Abstract
Background. Pulmonary complications are the most common cause of mortality and morbidity in post-operative populations, especially in cardiothoracic surgeries that may demand much attention from the therapist. Median sternotomy a gold standard incision for CABG, has been shown to have an inevitable adverse effect on the performance of the respiratory mus-cles. Moreover, reduced blood supply to these muscles following the han/est of left internal mammary artery (LIMA) for CABG has been well acknowledged. The reduction in blood supply may lead to respiratory muscle weakness and affects recovery. In the recent past inspiratory muscle training has gained momentum as an acceptable techniąue in the rehabilita-tion of the respiratory muscles. To determine the effectiveness of inspiratory muscle training on respiratory muscle strength and endurance in patients who have undergone CABG with LIMA. Experimental study design with simple randomization.Material and methods. Thirty patients who had undergone coronary artery bypass grafting with LIMA as one of the grafts were randomly divided into two groups. Postoperatively the experimental group received inspiratory muscle training with a flow dependent trainer for 15 days exclusively along with the standard hospital protocol while the control group received only the standard physiotherapy protocol. The primary outcome measures were Maximum Inspiratory Pressure (PIMax) and Maximal Voluntary Ventilation (MW), which are determinants of respiratory muscle strength and endurance, respectively The variables were measured preoperatively on the 4th day after the surgical procedurę prior to training and 15 days after the training. Results. The inspiratory muscle strength and endurance in the inten/entional group showed significant improvement as compared to the control group. Also the MW values in the inten/entional group reversed back to their presurgical levels and the improvement in the Pimax values of the experimental group was significantly higher than even the presurgical values. The control group did not display either of these two trends. Conclusions. The results of this study indicate that inspiratory muscle training is effective in improving the respiratory muscle strength and endurance in patients who have undergone coronary artery bypass graft. This study suggests that inspiratory muscle training represents a useful therapeutic adjunct in the rehabilitation protocol for patients who have undergone cardiac surgery.

Abductory twist” – an element of observational gait analysis in low back pain – a case study

Abstract
The objective of this case study is to explain the influence of the altered lower limb kinematics in subject with non-specific low back pain using simple video analysis and to emphasise the relationship between the altered lower limb kinematics and low back pain. Low back pain management is one of the most challenging tasks encountered by the therapist. Altered lower limb kinematics has been hypothesized to influence low back pain. A thorough biomechanical evaluation of the lower limbs and the pelvic girdle along with the spine is important in successful rehabilitation of non-specific low back pain.An 18 year old cricket player with the chief complaint of low back pain presented to the department of physiotherapy. Routine physiotherapy evaluation was carried out and the patient was diagnosed with non specific chronic low back pain. He had undergone specific low back rehabilitation protocol elsewhere. On observation it was found that there was a collapse of medial longitudinal arch during midstance. Following this, a through video analysis was done to identify the abnormalities in kinematics. There was a rapid medial movement of the heel at the instant of heel off which was coined as abductory twist in the literature.Abductory twist is an abnormal finding in which there is an excess and prolonged foot pronation beyond midstance. This alters the kinematics proximally leading to back pain. Observational gait analysis is mandatory in recurrent non-specific low back pain. This case study explains the pathomechanical relationship between the foot and low back pain.

摘要
该研究检查在选定的临床环境中接受物理治疗的受试者对家庭锻炼计划的依从程度。研究的调查结果将提供有关影响所规定家庭锻炼依从性的感知促成因素及障碍等方面的意见。
方法。该定性调查研究共采访了二十名受试者,包括12名男性及8名女性,它们在马来西亚的吉隆坡和森美兰州选定的私立医院和理疗诊所中接受各种门诊肌肉骨骼康复治疗。受试者对规定家庭锻炼的感知意见和经验的回答以半结构式访谈进行录音。所获得的回应经过关键字编码及内容分析进行分析。
结果和分析。自我激励、决心和承诺为积极参与的最佳推动因素,影响本试验中受试者脱离家庭锻炼的明显障碍包括缺乏时间、工作承担、个性、疾病性质或受伤状况及运动处方无效等。
结论。根据所记录回应的内容分析显示家庭锻炼依从性的预测结果为多因素,该试验提供了解参与家庭锻炼的感知促成因素和障碍的基本数据。这些调查结果将协助医疗保健专业人员实施措施,在最大限度上提高接受理疗患者的依从水平。

Abstract

Background.
Teaching face to face and partly online is called blended teaching. In practical oriented subjects like physiotherapy more hands on training is required. Hence online teaching may not help to meet the learning outcomes. But a combination of face to face and online may be very effective in delivering the course contents. In this paper the effect of blended teaching on students learning is discussed.
Aim.
The aim of this paper is to discuss the efficacy of blended learning to improve the quality of student learning in physiotherapy education.
Materials and methods.
At INTI International University, we delivered physiotherapy selected courses in blended mode using various tools in the Blackboard platform. We also used zoom and skype to deliver in-classroom lectures with international expert from overseas. The efficacy of this method was evaluated through student’s feedback and performance in the formative and summative assessments
Results.
The students participated in these courses perceived blended teaching strategies very effective and convenient. Blended learning encouraged more collaboration between students which was evident based on their summative exam scores.
Conclusion.
Blended teaching strategies may be effective in physiotherapy education to improve the quality of students learning. A higher quality studies will provide more evidence.
