Streszczenie

Background. Although stretching may play a role in treatment of muscle strain, its significance is still conflicting and previous studies show a low level of evidence. Objective. To find the latest evidence regarding the effectiveness of stretching as a treatment to patients with grade I&II muscle strain. Methods. A comprehensive electronic database search of PubMed, Cochrane Library Database, and the Physiotherapy Evidence Database (PEDro) was conducted for randomized controlled trials (RCTs) in English language from January 2008 to October 2019 on adults with muscle strain treated by stretching exercise. Manual searching was conducted for reference list of included studies. Two reviewers independently reviewed and assessed each article for inclusion. The Revised Cochrane risk-of-bias tool for randomized trials (RoB 2) was used to rate methodological quality and risk of bias. Results. A total of five RCTs with a total number of 197 patients were analyzed. Three trials used stretching exercises versus conventional physical therapy (basic range of motion exercise, postural stabilization, and concentric and eccentric exercises) showed the positive efficacy of static stretching on flexibility, strength and ROM. Two trials used stretching based rehabilitation versus other treatment (vibration and cryotherapy) revealed superior effect of stretching rather than any other treatment. Conclusion. The clinical efficacy of stretching may correlate with using static stretching in treatment of grade I&II muscle strain. The effects of combining stretching with other modalities may be superior to conventional physical therapy treatment and can result in better outcomes.

Streszczenie

Aim. To determine the effect of ultraviolet on vitamin D activation and balance in postmenopausal woman. Materials and methods. This study was conducted on forty postmenopausal women suffering from vitamin D deficiency (diagnosed by physician), with their age ranged from 50 to 60 years. They were randomly assigned into two equal groups. Group (A) received ultraviolet for 12 weeks (n = 20), while group (B) served as a control group (n = 20). Vitamin D was evaluated by blood analysis while balance was evaluated by Biodex balance system before and after treatment for both groups. Results. Comparing both groups post-treatment showed that there was a statistically significant increase in vitamin D (p < 0.05) and statistically significant reduction in all balance indices (p < 0.05) in favor of group (A). Conclusions. Ultraviolet radiation is effective, easy to apply, simple and successful method in improving vitamin D concentration and balance in postmenopausal women.

Streszczenie

Purpose. To determine the effect of interferential current augmented by strengthening exercises for core muscles on low back pain after delivery. Design. single blind randomized controlled trial. Methods. Thirty multiparous women suffering from postpartum low back pain (for at least 3 months after delivery) participated in the study, they were selected randomly from the outpatient clinic of obstetrics and gynecology in Police Authority Hospital at Naser City, Cairo, Egypt; their ages ranged from 25-40 years old, through a period of 33 months (from May 2017 to February 2020). Confidentiality was assured. They were assigned randomly into two groups equally in number; 15 patients each: Group (A) patients received interferential current on the lower back for 20 minutes, 3 times /week for 4 weeks, additionally to this, they were asked to perform strengthening exercises for back & abdominal muscles, posterior pelvic tilting and postural correction exercises for 60 minutes, 3 times / week for 4 weeks. Group (B) patients were asked to perform strengthening exercises for back& abdominal muscles, posterior pelvic tilting and postural correction exercises for 60 minutes, 3 times / week for 4 weeks. Results. Comparing both groups post-program revealed that there was a highly statistically significant reduction in present pain intensity scale and serum cortisol level (p < 0.05) in favor of the group (A). Conclusion. interferential current augmented by strengthening exercises for core muscles had a significant effect on low back pain after delivery, so this study supports the value of interferential current on low back pain after delivery patients.

Streszczenie

Wstęp. Nadwaga i otyłość wśród dzieci i młodzieży stanowi poważny problem wieku rozwojowego. Dotyczy on coraz to większej liczby dzieci. Coraz częściej dzieci spożywają dużą ilość przetworzonego i wysokokalorycznego pokarmu. Coraz mniej się ruszają, ponieważ swój czas wolny spędzają przed ekranem telefonu, telewizora czy komputera. Mała ilość ruchu sprzyja rozwojowi nadmiernej masy ciała u dzieci. Niska aktywność fizyczna powoduje również brak doskonalenia przez nie zdolności motorycznych. Ponadto wielu badaczy zwraca uwagę na negatywny wpływ nadmiernej masy ciała dziecka na jego sprawność fizyczną.
Cel pracy. Celem pracy była próba określenia poziomu wybranych zdolności motorycznych dzieci z nadwagą i otyłością oraz określenie zależności pomiędzy nadmierną masą ciała u dzieci a ich zdolnościami motorycznymi.
Materiał i metody. Grupę badawczą stanowiło 25 dzieci, w tym 12 dziewcząt i 13 chłopców w wieku od 12 do 18 lat. U badanych zdiagnozowano nadwagę i otyłość prostą. Do oceny sprawności motorycznej dzieci zastosowano próbę Romberga, testy tandemowe, rzut piłką lekarską, test palce-podłoga, bieg na 40 lub 60 m. Do oceny wydolności dzieci zastosowano próbę spiroergometryczną, 6MWT = 6-minutowy test marszowy i skalę Borga po wysiłku. Do oceny składu masy ciała wykorzystano wagę „Tanita”. U każdego dziecka wykonano to samo badanie i zastosowano te same testy, za pomocą których określono poziom wybranych zdolności motorycznych, wydolność oraz skład masy ciała.
Wnioski. Wyniki wskazują na taki sam rozwój zdolności motorycznych u dzieci z nadmierną, jak i prawidłowa masą ciała. Ponadto dzieci wykazywały taką samą tolerancję wysiłku.

