Masažo įtakos fiziologiniam ir psichologiniam veikimui apžvalga

Santrauka
Šiame darbe siekiama glaustai apibendrinti esamą literatūrą apie masažo poveikį sportui ir fiziniam veikimui, ypatingą dėmesį skiriant motorikos įgūdžiams, neurofiziologiniams veiksniams ir psichologiniams veiksniams. Apžvalga atitinka PRISMA gaires (Pageidaujami sisteminių apžvalgų ir meta-analizės ataskaitų elementai) ir apima iš viso 76 straipsnius. Rezultatai rodo, kad apskritai masažas nedaro reikšmingos įtakos motorikos įgūdžiams, išskyrus lankstumą. Nepaisant to, kai kurie tyrimai teigia, kad palankūs raumenų jėgos ir raumenų stiprumo pokyčiai gali būti pastebimi praėjus 48 valandoms po masažo. Kalbant apie neurofiziologinius aspektus, masažas atrodo neturi įtakos tokiais veiksniais kaip kraujo lakto rūgšties šalinimas, kraujotaka raumenyse, kraujo apytaka, temperatūra raumenų audinyje arba raumenų aktyvacija. Tačiau yra patikimų įrodymų, remiančių mintį, kad masažas gali palengvinti skausmą ir sumažinti vėluojančio raumenų skausmo atsiradimą, galimai mažinant kreatino kinazės fermento lygį ir per psichologinius procesus. Be to, apžvalga pabrėžia masažo psichologinius privalumus. Yra dokumentuota, kad masažo gydymas sumažina depresijos, streso, nerimo ir suvokiamo nuovargio jausmus, tuo pačiu metu skatindamas nuotaikos, atsipalaidavimo ir požiūrio į atsigavimo būsenas gerėjimą. Masažas gali neturėti tiesioginio poveikio tam tikriems veikimo aspektams, tačiau jis siūlo pastebimus psichologinius privalumus sportui, o fizinio veikimo pagerėjimas yra abejotinas. Jie taip pat atlieka netiesioginį vaidmenį kaip svarbi priemonė skatinant dėmesį, atsipalaidavimą ir atsigavimą sportininkams. Masažas gali padėti sportininkams išlikti psichiškai ir fiziškai pasirengusiems varžyboms ar treniruočių metu.

Santrauka
Sportininkų intensyvus treniravimasis gali lemti pernelyg didelį treniravimą ir sportininkai, kurie neoptimaliai prisimeną atlikti tempimo pratimus ir šildymą po treniruotės, gali patirti kojų blauzdos traumas. Tai neabejotinai trukdo sportininkų pasirodymui treniruotėse ir varžybose. Šio tyrimo tikslas yra nustatyti masažo terapijos ir mankštos terapijos poveikį moterims ilgųjų distancijų bėgimo sportininkių meistrų kategorijoje kojų blauzdos traumos atstatymui. Šio tyrimo tipas naudoja kvazi-eksperimentinį požiūrį su vieno kontrolinio grupės prieštesto-pažymėjimo tyrimo dizainu. Duomenų rinkimo technika naudoja testus ir matavimus su sąnarinių judesių diapazono matavimui naudojant goniometrą ir skausmo skalę, kad būtų nustatytas skausmo lygis. Šio tyrimo populiacija buvo moterys maratonininkės, sulaukusios 42 metų ir vyresnės. Duomenų analizės technika po normalumo ir homogeniškumo testų buvo naudojamas porinių t-testas. Rezultatai parodė, kad masažo terapijos ir mankštos terapijos gydymas turėjo reikšmingų skirtumų, būtent sumažėjo skausmas ir padidėjo sąnarių judesių diapazonas su p = 0,000 (p < 0,05) reikšme.

Streszczenie
Wprowadzenie. Rozwój zasobów ludzkich rozpoczyna się już przed narodzeniem poprzez skupienie się na dobrostanie przyszłych matek i trwa przez różne etapy życia: niemowlęctwo, wiek przedszkolny i szkolny, okres adolescencji, dorosłość i starość.
Cel. Celem tego badania jest zbadanie i ocena skuteczności określonego rodzaju masażu zdrowotnego w poprawie produktywności zawodowej u osób z niepełnosprawnościami fizycznymi. Ta produktywność obejmuje motywację, wiedzę, umiejętności, postawy i niezależność.
Metoda. W badaniu wykorzystano metodę quasi-eksperymentalną, korzystając z jednogrupowego projektu szeregów czasowych (pretest-posttest). Po początkowej obserwacji (pretest) przeprowadzono obserwację po zakończeniu interwencji (posttest). Grupa badana przeszła terapię masażem skierowanym na czynniki produktywności zawodowej u osób z niepełnosprawnościami fizycznymi, obejmującą pięć wpływowych elementów: (1) motywację, (2) wiedzę, (3) umiejętności, (4) postawy i (5) niezależność. Próbka została wybrana spośród populacji za pomocą nielosowej metody próbkowania celowego. Zbieranie danych obejmowało oceny i pomiarów, a kwestionariusz służył jako narzędzie testowe do pomiaru produktywności zawodowej. Analiza danych obejmowała analizę wariancji.
Wyniki. Wyniki badania ujawniają istotną różnicę między zmienną początkową a końcową, z wartością istotności (dwustronną) mniejszą niż 0,05, ustaloną za pomocą testów t-Studenta dla par. Jednak test ANOVA dał wynik wartości istotności wynoszący 0,267. Oznacza to istotny wpływ terapii masażem zdrowotnym na zmienne produktywności zawodowej, takie jak motywacja, wiedza, umiejętności, postawy i niezależność, u osób z niepełnosprawnościami fizycznymi.
Wnioski. Największy i najefektywniejszy wzrost produktywności zawodowej zaobserwowano w zmiennej motywacyjnej, z przyrostem procentowym wynoszącym 16,75%.

