Joanna Szczepańska, Joanna Kowalska, Grażyna Greń, Marek Woźniewski – Attitude of physiotherapists towards elderly patients with mental and behavioural disorders. Fizjoterapia Polska 2006; 6(3); 216-221

Abstract
Background. To verify whether cognitive deficits and mood disorders of older inpatients negatively influence the attitudes of physiotherapists towards them. Material and methods. Examination in Geriatric Rehabilitation Unit. 52 patients (78± ,4 years) and 7 physiotherapists. Cognitive status and mood of the patients were examined using the Mini Mental State Examination (MMSE) and the Geriatric Depression Scale (GDS) respectively. GDS was performed only in patients with MMSE 15. The therapists filled an anonymous questionnaire concerning the quality of working with particular patients. This enabled the following division: group (P+) patients working with whom the physiotherapists evaluated as pleasant, group (P-) patients who were negatively evaluated by the physiotherapists as working with them was difficult and tiresome. Results. In group (P-) MMSE values were significantly lower than in group (P+), and they were 15,2±7,5 vs. 23,1±3,7 respectively (p=0,0) and GDS values were significantly higher in comparison with group (P+), and they were 21,3±4,8 vs. 14,6±5,9 respectively (p=0,0). Conclusions. The patients evoking negative attitudes of physiotherapists (impatience, irritability, aversion) had mainly severe cognitive deficits (MMSE<19) and severe depressive symptoms (GDS>20). This may be an additional factor decreasing effectiveness of physiotherapy in this group of patients.

Mood disorders – significant factor in physiotherapy of elderly patients

Joanna Szczepańska, Grażyna Greń, Marek Woźniewski – Mood disorders – significant factor in physiotherapy of elderly patients. Fizjoterapia Polska 2008; 8(2); 170-178

Abstract
Background. Assessment of the prevalence of signs and symptoms of depressions among patients in a geriatric rehabilitation ward as well as an analysis of the physiotherapist-patient relationship where the patient suffers from a mood disorder. Material and methods. 15-item Geriatric Depression Scale (GDS), Mini Mental State Examination (MMSE), a questionnaire assessing the attitude of physiotherapists to working with specific patients. Results. 67.7% of the patients suffered from depression-type mood disorders. The presence of a mood disorder in chronically hospitalised elderly patients had a negative influence on the attitude of physiotherapists to those patients. Conclusions. Mood disorders are common in patients of long-term care institutions. Depression has a negative influence on the course of rehabilitation and the relationship between the physiotherapists and the patient. In order to increase the effectiveness of physiotherapy in geriatric rehabilitation wards, action must be taken to improve the emoctional health of elderly patients (diagnosis and treatment of depression).

Anna Skrzek, Małgorzata Mraz, Marek Woźniewski – Use of isokinetic tests and bmd in musculoskeletal system evaluation. Fizjoterapia Polska 2008; 8(2); 128-138

Abstract
Background. All elements of the locomotor system from an interconnected and interacting, biological and mechanical whole. The development, functioning and involution are mutually dependent. Studies of problems connected with the involution of the organs of locomotion help to explain and understand the most important determinants of these processes. The purpose of the present study was to demonstrate the utility of densitometric and isokinetic examinations for the assessment of changes in the organs of locomotion brought about by a pro-health exercise programme in women over 60 years old.Material and methods. The study group was composed of 46 women sged 60 or more who were subjected to a 4-monthpro-health exercise programme. Bone mineral density was measured in the proximal femur by DEXA. The densitometric studies were carried out in the Densitometry Unit of the Departament of Endocrinology, Diabetology and Isotope Therapy of Wroclaw Medical University. Strength measurements were performed using a Biodex System 3 Multi Joint isokinetic study unit (Biodex Medical System, New York, USA) located at the laboratory of the Physiotherapy Faculty of the University School of PHysical Education, Wroclaw. Concentric isokinetic tests for the trunk flexors and extensors and for the flexors and extenors of the knee were conducted in the sitting position. Results and Conclusions. The findings confirm a beneficial effect of this form of physical activity on the health of the components of the locomotor system examined in the study. There was a statistically significant improvement in most strength-velocity parameters examined in the muscles of the trunk and lower limbs. There was also a noticeable tendecy whereby the age-associated decrease in bone mineral density was slowed down.

Krystyna Boroń-Krupińska, Marek Woźniewski – The impact of relaxation on chronic pain intensity and level of depression. Fizjoterapia Polska 2012; 12(1); 49-57

Abstract
The objective of this study was to demonstrate the relationships between relaxation, pain intensity and level of depression. The study was conducted among forty five (N = 45) patients with pain, including 23 females and 22 males aged 60–80 years. The studied participants were randomized in three groups; each group consisted of fifteen subjects with equal number of males and females. The first group participated in relaxation training and standard physiotherapy, the second group participated only in standard physiotherapy and the control group was a „placebo” group who had conversations with relaxation trainer, without training and standard physiotherapy. Pain intensity was measured twice every day using VAS scale, before and after the activities. The level of depression was verified at the baseline and after two weeks using Geriatric Depression Scale. The analysis of the differences between average values for dependent samples was performed using Wilcoxon’s signed-rank test while the analysis for independent samples was carried out using Kruskal Wallis test. The correlations between the studied parameters were calculated using Spearman’s correlation ratio. The study showed pain intensity decrease in all studied groups, to the large extent in the relaxation and physiotherapy group (49.4% and 44.2% respectively). The level of depression significantly decreased in the relaxation group (7.9%). Depression level is a significant factor related to pain intensity. Relaxation resulted in the decrease in pain intensity and depression level.