Aerobic Versus Resistance Exercises on Liver Enzymes Following Burn Injuries

Streszczenie

Objective. The current study was conducted to evaluate which was more effective aerobic or resistance exercises in reducing plasma liver enzymes level following burn injuries. Materials and Methods. Sixty burned patients with burned body surface area about 30% to 50% participated in this study. Their ages ranged from 20-45 years. They were selected from Orabi Hospital and were divided randomly into two equal groups. Group (A) composed of 30 patients who received aerobic exercise in form of treadmill exercise for 45 minutes at 60-75% of maximum heart rate, 3times/week for 8 weeks beginning after their release from intensive care unit in addition to their traditional physical therapy program (splinting, stretching ex. and ROM ex.) and medical treatment. Group (B) composed of 30 patients who performed seven resistance exercises using dumbbells, sand bags and body weight for 2 and 3 series of 10 repetitions at intensity of 50%-70% of 1 repetition maximum for 45 minutes/session, 3times/week for 8 weeks beginning after their release from intensive care unit in addition to their traditional physical therapy program (splinting, stretching ex. and ROM ex.) and medical treatment. Method of evaluation was measurements of alanine transaminase (ALT) and aspartate transaminase (AST) liver enzymes by spectrophotometer device. Results. There was a_significant decrease in ALT and AST plasma liver enzymes levels in aerobic exercise group compared to that in resistance exercise group. Conclusion. Aerobic exercise was more effective than resistance exercise in reducing plasma liver enzymes level (AST and ALT) following burn injuries.

Streszczenie

High morning cortisol levels are associated with cognitive decline in diabetic polyneuropathy (DPN) patients. Cognitive decline is the fourth diabetic microvascular complication which has an effect on gait parameters in DPN patients. Objective. Our aim was to investigate influence of cognitive therapy (CT) on the cortisol levels and the spatiotemporal gait parameters during three different gait conditions (walking without cognitive task, walking with verbal fluency & walking with arithmetic task) in DPN patients. Materials and methods. Forty patients with type II diabetes mellitus (DM) with moderate polyneuropathy participated in this study. The patients group were assigned into two equal groups; study group (G1) treated by visuo-motor integration program via Rehacom system in addition to a selected physical therapy program for DPN patients, control group (G2) treated by the same selected physical therapy program. Rehacom procedure was used to assess cognitive function for all the patients and make CT for the study group (G1). Treatment was conducted three times per week for eight weeks. The cortisol level test was used to measure cortisol levels. The glycated hemoglobin (Hb1AC) test was used to detect the glycated Hb level. Different variables of spatiotemporal gait parameters (cadence, velocity & stride length) for all the patients were assessed using two dimension (2D) video-based motion analysis during the three different gait conditions. Results. The results showed significant reduction in cortisol levels post-treatment in (G1) compared to (G2) (P < 0.05). There was a significant improvement in all cognitive domains in (G1) compared to (G2) post-treatment (P < 0.05). Significant increase in all spatiotemporal gait parameters post-treatment in (G1) compared to (G2) during the three different gait conditions (P < 0.05). Conclusion. Visuo-motor integration activities have a positive effect on reducing serum cortisol levels and consequently improving cognitive function and gait parameters especially during dual task conditions while walking with verbal fluency task and walking with arithmetic task in DPN patients.

