Conclusions. Interferential current therapy more effective than ultrasound for the reduction of pain radiation to the lower extremities in patients with chronic low back pain in the course of spondyloarthrosis.