The mobility of the lumbar spine in persons with low back pain

Abstract

Background. The purpose of our study was to examine changes in the mobility of the lumbar spine in persons with L5-S1 diskopathy and the changes in mobility associated with different modalities of treatment. Material and methods. We studied 150 persons 40-50 years of age, 64 men and 86 women. Due to variations in management the material was divided into two groups: Group I, treated only pharmacologically, and Group II, treated with both pharmacological methods and physiotherapy. The patients were examined twice, once before treatment and again after 6 weeks of treatment. The research methods included clinical interviews and measurement of thoraco-lumbar and lumbar mobility on all planes. Non-steroid anti-inflammatory drugs were used in Group I. In Group II, comprehensive physiotherapy was also applied, including physicotherapy, kinesitherapy, and massage. Results. Our analysis of the material showed that the limitations in spinal mobility were similar in both groups at baseline. After a 6-week course of treatment, significant improvement in the mobility of the thoraco-lumbar and lumbar spine was noted only in Group II (pharmacotherapy and physiotherapy), involving all the tested ranges of movement. Conclusions. An evaluation of treatment outcome in persons with L5-S1 diskopathy indicates that persons treated both with pharmacotherapy and physiotherapy show greater improvement. The fact that significant improvement was seen only in this group may result from the fact that the drugs used prepared the way for better results in subsequent physiotherapy. These results indicate that pharmacological treatment in these patients should be supplemented with physiotherapy.

Medical massage in dysmenorrhoea

Abstract
Background. From 40 to 60 % of women aged 20-40 are affected by dysmenorrhoea. Such patients are usually prescribed pain-relieving and spasmolytic drugs. Massage is a non-invasive therapeutic modality whose effects are believed to include reduction of muscle tone. Therefore, the authors attempted to determine the efficacy of medical massage in allevi-ating dysmenorrhoeic pain. Material and methods. The participants of the study were 48 students from the University School of Physical Education, aged 22-25. To evaluate efficacy of medical massage, the researchers elaborated questionnaires. The objectives of the questionnaire were to: 1) select those participants complaining of menstruation-associated pain, and 2) to determine the intensity of the pain at baseline and after a series of massages. A total of 26 women with dysmenorrhea were selected and subsequently subjected to medical massage. Four 10-minute medical massage sessions were performed over the lumbar, sacral and gluteal areas. At the end of each procedure a hot water bottle with 45 degrees Celsius water was applied to the massaged area in order to preserve the changes in blood distribution within the pelvis. Results. Medical massage applied a few days before menstruation reduced dysmenorrhoeic pain in 24 out of the 26 women. Pain was alleviated in 16 women and completely disappeared in a further 8 patients. In 2 patients, the intensity of pain remained the same after application of medical massage. Conclusions. The findings indicate that medical massage can be an easy-to-use adjunctive therapeutic modality for reducing pain in women with dysmenorrhea.

Abstract

The main aim of postoperative management following surgery of the facial skeleton is to prevent potential postoperative complications. As a result of disruption of the continuity of tissue, there is swelling within the operated area, as well as subcutaneous extravasation of blood and hyperaesthesia of facial skin. The authors have developed an approach to reduce or completely eliminate those inconveniences and thus shorten convalescence time using medical massage. Initially, normal superficial sensitivity of facial skin is restored by stroking from the medial line of the face towards the ear concha, starting from the lower jaw line and ending in the forehead. Next, the peristaltic movement in lymphatic vessels diverting lymph within the head and neck is stimulated by moving the skin over deeper lying tissues within the physiological range of skin mobility. Circular movements are made with a brush for 2-4 min., and slight pressure is applied to the skin over the haematoma area in order to accelerate the resorption of subcutaneous discoloration ensuing from extravasation of blood. The last stage of the treatment is aimed at reducing pain and tenderness in the operated area. We use vibration of < 1 mm amplitude and frequency of 50-100 Hz, which guarantees analgesia for 2-3 hours. The treatment reduces postoperative discomfort afflicting patients already during the first 24 hours after surgery.