Streszczenie

Background. A burn is a type of injury to skin, or other tissues, caused by heat, cold, electricity, chemicals, friction, or radiation. Purpose. to investigate the effect isokinetic training on gait response in lower limb burned patients. Subjects and methods. Sixty patients were recruited from outpatient clinic of faculty of physical therapy, and El Kasr El-Ainy hospital in Cairo, all patients sign a consent form before starting; they wer randomly allocated into 2 equal groups and treated for 8 weeks as follow: Group A (Experimental group): 30 patients complaining of 2nd degree lower limb burns and treated with traditional physical therapy plus isokinetic training. Group B (Control group): 30 patients complaining of 2nd degree lower limb burns and treated with traditional physical therapy. Results. The results showed no significant difference in step length either in affected or non-affected limb within and between groups (P > 0.05), While for step time there was there was significant reduction in affected limb (P < 0.05) and significant increase in non-affected limb (P < 0.05) in favor to group A in compared to group B. Concerning for step width there was there was significant increase in affected limb (P < 0.05) and significant reduction in non-affected limb (P < 0.05) in favor group A in compared to group B. Conclusion. Isokinetic training had statistically significant effect on gait in lower limb burned patients in terms of step time and width of affected and non-affected limbs.

Posture deviation of head and neck in heavily using smart phone adolescents: An observational cross sectional study

Streszczenie

Objectives. To evaluate the head and neck posture in heavy using smart phone adolescents.
Methods. Two hundred twenty-four undergraduate male students were recruited from different faculties at Pharos University in Alexandria and assigned according to the number of hours spent daily on the smart phone into three groups. Group A used the smart phone for 10-16 hours/day (n = 25); group B used the smart phone for 5-9 hours/day (n = 64); group C used the smart phone for 1-4 hours/day (n = 11). The Postural assessment software (PAS/SAPO) was used to assess head and neck posture from digitalized pictures from anterior, right lateral and left lateral views.
Results. The horizontal alignment of the head and acromion from anterior view, and the vertical alignment of the head from right and left lateral views showed non-significant differences (p > 0.05) among three groups. However, the horizontal alignment of the head from right and left lateral views was significantly higher in group B compared to group A (p < 0.05), while there were non-significant differences (p > 0.05) between (group A versus C) and (group B versus C).
Conclusions. Heavily use of the smart phone negatively affects head and neck posture in adolescents and postural deviations in head and neck are directly proportional to the hours of using the smart phone daily.

Fizjoterapia po endoprotezoplastyce stawu biodrowego – analiza postępowania

Streszczenie

Wstęp. Choroba zwyrodnieniowa stawów biodrowych (ChZSB) stanowi poważny problem kliniczny i społeczny, została uznana za chorobę cywilizacyjną. Jest chorobą przewlekłą, niszczącą chrząstkę stawową, której objawy narastają powoli, w konsekwencji doprowadzając do znacznego ograniczenia ruchomości stawu biodrowego i wywołując silne dolegliwości bólowe. Zaawansowane zmiany zwyrodnieniowe to jedno z głównych wskazań do wykonania zabiegu endoprotezoplastyki.
Cel pracy. Ocena wpływu endoprotezoplastyki stawu biodrowego oraz prowadzonej fizjoterapii na funkcjonowanie pacjentów.
Materiał i metody. Badania zostały przeprowadzone wśród 30 chorych po zabiegu endoprotezoplastyki w Wojewódzkim Szpitalu Zespolonym w Kielcach w 2019 roku. Kwestionariusz ankiety WOMAC został wypełniany przez pacjentów dwukrotnie – przed i po leczeniu usprawniającym.
Wyniki. U wszystkich pacjentów nastąpiła znaczna poprawa sprawności fizycznej oraz ustąpienie odczuwalnego bólu podczas wykonywania codziennych czynności.
Wnioski. Zastosowanie endoprotezoplastyki oraz kompleksowego usprawniania wpłynęło na poprawę zakresów ruchomości stawów biodrowych u chorych, co przyczyniło się do poprawy chodu, zdolności funkcjonowania, zwiększyło sprawność fizyczną oraz znacząco polepszyło jakość życia.

Streszczenie

Purpose. This study was conducted to investigate the Effect of different postural correction exercises on forward head angle, degree of rounded shoulder in patients with forward head and rounded shoulder. Design. single blind randomized controlled trial Methods. 45 subjects were suffering from forward head and rounded shoulders. Their age ranged from 18 to 30 years. Participants were randomly divided into three equal groups. First group (A) received only forward head correction program. Second group (B) received only rounded shoulder correction program. Third group (C) received forward head correction program and rounded shoulder correction program. All participants received the treatment 3 times per week for 4 weeks. Craniovertebral angle, height of rounded shoulder was measured before and at the end of treatment program which continued for 12 sessions. Results. Statistical analysis using mixed design MANOVA showed that group (A), (B) and (C) revealed significant improvement in forward head and rounded shoulder post-treatment. Comparing the results among the three tested groups, it was revealed that there was no significant difference in the post-testing mean values of forward head and rounded shoulder between the three groups (A),(B) and (C) (p > 0.05) Conclusion: It was concluded that all interventions improved forward head and rounded shoulder so treatment of forward head may improve rounded shoulder deformity and treatment of rounded shoulder may improve rounded shoulder And that supports the theory that body segments affect each other.
