枕下肌抑制对年轻个体姿势稳定性的影响

摘要
引言。姿势稳定性是肌肉骨骼系统和神经系统合作的结果。站立姿势的稳定性确保了身体的空间排列,其中重心落在支撑基底的中心。前倾头姿势(FHT)增加了枕颈关节和上颈椎的伸展,加深了下颈椎段的屈曲,导致枕下肌的不当收缩。枕下肌的音调紊乱可能会影响平衡。
目的。研究假设是单次4分钟枕下肌抑制程序影响着开眼和闭眼状态下颅椎角(CVA)在正常范围内及以下个体的姿势稳定性值的变化。
材料与方法。健康参与者(10名女性和8名男性)平均年龄22.7 ± 0.58。参与者参加了这项研究。他们根据CVA角度评估被分入研究组。姿势稳定性分析使用Koordynacja稳定性平台。在枕下肌抑制前后立即进行开眼和闭眼的测量。
结果。观察到枕下肌抑制影响了A组(CVA < 50)前后(A-P)位移,p值=0.0168,而在B组(CVA ≥ 50)中无显著影响,p值=0.3695。
结论。单次4分钟枕下肌抑制程序对检查组的姿势稳定性参数没有显著影响,除了CVA < 50组开眼状态下的A-P位移。

Abstract
Background. Postural stability after anterior circulation stroke is considered a major cause of falling and functional dependence. Recently, special attention regarding mechanisms about standing balance recovery following stroke has been grown. Evidence supports the effectiveness of vestibular rehabilitation therapy for patients with central vestibular disorders. Objective. A randomized controlled trial was designed to investigate the effects of vestibular rehabilitation on postural stability in anterior circulation stroke patients. Methods. Thirty-six patients (27 males, 9 females) their age range from (44 to 72 years) diagnosed with anterior circulation stroke were enrolled and assigned randomly into two similar groups: experimental group (A) received vestibular rehabilitation therapy in addition to conventional treatment 3 sessions per week for six consecutive weeks, but the conventional group received only conventional treatment. All patients were assessed for postural stability using sensory organization test and berg balance scale and for gait performance using time up and go test at baseline and after 6 weeks at end of the rehabilitation process. Results. Statistical analyses using mixed design MANOVA showed that there was statistical significant difference (p < 0.05) in all outcome variables for both groups in favor to VR group especially in SOT5, SOT6 while there were non-significant differences between groups in SOT1 variable. Conclusion. Vestibular rehabilitation may be a promising approach if it is integrated as standard in the rehabilitation program for improving postural control after anterior circulation stroke affecting parieto insular region.

Effects of Thera Togs suit on postural stability in patients with central hypotonia. Preliminary report

Abstract
Postural hypotonia is one of the many factors adversely affecting postural stability, understood as the attitude of resistance to interference of endogenous and exogenous interferences. It evokes a number of compensatory responses allowing the stabilization of the body and entailing changes in the reciprocal positioning of the key points: the head, the shoulders and the pelvis. This affects the spatial position of the overall center of mass (COM), changing the base of the support and physiological limits of stability. Thera Togs suit is an orthotic equipment applicable in the treatment of postural hypotonia. Through the compression it provides incentives for proprioceptors, affecting body awareness and postural stability. The paper attempts to assess the impact of Thera Togs suit on postural stability of the studied sample. The sample comprised 8 of Rehabilitation-Education-Pedagogical Centre in Kielce, diagnosed with postural hypotonia who were able to maintain an upright position. In order to evaluate the therapeutic effect of Thera Togs suit, triple stability measurements was carried out, first before putting the suit on and next after one hour and six hours of wearing the suit. A statistically significant improvement in postural stability was observed in each patient, expressed by a decrease in size and area of the confidence ellipse and reduction of the number of postural sways. The corrective effect, resulting from application of the tape, aimed at restoring a symmetrical load distribution between the feet, was not obtained. The preliminary results show an improvement in postural stability in the study group of patients.

Wskaźnik masy ciała a stabilność posturalna i ryzyko upadku kobiet po 60. roku życia

Streszczenie
Cel pracy. Celem pracy była ocena stabilności posturalnej oraz ryzyka upadku kobiet po 60. roku życia, a także ocena współzależności pomiędzy wskaźnikiem masy ciała a stabilnością posturalną i ryzykiem upadku.
Materiał i metody. Badania zostały przeprowadzone na grupie 74 kobiet w wieku od 60 do 85 lat, będących uczestniczkami turnusu rehabilitacyjnego w sanatorium. Badane zostały podzielone na klasy według wskaźnika BMI. Do oceny równowagi wykorzystano specjalistyczne testy oceny równowagi. Za pomocą testu sięgania (Functional Reach Test, FRT) oraz próby Romberga oceniono stabilność posturalną oraz ryzyko upadku. Platforma stabilometryczna posłużyła do oceny równowagi na podstawie analizy parametrów COP. Natomiast test FRT wykorzystywany jest do oceny zdolności utrzymania równowagi podczas wychylenia ciała w przód oraz do oceny ryzyka upadku.
Wyniki. Analiza wyników Functional Reach Test nie wykazała istotnego wpływu współczynnika masy ciała na równowagę. Analiza wyników uzyskanych w próbie Romberga pokazała istotnie statystyczny wpływ współczynnika masy ciała na część zmierzonych parametrów.
Wnioski. Wskaźnik masy ciała BMI nie wpływa istotnie na wyniki uzyskane w teście FRT. Analiza przeprowadzonych testów wykazała, że osoby z nadwagą i otyłością mają większy problem z utrzymaniem równowagi w przypadku występowania czynnika zaburzającego.
W przypadku osób po 60. roku życia może to być dodatkowy czynnik zwiększający ryzyko upadku.

Abstract
The aim of this study is to explain the differences between balance and postural stability, and the changes which occur in these traits in patients after stroke.
Human being has the natural ability to stand on two feet and to move on the two feet across an unfamiliar terrain. However these abilities do constitute a big challenge for the balance control system, which is being controlled through the three separate systems, two of which are sight, responsible for planning of the locomotion activity, vestibular system, responsible for spatial orientation of the human body. The third, and the most complex is the sensory system (composed of proprioceptors and mechanoreceptors). In terms of biomechanics the balance is defined as the ability to maintain the center of gravity of the human body above the supporting plane.
One of the most important determinants of the correct body posture is the postural stability, which wider concept and denotes the ability to recover the balance. This is possible thanks to three strategies. Patients after a stroke have a problem with the ability of spontaneous kinesthetic response. The distribution of pressure forces of the plantar side of a foot on the ground changes, it becomes difficult to maintain balance, and the consequences are frequent falls, femur fractures and problems with the basic activities of daily living.