Improvement of spinal joint mobility and relief of low back pain by soft tissue massage of the spine

Abstract

Background. The aim of this paper is an evaluation of the effect of remedial massage on the improvement of mobility and reduction of pain in the lower back. Material and method. The study enrolled 25 outpatients treated at the Therapeutic Mobilization Laboratory, Ministry of Internal Affairs and Administration Medical Centre in Olsztyn. Of the 25 patients, 13 were diagnosed with cervical discopathy, including 4 with brachial neuralgia (with pain radiating to the arm in 2 patients and to the forearm in the other two), and 12 with lumbar discopathy, including 8 with pain radiating to the foot. Sources of data comprised linear measurements of spine mobility and a modified Melzack Pain Questionnaire. The data were obtained on the first medical day and at the end of a course of 10 procedures. Dry massage was applied to the entire spinal column with heavier pressure on the area where the pain originated. Results. The results indicate a statistically significant increase in mean spinal range of movement after the course of massage procedures. After the massage treatment, a definite change occurred in the location of pain in the patients with left-sided and right-sided sciatic neuralgia. There was also a definite improvement in the patients with brachial neuralgia. Conclusions. The massage treatment brought about a reduction in the pain and altered its quality.

Abstract
On the level of cells, tissues and organs alike, the structure of the human body enables the maintenance of constant spatial arrangement. This is possible owing to collagen, the most prevalent protein of the system. It is the basic building block of connective tissue, including the musculo-fascio-ligamentous system. The maintenance of proper spatial structure of the body relies on the maintenance of a balanced tonus of collagen, and thereby, of connective tissue. This balance is generated mainly by the resting tonus of skeletal muscles, which also contain collagen. On this basis, anatomical analysis of structural dependence at the musculo-fascio-ligamentous level has been performed, in accordance with the principle of tensegrity, on four fundamental systems. Each of these systems comprises structurally linked muscles, fasciae and ligaments forming an extensive pathway for the transmission of tensions over a large area of the body. Vessels and nerves course through these structures at particular points. Disorders may ensue as a result of persistently elevated resting tone of muscles and fasciae. The most common conditions of the motor system which can be a consequence of disturbances in the distribution of tensions within those four pathways are also given in the paper. This attempt at analysis of the anatomical structure of the human body according to the tensegrity principle ensures a better understanding of abnormal functioning of the body. Other benefits can include increased effectiveness of massage therapy in some conditions and better insight into structural and functional connections between various elements of the kinetic system on the one hand and vessels or nerves on the other.

Efficacy of medical massage after facial skeleton surgery

Abstract
Background. Appropriate physical therapy procedures, including medical massage, can be used as a therapeutic adjunct inpatients after facial skeleton surgery. Material and methods. 18 patients hospitalised for facial skeleton surgery participated in the study. The age range was 19-52 years. All patients had lymphoedema and dysaesthesia with hyperaesthesia. Blood extravasations were noted in two patients. The mean extent of oedema on the operated side was 120% compared to the healthy side and the mean intensity of pain was 5.6 degrees. Massage was performed every day (on average 4-5 procedures, 15 min. each), beginning during the first 24 hours after surgery and continuing until discharge. Changes in the size of oedema and haematoma were assessed together with any changes in the intensity of pain. The assessments were based on photographic documentation and an 11-degree scale of pain intensity. Changes in the size of swelling were measured with two scales: absolute (measured from the base of the nose to the point of maximum oedema, before and after the procedure) and relative (only the change in swelling was measured). Results. Mean oedema size after the rehabilitation programme was 103%, corresponding to an absolute reduction of 17.8%. The relative change in lymphatic swelling was 88.5%. Pain and tenderness were reduced by 4.9 degrees (87%) to 0.75 degrees. The haematomas were completely absorbed. Conclusions. Medical massage had a positive effect on the convalescence of facial skeleton surgery patients.

Trunk asymmetry in people with L5-S1 lumbar discopathy

Abstract
Background. The aim of this study was to investigate and describe trunk symmetry patterns via an analysis of changes in the position of selected anthropometric points and tracing these changes in time relative to type of treatment. Material and methods. The study enrolled 150 people aged 40-50 diagnosed with a chronic spinal pain syndrome inthe course of L5-S1 discopathy. The patients were divided into two groups corresponding to different treatments used. Patients in each group were further divided with respect to the direction of the hemia of the nucleus pulpous. Examinations were carried out twice: before starting treatment and after 6 weeks. Results. The degree of asymmetry in the frontal plane was similar between the study groups. Asymmetry occurred in a considerable percentage of the patients in both groups. Conclusions. Greater improvement of the asymmetry was noted in the patients receiving combination treatment, which may indicate a need for combining physiotherapy with pharmacological treatment in the treatment of discopathy. A complete cure is difficult to obtain in lumbar discopathy patients following a 6-week treatment. Long-term thearpy is needed as well as broadly understood prophylaxis to prevent recurrence of symptoms.
