Wpływ edukacji z zakresu neurobiologii bólu na niepełnosprawność u pacjentów z przewlekłym bólem dolnego odcinka pleców

Streszczenie Cel badania. Celem tego badania było zbadanie wpływu edukacji z zakresu neurobiologii bólu (PNE) na niepełnosprawność u pacjentów z przewlekłym bólem dolnego odcinka pleców w Arabii Saudyjskiej, skupiając się na takich zmiennych pośredniczących jak wiara w sobie w kontekście bólu, katastrofizacja bólu oraz przekonania o bólu pleców. Metody. Zastosowano obserwacyjne badanie wieloośrodkowe przed i po. Badanie przeprowadzono w klinikach fizjoterapeutycznych podległych Ministerstwu Zdrowia w Arabii Saudyjskiej, w okresie od grudnia 2020 do grudnia 2021. Pacjenci z przewlekłym bólem dolnego odcinka pleców zostali włączeni do dwutygodniowego programu fizjoterapeutycznego. W badaniu wzięło udział dziesięciu fizjoterapeutów z sześciu klinik, którzy leczą pacjentów z zaburzeniami mięśniowo-szkieletowymi oraz bólem dolnego odcinka pleców. Utworzono dwie grupy: zwykłą opiekę fizjoterapeutyczną (UC-PT) oraz grupę otrzymującą edukację z zakresu neurobiologii bólu (PNE). Zmienne pośredniczące oraz stopień niepełnosprawności oceniano na początku badania i po dwóch tygodniach. Wyniki. W badaniu wzięło udział 249 pacjentów, którzy spełnili kryteria dołączenia i wyrazili zgodę na uczestnictwo. 131 pacjentów otrzymało tylko zwykłą opiekę fizjoterapeutyczną, a 118 uczestników otrzymało dodatkowo PNE. Analiza wykazała, że zwiększenie wiary w siebie w kontekście bólu, zmniejszenie katastrofizacji bólu oraz zmniejszenie przekonań o własnej niepełnosprawności związanej z bólem skutecznie pośredniczyły w redukcji niepełnosprawności dzięki zastosowaniu PNE. Wnioski. Uczestnicy otrzymujący edukację z zakresu neurobiologii bólu wykazali większą poprawę we wszystkich badanych czynnikach poznawczych, co przyczyniło się do zmniejszenia niepełnosprawności. Wyniki te dostarczają wskazówek, jak można ukierunkować zalecenia lecznicze, aby maksymalnie wykorzystać korzyści płynące z PNE. Badanie to dostarcza również cennych informacji dla fizjoterapeutów i badaczy, którzy dążą do optymalizacji wyników leczenia pacjentów z przewlekłym bólem dolnego odcinka pleców.

摘要

目标. 尽管肌肉骨骼疾病（MSD）是全球物理治疗师面临的重要问题之一，但有关在沙特阿拉伯塔伊夫市执业的治疗师的具体特征知之甚少。本研究调查了塔伊夫市物理治疗师的肌肉骨骼疾病（WRMDs）的患病率、工作相关变量以及他们对这些疾病的反应。
方法. 物理治疗师在塔伊夫市参加了包含从标准化北欧问卷衍生的问题的调查问卷，作为横断面调查的一部分。共发放了120份自填问卷，回收了96份，回收率为80%。
结果. 在塔伊夫市，沙特阿拉伯的物理治疗师报告了91.7%的WMSDs患病率。过去一年中最常见的解剖区域包括下背部（24%）、颈部（18%）和肩部（17%）。分别以16.7%、15.6%和12.5%的比例，单日治疗更多患者、搬运患者以及使用手法治疗技术是最主要的风险因素。
讨论. 这些结果强调了为沙特阿拉伯的物理治疗师提供符合人体工学的有效干预培训计划的重要性，以减少WMSDs的发生并改善工作条件。

摘要
下肢静脉曲张和慢性静脉功能不全是影响工作年龄人群的常见疾病，其症状往往给患者带来不适，甚至可能妨碍或阻止其完成工作相关任务。多种因素可能促成这些疾病的发展。物理治疗师，尤其是那些大部分时间处于站立姿势的从业者，应特别关注静脉曲张和静脉功能不全的风险因素和预防措施。这种意识有助于预防该类疾病的发生。本文综述了下肢静脉曲张和静脉功能不全的患病率、风险因素及其预防方法。

Streszczenie:

Fizjoterapeuci ze względu na specyfikę wykonywanego zawodu są szczególnie narażeni na wypadki przy pracy. Istniejące w Polsce ubezpieczenie społeczne umożliwia im jednak zabezpieczenie na wypadek niezdolności do pracy. W niniejszym artykule zawarto najważniejsze kwestie dotyczące definicji i rodzajów ubezpieczeń społecznych, z jakich mogą korzystać fizjoterapeuci. Opisano powiązanie między prawem pracy a ubezpieczeniem społecznym. Przedstawiono również świadczenia, jakie przysługują pracownikowi z tytułu wypadku przy pracy lub choroby zawodowej. Ze względu na wymagającą pracę oraz ryzyko wypadku przy pracy istotne jest, aby fizjoterapeuci byli świadomi swoich praw i obowiązków, a w razie potrzeby – by korzystali z ubezpieczenia społecznego.

Abstract

A physiotherapist running or thinking of starting his/her own business must take into account many factors affecting business profitability. All predictable costs, as well as sudden crises and geopolitical changes should be considered due to their significant impact on the ability to continue running the business. Recent changes in tax policy and geopolitical turmoil make the analysis even more difficult, and its results not always optimistic.

Abstract
Introduction. Professional development is crucial for specialists in all fields. Physiotherapists also should take care of their professional and educational development. Coaching is able to help them set and achieve goals.
Objectives. This article was focused on characterization of coaching and its possible use among physiotherapists.
Methods. Review of the literature was used, consisting of articles about coaching among medical professionals.
Results. The vast majority of articles emphasize coaching’s positive qualities and effectiveness in medical professionals’ professional, educational and personal development. Few drawback were mentioned – mostly the need to appropriately match time and theme of the meetings with specific medical professions.
Conclusions. Coaching is still not a popular topic around physiotherapists. It should be more widespread, to encourage further development, independently from the level of education or professional experience.

Abstract
Introduction. Due to constant need for protection of patients’ personal data and for up-to-date knowledge about personal data protection law and Data Protection Officer’s (DPO) role in healthcare it is crucial for physiotherapists that work in their profession to have actual knowledge about the law and individuals they can ask for help with data protection issues.
Aim. The aim of this research was to assess whether there exists a statistically important correlation between respondents’ sociodemographic characteristics and their opinions and knowledge about personal data protection law and DPO’s functions.
Methods. An author’s custom survey form was used. The survey was conducted on a group of 101 physiotherapists actively working in their profession.
Results. Physiotherapists questioned felt the need to refresh their knowledge about medical law. They did not have faith in DPO regarding his possibility to improve data security and quality of services offered. At the same time, they witnessed data security breach. Most of the variables used were statistically significantly correlated.
Summary. Education programs concerning personal data protection, medical law and DPO’s roles should be conducted at medical facilities. Employees’ higher knowledge about this topic allows to secure both physiotherapists’ and patients’ interests.

Knowledge of physiotherapists on health promotion

Abstract
Introduction. A high level of knowledge of physiotherapists in the field of health promotion and proper body parameters is necessary for the correct transmission of information to patients. Physiotherapists should also use their lifestyle to set a good example to patients and encourage them to change their habits and know correct body parameters. A disturbingly small percentage of physiotherapists promoted a healthy lifestyle among patients. The answers to most of the questions were correlated with sociodemographic features.
Conclusions. Additional programmes promoting the transfer of knowledge to patients by physiotherapists should be introduced in most medical facilities. Physiotherapists should be encouraged to share their knowledge of a healthy lifestyle.

Professional situation of physiotherapists during the COVID-19 pandemic

Abstract
Introduction. COVID-19 epidemy impacted the economic situation in Poland and most of the world’s countries. It caused vast changes in citizens’ professional situation and leaded to employees fears about the future. Significant professional changes were also prepared for physiotherapists.
Aim This research was focused on assessing physiotherapists’ opinions about their current professional situation. Existence of statistically significant correlation between respondents’ answers and sociodemographic characteristics was determined.
Methods Author’s custom survey form was used, consisting of questions about professional situation of respondents. The survey was conducted on a group of 101 physiotherapists.
Results The majority of physiotherapists questioned had not suffered from changes implemented during COVID-19 pandemic, but they would like to receive additional benefits like other medical professions. There was a statistically significant correlation between respondents’ answers and sociodemographic characteristics.
Summary Medical facilities should continue to care about their employees, by offering physiotherapists similar benefits like those available to other medical employees. Despite the epidemiology situation, physiotherapists’ salary and working hours should not be cut.

Abstract
Introduction For medical facilities, violation of las and ethical rules might bring strong stigmatization by patients. Because of that, it is recommended to implement Compliance system in medical facilities and to give medical employees all information about Compliance Manager role.
Aim This research was focused on assessing the current knowledge and opinion of physiotherapists about Compliance Manager role. Existence of statistically significant correlation between respondents’ answers and sociodemographic characteristics was determined.
Methods Author’s custom survey form was used, consisting of questions about Compliance Manager role. The survey was conducted on a group of 101 physiotherapists, actively working in their profession.
Results Majority of respondents knew about Compliance Manager role and wanted such person to be employed in facility they worked for. There is a lack of trust among employees towards effectiveness of such professional’s actions. Most of the answers were significantly correlated with sociodemographic characteristics of respondents.
Summary Compliance system and the role of Compliance Manager should be implemented in polish medical facilities. Additional workshops and trainings are needed to strengthen medical staff trust towards Compliance Manager actions.
